      A patient presenting with persistent pain is generally frustrated and distressed. Regardless of cause, persistent pain, which resists treatment, will result in loss of confidence and lead to frustration and impatience on the patient's part. Often, the same is true for the GP and anyone else involved, including the patient's family members. What follows for the patient however, can be diminishing quality of life including deterioration in sleep, activity levels, interpersonal relationships and mood.

  • Volume 1 Low-level Laser Therapy in Pain Management

      Low-level laser therapy (LLLT) is the use of non-thermal, visible or infrared laser irradiation (LI) to treat a variety of medical conditions, including acute or chronic pain. Although initially regarded with some scepticism, strong evidence of efficacy from systematic reviews and meta-analyses for clinically significant pain relief in many common conditions such as neck pain, chronic joint disorders, chronic pain and lateral epicondylitis has steadily accumulated. Indeed the World Health Organization Committee of the Decade of the Bone and Joint has added LLLT to guidelines for treatment of neck pain and the IASP fact sheet on myofascial pain also supports LLLT.

      When pain persists and there are no further investigations or treatment options worth exploring the practitioner is faced with advising their patient that "they'll just have to learn to live with it". The options then boil down to either pharmacological pain relief measures (ongoing analgesics, antidepressants, anticonvulsants, etc.) or non-pharmacological strategies. Often, it's a bit of both, but both have implications.

      The medical literature characterises migraine as a neurovascular disorder, emphasises the importance of genetics, and advocates pharmacological treatment. Whilst there is empirical evidence to support all these propositions, it does not alter the fact that headache disorders generally and migraine specifically occur in a developmental and psychosocial context. That is, whether you experience migraines depends on how you lead your life - family relationships, work environment, exercise patterns, diet, sleep routines, and so forth.

  • Volume 1 Exercise in Musculoskeletal Pain Management

      Persisting pain, pain that persists for longer than the expected healing time, is having a significant impact on the daily life of around 10 percent of Australians at any one time. The physical origins of persisting musculoskeletal pain are varied and include non-specific back pain, neck and shoulder pain and orthopaedic post-operative pain. As pain duration increases, psychosocial factors become more prevalent in the cycle of pain and impaired function. Common indicators for problematic persisting pain are: ineffective multiple medical interventions and treatment modalities; heightened levels of anxiety and distress; low activity levels for fear of pain and injury; catastrophic thinking; managing and coping poorly; and medication dependency. Early identification of these patients and progression to exercise treatment using a bio-psychosocial approach, in conjunction with appropriate drug therapy when necessary, is an effective management tool.

      Chronic pain is a common and complex challenge - to patients, practitioners and modern medicine itself. Although pain is conceptualised in a 'biopsychosocial' framework that identifies somatic, psychological and socioenvironmental contributions, the person in pain is usually processed through a narrow biomedical model, where the emphasis is on finding - and treating - an underlying disease state that 'causes' the pain. This is the so-called "broken part" approach, extrapolating from the acute to the chronic situation. However by the time pain has been present daily for three months and "red-flag" conditions have been excluded, it is unlikely that a treatable underlying somatic condition will be found. Evolving understanding suggests that a permanent change in nociceptive ("damage-signalling") networks in the person's central nervous system may have occurred.

      In the past neuropathic pain was defined as being initiated or caused by a primary lesion or dysfunction in the nervous system., However recently it has been more tightly defined as "pain arising as a direct consequence of a lesion or disease affecting the somatosensory system". In a significant percentage of patients neuropathic pain will arise where disease or trauma affects the sensory peripheral nerves or somatosensory regions of the spinal cord or brain.
