  • Spiritual Support in Advanced Heart Failure : A Randomized Controlled
           Feasibility Study
    • Heart failure is a complex, progressive disease with an uncertain trajectory.  Those with advanced heart failure (AHF) experience substantial spiritual needs.  Spiritual interventions may enhance quality of life and reduce anxiety and depression,  but studies are limited and none have focused exclusively on the AHF patient population.  This is the first feasibility randomized controlled trial (RCT) to ascertain the clinical and cost effectiveness of a spiritual intervention (spiritual support) in AHF  patients. A total of 47 AHF patients were randomized to control (standard care, n = 25)  or intervention (standard care plus spiritual support, n = 22) groups. Spiritual support  consisted of a one-hour discussion facilitated by trained volunteers using a “Spiritual  Enquiry Tool” at two-monthly intervals over six months. Participants completed validated  measures of spiritual well-being, depression/anxiety, and health-related quality  of life (QoL). Purpose-designed questionnaires gathered information on demographics,  NHS resource use, confounding factors, and satisfaction with spiritual support. The  new information was to help researchers design an RCT to determine the clinical and  cost effectiveness of spiritual support within a holistic model of care for AHF patients.  Future trends worthy of further investigation include (i) the possible positive effect of  spiritual support on QoL and anxiety, and (ii) possible lower NHS resource use and cost  savings in patients receiving spiritual support. Overall, the key message of this study is  that researchers must evaluate whether the cost of running a well-designed trial of this  nature is justified in the current economic climate, where funding bodies are looking  for value for money.

  • Making Courageous Conversation in Healthcare (Part II) : Exploring the
           Impact of Values Based Reflective Practice® on Professional Practice
    • This article is the second in a two-part series describing the evidence base for Values Based Reflective Practice (VBRP®), as it is used within the National Health Service (NHS) in Scotland (Bunniss, 2020). Here, the author discusses the conceptual underpinnings of VBRP®, considering the literature concerned with “soul and role” in professional education, and asks whether health and social care professionals seeking to rediscover the vocational hopefulness behind their working day might find some encouragement through this new reflective practice model. Qualitative evidence is presented from ten semi-structured interviews with a range of healthcare professionals who are using VBRP® in practice.

  • Making Courageous Conversation in Healthcare (Part I) : Designing and
           Evaluating Values Based Reflective Practice® Training for Healthcare
           Professionals in Scotland
    • Values Based Reflective Practice (VBRP®) was first developed by National  Health Service (NHS) Education for Scotland in 2011 to help health and social care staff  have courageous conversations about the relationship between who they are and what  they do at work. The stated hope of VBRP® is to help staff reclaim a sense of meaning  and purpose in their professional lives, and reawaken the memory of why they chose to  work in a caring vocation. This article describes the history and development of VBRP®,  the elements of this reflective practice model, and presents the first quantitative evaluation  of VBRP® training with evidence gathered from 138 healthcare, social care, and  third sector workers. This study is the first of a two-part series; the second article (Part  II) locates VBRP® within healthcare literature concerned with “soul and role,” and it  presents further qualitative evidence for and wider discussion of the model.

  • “Dog” is “God” Spelled Backward: “Poppy Jingles,” the Staff
           Well-being Spaniel
    • This brief report describes the unique and innovative work of a hospital chaplaincy  “Good Samaritan” staff welfare dog named “Poppy Jingles.” As the importance of  stress prevention and burnout among National Health Service staff (UK) has been highlighted  in current policy, the initiative of animal-assisted therapy is both timely and  significant. The relevance of inter-species well-being is detailed in this article, describing  what can be considered as Poppy’s “nonhuman charisma.” This includes eliciting  specific aspects of well-being which can be thought to correspond to eudaimonic states  of human flourishing.

  • Validation of the Patient Reported Outcome Measure of Spiritual Care
           (PROM) in an Australian Setting
    • A multi-site study across five hospitals in Australia, undertaken as part of the study to evaluate the validity and reliability of the Patient Reported Outcome Measure of spiritual care (PROM) scale. Two hundred and eighty-one patients participated in the study. Data were analysed using AMOS (version 24) to assess the validity and reliability of the PROM using confirmatory factor analysis (CFA). Predictive (criterion) validity of the scale was also assessed by evaluating the relationship of the PROM scale with the quality of spiritual care. The results demonstrated acceptable validity, model-based reliability and predicative validity for the PROM scale. It is recommended that a further study for cross-validation of the scale in different samples and populations is undertaken. Controlling for other variables such as (positive-negative affect or personality characteristics) for future studies might shed more light on the content validity of  the PROM. The study has implications for the longer-term goal of building an evidence base for a spiritual care framework for Australian healthcare organizations and beyond.  It is hoped that the data collected will enable spiritual health organizations to contrib- ute to a larger database of evidence, both nationally and internationally.

  • A Timeless God' A Rejoinder to van Holten and Walton
    • In this article John Swinton responds to the arguments detailed by Wilko van Holten2 and Martin Walton3 regarding the theology of the “Timelessness of God” and its relationship to disability and dementia (see ‘A Critical Appraisal of John Swinton’s Theology of Time and Memory’ https://doi.org/10.1558/hscc.40137). Swinton acknowledges the value of van Holten and Walton’s critique but points out that, by ignoring the genre of his work – pastoral theology – they have misunderstood critical aspects and failed to acknowledge important issues around the relationship between concepts and practice. The Response acknowledges that the argument for the timelessness of God is complex and problematic but argues that reflection on this requires practical exploration as well as theoretical conjecture. Swinton offers insights into how the reflection on the nature of God’s involvement with time, properly construed, can throw important practical and theological light on to the lives of people with dementia and people with brain injuries.

  • A Timeless God' A Critical Appraisal of John Swinton's Theology of
           Time and Memory
    • In two recent publications the practical theologian John Swinton has given  extensive attention to the concepts of time and memory. He considers in what way these  have a bearing on how, in Western culture, we view and treat people with disabilities or  other kinds of impairment. The authors of this article argue that despite the many pastoral  merits of Swinton’s thinking on these subjects, his theology of time and memory is  unsatisfactory as far as his interpretation of the relevant terms is concerned. Especially his  appeal to the doctrine of divine timelessness which is shown to be flawed and unnecessary  for his overall argument.   • Why are Some Healthcare Chaplains Registered Professionals and Some are
           Not' A Survey of Healthcare Chaplains in Scotland
    • The professional status of UK healthcare chaplains remains partial, with voluntary accreditation effective in achieving around 50% registration. This study set out to elicit reasons for this by surveying healthcare chaplains working in Scotland. An online survey was created to gather demographic details and chaplains’ opinions on the importance of five key elements of professional status: A body of knowledge that underpins practice; A code of professional ethics; An occupational organization controlling the profession; Substantial intellectual and practical training; and Provision of a specialized skill or service. Most respondents (38/43) agreed that chaplains should belong to a professional body in order to maintain standards, ensure accountability and formalize professional development. They said that registration reinforced their professional status, added credibility and a clear governance structure to protect the public. However, a minority felt disconnected from the professionalization agenda. This paper discusses the reasons for this. Further UK and international studies into the professional status of chaplains are planned.
