    • Abstract: Resumen Objetivo: El objetivo es valorar la efectividad de una intervención de mejora de hábitos posturales para prevenir los trastornos musculoesqueléticos en el sector limpieza. Método: Se diseñó una intervención específica para el colectivo de limpieza, mediante una serie de acciones continuadas en el tiempo dirigidas a fomentar una buena higiene postural y el ejercicio físico. Se aplicó la intervención en dos años distintos (estudio original y estudio de replicación), a una muestra de trabajadores del subsector Limpieza general de edificios, pertenecientes a distintos centros de trabajo de una misma empresa. Se incorporaron herramientas de valoración de las intervenciones basadas en la autopercepción y el registro de los hábitos por un observador externo. Resultados: En el primer estudio se obtuvo un aumento del 92% de las personas que manifestaban adoptar siempre o casi siempre buenos hábitos posturales durante las tareas de limpieza. En el estudio de replicación esta mejora fue del 67%. Las principales mejoras se observaron en: flexionar las piernas, mantener la espalda recta, alternancia de brazos y buscar puntos de apoyo. En la valoración observacional de los hábitos posturales se constató la mejoría autopercibida. Conclusiones: El modelo de intervención fundamentado en una serie de acciones continuadas en el tiempo, favoreciendo el empoderamiento de los trabajadores puede ser efectivo para mejorar, a corto plazo, los hábitos posturales del colectivo de limpieza. Se precisa de más estudios para valorar si el cambio de hábitos se mantiene a largo plazo y su efecto sobre la disminución de los trastornos musculoesqueléticos.Abstract Introduction: The aim of this study was to assess the effectiveness of an intervention for improving postural habits among cleaning personnel, to prevent musculoskeletal disorders. Methods: We designed a specific intervention for cleaning personnel, consisting of a set of continuous actions focused on promoting good postural habits and physical activity. The intervention was implemented over two different years (initial implementation and replication phases) in a sample of workers in the general building cleaning subsector who belonged to different work centers of the same company. The evaluation of the intervention consisted of self-reported worker feedback and recording of habits by an external observer. Results: In the initial implementation phase, we observed a 92% increase in participants who reported they always or almost always adopt good postural habits during cleaning tasks. In the replication phase, this improvement was sustained by about 67% of participants. The main improvements were observed in: bending knees and hips, keeping the back straight, alternating use of arms and using support while bending forward. The external observation of postural habits corroborated the improvement reported by the workers. Conclusions: This intervention approach, consisting of different actions, could be continued over time, facilitating worker empowerment, and may be effective in improving the postural habits of cleaning workers in the short term. More research is needed to assess whether the change in habits is maintained over the long term and what its effect would be on the reduction of musculoskeletal disorders.
       
  • Evolución temporal de la incidencia por incapacidad temporal de origen
           común según tramos de duración (2009-2018)

    • Abstract: Resumen Objetivo: Describir la evolución temporal (2009-2018) de la incidencia de la incapacidad temporal por contingencia común (ITCC) según tramos de duración en España. Métodos: Se consideraron casos de ITCC de la población afiliada de una mutua laboral. Se obtuvo la incidencia global de la incapacidad temporal por ITCC y por tramos de duración (1-3 días, 4-15 días, 16-30 días, 31-90 días y >90 días). Se realizó un estudio ecológico de series temporales considerando un punto de inflexión en 2013. Se calculó el porcentaje de cambio anual e intervalo de confianza del 95%. Los análisis se estratificaron por edad y sexo. Resultados: La incidencia global pasó de 35,3 casos por cada 100 trabajadores-año en 2009 a 25,2 en 2013. A partir del 2014 se observa un ascenso sostenido de la incidencia de ITCC, finalizando el 2018 con 34,1 casos por cada 100 trabajadores-año. La incidencia global está determinada fundamentalmente por procesos menores a 16 días en población joven. El descenso en 2009-2013 se produjo en todos los tramos de duración, en especial en 4 a 15 (PCA=-11,2; IC 95%=(-14,1 a -8,2)) y más de 90 días (PCA=-9,4; IC 95%=(-15,5 a -2,8)), principalmente en jóvenes. El ascenso en 2013-2018 se observó en todos los tramos, siendo el incremento más significativo en tramos de 1-3 días (trabajadores de menor edad: PCA=18,9; IC 95%=(14,8 a 23,2)) y en más de 90 días (principalmente en mayores). La evolución de la ITCC presentó un patrón similar en ambos sexos. Conclusiones: La descripción temporal de la incidencia de ITCC por tramos de duración ofrece una información detallada de la incapacidad temporal. Estos resultados son útiles para profesionales de la prevención y gestión de la ITCC.Abstract Objective: We describe time trend incidence (2009-2018) of non-work related Sickness Absence (SA) segmented by duration of episodes in Spain. Methods: We used SA cases from a health insurance company (“mutua”) in Spain. Overall non-work related SA incidence and incidence by duration of episodes (1-3 days, 4-15 days, 16-30 days, 31-90 days and> 90 days) were obtained. A time series ecological study was carried out with an inflection point in 2013. The annual percentage of change and 95% confidence interval were obtained. The analyses were stratified by age and sex. Results: Overall incidence went from 35.3 cases per 100 workers-years in 2009 to 25.2 in 2013. From 2014, there is a sustained rise in the incidence of ITCC, ending 2018 with 34.1 cases per 100 workers-years. The overall incidence is determined mainly by processes less than 16 days in young population. The decrease in 2009-2013 occurred in all the duration segments, especially in 4 to 15 (APC=-11,2; 95% CI=(-14,1 a -8,2)) and more than 90 days (APC =-9,4; 95% CI =(-15,5 a -2,8)), mainly in young people. The rise in 2013-2018 was observed in all the segments, with the largest significant increase in sections of 1-3 days (younger workers: APC =18,9; 95% CI =(14,8 a 23,2)) and in more than 90 days (mainly in older ones). Time trend of SA showed similar pattern in both sexes. Conclusions: Time trend analysis of SA incidence by duration segments offers a detailed information of SA. These results are useful for professionals in the prevention and management of SA.
       
  • Influencia de las medidas preventivas ergonómicas en el desarrollo de
           secuelas por patología no traumática del hombro

    • Abstract: Resumen Objetivo: Describir las secuelas referidas en los casos atendidos por patología no traumática del hombro . Y conocer cómo se asocian con la presencia de secuelas las características sociodemográficas, disponer de conocimientos en prevención de riesgos laborales y los requerimientos ergonómicos del puesto de trabajo. Métodos: Estudio observacional transversal en una población de 345 pacientes atendidos durante un año en las consultas de un centro de atención primaria de salud en Alicante. Mediante la prueba de la Ji-cuadrado se estudiaron las asociaciones entre las secuelas y el resto de variables. Resultados: El 53,9% (n=186) de los participantes refirieron presentar secuelas derivadas, sobre todo dolor ocasional asociado o no a limitación de la movilidad. Son factores de riesgo: un bajo nivel de estudios, actividades de limpieza y sector industrial, alzar los brazos por encima del hombro y la monotonía en las tareas. Sin embargo, son factores protectores: el sexo masculino, disponer de información sobre los riesgos laborales, equipos de protección individual, existencia de Servicio de Prevención de Riesgos Laborales, evaluación de riesgos, plan de prevención y los reconocimientos médicos periódicos. Conocer los riesgos del puesto se mostró como factor de protección independiente frente al desarrollo de secuelas (ORa=0,2; IC95%=0,1-0,4). Sin embargo, alzar el brazo por encima del hombro, se comporta como factor de riesgo independiente para el desarrollo de secuelas por patología del hombro (ORa=2,1; IC95%=1,0-4,3). Conclusiones: La combinación de estrategias ergonómicas y formativas parecen útiles para la prevención del desarrollo de secuelas a nivel del hombro.Abstract Objective: To describe the sequelae referred to the cases attended by non-traumatic diseases of the shoulder . To know how sociodemographic characteristics, preventive knowledge about the workplace and ergonomic requirements are associated with the presence of sequelae. Methods: Cross-sectional observational study in a population of 345 patients attended for a year in the consultations of a primary health care center in Alicante. Through the Chi-square test, the associations between the sequels and the rest of the variables were studied. Results: 53.9% (n=186) of the participants reported having sequelae derived, especially occasional pain associated or not with limitation of joint mobility. They are risk factors: a low level of studies, cleaning activities and industrial sector, raising the arm over the shoulder and the monotony in the workplace. However, they are protective factors: male sex, having information on occupational risks, Personal Protective Equipment, the existence of a Labor Risk Prevention Service, Risk Assessment, Prevention Plan and periodic medical examinations. Knowing the risks of the position was shown as an independent protection factor against the development of secondary sequelae in the shoulder (ORa=0,2; IC95%=0,1-0,4). However, to raise the arm above the shoulder, behaving as an independent risk factor for the development of sequelae due to shoulder pathology (ORa=2,1; IC95%=1,0-4,3). Conclusions: The combination of ergonomic and training strategies seems useful in the prevention of the development of shoulder-level sequelae.
       
  • Revisión bibliográfica sobre la eficacia del ejercicio excéntrico como
           tratamiento para la tendinopatía del tendón de Aquiles

    • Abstract: Resumen Introducción: las tendinopatías son consecuencia de cargas excesivas en el tendón, por lo que son frecuentes en el ámbito laboral debido a movimientos repetitivos y posturas forzadas. La tendinopatía de Aquiles se caracteriza por una respuesta de curación desorganizada, asociada a cierto grado de neovascularización, siendo el tratamiento conservador la estrategia inicial. Objetivos realizar una revisión bibliográfica para analizr la efectividad de los tratamientos utilizados en la tendinopatía aquilea a partir de los estudios incluidos. Material y métodos: revisión bibliográfica de artículos científicos consultando las bases de datos PubMed y The Cochrane Library utilizando las palabras clave “((tendinopathy) AND (Achilles Tendon) AND (exercise OR exercise therapy OR Muscle Stretching Exercises OR physical therapy modalities OR rehabilitation))”. Resultados: Se obtuvieron 635 refrencias en Pubmed y 134 en Cochrane Library, de los cuales 14 cumplieron los criterios de inclusión. Los estudios seleccionados mostraron que, en general, los ejercicios excéntricos son una terapia efectiva aplicada entre 6 y 12 semanas en la tendinopatía de Aquiles, sin combinar o combinada con la plataforma de vibración, complementos nutricionales, tratamiento de tejidos blandos, terapia de láser o terapia de ondas de choque. Conclusión: la revisión muestra que el ejercicio excéntrico presenta los mejores resultados en el tratamiento de la tendinopatía aquilea.Abstract Introduction: Tendinopathies are a consequence of excessive tendon loads, so they are frequent both in athletes and in the workplace due to repetitive movements and forced postures. Achilles tendinopathy is characterized by a disorganized healing response, associated with a certain degree of neovascularization, with conservative treatment being the initial strategy. Objectives: To carry out a bibliographic review in order to identify the different eccentric exercise protocols and their effectiveness in Achilles tendinopathy. Methods: A literature review of scientific articles has been implemented in the databases of PubMed and The Cochrane Library databases using the key words “((tendinopathy) AND (Achilles Tendon) AND (exercise OR exercise therapy OR Muscle Stretching Exercises OR physical therapy modalities OR rehabilitation))” Results: 635 studies were obtained from Pubmed and 134 from Cochrane Library of which 14 met all the inclusion criteria. Selected studies showed that, in general, eccentric exercises are an effective therapy applied between 6 and 12 weeks in Achilles tendinopathy, either on their own or combined with the vibration platform, nutritional supplements, soft tissue treatment, laser therapy or shock wave therapy. Conclusion: this review showed eccentric exercise has a great scientific evidence in Achilles tendinopathy treatment.

    • Abstract: Resumen Introducción: las tendinopatías son consecuencia de cargas excesivas en el tendón, por lo que son frecuentes en el ámbito laboral debido a movimientos repetitivos y posturas forzadas. La tendinopatía de Aquiles se caracteriza por una respuesta de curación desorganizada, asociada a cierto grado de neovascularización, siendo el tratamiento conservador la estrategia inicial. Objetivos realizar una revisión bibliográfica para analizr la efectividad de los tratamientos utilizados en la tendinopatía aquilea a partir de los estudios incluidos. Material y métodos: revisión bibliográfica de artículos científicos consultando las bases de datos PubMed y The Cochrane Library utilizando las palabras clave “((tendinopathy) AND (Achilles Tendon) AND (exercise OR exercise therapy OR Muscle Stretching Exercises OR physical therapy modalities OR rehabilitation))”. Resultados: Se obtuvieron 635 refrencias en Pubmed y 134 en Cochrane Library, de los cuales 14 cumplieron los criterios de inclusión. Los estudios seleccionados mostraron que, en general, los ejercicios excéntricos son una terapia efectiva aplicada entre 6 y 12 semanas en la tendinopatía de Aquiles, sin combinar o combinada con la plataforma de vibración, complementos nutricionales, tratamiento de tejidos blandos, terapia de láser o terapia de ondas de choque.   • Denegación de incapacidad versus ineptitud laboral

    • Abstract: Resumen El retorno al trabajo tras una baja médica prolongada comporta una valoración tanto de la capacidad laboral como de la aptitud para el trabajo. Nos encontramos con una trabajadora auxiliar administrativa con lesión congénita en cuerdas vocales, a quien se le deniega la incapacidad permanente total. Es valorada en el reconocimiento médico tras ausencia prolongada resultando apta con limitaciones. Tras la revisión de la evaluación de riesgos de su puesto y de todos los compatibles con su categoría profesional se procede a despido objetivo por ineptitud sobrevenida. La valoración de la capacidad/incapacidad laboral debe ser una valoración multidisciplinar, contemplando de forma integrada el diagnóstico, el tratamiento, la evolución, la perspectiva preventiva y la laboral.Abstract The return to work process following a prolonged medical leave includes an assessment of both work capacity and ability to meet essential job functions. We present the case of an administrative assistant with a congenital vocal cord injury who was denied total permanent disability. The initial fitness for duty examination after her prolonged absence resulted in a determination of fitness, but with limitations. After performing a risk assessment and review of the essential job functions of her occupation, she was dismissed based on inability to meet the essential demands of the job. Assessments of work capacity/disability must be multidisciplinary, integrating the diagnosis, treatment, course of the illness, and conducted from both a preventive and work perspective.
