  • Alteraciones clínicas y humorales como factores pronóstico en pacientes
           con traumatismo craneoencefálico grave

      Abstract: Introducción: El traumatismo craneoencefálico grave (TCEG) es el resultado de la aparición brusca sobre la cabeza de una fuerza física de tal magnitud que provoque lesiones en el cuero cabelludo, cráneo, meninges y tejido encefálico. Objetivo: Determinar el valor pronóstico de las alteraciones clínicas y humorales en pacientes con traumatismo craneoencefálico grave. Diseño Metodológico: Estudio observacional, analítico de cohortes en la Unidad de Cuidados Intensivos (UCI) del Hospital ‘’Carlos Manuel de Céspedes‘’ de Bayamo, Granma en el período del 1 de enero de 2007 al 31 de diciembre de 2011. Se seleccionó una muestra de 100 pacientes. Se realizó un análisis univariado con cada una de las variables estimando el riesgo relativo de morir (RR) y su intervalo de confianza al 95% (IC 95%). Se aplicó la estrategia multivariada basada en el ajuste de un modelo de regresión logística con todas las variables. Resultados: El análisis estadístico multivariado demostró que la hipoxemia (OR ajustado 2.3; IC 95 % 2.5-208,6) y la escala de coma de Glasgow (OR ajustado 4,9 IC 95 % 1,7-11,8) tuvieron una relación significativa con la muerte. Los pacientes masculinos mayores de 55 años sin antecedentes de alcoholismo ni tabaquismo fueron los que con mayor frecuencia fallecieron como consecuencia de un traumatismo cráneo encefálico grave. Conclusiones: De las variables clínicas y humorales estudiadas, sólo la hipoxemia y la escala de coma del Glasgow tuvieron valor para predecir el pronóstico de pacientes con traumatismo craneoencefálico grave.ABSTRACTIntroduction: Severe head injuries is the result of the abrupt appearance on the head of a physical force of such a magnitude that provokes lesions in the hairy leather, skull, meninges and knitted brain. Objective: To determine the value presage of the clinical and humoral alterations in patient with severe head injuries. Methodological design: An observational, analytic of cohorts study in the Unit of Intensive Cares of the ''Carlos Manuel de Céspedes'' Hospital of Bayamo, Granma in the period of January 1 of 2007 at December 31 of 2011 was carried out. A universe of 100 patients was selected. An univariado analysis with each one of the variables estimating the relative risk of dying (RR) and their interval of trust to 95% (IC 95%) was carried out. The multivariate strategy was applied based on the adjustment of a model of logistical regression with all the variables. Results: The statistical multivariate analysis demonstrated that the hypoxemia (adjusted OR 2.3; IC 95% 2.5-208,6) and the scale of coma of Glasgow (adjusted OR 4,9; IC 95% 1,7-118) had a significant relationship with the death. The masculine patients older than 55 years without antecedents of alcoholism neither smoking habit were those that most often died as consequence of a severe head injuries. Conclusions: Of the clinical and humoral variables studied, only the hypoxemia and the scale of coma of Glasgow had value to predict the presage of patient with severe head injuries.

  • Consumo de antihipertensivos en tres años consecutivos en el Hospital
           Universitario “Manuel Fajardo”

      Abstract: Introducción: El estudio fármaco-económico del consumo de antihipertensivos (AH) en una institución de salud deviene en importante indicador de eficiencia y retroalimentación de las posibles implicaciones de su uso para los facultativos. Objetivo: Caracterizar los niveles de empleo de antihipertensivos en nuestro medio como indicador del uso racional de medicamentos por parte de los profesionales de la salud con facultad prescriptiva. Materiales y método: Se realizó un estudio observacional, descriptivo, retrospectivo y longitudinal en el Hospital “Manuel Fajardo” en La Habana, durante los años 2012, 2013 y 2014. El universo estuvo determinado por todos los antihipertensivos que tuvieron indicación facultativa intrahospitalaria en los años comprendidos (N=14). No se utilizó técnica de muestreo, pues se emplearon todos los antihipertensivos en uno u otro análisis dentro de la investigación. Los datos se obtuvieron en la farmacia del hospital, en los índices de consumo de estos medicamentos por los pacientes hospitalizados, así como el costo unitario en moneda nacional de cada droga. Resultados: El antihipertensivo más utilizado fue el Enalapril (80340 unidades). Los siete medicamentos más empleados significaron un costo cercano a los cien mil pesos. Conclusiones: Los antihipertensivos más empleados durante los años del estudio fueron el Enalapril, la Espironolactona y el Captopril, cabe referir que el primero de estos generó el mayor gasto económico. Introduction: The drug-economic study of the consumption of antihypertensives (AH) in a health institution becomes an important indicator of efficiency and feedback of the possible implications of its use for doctors. Objective: To characterize the levels of antihypertensive use in our environment as an indicator of the rational use of medicines by health professionals with a prescriptive capacity. Materials and methods: An observational, descriptive, retrospective and longitudinal study was conducted at the "Manuel Fajardo" Hospital in Havana, during the years 2012, 2013 and 2014. The universe was determined by all antihypertensive patients who had an inhospital Years included (N = 14). No sampling technique was used, since all antihypertensives were used in one or the other analysis within the investigation. The data were obtained at the hospital pharmacy, the rates of consumption of these drugs by hospitalized patients, as well as the unit cost in national currency of each drug. Results: The most commonly used antihypertensive was Enalapril (80,340 units). The seven most used drugs meant a cost close to one hundred thousand pesos. Conclusions: The most commonly used antihypertensive agents during the study years were Enalapril, Spironolactone and Captopril. It is worth noting that the first of these drugs generated the highest economic expenditure.

  • Prolapso de la válvula mitral, una revisión bibliográfica

      Abstract: Debido a la prevalencia del prolapso de la válvula mitral (PVM) en la población y la diversidad de criterios acerca de ella, se realizó una revisión bibliográfica con el objetivo de caracterizar el prolapso de la válvula mitral, donde se analizaron 30 referencias bibliográficas. La distribución del PVM respecto al sexo, aunque es cuestionada, parece afectar con mayor severidad a los hombres. Esta enfermedad ha sido considerada como una de las valvulopatía benignas más frecuentes en la edad pediátrica. Existe un subgrupo de pacientes con peor pronóstico, fundamentalmente aquellos en los que el PVM se acompaña de válvulas muy afectadas. En pacientes asintomáticos se aconseja realizar un seguimiento clínico-ecocardiográfico que permita estratificar el riesgo de complicaciones. ABSTRACTBecause of the prevalence of mitral valve prolapse in the population and the diversity of views about it, a literature review was conducted with the aim of characterizing the mitral valve prolapse, where 30 references were analyzed. PVM distribution regarding sex, although it is questioned, seems to affect more severely men. This disease has been considered one of the most frequent benign valve disease in children. There is a subgroup of patients with worse prognosis, mainly those in which the PVM is accompanied by very affected valves. In asymptomatic patients are advised to conduct a clinical-echocardiographic follow-up that allows stratify the risk of complications.

      Abstract: Presentamos un caso clínico de fractura de pene no asociada a ruptura de uretra.  Se trata de un paciente masculino, seropositivo al VIH, que luego de un coito vaginal, comenzó a manifestar signos y síntomas flogísticos y deformidad en el miembro viril. Hecho el examen físico, se concluye que padece una fractura de pene ante la cual se adopta una conducta adecuada a las normas del hospital. :We report a case of penile fracture not associated with rupture of the urethra. It is a male patient, HIV positive, who after a vaginal intercourse, began to manifest signs and symptoms of flogistics and deformity in the manly limb. After the physical examination, it is concluded that he suffers from a fractured penis before which he adopts a behavior appropriate to the hospital's standards.

      Abstract: El análisis de la situación de salud, constituye una necesaria contribución al desarrollo del sistema de salud, además de considerarse como una de las competencias básicas del médico general. El desarrollo de las ciencias médicas en la medida que transcurren los años trae consigo cambios sustantivos en el modo de actuar del médico y por ende la forma de resolutividad a los problemas identificados en el primer nivel de atención es diferente. Esto conllevó a proporcionarle al estudiante de medicina desde su formación básica las herramientas necesarias para lograr su interpretación como la investigación más importante que se desarrolla en la Atención Primaria de Salud. En este orden lógico del pensamiento se concedió a los estudiantes del segundo año una metodología a seguir para realizar el trabajo investigativo final de la asignatura de Medicina Comunitaria que le permita identificar los problemas de salud en la comunidad que se le asignó hasta la búsqueda de prioridad de los mismos.ABSTRACTThe analysis of the health situation is a necessary contribution to the development of the health system also regarded as one of the core competencies of the general practitioner. The development of medical science as far as the years pass brings substantial changes in the mode of action of the doctor and therefore the form of problem-solving to the problems identified at the primary care level is different. This led to providing the medical student from basic training necessary to achieve its interpretation as the most important research carried out in the Primary Health Care tools. In this logical order of thought to the students of the second year it was awarded a methodology for making the final investigative work on the subject of Community Medicine to help you identify health problems in the community that was assigned to the search priority thereof.

      Abstract:  La vida de José Martí ha sido ampliamente estudiada, aunque existen elementos dispersos en la literatura y escasamente explorados que resultan novedosos. La presente revisión tiene como objetivo describir aspectos curiosos y poco conocidos del universo martiano, para lo cual se revisaron 21 referencias bibliográficas. Se concluyó que: el Apóstol utilizó diferentes pseudónimos por motivos políticos, alcanzó el mayor grado militar del Ejército Libertador, su hijo formó parte las luchas por la independencia de Cuba; además, padeció de enfermedades como sarcoidosis, laringitis aguda y un tumor testicular, por lo que fue atendido por varios médicos como su amigo Fermín Valdés Domínguez. Su cadáver tuvo un total de cinco entierros.ABSTRACTThe life of José Martí has been widely studied, however, there are scattered and barely explored elements in literature that prove to be novelty. This literature review aims to describe curious and barely known elements in the universe of José Martí. 21 bibliographies were reviewed. It was concluded that: the Apostle used several pseudonyms due to political reasons; he reached the highest military grade in the Liberation Army; his son contributed in the battles to achieve the independence of Cuba; Martí also suffered Martí also suffered various diseases like sarcoidosis, acute laryngitis and testicular tumor and several physicians treated himlike his friend Fermín Valdés Domínguez. His body was buried five times.

      Abstract: El Apóstol le concedía un valor indiscutible a la ciencia, donde en sus análisis y reflexiones en este campo realizó algunas valoraciones propias de las ciencias médicas. El Maestro tenía un alto concepto de la misión de las mismas,tema que trató no sólo en su obra literal,sino que también puso en práctica en la manigua redentora. En sus escritos hizo referencia al empleo de la medicina verde y a la higiene del trabajo,confiriéndole un especial significado a todo lo relacionado con la medicina preventiva, a la vez que se preocupó por resaltar el avance de las ciencias médicas cubanas de su época. Este aspecto de su vida motivó la presente investigación, que tiene como objetivo valorar algunas de las concepciones de José Martí sobre las ciencias médicas. Para el desarrollo de este tema se revisaron 12 artículos localizados en  libros y revistas y en formato digital.ABSTRACTThe Apostle granted an unquestionable value to the science, where in his analyses and reflections in this field carried out some valuations characteristic of the medical sciences. The Teacher had a high concept of the mission of the same ones, fear that it not only tried in his literal work, but rather it also applied in the battle fields. In their writings he makes reference to the employment of the green medicine and the hygiene of the work, conferring a special meaning to all the related with the preventive medicine, at the same time that he worried to stand out the advance of the Cuban medical sciences of their time. This aspect of its life motivated the current investigation that has as objective to value some of José Martí's conceptions about medical sciences. For the development of this topic 12 articles located in books and magazines and in digital formatwere revised.

      Abstract: La universidad contemporánea exige cambios y consecuentemente la búsqueda de nuevas estrategias de enseñanza-aprendizaje, de manera que las habilidades y capacidades de los estudiantes se avengan con las exigencias de los nuevos retos en su preparación integral en las diferentes ramas de la ciencia y la técnica. Es por ello que las ciencias Administrativas y Humanísticas deben contribuir al desarrollo de habilidades que conduzcan al educando a un desempeño profesional independiente y creativo. Este estudio tiene como objetivo describir algunos posicionamientos teóricos sobre las estrategias didácticas de evaluación como una herramienta eficaz y su influencia en el desarrollo del proceso de enseñanza aprendizaje del estudiante. Este proceso ha estado vinculado directamente a los cambios políticos, económicos y sociales que se han generado en los diferentes países, donde el desarrollo social, de la ciencia, la técnica, la práctica y la investigación, han obligado a aplicar -no en el discurso, sino en la práctica efectiva- los conceptos de eficiencia, calidad y exigencia en los procesos educativos que realizan las universidades, cada vez más comprometidas y en interacción con la sociedad. Los docentes en su función tienen que mejorar la manera de enseñar, para lo cual deben prepararse para asumir ese reto y mantenerse siempre competitivos, sin miedo a los cambios tecnológicos que se estén generando, esta preparación e innovación debe ir en el sentido de estar al día con el uso y manejo de las Tecnologías de la Informática y las Comunicaciones, para tener una educación superior de excelencia y calidad. ABSTRACT
