      Abstract: Program of One House One Larvae Monitoring Specialist (or G1R1J) is a community empowerment endeavor to eradicate mosquito nest of Dengue Hemorrhagic Fever. Program involves one of the family members to be a Larvae Monitoring Specialist at home. Factors influencing community behavior to implement a program are driving factors such as attitudes, and community leader's behavior, health staff, and other health staff that is a community reference group. The study's objective is to know the cadres' perception of the G1R1J Program as an agent of change to control DHF and their roles in a community. A qualitative study with Health Believe Model Approach. Data Collection conducted in the year 2019 using Focus Group Discussion and In-Depth Interview methods. Informants are cadres in Pondok Betung Sub-District Pondok Aren District, which a DBD endemic area in South Tangerang City. Data obtained were then processed according to thematic analysis. The study results showed cadre as a frontline implementation of G1R1J Program, who have a good understanding concerning program technical in Community. Risk perception related to disease and benefit from G1R1J Program is one of cadre motivation to run roles. Lack of response and dependency of the community about cadre are obstacles to conduct G1R1J Program. Efforts can do socialization of the G1R1J Program at the family level, which involves across sectors. Abstrak Program Gerakan 1 Rumah 1 Jumantik (G1R1J) adalah satu upaya pemberdayaan masyarakat dalam pelaksanakan pemberantasan sarang nyamuk Demam Berdarah Dengue (DBD). Program tersebut melibatkan salah satu anggota keluarga menjadi jumantik rumah. Salah satu faktor yang memengaruhi perilaku masyarakat dalam pelaksanaan suatu program adalah adanya faktor pendorong berupa sikap dan perilaku tokoh, petugas kesehatan atau petugas lain yang merupakan kelompok referensi masyarakat. Tujuan penelitian ini adalah mengetahui persepsi kader terhadap Program G1R1J; sebagai agen perubahan dalam pengendalian DBD dan perannya di masyarakat. Penelitian ini merupakan penelitian kualitatif dengan pendekatan Health Belief Model (HBM). Pengumpulan data dilakukan pada tahun 2019 melalui Diskusi Kelompok Terarah (DKT) dan wawancara mendalam (In-Depth Interview/IDI). Informan penelitian ini adalah kader yang berada di Kelurahan Pondok Betung Kecamatan Pondok Aren yang merupakan wilayah endemis DBD di Kota Tangerang Selatan. Data yang diperoleh kemudian diolah berdasar analisis tematik. Hasil penelitian menunjukkan bahwa kader sebagai ujung tombak implementasi program G1R1J sudah memiliki pemahaman yang baik tentang teknis program di masyarakat. Persepsi risiko terkait penyakit dan manfaat dari program G1R1J menjadi salah satu motivasi kader dalam menjalankan perannya. Kurangnya respon dan ketergantungan masyarakat terhadap kader menjadi hambatan dalam pelaksanaan program G1R1J. Upaya yang dapat dilakukan adalah sosialisasi program GIRIJ di tingkat keluarga dengan melibatkan lintas sektor.

      Abstract: Midwifery education graduates ideally work as midwives. This study explored the relationship between characteristics and suitability the field of work graduates of DIII midwifery study program in 2019. This study applied a cross-sectional design with a quantitative approach. The population was graduates in 4 DIII midwifery study programs in West Java and the sample whose access the google form as many as 442 respondents. Google forms distributed by email, Facebook, and WhatsApp group graduates within one month. The dependent variable was the suitability the field of work graduate. In contrast, the independent variables were characteristics of graduate (the status of the study program, age, marital status, year of graduation, the value of IPK, and additional education after graduation). Analysis of data used chi-square. The results showed 10.2% of midwifery study program graduates worked not as midwives, 37.8% of them said that salary was the reason they did outside the midwife profession and another 15.6% expressed no interest as midwives. The results of the bivariate analysis showed that marital status and IPK at graduation had a significant relationship with the suitability of the graduate work field (p <0.005). The commitment of graduates as midwives is the main focus to enhance midwifery professionalism. The existence of graduates whose work outside the midwife profession along with their reasons provided information on the need for strengthening midwife salary standards based on labour regulations.  Abstrak Lulusan pendidikan kebidanan idealnya bekerja sebagai bidan. Penelitian ini dilakukan pada tahun 2019 untuk menilai hubungan antara karakteristik dengan kesesuaian bidang kerja lulusan prodi DIII kebidanan. Penelitian menggunakan desain penelitian potong lintang dengan pendekatan kuantitatif. Populasi adalah lulusan di 4 prodi DIII kebidanan di Jawa Barat dan sampel adalah yang mengakses google form. Total sampel sebanyak sebanyak 442 responden. Google form yang disebarkan melalui email, facebook, dan whatsapp group lulusan dalam jangka waktu 1 bulan. Sebagai variabel terikat adalah kesesuaian bidang kerja lulusan, variabel bebas adalah karakteristik lulusan (status program studi, umur, status pernikahan, tahun lulus, nilai IPK, dan pendidikan tambahan setelah lulus). Data dianalisis menggunakan chi-square. Hasil penelitian menunjukkan 10,2% lulusan prodi DIII kebidanan bekerja bukan sebagai bidan, 37,8% diantaranya menyampaikan bahwa gaji merupakan alasan mereka bekerja di luar profesi bidan dan 15,6% lainnya menyatakan tidak berminat sebagai bidan. Hasil analisis bivariat memperlihatkan status pernikahan dan IPK saat lulus memiliki hubungan yang bermakna dengan kesesuaian bidang kerja lulusan (p<0,005). Komitmen lulusan sebagai bidan merupakan fokus utama untuk meningkatkan profesionalisme bidan. Keberadaan lulusan yang bekerja di luar profesi bidan beserta alasannya memberikan informasi perlunya penetapan standar gaji bidan dengan berpedoman pada aturan ketenagakerjaan.

  • Kajian Kebijakan Pemanfaatan Obat Tradisional Di Fasilitas Pelayanan
           Kesehatan Pada Era Jaminan Kesehatan Nasional

      Abstract: Since the establishment of the Healthcare Social Security Agency (BPJS), there has been a low demand of patient demand for traditional medicine (TM) services in health care facilities. This study examined various aspects, situations/conditions and the potential use of TM in health facilities. Secondary sources, such as policies and regulations, and in-depth interviews, became the primary data. Those who in charge of the BPJS program was the key informant. The study focused on informants, particularly either Semarang or Surakarta. This study analysed the data, both descriptively and inductively. The aspects examined were services, the implementation of OT guarantees with BPJS, clinicians' readiness and government supports. The existence of JKN implies the use of OT to decrease dramatically. The health facilities did not fully utilise the presence of special allocation funds (DAK) and capitation arrangements. The list of TM was to contain the choices for preventive, promotive, curative, and palliative efforts either a compliment or an adjuvant. TM services involved in the path of preventive, promotive or curative actions either as an adjuvant, or complement with conventional medicine. TM financing may be from DAK, capitation funds or APBN II; however, these sources did not optimise yet. This condition happened due to the lack of political will from stakeholders, as well as the absence of standard reference for the Traditional Medicine Formulary, especially BPJS enrollment by the future. Therefore, the Ministry of Health RI has to issue a decree for health facilities obliged to provide services with traditional medicines.  Abstrak   Sejak dibentuknya Badan Pengelola Jaminan Sosial (BPJS),terjadi penurunan permintaan pasien atas layanan  obat tradisional di fasilitas pelayanan kesehatan (fasyankes). Tujuan  menganalisis berbagai aspek, situasi /kondisipemanfaatan obat tradisional dalam pelayanan di fasyankes. Mengkaji peraturan perundang-undangan dan kebijakan terkait pemanfaatan obat tradisional di fasyankes,dan melakukan wawancara mendalam dengan nara sumber relevan di fasilitas pelayanan kesehatan dan dinas kesehatan terpilih (Kota Surakarta dan Semarang). Analisis dilakukan secara deskriptif dan induktif. Terdapat lebih  dari 20 peraturan perundang-undangan dan kebijakan terkait pemanfaatanobat tradisional di fasyankes.Aspek yang dikaji adalah pelayanan, implementasi jaminan OT dengan BPJS, kesiapan klinisi dan dukungan pemerintah. Adanya JKN menyebabkan penggunaan  OT mengalami penurunan drastis, adanya pengaturan dana alokasi khusus (DAK) dan kapitasi, tidak dimanfaatkan fasyankes.  Komite medik rumah sakit, puskesmas serta dinas kesehatan membutuhkan adanya daftar obat tradisional yang dapat digunakan di fasyankes yang ditetapkan pemerintah. Daftar obat tradisional  diharapkan berisi pilihan obat tradisional untuk upaya preventif, promotif, kuratif ringan dan paliatif sebagai komplemen maupun sebagai adjuvan. Pelayanan dengan obat tradisional masuk jalur upaya preventif, promotif atau kuratif ringan baik sebagai adjuvant maupun komplemen dengan obat konvensional. Pembiayaan obat tradisional dapat dilakukan dengan DAK, dana Kapita si atau APBN II, namun belum dimanfaatkan. Kondisi ini karena kurangnya political will dari stake holder dan belum adanya acuan standar Formularium Obat Tradisional, terutama jika kemudian hari dapat dijamin oleh BPJS.  Masih dibutuhkan adanya regulasi ketetapan menteri bahwa fasyankes wajib melakukan pelayanan dengan obat tradisional.

      Abstract: Young marriage still occur in Indonesia. About 0.2% (22,000) teenager 10-14 years were married. Though, Law of marriage the Republic of Indonesia onage requirements are 19 years for men and 16 years for women.Two things for unexpected pregnancy are maintaining or ending a pregnancy by adolescents. The study aimed to determine teenager behaviors and teenager pregnancies in Limakoli village, Rote Ndao District. It was an explorative ethnographic. Inforrmants were selected by snowball  sampling. Data were analyzed by thematic analysis. Girls used to do houseworks. Meanwhile, boys had sports in the afternoon. Tenageers got knowledge and information on reproductive health but discussions of pregnancy and the risks were limited. The pregnant teenagers faced gossips and social stigma. One teen pregnacy was faced angry by their parents and families, aborted, quiet, drop out from school. The other teenager covered her second pregnancy, though her parents likely accept her pregnancy. Teenager pregnancies tended not to have antenatal cares. The traditional marriage ‘Terang Kampung’ was not done by underages, likely it included family and extended families. Pregnant teenagers dropped out from schools, in contrast boys did not. Girls had most impact on teenager pregnancies of physical, psychological, and social risks. Hence, an integrative sociocultural intervention for dating with no sexual relations, no adolescence pregnancies and marriage by school, church, primary health center, traditional leaders, village staffs should be developed. Abstrak Pernikahan muda masih terjadi di Indonesia. Sekitar 0,2% (22.000) remaja 10-14 tahun menikah. Padahal, UU Perkawinan Republik Indonesia tentang persyaratan usia adalah 19 tahun untuk pria dan 16 tahun untuk wanita. Dua hal untuk kehamilan yang tak terduga adalah mempertahankan atau mengakhiri kehamilan oleh remaja. Penelitian ini bertujuan menentukan perilaku remaja dan kehamilan remaja di Desa Limakoli, Kabupaten Rote Ndao. Studi ini adalah etnografi eksploratif. Informan dipilih secara snowball sampling. Data dianalisis dengan analisis tematik. Remaja perempuan biasanya melakukan pekerjaan rumah. Sementara, remaja laki-laki berolahraga di sore hari. Remaja memperoleh pengetahuan dan informasi tentang kesehatan reproduksi, tetapi diskusi tentang kehamilan dan risikonya terbatas. Remaja yang hamil menghadapi gosip dan stigma sosial. Seorang remaja hamil menghadapi kemarahan orang tua dan keluarganya, mengalami aborsi, menjadi pendiam, dan putus sekolah. Remaja lainnya menutupi kehamilannya yang kedua, walaupun kemungkinan orang tuanya menerima kehamilannya. Kehamilan remaja cenderung tidak mendapat antenatal care. Perkawinan tradisional 'Terang Kampung' tidak dilakukan oleh anak-anak di bawah umur, kemungkinan karena melibatkan keluarga dan keluarga besar. Remaja yang hamil mengalami drop out dari sekolah, sebaliknya remaja laki-laki tidak. Remaja perempuan memiliki dampak paling besar pada kehamilan remaja terhadap risiko fisik, psikologis, dan sosial. Sehingga perlu dikembangkan, intervensi sosiokultural integratif untuk berpacaran tanpa hubungan seksual, tidak ada kehamilan remaja dan pernikahan, dengan melibatkan sekolah, gereja, pusat kesehatan primer, pemimpin tradisional, staf desa.

           Keluarga (PISPK) Di Puskesmas

      Abstract: The Healthy Indonesia Program with a Family Approach (PISPK) is conducted by the health center (puskesmas), which addressed to closer the public access to health services with a family target. Puskesmas would accept data and information from family health profiles (Prokesga) based on evidence, that needs to manage adequately so can be utilized as a document of drafting the action plan. Evaluation of the PISPK implementation in 2019 shows that the majority of puskesmas do not conduct and utilize PISPK Data. This study aims to describe how to manage, analyze, and utilization PISPK Data at puskesmas. The analysis is part of the PISPK implementation research conducted by the Center for Research and Development of Health Resources and Services through Participatory Action Research (PAR) approach in 8 puskesmas in Indonesia (2017-2018) using mixed methods. We had performed a Focus Group Discussion for the surveyor and an in-depth interview with the Head of Health Centers. PISPK Data analysed using Excel and SPSS. Results showed that still not proficient in data management and utilization at puskesmas sites. It is caused by several obstacles involve an administrative change of city or district; update of the healthy family application; restricted access of raw data, internet connection, and Prokesga storage, including limitations of data analysis skill also. Barriers can be minimalized use manual analysis and special training for management and data analysis. Results of data analysis for PISPK can also be used to determine program targets, to make a map of the individual problem, family, and area, which is utilized to arrange a draft of the Action Plan. Abstrak Program Indonesia Sehat dengan Pendekatan Keluarga (PISPK) dilaksanakan oleh puskesmas untuk mendekatkan akses pelayanan kesehatan dengan target keluarga. Puskesmas akan mendapatkan data dan informasi dari profil kesehatan keluarga (Prokesga) berdasar evidence yang perlu dikelola dengan baik sehingga dapat dimanfaatkan sebagai bahan penyusunan rencana usulan kegiatan. Evaluasi pelaksanaan PISPK 2019 menunjukkan mayoritas puskesmas belum melakukan analisis dan pemanfaatan data PISPK. Untuk itu, tulisan ini bertujuan menggambarkan bagaimana pengelolaan, analisis, dan pemanfaatan data PISPK di puskesmas. Analisis merupakan bagian dari riset implementasi PISPK yang dilakukan oleh Puslitbang Sumber Daya dan Pelayanan Kesehatan melalui pendekatan Parcipatory Action Research (PAR) di 8 puskesmas di Indonesia pada tahun 2017-2018 dengan metode kualitatif dan kuantitatif. Dilakukan Focus Group Discussion (FGD) terhadap petugas puskesmas dan wawancara mendalam kepada kepala puskesmas. Data PISPK diolah dengan menggunakan excel dan SPSS. Hasil menunjukkan bahwa pengelolaan dan pemanfaatan data PISPK di puskesmas lokus belum optimal. Hal ini terkendala, antara lain: perubahan administratif kota/kabupaten, perubahan versi aplikasi KS; tidak ada akses terhadap raw data; terbatasnya sinyal internet dan tempat penyimpanan Prokesga, termasuk juga keterbatasan kemampuan analisis data. Kendala dapat diminimalisir dengan analisis manual, dan pelatihan khusus manajemen dan analisis data. Hasil analisis data PISPK dapat digunakan dalam menentukan sasaran program, membuat peta masalah individu, keluarga dan wilayah yang dapat dimanfaatkan untuk penyusunan rencana usulan kegiatan.

  • Indikator Antropometri Sebagai Prediktor Glukosa Darah Puasa Terganggu
           Pada Usia Dewasa Di Tangerang Selatan

      Abstract: In recent decades, the prevalence of Diabetes Mellitus (DM) type-2 has significantly increased globally and nationally in Indonesia. High risk of DM type-2 is associated with Impaired Fasting Glucose (IFG), and IFG changes conditions are closely related to increasing anthropometric indicators. This study aimed to assess the association between anthropometric indicators and IFG status in adults aged ≥40 years in South Tangerang City. A cross-sectional study was conducted among 320 respondents aged ≥40 years and lived in Sub District of East Ciputat, South Tangerang City. The results of the study indicated that 17.5% of 320 adults had IFG conditions. Anthropometric indicators for central obesity were significantly associated with IFG status. Multivariate analysis indicated that there were significant associations between anthropometric indicators (waist circumference/WC, waist to hip ratio/WHR, waist to height ratio/WHtR), and IFG status (p<0.05). The multivariate-adjusted odds ratios (95% confidence interval) of IFG status for anthropometric indicators were 3.43(1.52-7.71), 7.71(1.79-33.24), and 3.11 (1.04-9.24), respectively. Body Mass Index/BMI was not significantly associated with IFG status (p>0,05). This study noted that examining central obesity indicators regularly merits consideration for the prevention of IFG conditions among adults. We need to increase community awareness to prevent IFG and DM type-2 through health promotion of controlling adults’ body mass and cholesterol with normal fasting blood glucose. Abstrak Pada beberapa dekade terakhir, prevalensi Diabetes Melitus tipe-2 (DM tipe-2) di dunia dan khususnya di Indonesia mengalami peningkatan. Risiko tinggi untuk DM tipe-2 berhubungan dengan Glukosa Darah Puasa Terganggu (GDPT). Perkembangan kondisi GDPT dikaitkan dengan peningkatan indikator antropometri. Penelitian ini bertujuan untuk mengetahui hubungan antara indikator antropometri dengan GDPT pada orang dewasa usia ≥40 tahun di Tangerang Selatan. Penelitian ini menggunakan desain studi cross-sectional dengan jumlah responden sebanyak 320 orang yang berusia ≥40 tahun dan tinggal di Kecamatan Ciputat Timur, Kota Tangerang Selatan. Hasil penelitian ini menemukan sebanyak 17,5% dari 320 orang dewasa mengalami kondisi GDPT. Indikator antropometri untuk obesitas sentral berhubungan bermakna dengan GDPT. Hasil analisis multivariat menunjukkan bahwa lingkar pinggang, Rasio Lingkar Pinggang Panggul dan Rasio Lingkar Pinggang terhadap Tinggi Badan berhubungan dengan kejadian GDPT (Nilai-P <0,05). Setelah disesuaikan dengan faktor perancu, dihasilkan Rasio Odds (95% Confidence Interval) masing-masing sebesar 3,43 (1,52-7,71), 7,71 (1,79-33,24) dan 3,11 (1,04-9,24). Indeks Massa Tubuh tidak ditemukan mempunyai hubungan dengan GDPT. Penelitian ini menunjukkan pentingnya pengecekan indikator obesitas sentral secara berkala untuk mencegah kondisi GDPT pada orang dewasa. Diperlukan promosi kesehatan untuk meningkatkan kesadaran masyarakat dalam mengontrol massa tubuh dan lemak tubuh orang dewasa dengan glukosa darah puasa normal untuk mencegah GDPT dan DM tipe-2.

      Abstract: The negative impact of early age marriage in Indonesia is the risk maternal and infant mortality by 30%, as many as 56% of adolescent girls experienced domestic violence, only 5.6% of adolescents with early marriage still continue schooling after marriage. This article arrange by further analysis and literature review of the ethnographic research book series on the Center for Research and Development in Humanities and Management Health, NIHRD. This article aim to determine the impact of early marriage on the maternal and child health in several ethnic groups in Indonesia and about the prevention. Cases of early marriage still occur in many ethnic groups in Indonesia and contribute to mortality and morbidity for mothers and children. The impact of early marriage on maternal and child health includes miscarriage, premature, bleeding and maternal death. They must get a education health about repoduction health from reliable source. It is necessary for the government’s role to provide education about reproductive health and positive activities to avoid early age marriage. Especially in districs that have high rates of early age marriage. Abstrak Dampak negatif dari pernikahan dini di Indonesia  adalah risiko kematian ibu dan bayi sebesar 30 %, 56% remaja perempuan mengalami Kekerasan Dalam Rumah Tangga, dan hanya 5,6% remaja dengan pernikahan dini yang masih melanjutkan sekolah setelah kawin.  Tujuan penulisan artikel ini adalah untuk mengetahui dampak pernikahan usia dini terhadap kesehatan ibu dan anak di beberapa etnis di Indonesia serta pencegahannya. Artikel dibuat berdasarkan analisis lanjut dan kajian literatur dari buku seri riset etnografi Pusat Penelitian dan Pengembangan Humaniora dan Manajemen Kesehatan, Badan Litbang Kesehatan. Kasus pernikahan dini masih banyak terjadi di berbagai etnis di Indonesia dan menyumbang angka kematian dan kesakitan bagi Ibu dan Anak. Dampak pernikahan dini terhadap kesehatan ibu dan anak antara lain, terjadinya keguguran, kelahiran premature, perdarahan hingga kematian ibu. Sebaiknya remaja memiliki pengetahuan mengenai pentingnya kesehatan reproduksi dan mendapatkan pendidikan kesehatan reproduksi yang benar dan layak dari sumber yang terpercaya. Perlu peran pemerintah untuk memberikan edukasi tentang kesehatan reproduksi dan kegiatan-kegiatan yang positif untuk menghindari pernikahan dini.  Edukaski tersebut khususnya untuk masyarakat di daerah yang memiliki angka pernikahan dini yang cukup tinggi.

      Abstract: Gerakan Memasyarakatkan Makan Ikan (GEMARIKAN) is launched as an effort to increase fish consumption and improve people’s nutrition through the consumption of fishery products. Food processing modification is an effort to support GEMARIKAN by increasing food processing method to improve fishery products’ nutrition. Tilapia and tofu waste flour are used as ingredients in making tilapia fish meatballs. This study aims to analyze the effect of the addition of tofu waste flour on protein, fiber, water content, and acceptability of tilapia fish meatballs. This research was experimental research with a quasi-experimental type and used a posttest only control group research design. The addition of tofu waste flour (X0: 0 gram; X1; 5 grams; X2: 10 grams; X3: 15 grams) caused the increasing level of protein, fiber, and water of tilapia fish meatballs. Protein content on tilapia fish meatballs are X0: 16.59%; X1: 16.88%; X2: 17.24%; X3: 17.42%, fiber content are X0: 0.08%; X1: 0.14%; X2: 0.25%; X3: 0.35%) and water content were X0: 63.63%; X1: 63.55%; X2: 63.75%; X3: 64.07%). Tilapia fish meatballs with 10 grams addition of tofu waste flour were the most acceptable product. This innovation produced the most preferred meatball product by panelists, and the nutritional content had met the fish meatball's quality standards. Abstrak Gerakan Memasyarakatkan Makan Ikan (GEMARIKAN) merupakan suatu usaha untuk meningkatkan konsumsi ikan serta perbaikan gizi masyarakat melalui konsumsi produk perikanan. Upaya yang dapat dilakukan untuk mendukung GEMARIKAN adalah dengan melakukan modifikasi pengolahan ikan untuk menambah variasi produk perikanan maupun memperkaya kandungan gizinya. Bahan yang digunakan dalam modifikasi produk perikanan pada penelitian ini adalah ikan nila dan tepung ampas tahu yang kemudian diolah menjadi bakso ikan nila. Penelitian bertujuan menganalisis pengaruh penambahan tepung ampas tahu terhadap kadar protein, kadar  serat, kadar air dan daya terima pada bakso ikan nila. Penelitian ini merupakan penelitian eksperimen dengan jenis quasi eksperimental menggunakan desain penelitian posttest only control group design. Hasil penelitian menunjukkan seiring peningkatan jumlah penambahan tepung ampas tahu (X0: 0 gram; X1: 5 gram; X2: 10 gram; X3: 15 gram) meningkatkan kadar protein, serat dan air bakso ikan nila. Kadar protein menjadi X0: 16,59%; X1: 16,88%; X2: 17,24%; X3: 17,42%; kadar serat menjadi X0: 0,08%; X1: 0,14%; X2: 0,25%; X3: 0,35%; dan kadar air menjadi X0: 63,63%; X1: 63,55%; X2: 63,75%; X3: 64,07%. Bakso yang disarankan adalah bakso ikan nila dengan penambahan tepung ampas tahu sebanyak 10 gram. Perlakuan ini menghasilkan bakso ikan nila yang paling banyak disukai oleh panelis dan dari segi mutu gizinya telah sesuai dengan standar mutu bakso ikan.
