  • Misoprostol for treatment of incomplete abortion- providers practice in
           Dodoma region, central Tanzania

      Abstract: Remnant products of conception in the uterine cavity following a  miscarriage or induced abortion presents a serious threat to a woman   because it increases the risk of infection or continued bleeding and therefore uterine evacuation in such cases is imperative. Surgical and  medical methods using drugs like misoprostol may be used for complete the evacuation of the remaining products of conception (POC).This study aimed to examine health care providers practice regarding the use of misoprostol for post abortion care in Dodoma region of Tanzania. This was a quantitative prospective cross-section study which was done in three districts of Dodoma region. It involved 126 health care providers from health facilities in the three districts. Information regarding use of  misoprostol in post abortion care (PAC) was collected using pretested self-administered questionnaire to 126 randomly selected health care providers. 28.6% of interviewed health care workers revealed that they were using misoprostol for post abortion care while they had no formal training regarding the use of the drug for this indication. Most of them (65%) got information from co-workers. A remarkable proportion of health care workers were using misoprostol though they lack relevant knowledge and guidelines for using the drug for PAC and the drug was yet to be approved by Ministry of Health and Social Welfare for this indication.KEY WORDS Misoprostol, Incomplete abortion, Uterine evacuation, Post abortion care

  • Maternal near-miss in a great grandmultipara following unsafe abortion: A
           focus on the uncommon contributing factors

      Abstract: Unsafe abortion and its complications have been limited to adolescents lacking family and partner support without attention to older, married  women and their peculiarities regarding unsafe abortions. Main objective of the study was to focus on uncommon contributing factors to unsafe abortion in older women. A 40year old, great grandmultipara [Para 10+1,8 alive] with awareness on modern contraception underwent unsafe abortion following in severe morbidity [severe anaemia, perforated gangrenous uterus and septicaemia] which necessitated exploratory laparotomy and  subtotal hysterectomy. Adequate puerperal care, cultural reorientation, review of information in family planning jingles and liberalization of abortion law will assist in preventing unsafe abortions among older women.Key words: Maternal near-miss, unsafe abortion, great grandmultipara, contributing factors.

  • Soil transmitted helminths infections, malnutrition and anaemia among
           primary school children in northern Tanzania

      Abstract: Soil-transmitted helminths (STHs) are a major public health problem in many developing countries. Establishment of prevalence and intensity of infections is important in designing, implementating and evaluating control programs. This study aimed at determining the prevalence and intensity of STH infections, malnutrition and anaemia among primary school children in Same district. A descriptive cross-sectional study was carried out on 579 school children. Kato-katz technique was used to determine status and intensity of STH infection. Weight and height were determined using  weighing The prevalence of STH was 0.9%. The prevalence of stunting was 42.3%, male were more stunted than female at 48.6% and 37.8%  respectively (P=0.015). Higher rates of stunting were observed in the upland plateau zone (48.4%) than in the other zones (P=0.029). The prevalence of thinness was 11.7%. Prevalence of thinness was highest in the lowlands zone (15.8%) (P=0.013). The prevalence of anaemia was 3.1%. The prevalence of STH was very low, but malnutrition was observed to be a problem in the district with nearly half of the children being stunted.

  • Langerhans’ cell histiocytosis with twenty-nail dystrophy and a rare
           cutaneous manifestation

      Abstract: Langerhans’ Cell Histiocytosis (LCH) constitutes a rare group of idiopathic disorders, characterized by proliferation and infiltration of various tissues by clonal population of cells with phenotype of langerhans’ cell. We report a 2 year old female child, a product of consanguinous marriage who presented with seborrhoeic dermatitis like eruption involving scalp, twenty nail dystrophy, oral lesions and loosened teeth, mild hepatomegaly, low RBC count, multiple osteolytic lesions in skull and pelvic bones thus falling into the category of high-risk multisystem disease. Following the clinical suspicion of LCH in the child, a battery of cost-effective investigative tools were employed primarily to confirm the diagnosis and to decide the extent of systemic involvement. This case is being reported from the view point of its rarity and the diagnostic challenges posed to some dermatologists before the child was brought to our department.

  • A clinical study comparing clonidine with dexmedetomidine as adjuvants to
           local anaesthetic agent in brachial plexus block via inter-scalene
           approach for early discharge

      Abstract: It is a known fact that use of peripheral nerve block with local anaesthetics like lignocaine and bupivacaine for anaesthesia reduces both operating room time and length of hospital stay and provide good postoperative analgesia. Various additives can be added to local anaesthetics to accelerate the onset time, to reduce the systemic absorption (thus the risk of local anaesthetic related toxicity) and prolong the duration of nerve block/pain relief. Yet the quest continued for better drug which meets the above criteria and even then keeping the patient awake and alert to make him fit for early ambulation and discharge. 60 patients aged 18 - 65 years with ASA grade I were randomly divided into group I and group II to receive 10 ml of lignocaine 2% and 20 ml of bupivacaine 0.5% with 90µg of clonidine or dexmedetomidine 25µg respectively. Onset of sensory blockade was determined by pinprick method by three point score and motor blockade by three point scale. Duration of postoperative analgesia, the hemodynamic changes, sedation scores, any adverse effects and recovery criteria were observed. Statistical analysis was done by student’s “t” test and p < 0.05 was considered significant. It was found that there was faster onset of sensory and motor blockade, the postoperative analgesia was prolonged and the amount of sedation was profound in group II as compared to group I. All the above findings were statistically significant. Many of the patients in group II were found to be alert and fit for discharge on 2nd day as compared to group I, Therefore, it was recommended that Dexmedetomidine 25µg is a better option as an additive than clonidine 90µg for hastening the onset of sensory and motor block with prolonged postoperative analgesia, sedation  and alert &awake patient ready for discharge on the 2nd day.KEY WORDS: Inter-Scalene Brachial Plexus Block, Clonidine, Dexmedetomidine

  • Compliance to infection prevention and control guidelines among health
           care workers at Mnazi Mmoja Hospital Zanzibar

      Abstract: Nosocomial infections ,commonly known as hospital acquired infections (HAI) include several pathogens like Escherichia coli, Hepatitis viruses, HIV, Pseudomonas and Staphylococcus etc. These agents are transmitted directly or indirectly. Prevention and control of Nosocomial infections is the most important approach in all hospitals and other health care settings. Infection prevention and control (IPC) of HAIs is determined by knowledge, practice and level of commitment invested by health care organizations to bring the hospital acquired infections under control. Though hospitals in developed countries acquire very few HAIs but in developing countries HAI has been attaining much attention because it was found that up to 5%–10% of patients admitted for acute care are suffering from HAI. A descriptive cross-section study was designed to determine the level of compliance towards IPC guidelines among 186 health care workers at Mnazi Mmoja Hospital Zanzibar (MMHZ) .It was found that compliance to IPC in MMHZ was inadequate. Of 185 respondents, a considerable proportion 83 (44.8%) indicated lack of regular standard procedures. Although 166 (89.7%) strongly agreed that IPC is beneficial, surprisingly 73 (39.4%) reported to wash their hands only after the procedure and not otherwise. Moreover 55 (30%) reported on inadequate segregation containers, justifying unsatisfactory health care wastes segregation, thereby causing indirect /direct community exposure to hazardous substances. It was recommended that Hospital Management Team in MMHZ must establish IPC Committees to monitor, and evaluate the compliance with the guidelines ensuring that IPC is prioritized by all health staff . Interventions of standard precautions, such as hand hygiene, provision of personal protective equipments, and safe disposal of infectious material must also be adequately addressed. In-service education programs on IPC should be arranged for all categories of staff aiming at imparting basic theoretical concepts on key components of selected interventions of standard precautions.

  • Drug utilization study of antibiotics for urinary tract infections in a
           tertiary care hospital

      Abstract: A drug utilization pattern of antibiotics for urinary tract infections in 200 cases above 18 years of age was done at Katuri Medical College Hospital, Guntur, India. The antibiotic sensitivity profile of the microorganism causing urinary tract infections was studied in cases diagnosed as urinary tract infection. The patients with significant bacteriuria (> 105 CFU/ ml) were treated with antibiotics Cephalosporin’s, Amikacin, Fluroquinolones, Amoxicillin + Clavulanic acid and Linezolid. The incidence of urinary tract infections was more or less equal in either sex. More than 2/3 of these cases were found in the age group of 18 to 60 years. The most common organisms isolated from the urine culture and sensitivity test was E.coli and the most commonly prescribed antibiotic was
      amikacin.

      Present study was aimed to study the role of Yoga in improvement of Quality of Life among Schizophrenia patients.
      70 Schizophrenic patients, diagnosed according to ICD-10 criterion were recruited and randomized into two groups. Group-A patients received only pharmacotherapy and Group-B patients received pharmacotherapy and Yoga. Response was measured and compared by using Quality of Life Instrument (WHO QOL - BREF).
      Group-B patients, who received Yoga plus pharmacotherapy, had shown much improvement in all domains of WHO-QOL BREF scale. The improvement is more in psychological domain followed by social domain.
      Schizophrenics, who received Yoga plus pharmacotherapy, had shown much improvement in all domains of WHO-QOL BREF scale. Studies like this with various forms of Yoga will improve the awareness in medical fraternity and Quality of life in patients.

  • Assessing birth asphyxia using strictly observational signs, the ARC and
           AC scores versus Apgar score

      Abstract: The Apgar Scoring system is the sole assessment tool for newborn asphyxia,but the Apgar scoring system has many limitations as it does not assess conditions before and after delivery, that might trigger asphyxia. Moreover, the score combine subjective and objective signs as well as observations and measurements which need resources, skills and time that are not always sufficiently available to out health workers and health facilities. So far, there is no other alternative score system, convenient to health workers. The AC score, a simple tool along with the ARC score, strictly observational ,developed by Dr Sultan Omar Sultan was pre-tested at St Francis Hospital Ifakara in 2008 , then tested on 340 newborns in Temeke Hospital and Mnazi Mmoja Hospital. It was found be more sensitive, more specific and more reliable and ultimately easy to use for newborn asphyxia assessment (P:0,03<0,05) using Chi square test) compared to the Apgar Score. The specificity, sensitivity and reliability of the ARC and AC score was 100%. There was no significant difference between the results of ARC and AC scores (similar results).Considering the simplicity of the AC score (2 variables compared to 3 in ARC score), it was suggested, with confidence that the AC score is a valid alternative and easy tool in asphyxia assessment at birth that can be utilized both at health facility and at household by any trained health worker.

  • The magnitude of widow ritual cleansings among Kara tribe in Ukerewe
           Ditrict, Mwanza Region

      Abstract: Widow ritual cleansing has been documented in many countries in Africa. The ritual refers to the traditional practice of sexual intercourse with the widow of a deceased male, to prepare her for acceptance to the community and or remarriage. Several reasons have been found to account for the existence of this kind of ritual, mainly, low income, socio-culture and taboos.
      Objective of the study was to determine the magnitude and reasons for widow cleansing practice among the Kara Tribe in Nyangombe Village, Ukara Division, Ukerewe District , Tanzania. A cross-sectional descriptive study in June 2013 was conducted in which ,91 widows at Nyangombe Village, who belong to the Kara tribe were included. Data were collected using semi-structured questionnaire and in depth interviews. It was found that Widow cleansing among Kara Tribe of Nyangome Village, Ukerewe Distric is a ritual involving sexual intercourse with a male cleanser, unknown to the woman, a traditional custom. Of the 91 Kara widows found in this village, majority (94%) had already underwent this ritual, while 6% were not aware and 5% were aware but had not practiced. Again majority (66%) of them had information on cleansing ritual through their aunts/ uncles , 17% from their mother and 4% from their neighbors. Although 66% of the widows were aware about STIs/HIV/AIDs, the ritual cleanser was not aware and did not use a condom during cleansing, though he was aware about gonorrhea as a sexually transmitted disease (STI).It was recommended that Major efforts should be done to ensure residents of Ukara Division are given health education on HIV transmission. Welfare institutions in Ukerewe should take note of these practices and strive to raise the levels of education and social economic status among the Kara people.

  • Food fortification: Not too late in Tanzania – A call for action

      Abstract: The paper gives an overview of micronutrients deficiency effects and the recent maiden steps taken for food fortification in Tanzania. Tanzania is commended for starting a food fortification program. In 2013 a pilot project was introduced for grain fortification with iron, zinc and folic acid. It has been implemented in Manzese and Tandale Wards of Dar-es- Salaam. The project is aligned to the Scaling Up of Nutrition initiative (SUN) launched by President Jakaya Kikwete in 2012. This means the President declared a war against malnutrition and food fotification as one of the strategies. It is expected that the food fortification pilot project will succeed and will be emulated in the rest of Tanzania, Africa and in the whole developing world.
