  • Sensitifitas dan Spesifisitas Pertanyaan Gejala Saluran Pernapasan dan
           Faktor risiko untuk Kejadian Penyakit Paru Obstruktif Kronik (PPOK)

      Abstract: Chronic obstructive pulmonary disease (COPD) is a chronic lung disease characterized by progressive non-reversible or partially reversible airflow obstruction in the airway. The aim of this study was to assess the sensitivity and specificity of questions on respiratory symptoms and risky behavior for diagnosing COPD. Method. This study was a further analysis of Bogor Cohort Study on Non-Communicable Risk Factors 2011-2012 data. COPD was diagnosed by spirometry examination with bronchodilator. Twelve questions on COPD respiratory symptoms and two questions on rizky behavior were analyzed using crossed tabulation with 5 % significance level. Results. The sensitivity and specificity of several aggregate questions on respiratory symptom and COPD risk factors ranged from 12.5% to 94.3% and 2.2% to 94.4%. Aggregate of 7 questions on 6 respiratory symptoms and 1 smoking behavior with either one was answered “Yes” had the optimal sensitivity (68.1 %) and specificity (59.7 %). Those questions were coughing more than 1 month, excreting sputum almost every day, easily feeling fatigue or dyspnea while doing activity, dyspnea > 3 months that restricted daily activities, worsened fatigue/dyspnea, whizzing, and smoking behavior. Conclusion. Questions on respiratory symptoms were neither sensitive nor specific for diagnosing COPD. Keywords: chronic obstructive pulmonary disease, sensitivity, specificity. AbstrakPenyakit paru obstruktif kronis (PPOK) adalah penyakit paru kronik, ditandai dengan adanya hambatan aliran udara di saluran pernapasan, dan bersifat progressif nonreversibel atau reversibel parsial. Tujuan penelitian untuk mendapatkan sensitifitas dan spesifisitas pertanyaan gejala saluran pernapasan dan faktor risiko terhadap PPOK. Metode. Penelitian ini merupakan analisis lanjut sampel Studi Kohor Faktor Risiko PTM di Bogor tahun 2011-2012. Variabel yang dianalisis terdiri dari 12 pertanyaan gejala saluran pernapasan dan dua pertanyaan faktor risiko PPOK. Diagnosis PPOK berdasarkan hasil spirometri dengan bronchodilator. Analisis untuk mendapatkan nilai sensitifitas dan spesitifitas dilakukan tabulasi silang dengan tingkat kemaknaan 5%. Hasil. Dari kombinasi antara pertanyaan gejala saluran pernapasan dan faktor risiko terhadap PPOK, didapatkan range sensitifitas antara 12,5%-94,3% dan spesifitas 2,2%- 94,4%. Sensitifitas dan spesifisitas tertinggi adalah adanya salah satu dari 7 pertanyaan saluran pernapasan dan faktor risiko merokok terhadap PPOK yaitu dengan sensitifitas 68,1% dan dengan spesifisitas 59,7%. Tujuh pertanyaan tersebut adalah batuk setiap hari satu bulan lebih, mengeluarkan dahak/reak hampir setiap hari, cepat lelah/sesak napas bila beraktivitas, keluhan sesak napas >3 bulan yang membatasi aktivitas sehari-hari, cepat lelah/sesak napas semakin memberat, mengi, dan merokok. Kesimpulan. Pertanyaan gejala saluran pernapasan kurang sensitif dan kurang spesifik untuk mendiagnosis PPOK.Kata kunci: penyakit paru obstruksi kronis, sensitifitas, spesifisitas.

  • Karakterisasi Mutu Ekstrak Daun Sirsak (Annona muricata L.) dari Tiga
           Tempat Tumbuh

      Abstract: The soursop leaves (Annona muricata L.) are part of soursop plants that have a potential to be developed as anticancer as it contains annonacin. Annonacin is one of the acetogenins compounds group that are cytotoxic and mostly can be found in soursop plants. In the development of pharmaceutical preparation, apart from their safety and efficacy, quality of medicinal raw materials has been one of the important factors. Soursop leaf extract quality can be affected by their types of growing places. Therefore, the research of the quality characterization of soursop leaf extracts from three different growing places needs to be done in order to identify the quality of the extract used as herbal medicines. Quality inspection of soursop leaf extracts was including non-specific and specific parameters. Soursop leaf samples were obtained from three growing places with different altitude, which were from Tawangmangu, Central Java,; Pasuruan, East Java; and Bogor, West Java.The results showed that the best leaf extracts of soursop was from Pasuruan with non-specific parameters: (water content of 7,15% , ash content of 3,59% and 0,44 ppb levels of aflatoxin G2, and specific parameters: (the concentration of annonacin is 12,80%). Keywords: soursop leaf, annonacin, characterization, quality extractsAbstrakDaun sirsak (Annona muricata L.) merupakan bagian dari tanaman sirsak yang berpotensi untuk dikembangkan sebagai antikanker karena mengandung annonacin. Annonacin adalah salah satu senyawa golongan acetogenins yang bersifat sitotoksik dan paling banyak terkandung dalam tanaman sirsak. Dalam pengembangan sediaan obat, mutu bahan baku obat merupakan salah satu faktor penting, selain keamanan dan efektifitas. Mutu ekstrak daun sirsak dapat dipengaruhi oleh perbedaan tempat tumbuh. Oleh karena itu, penelitian karakterisasi terhadap mutu ekstrak daun sirsak pada tiga tempat tumbuh perlu dilakukan untuk mengidentifikasi mutu ekstrak yang digunakan sebagai obat herbal. Pemeriksaan mutu ekstrak daun sirsak mencakup parameter non spesifik dan spesifik. Sampel daun sirsak diperoleh dari tiga tempat tumbuh dengan ketinggian yang berbeda, yaitu daerah Tawangmangu, Jawa Tengah; Pasuruan, Jawa Timur; dan Bogor, Jawa Barat. Hasil penelitian menunjukkan bahwa ekstrak daun sirsak yang paling baik adalah ekstrak dari daerah Pasuruan dengan parameter non spesifik: (kadar air 7,15%, kadar abu 3,59%, kadar aflatoksin G2 0,44 ppb), dan parameter spesifik: (kadar annonacin sebesar 12,80%).Kata kunci: daun sirsak , annonacin, karakterisasi, mutu ekstrak

  • Pelaksanaan Program Pelayanan Obstetri dan Neonatal Emergensi Dasar
           (PONED) di Kabupaten Karawang

      Abstract: One of the Ministry of Health efforts to support the decrease of maternal and child deaths rates is optimizing the handling of Obstetrics and Neonatal Emergency /complications at the level of basic services through the Basic Emergency Obstetric Neonatal Care (BEONC). A study was done in Karawang in 2014 to obtain input dan output from the implementation of the BEONC program and it’s financing from secondary data and policy. There has been an increase of budget of the construction of health centers which were capable of being BEONC since 2011, but it was not sufficient for the completeness of infrastructure and trained personnel .The increasing number of health centers capable of being BEONC was accompanied by increased number of handling of pregnant women and postpartum mothers at health centers. However, there were some authorities that have not been executed as to comply with the BEONC guidelines.. Financing of health centers capable of being BEONC should be allocated to meet the needs of health centers’ infrastructures and skilled personnel, so that they can run the appropriate implementation of BEONC Guidelines. Keywords: BEONC, financing, health centerAbstrakSalah satu upaya Kementerian Kesehatan dalam mendukung percepatan penurunan Angka Kematian Ibu dan Angka Kematian Neonatal adalah melakukan optimalisasi penanganan Obstetri dan Neonatal emergensi/komplikasi di tingkat pelayanan dasar melalui Pelayanan Obstetri Neonatal Emergensi Dasar (PONED). Kajian ini dilakukan di Kabupaten Karawang pada tahun 2014 untuk memperoleh masukan dan luaran pelaksanaan program PONED dan pembiayaannya. Penelusuran dokumen dan observasi dilakukan di puskesmas PONED. Hasil kajian menunjukkan peningkatan biaya pembangunan puskesmas mampu PONED sejak tahun 2011 sampai 2013, namun tidak diimbangi dengan kelengkapan sarana prasarana dan tenaga terlatih. Peningkatan jumlah Puskesmas mampu PONED juga diiringi dengan meningkatnya penanganan ibu hamil dan ibu nifas di Puskesmas mampu PONED, namun masih ada beberapa kewenangan Puskesmas PONED yang belum dijalankan sesuai Pedoman Penyelenggaraan Puskesmas Mampu PONED. Pembiayaan puskesmas mampu PONED seharusnya dialokasikan untuk memenuhi prasarana puskesmas mampu PONED dan kompetensi petugas PONED sehingga dapat menjalankan kewenangan sesuai Pedoman Penyelenggaraan Puskesmas Mampu PONED.Kata kunci :PONED, pembiayaan, puskesmas

  • Bagaimana Kebijakan Pemerintah Daerah di Provinsi Jawa Barat Dalam
           Implementasi Layanan Pencegahan Penularan HIV-AIDS dari Ibu ke Anak (PPIA)

      Abstract: Increasing cases of HIV-AIDS transmission in women and children were becoming more and more alarming The central government policy to prevent the transmission of HIV-AIDS had been carried out since 2002, but its implementation has not been satisfactory. Therefore, it is needed to review the local government policy in the prevention and control of HIV-AIDS transmission from mother to child (PMTCT). The study was carried out in West Java province and its 3 cities/districts as the province was one of the five provinces with highest HIV-AIDS cases in Indonesia using. qualitative study through in-depth interviews with the stakeholders of HIV-AIDS programs .The study revealed that West Java province had no adequate local policies and its current policy was not specifically for the PMTCT but it remained in general about HIV-AIDS. Moreover, the policies in the three districts and municipality were the same as of the central and provincial .Clear policy from the local government on PMTCT should be formulated through District Health Office for the implementation of the PMTCT services, including the provision of PMTCT budget ranging from promotive, preventive, currative, and rehabilitative programs. Kebijakan pemerintah pusat untuk mencegah penularan HIV-AIDS sudah digulirkan sejak tahun 2002, namun implementasinya di lapangan masih belum sesuai dengan yang diharapkan. Penelitian ini dilakukan untuk mengetahui bagaimana kebijakan pemerintah daerah provinsi dan 3 kota di Jawa Barat dalam implementasi layanan pencegahan penularan HIV-AIDS dari ibu ke anak (PPIA). Metodenya adalah kualitatif dengan melakukan wawancara mendalam pada pemegang program HIV-AIDS di Dinas Kesehatan Provinsi dan Kota. Provinsi Jawa Barat dan 3 kota/kabupaten lainnya belum terdapat kebijakan yang adekuat, karena belum ada kebijakan secara khusus untuk PPIA. Disamping itu tiga kota/kabupaten masih menggunakan kebijakan dari pusat dan provinsi. Sebaiknya ada kebijakan tertulis dari pemerintah daerah melalui dinas kesehatan untuk implementasi layanan PPIA, misalnya dengan memasukkan anggaran PPIA secara khusus ke dalam APBD, baik untuk kegiatan promotif, preventif, kuratif, maupun rehabilitatif. Dengan adanya kebijakan pemerintah daerah dapat memberikan lingkungan yang kondusif bagi terselenggaranya upaya pencegahan dan penanggulangan penularan HIV dari ibu ke anak.Kata kunci : PPIA, HIV-AIDS, Pemerintah Daerah, KIA

  • Pencegahan Penularan HIV/AIDS: Efektivitas Metode KIE “Aku Bangga
           Aku Tahu (ABAT)”

      Abstract: The epidemic of HIV/AIDS in Indonesia is one of fastest growing among Asian countries. One of the efforts of the government to reduce HIV/AIDS cases is by implementing Information Education and Communication (IEC) which is so-called “Aku Bangga Aku Tahu (ABAT)”. However, the effectiveness of ABAT and its methods have not been evaluated. The aim of this study is to analyze the effectiveness of IEC of ABAT on knowledge, perception, stigma and HIV/AIDS behavioral prevention among selected high school students. This is a quantitative research, using pre-test, post-test, and control group of Quasi Experimental Design. A number of 221 students were chosen using multistage random sampling, divided into groups of single, multiple and control. The results of this study showed that the IEC of ABAT effectively increased knowledge, perception and preventive behavior, but it had not been able to reduce stigma. IEC of ABAT multiple sessions provided higher impact thanthat of the single session (p < 0.05). It is necessary to improve methods of IEC to encourage the improvement of knowledge, perception, and reduced stigma and HIV/AIDS risk behavior. Sebagai upaya pengendalian HIV/AIDS, pemerintah meluncurkan modul komunikasi informasi dan edukasi Aku Bangga Aku Tahu (KIE ABAT). Efektivitas metode dan KIE ABAT sampai saat ini belum pernah dilakukan evaluasi. Tujuan penelitian ini untuk menganalisis efektivitas KIE ABAT terhadap pengetahuan, persepsi, stigma dan perilaku pencegahan HIV/AIDS pada siswa/siswi SMA. Penelitian ini merupakan penelitian kuantitatif dengan rancangan penelitian Quasi Experiment Design dengan Pre-test Post-test Control Group Design. Sebanyak 221 siswa berpartisipasi dalam penelitian yang dipilih menggunakan multistage random sampling, kemudian dibagi kedalam kelompok single, multiple dan kontrol. Hasil penelitian menunjukkan bahwa KIE ABAT efektif meningkatkan pengetahuan, persepsi dan perilaku pencegahan, akan tetapi belum mampu menurunkan stigma. Simpulan berikutnya adalah KIE ABAT multiple session memberikan pengaruh yang lebih tinggi dibandingkan dengan single session. Untuk itu perlu penyempurnaan metode KIE agar dapat mendorong terjadinya peningkatan pengetahuan, persepsi, penurunan stigma dan peningkatan perilaku pencegahan HIV/AIDS.
