  • Sustainable development goals and the human resources crisis

      Abstract: Achieving universal health coverage by 2030 requires that lessons from the Millennium Development Goals must be heeded. The most important lesson is that the workforce underpins every function of the health system, and is the rate-limiting step. The three dimensions that continue to limit the success of the development agenda are availability, distribution and performance of health workers – and the Sustainable Development Goals cannot be achieved without addressing all three. Hence, the traditional response of scaling up supply is inadequate: a paradigm shift is required in the design of systems that can properly identify, train, allocate and retain health workers.

  • Prevalence and correlates of self-reported chronic non-communicable
           diseases in Botswana: a cross-sectional study

      Abstract: BackgroundThe purpose of this paper is to estimate the prevalence of self-reported chronic non-communicable diseases and their correlates in Botswana. This is a nationally representative, cross-sectional survey.MethodsThis is a cross-sectional study of respondents aged 10–64 years using data from the Botswana AIDS Impact Survey IV conducted in 2013. Three self-reported non-communicable diseases, namely, hypertension, diabetes and asthma were used. Multivariate logistic regression models were used to identify their correlates.ResultsOut of the 2153 participants, the prevalence rates of hypertension, diabetes and asthma were 14.2%, 3.3% and 5.3%, respectively. The study found that among other factors, older populations are at a much higher risk of having more than one non-communicable disease.   • Child marriage and women's attitude towards wife beating in a nationally
           representative sample of currently married adolescent and young women in
           Pakistan

      Abstract: BackgroundChild marriage (before 18 years) is widely prevalent in Pakistan, and disproportionately affects young girls in rural, low income and poorly-educated households. Our study aims to determine the association of child marriage and attitude towards wife beating among currently married Pakistani women aged 15–24 after controlling for social equity indicators (education, wealth index, rural residence).MethodsWe limited the data from Pakistan Demographic and Health Survey, 2012–2013 to currently married women aged 15–24 years (n=2648). Five specified dichotomous variables indicating women's attitude towards wife beating (goes out without telling husband, neglects the children, argues with husband, refuses to have sex with husband, burns the food) were considered as outcome variables. The likelihood (OR and 95% CI) of each outcome variable for the child marriage group was estimated using logistic regression models.ResultsThe prevalence of child marriage was significantly higher among women having no education and Balochi ethnicity, living in Khyber Pakhtunkhwa region and rural area, and belonging to the poorest quintile of wealth index. Women married as children compared with women married as adults were more likely to justify wife beating for all five specified reasons. However, these associations were lost when social equity indicators and national region of residence were adjusted in the regression models.ConclusionsHighly prevalent child marriage practice among women can be minimized by promoting education and providing economic opportunities in Pakistan.

  • Does tax-based health financing offer protection from financial
           catastrophe? Findings from a household economic impact survey of ischaemic
           heart disease in Malaysia

      Abstract: BackgroundMalaysia is an upper-middle income country with a tax-based health financing system. Health care is relatively affordable, and safety nets are provided for the needy. The objectives of this study were to determine the out-of-pocket health spending, proportion of catastrophic health spending (out-of-pocket spending >40% of non-food expenditure), economic hardship and financial coping strategies among patients with ischaemic heart disease (IHD) in Malaysia under the present health financing system.MethodsA cross-sectional study was conducted at the National Heart Institute of Malaysia involving 503 patients who were hospitalized during the year prior to the survey.ResultsThe mean annual out-of-pocket health spending for IHD was MYR3045 (at the time US$761). Almost 16% (79/503) suffered from catastrophic health spending (out-of-pocket health spending ≥40% of household non-food expenditures), 29.2% (147/503) were unable to pay for medical bills, 25.0% (126/503) withdrew savings to help meet living expenses, 16.5% (83/503) reduced their monthly food consumption, 12.5% (63/503) were unable to pay utility bills and 9.0% (45/503) borrowed money to help meet living expenses.ConclusionsOverall, the economic impact of IHD on patients in Malaysia was considerable and the prospect of economic hardship likely to persist over the years due to the long-standing nature of IHD. The findings highlight the need to evaluate the present health financing system in Malaysia and to expand its safety net coverage for vulnerable patients.

  • Sex disparities in outcomes among adults on long-term antiretroviral
           treatment in northern Nigeria

We investigated sex disparities in treatment outcomes for adults on first line antiretroviral treatment (ART) in Nigeria.MethodsWe compared clinical and immunologic responses to ART between HIV-infected men (n=205) and women (n=140) enrolled in an ART program between June 2004 and December 2007, with follow-up through June 2014.   • Financial burden of health care for Buruli ulcer patients in Nigeria: the
           patients’ perspective

      Abstract: BackgroundThe economic burden of Buruli ulcer for patients has not been well-documented. This study assessed the costs of Buruli ulcer care to patients from the onset of illness to diagnosis and to the end of treatment.MethodsThis was a cross-sectional cost of illness study conducted among patients with Buruli ulcer in four States in Nigeria between July and September 2015. A structured questionnaire was used to collect data on the patients’ characteristics, household income and out-of-pocket costs of care.ResultsOf 92 patients surveyed, 54 (59%) were older than 15years, 49 (53%) were males, and 86 (93%) resided in a rural area. The median (IQR) direct medical and non-medical cost per patient was US$124 (50–282) and US$3 (3–6); corresponding to 149% and 4% of the patients’ median monthly household income, respectively. The overall direct costs per patient was US$135 (58–327), which corresponded to 162% of median monthly household income, with pre-diagnosis costs accounting for 94.8% of the total costs. The direct costs of Buruli ulcer care were catastrophic for 50% of all patients/households – the rates of catastrophic costs for Buruli ulcer care was 66% and 19% for patients belonging to the lowest and highest income quartiles, respectively.ConclusionsDirect costs of Buruli ulcer diagnosis and treatment are catastrophic to a substantial proportion of patients and their families.

  • Multicenter prospective study on device-associated infection rates and
           bacterial resistance in intensive care units of Venezuela: International
           Nosocomial Infection Control Consortium (INICC) findings

      Abstract: BackgroundDevice-associated healthcare-acquired infections (DA-HAI) pose a threat to patient safety in the intensive care unit (ICU).MethodsA DA-HAI surveillance study was conducted by the International Nosocomial Infection Control Consortium (INICC) in two adult medical/surgical ICUs at two hospitals in Caracas, Venezuela, in different periods from March 2008 to April 2015, using the US Centers for Disease Control and Prevention's National Healthcare Safety Network (CDC/NHSN) definitions and criteria, and INICC methods.ResultsWe followed 1041 ICU patients for 4632 bed days. Central line-associated bloodstream infection (CLABSI) rate was 5.1 per 1000 central line days, ventilator-associated pneumonia (VAP) rate was 7.2 per 1000 mechanical ventilator days, and catheter-associated urinary tract infection (CAUTI) rate was 3.9 per 1000 urinary catheter days, all similar to or lower than INICC rates (4.9 [CLABSI]; 16.5 [VAP]; 5.3 [CAUTI]), and higher than CDC/NHSN rates (0.8 [CLABSI]; 1.1 [VAP]; and 1.3 [CAUTI]). Device utilization ratios were higher than INICC and CDC/NHSN rates, except for urinary catheter, which was similar to INICC. Extra length of stay was 8 days for patients with CLABSI, 9.6 for VAP and 5.7 days for CAUTI. Additional crude mortality was 3.0% for CLABSI, 4.4% for VAP, and 16.9% for CAUTI.ConclusionsDA-HAI rates in our ICUs are higher than CDC/NSHN's and similar to or lower than INICC international rates.

  • Dedicated health systems strengthening of the Global Fund to Fight AIDS,
           Tuberculosis, and Malaria: an analysis of grants

      Abstract: BackgroundThis study aims to understand the determinants of the Global Fund to Fight AIDS, Tuberculosis, and Malaria's dedicated channel for health systems strengthening (HSS) funding across countries and to analyze their health system priorities expressed in budgets and performance indicators.MethodsWe obtained publicly available data for disease-specific and HSS grants from the Global Fund over 2004–2013 prior to the new funding model. Regression analysis was employed to assess the determinants of dedicated HSS funding across 111 countries. Documents for 27 dedicated HSS grants including budgets and performance indicators were collected, and activities were analyzed by health system functions.ResultsHSS funding per capita is significantly associated with TB and HIV funding per capita, but not per capita income and health worker density. Of 27 dedicated HSS grants, 11 had line-item budgets publicly available, in which health workforce and medical products form the majority (89% or US$132 million of US$148 million) of funds. Yet these areas accounted for 41.7% (215) of total 516 performance indicators.ConclusionsHealth worker densities were not correlated with HSS funding, despite the emphasis on health workforce in budgets and performance indicators. Priorities in health systems in line-item budgets differ from the numbers of indicators used.

      Abstract: BackgroundSelf-rated health and life satisfaction are two subjective measures for assessing overall health status. This study aims to investigate the association of self-rated health and life satisfaction with physical activity and screen time.MethodsAs part of the fourth survey of a national surveillance program in Iran (CASPIAN-IV study), 14 880 students aged 6 to 18 years were selected via multi-stage cluster sampling from 30 provinces. Data were obtained from the WHO Global School-Based Student Health Survey questionnaire.ResultsA total of 13 486 students with mean age of 12.47 (SD 3.36) completed the study. In crude model both prolonged screen time and physical activity were associated with favorable life satisfaction and self-rated health. However, in multivariate analysis only high physical activity was associated with good self-rated health (OR 1.37) and life satisfaction (OR 1.39), while prolonged screen time was not associated with good self-rated health (OR 1.02) and life satisfaction (OR 0.94). For combined screen time-physical activity variable, low screen time-high physical activity combination had the highest OR for both good self-rated health (OR 1.37) and life satisfaction (OR 1.43) in multivariate analysis.ConclusionsOur findings suggest that increasing physical activity is more crucial than emphasizing reducing screen time in improving the well-being of children and adolescents.

  • Assessing the suitability of sets-based approaches: estimating the
           discriminative power of risk models for ordinal outcome treatments

      Abstract: BackgroundIn order to evaluate the discrimination performance of an ordinal model for improved disease screening, a new test was proposed where information was obtained across all samples simultaneously.MethodsThe ordinal c-index builds upon the volume under the surface methodology without focusing on the accompanying receiver operating characteristic surfaces. However, it can be simplified to an average of pairwise c-indexes. In this paper, a set-based estimate (information was obtained across all samples simultaneously) was proposed by summing all correctly ordered groups. The asymptotic distribution of this proposed estimate was derived using U-statistics.ResultsA predictive model was applied using the blood urea nitrogen/creatinine ratio to discriminate stroke in evolution in acute ischemic stroke patients, which could potentially be life-saving in emergency departments.ConclusionsBy conducting Monte Carlo simulations, it was concluded that the measure proposed herein is a better choice for clinical use because of the asymmetry of the predicted probabilities of groups.
