  • Wireless Capsule Gastrointestinal Endoscopy: Direction-of-Arrival
           Estimation Based Localization Survey

      Abstract: One of the significant challenges in capsule endoscopy (CE) is to precisely determine the pathologies location. The localization process is primarily estimated using the received signal strength (RSS) from sensors in the capsule system through its movement in the gastrointestinal (GI) tract. Consequently, the wireless CE (WCE) system requires improvement to handle the lack of the capsule instantaneous localization information and to solve the relatively low transmission data rate challenges. Furthermore, the association among the capsule's transmitter position, capsule location, signal reduction, and the capsule direction should be assessed. These measurements deliver significant information for the instantaneous capsule localization systems based on time-of-arrival approach, phase difference of arrival, RSS, electromagnetic, direction of arrival (DOA), and video tracking approaches are developed to locate the WCE precisely. This review introduces the acquisition concept of the GI medical images using the endoscopy with a comprehensive description of the endoscopy system components. Capsule localization and tracking are considered to be the most important features of the WCE system, thus this paper emphasizes the most common localization systems generally, highlighting the DOA-based localization systems and discusses the required significant research challenges to be addressed.

  • Endomicroscopy for Computer and Robot Assisted Intervention

      Abstract: Endomicroscopy is a new technique that allows human tissue to be characterized in vivo and in situ, circumventing the need for conventional biopsy and histology. Despite increased application and growing research interests in this area, the clinical application of endomicroscopy, however, is limited by difficulties in ergonomic control, consistent probe-tissue contact, large area surveillance, and retargeting. Recently, advances in high-speed imaging, mosaicing, and robotics have aimed to address these difficulties. The development of robot-assisted devices in particular has shown great promises in extending the clinical potential of endomicroscopy. Issues related to miniaturization, adaptation to tissue deformation, control stability, force and position compensation, cost, and sterility are being pursued by both research and commercial communities. In this review, recent clinical and technical developments in different aspects of computer and robotic assisted endomicroscopy interventions including instrumentation, multiscale integration, and high-speed imaging techniques are presented. We further address emerging trends and new research opportunities toward more widespread clinical acceptance of robotically assisted endomicroscopy technologies.

  • Are Current Advances of Compression Algorithms for Capsule Endoscopy
           Enough' A Technical Review

      Abstract: The recent technological advances in capsule endoscopy system have revolutionized the healthcare system by introducing new techniques and functionalities to diagnose gastrointestinal tract. These techniques improve diagnostic accuracy and reduce the risk of hospitalization. Although many benefits of capsule endoscopy are known, there are still limitations including lower battery life, higher bandwidth, poor image quality and lower frame rate, which have restricted its wide use. In order to solve these limitations, the importance of a low-cost compression algorithm, that produces higher frame rate with better image quality and yet consumes lower bandwidth and transmission power, is paramount. While several review papers have been published describing the capability of capsule endoscope in terms of its functionality and emerging features, an extensive review on the compression algorithms from past and for future applications is still of great interest. Hence, in this review, we aim to address the issue by exploring the characteristics of endoscopic images, analyzing the strengths and weaknesses of useful compression techniques, and making suggestions for possible future adaptation.

  • A Review of Safety and Hazards Associated With the Artificial Pancreas

      Abstract: The artificial pancreas (AP) is a closed-loop device with the potential to reduce the complications associated with type 1 diabetes mellitus by maintaining euglycemia in patients. The AP encompasses an algorithm that determines the amount of insulin (and other hormones) to be administered to the patient via a continuous subcutaneous insulin infusion pump using information provided by a continuous glucose monitor and other sensors. As the AP approaches commercialization, special attention must be given to safety within all the individual components, including physiological changes in the patient, as well as safety issues that can arise when these components are combined into a single system. Therefore, we analyzed the specific hazards applicable to the AP with the aim of exposing areas of safety that are yet to be addressed.

  • Haptic Feedback in Needle Insertion Modeling and Simulation

      Abstract: Needle insertion is the most basic skill in medical care, and training has to be imparted not only for physicians but also for nurses and paramedics. In most needle insertion procedures, haptic feedback from the needle is the main stimulus in which novices need training. For better patient safety, the classical methods of training the haptic skills have to be replaced with simulators based on new robotic and graphics technologies. This paper reviews the current advances in needle insertion modeling, classified into three sections: needle insertion models, tissue deformation models, and needle-tissue interaction models. Although understated in the literature, the classical and dynamic friction models, which are critical for needle insertion modeling, are also discussed. The experimental setup or the needle simulators that have been developed to validate the models are described. The need of psychophysics for needle simulators and psychophysical parameter analysis of human perception in needle insertion are discussed, which are completely ignored in the literature.

  • Virtual and Augmented Reality Systems for Renal Interventions: A
           Systematic Review

      Abstract: Purpose: Many virtual and augmented reality systems have been proposed to support renal interventions. This paper reviews such systems employed in the treatment of renal cell carcinoma and renal stones. Methods: A systematic literature search was performed. Inclusion criteria were virtual and augmented reality systems for radical or partial nephrectomy and renal stone treatment, excluding systems solely developed or evaluated for training purposes. Results: In total, 52 research papers were identified and analyzed. Most of the identified literature (87%) deals with systems for renal cell carcinoma treatment. About 44% of the systems have already been employed in clinical practice, but only 20% in studies with ten or more patients. Main challenges remaining for future research include the consideration of organ movement and deformation, human factor issues, and the conduction of large clinical studies. Conclusion: Augmented and virtual reality systems have the potential to improve safety and outcomes of renal interventions. In the last ten years, many technical advances have led to more sophisticated systems, which are already applied in clinical practice. Further research is required to cope with current limitations of virtual and augmented reality assistance in clinical environments.

  • Advances in Monte Carlo Simulation for Light Propagation in Tissue

      Abstract: Monte Carlo (MC) simulation for light propagation in tissue is the gold standard for studying the light propagation in biological tissue and has been used for years. Interaction of photons with a medium is simulated based on its optical properties. New simulation geometries, tissue-light interaction methods, and recording techniques recently have been designed. Applications, such as whole mouse body simulations for fluorescence imaging, eye modeling for blood vessel imaging, skin modeling for terahertz imaging, and human head modeling for sinus imaging, have emerged. Here, we review the technical advances and recent applications of MC simulation.

  • Wireless Power Transfer Strategies for Implantable Bioelectronics

      Abstract: Neural implants have emerged over the last decade as highly effective solutions for the treatment of dysfunctions and disorders of the nervous system. These implants establish a direct, often bidirectional, interface to the nervous system, both sensing neural signals and providing therapeutic treatments. As a result of the technological progress and successful clinical demonstrations, completely implantable solutions have become a reality and are now commercially available for the treatment of various functional disorders. Central to this development is the wireless power transfer (WPT) that has enabled implantable medical devices (IMDs) to function for extended durations in mobile subjects. In this review, we present the theory, link design, and challenges, along with their probable solutions for the traditional near-field resonant inductively coupled WPT, capacitively coupled short-ranged WPT, and more recently developed ultrasonic, mid-field, and far-field coupled WPT technologies for implantable applications. A comparison of various power transfer methods based on their power budgets and WPT range follows. Power requirements of specific implants like cochlear, retinal, cortical, and peripheral are also considered and currently available IMD solutions are discussed. Patient's safety concerns with respect to electrical, biological, physical, electromagnetic interference, and cyber security from an implanted neurotech device are also explored in this review. Finally, we discuss and anticipate future developments that will enhance the capabilities of current-day wirelessly powered implants and make them more efficient and integrable with other electronic components in IMDs.

  • A Survey on Biofeedback and Actuation in Wireless Body Area Networks

      Abstract: Wireless body area networks (WBANs) have attained increasing popularity as the next generation framework of wearable technologies for human monitoring. Invasive or noninvasive wearable sensors designed in a WBAN are worn to gather vital information. Biofeedback is a recent concept where collected data are used to generate actuation signals in WBANs. Applications can be seen in various areas such as sports (e.g., locomotor velocity) or medicine (e.g., blood pressure measurement). However, since the body is closely regulated, the next generation WBAN technology must be smart enough to react to monitored data. The main aim of this paper is to review the current state of biofeedback and actuation technology on WBANs in terms of its structure, applications, benefits, and control approaches. The emphasis on the specific requirements when applying biofeedback to humans will be highlighted and discussed. Challenges and open research issues will be concluded at the end.

  • Stability of Enzymatic Biosensors for Wearable Applications

      Abstract: Technological evolution in wearable sensors accounts for major growth and transformation in a multitude of industries, ranging from healthcare to computing and informatics to communication and biomedical sciences. The major driver for this transformation is the new-found ability to continuously monitor and analyze the patients' physiology in patients' natural setting. Numerous wearable sensors are already on the market and are summarized. Most of the current technologies have focused on electrophysiological, electromechanical, or acoustic measurements. Wearable biochemical sensing devices are in their infancy. Traditional challenges in biochemical sensing such as reliability, repeatability, stability, and drift are amplified in wearable sensing systems due to variabilities in operating environment, sample/sensor handling, and motion artifacts. Enzymatic sensing technologies, due to reduced fluidic challenges, continue to be forerunners for converting into wearable sensors. This paper reviews the recent developments in wearable enzymatic sensors. The wearable sensors have been classified in three major groups based on sensor embodiment and placement relative to the human body: 1) on-body, 2) clothing/textile-based biosensors, and 3) biosensor accessories. The sensors, which come in the forms of stickers and tattoos, are categorized as on-body biosensors. The fabric-based biosensor comes in different models such as smart-shirts, socks, gloves, and smart undergarments with printed sensors for continuous monitoring.

  • Active Electrodes for Wearable EEG Acquisition: Review and Electronics

      Abstract: Active electrodes (AEs), i.e., electrodes with built-in readout circuitry, are increasingly being implemented in wearable healthcare and lifestyle applications due to AEs' robustness to environmental interference. An AE locally amplifies and buffers μV-level EEG signals before driving any cabling. The low output impedance of an AE mitigates cable motion artifacts, thus enabling the use of high-impedance dry electrodes for greater user comfort. However, developing a wearable EEG system, with medical grade signal quality on noise, electrode offset tolerance, common-mode rejection ratio, input impedance, and power dissipation, remains a challenging task. This paper reviews state-of-the-art bio-amplifier architectures and low-power analog circuits design techniques intended for wearable EEG acquisition, with a special focus on an AE system interfaced with dry electrodes.

      Abstract: Apnea in the pediatric population is associated with increased morbidity and mortality in a large number of developed as well as developing countries. It is even more prominent in preterm newborn infants and is commonly referred to as apnea of prematurity. Its current diagnosis and therapy involve the use of traditional technologies, which often result in discomfort to the infants due to the use of invasive devices attached to their sensitive skin, especially in overnight clinical sleep analysis (for over a 12- or 24-h period). Emerging trends for the point-of-care diagnosis of this sleep disorder are focused on the design of integrated devices for less complex and noninvasive monitoring. This paper presents a review of the state of the art of clinical technologies and methodologies for sleep apnea detection and their pros and cons, with particular focus on their working principles and relevance to pediatrics. Moreover, an in-depth discussion on emerging future technologies envisioned to be integral parts of the daily home-based applications is included in the paper.

  • Multiple-Instance Learning for Medical Image and Video Analysis

      Abstract: Multiple-instance learning (MIL) is a recent machine-learning paradigm that is particularly well suited to medical image and video analysis (MIVA) tasks. Based solely on class labels assigned globally to images or videos, MIL algorithms learn to detect relevant patterns locally in images or videos. These patterns are then used for classification at a global level. Because supervision relies on global labels, manual segmentations are not needed to train MIL algorithms, unlike traditional single-instance learning (SIL) algorithms. Consequently, these solutions are attracting increasing interest from the MIVA community: since the term was coined by Dietterich et al. in 1997, 73 research papers about MIL have been published in the MIVA literature. This paper reviews the existing strategies for modeling MIVA tasks as MIL problems, recommends general-purpose MIL algorithms for each type of MIVA tasks, and discusses MIVA-specific MIL algorithms. Various experiments performed in medical image and video datasets are compiled in order to back up these discussions. This meta-analysis shows that, besides being more convenient than SIL solutions, MIL algorithms are also more accurate in many cases. In other words, MIL is the ideal solution for many MIVA tasks. Recent trends are discussed, and future directions are proposed for this emerging paradigm.

  • State-of-the-Art Methods for Brain Tissue Segmentation: A Review

      Abstract: Brain tissue segmentation is one of the most sought after research areas in medical image processing. It provides detailed quantitative brain analysis for accurate disease diagnosis, detection, and classification of abnormalities. It plays an essential role in discriminating healthy tissues from lesion tissues. Therefore, accurate disease diagnosis and treatment planning depend merely on the performance of the segmentation method used. In this review, we have studied the recent advances in brain tissue segmentation methods and their state-of-the-art in neuroscience research. The review also highlights the major challenges faced during tissue segmentation of the brain. An effective comparison is made among state-of-the-art brain tissue segmentation methods. Moreover, a study of some of the validation measures to evaluate different segmentation methods is also discussed. The brain tissue segmentation, content in terms of methodologies, and experiments presented in this review are encouraging enough to attract researchers working in this field.

      Abstract: This paper provides a focused and organized review of the research progress on neurophysiological indicators, also called “neurometrics,” to show how they can effectively address some of the most important human factors (HFs) needs in the air traffic management (ATM) field. In order to better understand and highlight available opportunities of such neuroscientific applications, state of the art on the most involved HFs and related cognitive processes (e.g., mental workload and cognitive training) are presented together with examples of possible applications in current and future ATM scenarios. Furthermore, this paper will discuss the potential enhancements that further research and development activities could bring to the efficiency and safety of the ATM service.

      Abstract: There is a growing body of research focusing on automatic detection of ischemia and myocardial infarction (MI) using computer algorithms. In clinical settings, ischemia and MI are diagnosed using electrocardiogram (ECG) recordings as well as medical context including patient symptoms, medical history, and risk factors-information that is often stored in the electronic health records. The ECG signal is inspected to identify changes in the morphology such as ST-segment deviation and T-wave changes. Some of the proposed methods compute similar features automatically while others use nonconventional features such as wavelet coefficients. This review provides an overview of the methods that have been proposed in this area, focusing on their historical evolution, the publicly available datasets that they have used to evaluate their performance, and the details of their algorithms for ECG and EHR analysis. The validation strategies that have been used to evaluate the performance of the proposed methods are also presented. Finally, the paper provides recommendations for future research to address the shortcomings of the currently existing methods and practical considerations to make the proposed technical solutions applicable in clinical practice.

      Abstract: Gait is an important part of our life, as it affects many daily activities. Special footwear is fundamental to obtain an ergonomic gait and to extract data for analysis. The plantar foot pressure can be employed to detect many kinds of disorders, suggest improvements in treatments, rehabilitation tasks, patient monitoring, development of orthopedic devices, and other applications. In recent years, attention to this topic has grown and is reflected in many works issued in both commercial and academic groups, and has focused on the development of devices for foot plantar pressure measurement with applications in medicine, sports, and research. First works on this subject appeared around 1963 and have continuously evolved with emerging technologies. This paper reviews the reported developments in the field of footwear-embedded sensors for gait measurement, monitoring, diagnosis, and analysis in rehabilitation. Future work is proposed to improve the field of measurement of the footprint with electronic devices.

  • The Role of a Dynamic Craniospinal Compliance in NPH—A Review and
           Future Challenges

      Abstract: Despite the normal mean intracranial pressure in normal pressure hydrocephalus, the cerebral ventricles enlarge. Many hypotheses exist as to why, and these have lately been investigated in simulation. These can be grouped into one of two categories: 1) Tissue damage is caused by a transmantle pressure gradient widening the ventricles mechanically. 2) The overall cerebrospinal fluid dynamics are disturbed resulting from various pathologies. This paper reviews the literature regarding the computational simulation models investigating the development of enlarging ventricles in connection with the onset of hydrocephalus. The models are categorized by the underlying hypothesis and their results are contrasted with clinical findings in the field. Finally, open questions are identified for future modeling approaches.

  • Diving Into Research of Biomedical Engineering in Scuba Diving

      Abstract: The physiologic response of the human body to different environments is a complex phenomenon to ensure survival. Immersion and compressed gas diving, together, trigger a set of responses. Monitoring those responses in real time may increase our understanding of them and help us to develop safety procedures and equipment. This review outlines diving physiology and diseases and identifies physiological parameters worthy of monitoring. Subsequently, we have investigated technological approaches matched to those in order to evaluated their capability for underwater application. We focused on wearable biomedical monitoring technologies, or those which could be transformed to wearables. We have also reviewed current safety devices, including dive computers and their underlying decompression models and algorithms. The review outlines the necessity for biomedical monitoring in scuba diving and should encourage research and development of new methods to increase diving safety.

  • Evolutionary Computing Enriched Computer-Aided Diagnosis System for
           Diabetic Retinopathy: A Survey

      Abstract: Complications caused due to diabetes mellitus result in significant microvasculature that eventually causes diabetic retinopathy (DR) that keeps on increasing with time, and eventually causes complete vision loss. Identifying subtle variations in morphological changes in retinal blood vessels, optic disk, exudates, microaneurysms, hemorrhage, etc., is complicated and requires a robust computer-aided diagnosis (CAD) system so as to enable earlier and efficient DR diagnosis practices. In the majority of the existing CAD systems, functional enhancements have been realized time and again to ensure accurate and efficient diagnosis of DR. In this survey paper, a number of existing literature presenting DR CAD systems are discussed and analyzed. Both traditional and varoius evolutionary approaches, including genetic algorithm, particle swarm optimization, ant colony optimization, bee colony optimization, etc., based DR CAD have also been studied and their respective efficiencies have been discussed. Our survey revealed that evolutionary computing methods can play a vital role for optimizing DR-CAD functional components, such as proprocessing by enhancing filters coefficient, segmentation by enriching clustering, feature extraction, feature selection, and dimensional reduction, as well as classification.
