  • Movement Quantification in Neurological Diseases: Methods and Applications

      Abstract: The movement of the human body offers neurologists important clues for the diagnosis and follow-up of many neurological diseases. The typical diagnosis approach is accomplished through simple observation of movements of interest (MOI) associated with a specific neurological disease. This approach is highly subjective because it is mainly based on qualitative evaluation of MOIs. Quantitative movement techniques are then obvious diagnosis-aid systems to approach these cases. Nevertheless, the use of motion quantification techniques in these pathologies is still relatively rare. In this paper, we intend to review this area and provide a clear picture of the current state of the art, both in the methods used and their applications to the main movement-related neurological diseases. We approach some historic aspects and the current state of the motion capture techniques and present the results of a survey to the literature that includes 82 papers, since 2006, covering the usage of these techniques in neurological diseases. Furthermore, we discuss the pros and cons of using quantitative approaches in these clinical scenarios. Finally, we present some conclusions and discuss the trends we foresee for the future.

  • Treatment of the Partial Hand Amputation: An Engineering Perspective

      Abstract: Partial hand amputation is perhaps the most frequent amputation level, worldwide. Although its annual incidence in western countries is roughly 1:18 000 inhabitants, the treatments of partial hand amputations have modestly progressed so far. We have identified three main limitation factors to this progress: 1) the wide range of anatomical and functional presentations, which makes difficult to find standardized and scalable solutions; 2) the technological complexity in replacing the motor and sensory function of a lost digit in the size of a digit; and 3) the fact that a partial hand can be functionally successful mostly when it restores a lost opposition movement, i.e., the ability to oppose the thumb against the fingers while providing enough grasping force and aperture width. This review presents an overview of the existing surgical and technological solutions for treating partial hand amputations, and is specifically targeted to (biomedical) engineers. We critically highlighted the advantages, limitations, and open challenges. Remarkably, current fitting procedures rely on manual approaches by skilled prosthetic technicians rather than on modern engineering methods. Hence, the objective of this study is to comprehensively but concisely overview the field in order to inspire engineers to develop new systems and procedures able to address current open issues.

  • Magnetic Surgical Instruments for Robotic Abdominal Surgery

      Abstract: This review looks at the implementation of magnetic-based approaches in surgical instruments for abdominal surgeries. As abdominal surgical techniques advance toward minimizing surgical trauma, surgical instruments are enhanced to support such an objective through the exploration of magnetic-based systems. With this design approach, surgical devices are given the capabilities to be fully inserted intraabdominally to achieve access to all abdominal quadrants, without the conventional rigid link connection with the external unit. The variety of intraabdominal surgical devices are anchored, guided, and actuated by external units, with power and torque transmitted across the abdominal wall through magnetic linkage. This addresses many constraints encountered by conventional laparoscopic tools, such as loss of triangulation, fulcrum effect, and loss/lack of dexterity for surgical tasks. Design requirements of clinical considerations to aid the successful development of magnetic surgical instruments, are also discussed.

  • A Review of Engineering Approaches for Lymphedema Detection

      Abstract: Edema is a condition characterized by excessive swelling of a tissue due to an abnormal accumulation of interstitial fluid in the subcutaneous tissue. More specifically, disruption of the lymphatic system causes what is known as lymphedema. This condition is commonly seen in breast cancer survivors postradiotherapy treatment, chemotherapy, and surgeries; this population has shown high risk of developing lymphedema in the limbs. Throughout the years, several techniques have been developed and implemented for the detection and measurement of lymphedema, including techniques to measure the diseased limb volume, electrical techniques to measure the water content in tissues, and optical techniques to measure either tissue absorbance or limb volume. However, there is still no method that allows for continuous monitoring of the disease and provides a better understanding of its progression. This study describes the different approaches that have been used and that could be used for lymphedema measurement.

  • Laser Speckle Imaging to Monitor Microvascular Blood Flow: A Review

      Abstract: Laser speckle is a complex interference phenomenon that can easily be understood, in concept, but is difficult to predict mathematically, because it is a stochastic process. The use of laser speckle to produce images, which can carry many types of information, is called laser speckle imaging (LSI). The biomedical applications of LSI started in 1981 and, since then, many scientists have improved the laser speckle theory and developed different imaging techniques. During this process, some inconsistencies have been propagated up to now. These inconsistencies should be clarified in order to avoid errors in future works. This review presents a review of the laser speckle theory used in biomedical applications. Moreover, we also make a review of the practical concepts that are useful in the construction of laser speckle imagers. This study is not only an exposition of the concepts that can be found in the literature but also a critical analysis of the investigations presented so far. Concepts like scatterers velocity distribution, effect of static scatterers, optimal speckle size, light penetration angle, and contrast computation algorithms are discussed in detail.

  • Mathematical Models of Contrast Transport Kinetics for Cancer Diagnostic
           Imaging: A Review

      Abstract: Angiogenesis plays a fundamental role in cancer growth and the formation of metastasis. Novel cancer therapies aimed at inhibiting angiogenic processes and/or disrupting angiogenic tumor vasculature are currently being developed and clinically tested. The need for earlier and improved cancer diagnosis, and for early evaluation and monitoring of therapeutic response to angiogenic treatment, have led to the development of several imaging methods for in vivo noninvasive assessment of angiogenesis. The combination of dynamic contrast-enhanced imaging with mathematical modeling of the contrast agent kinetics enables quantitative assessment of the structural and functional changes in the microvasculature that are associated with tumor angiogenesis. In this paper, we review quantitative imaging of angiogenesis with dynamic contrast-enhanced magnetic resonance imaging, computed tomography, and ultrasound.

  • Quantitative Laughter Detection, Measurement, and Classification—A
           Critical Survey

      Abstract: The study of human nonverbal social behaviors has taken a more quantitative and computational approach in recent years due to the development of smart interfaces and virtual agents or robots able to interact socially. One of the most interesting nonverbal social behaviors, producing a characteristic vocal signal, is laughing. Laughter is produced in several different situations: in response to external physical, cognitive, or emotional stimuli; to negotiate social interactions; and also, pathologically, as a consequence of neural damage. For this reason, laughter has attracted researchers from many disciplines. A consequence of this multidisciplinarity is the absence of a holistic vision of this complex behavior: the methods of analysis and classification of laughter, as well as the terminology used, are heterogeneous; the findings sometimes contradictory and poorly documented. This survey aims at collecting and presenting objective measurement methods and results from a variety of different studies in different fields, to contribute to build a unified model and taxonomy of laughter. This could be successfully used for advances in several fields, from artificial intelligence and human-robot interaction to medicine and psychiatry.

  • Treating Total Occlusions: Applying Force for Recanalization

      Abstract: Goal: The aim of this review was to analyze and review the state of the art devices for the endovascular treatment of chronic and acute total occlusions in the different vascular beds (coronary, peripheral, carotid, and intracranial arteries) based on the forces exerted on the occlusion during treatment. Methods: The literature search was executed in the databases of Scopus and Espacenet. Overall, 120 patents and 49 articles have been reviewed and categorized. Results: The identified treatment tools were divided into three treatment modalities: compression, resection, and conversion. A further subdivision was made based on the force direction, i.e., axial, radial, tangential, or energy conversion process (dissolving or state change). Conclusion: The working principles of the identified treatment tools relate strongly to the (biomechanical) properties of the occlusion and environment. In order to increase procedural success, especially in chronic occlusions, the applied force should be increased by applying buckling prevention methods, the penetration force decreased by smart tip design or dynamic loading, and the energy dissipation to the environment minimized. Significance: Insight in the working principles of the treatment tools and their effect on the environment can in future aid designers to overcome current challenges and develop new, innovative treatment tools.

  • State of the Art for Deep Brain Stimulation Therapy in Movement Disorders:
           A Clinical and Technological Perspective

      Abstract: Deep brain stimulation (DBS) therapy is a widely used brain surgery that can be applied for many neurological and psychiatric disorders. DBS is American Food and Drug Administration approved for medication refractory Parkinson's disease, essential tremor and dystonia. Although DBS has shown consistent success in many clinical trials, the therapy has limitations and there are well-recognized complications. Thus, only carefully selected patients are ideal candidates for this surgery. Over the last two decades, there have been significant advances in clinical knowledge on DBS. In addition, the surgical techniques and technology related to DBS has been rapidly evolving. The goal of this review is to describe the current status of DBS in the context of movement disorders, outline the mechanisms of action for DBS in brief, discuss the standard surgical and imaging techniques, discuss the patient selection and clinical outcomes in each of the movement disorders, and finally, introduce the recent advancements from a clinical and technological perspective.

  • Robust Nucleus/Cell Detection and Segmentation in Digital Pathology and
           Microscopy Images: A Comprehensive Review

      Abstract: Digital pathology and microscopy image analysis is widely used for comprehensive studies of cell morphology or tissue structure. Manual assessment is labor intensive and prone to interobserver variations. Computer-aided methods, which can significantly improve the objectivity and reproducibility, have attracted a great deal of interest in recent literature. Among the pipeline of building a computer-aided diagnosis system, nucleus or cell detection and segmentation play a very important role to describe the molecular morphological information. In the past few decades, many efforts have been devoted to automated nucleus/cell detection and segmentation. In this review, we provide a comprehensive summary of the recent state-of-the-art nucleus/cell segmentation approaches on different types of microscopy images including bright-field, phase-contrast, differential interference contrast, fluorescence, and electron microscopies. In addition, we discuss the challenges for the current methods and the potential future work of nucleus/cell detection and segmentation.

  • Review on Lithotripsy and Cavitation in Urinary Stone Therapy

      Abstract: Cavitation is the sudden formation of vapor bubbles or voids in liquid media and occurs after rapid changes in pressure as a consequence of mechanical forces. It is mostly an undesirable phenomenon. Although the elimination of cavitation is a major topic in the study of fluid dynamics, its destructive nature could be exploited for therapeutic applications. Ultrasonic and hydrodynamic sources are two main origins for generating cavitation. The purpose of this review is to give the reader a general idea about the formation of cavitation phenomenon and existing biomedical applications of ultrasonic and hydrodynamic cavitation. Because of the high number of the studies on ultrasound cavitation in the literature, the main focus of this review is placed on the lithotripsy techniques, which have been widely used for the treatment of urinary stones. Accordingly, cavitation phenomenon and its basic concepts are presented in Section II. The significance of the ultrasound cavitation in the urinary stone treatment is discussed in Section III in detail and hydrodynamic cavitation as an important alternative for the ultrasound cavitation is included in Section IV. Finally, side effects of using both ultrasound and hydrodynamic cavitation in biomedical applications are presented in Section V.
