  • Long-term effectiveness of group cognitive-behavioral therapy for
           alcoholism: Impact of dual diagnosis on treatment outcome

    • Abstract: Resumen: El objetivo de este estudio fue analizar el resultado del tratamiento cognitivo-conductual grupal llevado a cabo en un centro ambulatorio para el tratamiento de las adicciones. La muestra estuvo compuesta por 130 hombres con trastorno por dependencia al alcohol. Se evaluaron los casos que reinician tratamiento tras el alta terapéutica en un rango de tiempo desde los dos años como mínimo hasta los 18. El 56.2% de la muestra finalizó el tratamiento con alta terapéutica, de los cuales el 75.3% no volvieron a iniciar tratamiento tras el alta, aumentando a 77.7% cuando el paciente completaba el periodo de revisión tras el alta. El 58% de los pacientes con trastornos de la personalidad finalizaron el tratamiento con alta terapéutica. El tratamiento cognitivo-conductual, cuyos componentes esenciales se detallan en este trabajo, es efectivo para el tratamiento grupal de los trastornos por dependencia del alcohol con comorbilidad de trastornos de la personalidad. : The aim of the present study was to analyze the results of a cognitive-behavioral group therapy applied in an outpatient unit to treat drug addiction. The sample consisted of 130 males with an alcohol dependence disorder. Around 56% of the sample completed treatment - they were given therapeutic discharge. From those, 75.3% never resumed treatment, increasing to 77.7% when patients completed the check-up period after discharge. Follow-up from therapeutic discharge varies from 2 to 18 years; 58% of patients with personality disorders completed treatment with therapeutic discharge. Cognitive-behavioral treatment, whose essential components are detailed in the present study, is effective for the group treatment of patients with alcohol dependence and a comorbid personality disorder.
       
  • Because time does not heal all wounds: Efficacy of trauma-focused
           cognitive behavioral therapy for very long-term posttraumatic stress in
           victims of terrorism

    • Abstract: Resumen: Ciento veinte víctimas del terrorismo que habían sufrido un atentado como media 18 años antes y que presentaban sintomatología o trastorno de estrés postraumático (TEPT) fueron aleatorizadas a 16 sesiones de terapia cognitivo-conductual centrada en el trauma (TCC-CT) o a lista de espera. Ninguna de las víctimas que completó el tratamiento presentaba un diagnóstico de TEPT en el postratamiento; estas víctimas mostraban niveles de sintomatología de estrés postraumático significativamente menores en el postratamiento que en el pretratamiento, con una diferencia pre-post grande (d = 1.39) y un 78.3% de ellas mostraban niveles normales en el postratamiento. Estos resultados fueron significativamente mejores que los del grupo control, siendo la diferencia tratamiento-control grande (d = 0.91). La TCC-CT es eficaz y representa actualmente el tratamiento de elección para víctimas del terrorismo que sufren TEPT o sintomatología de estrés postraumático, incluso a muy largo plazo, entre 6 y 51 años después del atentado. : One hundred and twenty victims of terrorism who had suffered a terrorist attack an average of 18 years ago and who mainly had posttraumatic stress disorder (PTSD) or posttraumatic stress symptomatology were randomized to a 16-session trauma-focused cognitive-behavioral therapy (TF-CBT) or to a waiting-list control group. No victim who completed treatment had a PTSD diagnosis at posttreatment; these victims showed significantly lower levels of posttraumatic stress symptomatology at posttreatment than at pretreatment, with a large pre-posttreatment difference (d = 1.39), and 78.3% of them showed normal levels of symptomatology at posttreatment. These results were significantly better than those of the control group, with a large treatment-control difference (d = 0.91). TF-CBT is efficacious and is currently the treatment of choice for victims of terrorism who suffer from PTSD or posttraumatic stress symptomatology, even in the very long term, between 6 and 51 years after the terrorist attack.
       
  • Prevention of depression in caregivers through conference calls

    • Abstract: Resumen: El objetivo del estudio fue evaluar una intervención breve, en formato telefónico, grupal, dirigida a cuidadores con síntomas depresivos elevados. Se aplicó la intervención de prevención indicada a 61 cuidadores (M = 55.5 años, 90.2% mujeres). Evaluadores independientes valoraron la incidencia de depresión, la sintomatología depresiva, el riesgo de desarrollar depresión y las variables del modelo teórico en que se basó la intervención en la preintervención, posintervención y seguimientos a 1 y 3 meses. La incidencia de depresión hasta 3 meses de seguimiento fue de 4.9%. Hubo una reducción significativa de sintomatología depresiva (p < .001) y del riesgo de desarrollar depresión (p < .001) en la posintervención y los seguimientos a 1 y 3 meses. Las variables del modelo mejoraron significativamente tras la intervención y estuvieron asociadas con los síntomas depresivos posintervención. La adherencia y satisfacción fueron elevadas. Los resultados alientan la realización de un ensayo clínico controlado aleatorizado. : The aim of this study was to evaluate a brief intervention, implemented via conference call, for non-professional caregivers with high depressive symptoms. Sixty-one caregivers (M = 55.5, 90.2% women) underwent an intervention of indicated prevention. The incidence of depression, depressive symptoms, the risk of developing depression, and model variables in which the intervention was based were assessed by independent evaluators at the pre-intervention, post-intervention, and 1- and 3-months of follow-up. The incidence of depression at 3-month follow-up was 4.9%. There was a significant reduction in depressive symptomatology (p < .001) and the risk of developing depression (p < .001) after the intervention and at 1- and 3-months of follow-up. The model variables improved significantly after the intervention and were associated with post-intervention depressive symptoms. Adherence and satisfaction with the intervention were high. The results encourage the performance of a randomized controlled trial.
       
  • Videoconference-delivered psychological treatment for a victim of
           terrorism with posttraumatic stress disorder and other comorbid emotional
           disorders 30 years after the terrorist attack

    • Abstract: Resumen: En este trabajo se muestra la efectividad o utilidad clínica de la terapia cognitivo-conductual centrada en el trauma (TCC-CT) cuando se aplica por videoconferencia a un paciente de 52 años víctima directa de un atentado terrorista sufrido hace 30 años, que presentaba trastorno por estrés postraumático (TEPT), trastorno depresivo mayor, fobia social y trastorno obsesivo-compulsivo. Tras 36 sesiones de tratamiento, se produjo una reducción clínicamente significativa de la sintomatología postraumática, ansiosa y depresiva, la cual se mantuvo estable a largo plazo. La TCC-CT aplicada por videoconferencia parece ser efectiva cuando se utiliza con víctimas del terrorismo que sufren TEPT y otros trastornos comórbidos a muy largo plazo, aunque la comorbilidad y la cronicidad parecen requerir un aumento en el número de sesiones. : This paper shows the effectiveness or clinical utility of trauma-focused cognitive-behavioral treatment (TF-CBT) that was delivered by videoconference in a 52 year old patient who was victim of a terrorist attack 30 years ago and suffered from posttraumatic stress disorder (PTSD) comorbid with major depressive disorder (MDD), social phobia, and obsessive-compulsive disorder (OCD). After 36 sessions, a clinically significant decrease in posttraumatic, depressive and anxious symptomatology was obtained and remained stable in the long term. TF-CBT delivered by videoconference seems to be effective when is used with victims of terrorism suffering from PTSD and comorbid emotional disorders in the very long term, although comorbidity and chronicity seem to require an increase in the number of treatment sessions.
       
  • Characteristics of alcohol use disorder screening tests in young
           university students in Argentina

    • Abstract: Resumen: Los trastornos por uso de alcohol son preocupantemente altos en la población universitaria. Contar con instrumentos de tamizaje es fundamental para la medición epidemiológica y la práctica clínica. El objetivo de este trabajo es comparar el desempeño de los instrumentos de tamizaje más usados a nivel internacional (AUDIT, AUDIT-C, TWEAK, RAPS-QF y CAGE) en estudiantes universitarios de Argentina. Los instrumentos de tamizaje, junto a la Entrevista Diagnóstica Compuesta Internacional, que permite obtener diagnósticos compatibles con el DSM 5, fueron administrados a una muestra probabilística (N = 2,430) de estudiantes de una universidad pública nacional. Entre otros, se evaluaron los puntos de corte óptimos, la sensibilidad, la especificidad, la consistencia interna y la estabilidad temporal, así como la presencia y la capacidad de discriminación de los ítems. La apreciación global de los resultados sugiere que tanto el AUDIT como el RAPS-QF serían los instrumentos preferibles para detectar la presencia de trastorno por uso de alcohol en esta población. : The prevalence of alcohol use disorders in university students is alarmingly high. Screening instruments are fundamental for both, epidemiological research and clinical practice. The aim of this article is to compare the performance of the most widely used screening instruments (AUDIT, AUDIT-C, TWEAK, RAPS-QF, and CAGE) in Argentinean university students. Screening tests, along with the Alcohol Section of the Composite International Diagnostic Interview that yields DSM 5 compatible diagnoses were administered to a probabilistic sample (N = 2,430) of university students from a national public university. The following characteristics were evaluated among others: optimal cut-off points, sensitivity, specificity, internal consistency, temporal stability, and item presence and discrimination capacity. The global appraisal of results suggests that the AUDIT and RAPS-QF would be preferable for evaluating AUD in this population.
       
  • Prevalent Tendencies for Mental Disorders in Pakistan

    • Abstract: : Psychological problems and mental disorders are visibly prevalent all around the globe. The current survey intended to analyze the latest situation of the prevailing variety of mental disorders in the country by involving 3,500 participants from 5 major cities. The sample of the study was sufficiently rich to represent Pakistanis based on gender, age, education, profession, and income. Data was gathered through interviews and a self-reported questionnaire. The study revealed that 27% of Pakistanis possess tendencies for different mental disorders. Depression and sleep-related problems are the most prevalent psychological problems in the country. Females, unmarried, people below 40 years of age, people with primary level of education, and people belonging to the upper-middle class possess significantly higher tendencies towards mental disorders as compared with their counterparts. The findings will be helpful for researchers, mental health practitioners, and other stakeholders in assessing the state of mental health in the country.Resumen: Los problemas psicológicos y los trastornos mentales son visiblemente prevalentes en todo el mundo. El estudio actual pretende analizar la situación más reciente de la variedad predominante de los trastornos mentales en el país con la participación de 3,500 participantes de 5 grandes ciudades. La muestra del estudio fue suficientemente rica para representar a los paquistaníes en función del sexo, la edad, la educación, la profesión y la renta. La información se obtuvo a través de entrevistas y un cuestionario autoinformado. El estudio reveló que el 27% de los paquistaníes tienen tendencia a padecer diferentes trastornos mentales, siendo la depresión y los problemas relacionados con el sueño los problemas psicológicos más prevalentes en el país. Las mujeres, solteras, las personas menores de 40 años de edad, las personas con estudios primarios y las personas que pertenecen a la clase media-alta tienen una tendencia significativamente más elevada a sufrir trastornos mentales en comparación con sus homólogos. Los resultados serán útiles para los investigadores, los profesionales de salud mental y otras partes interesadas en la evaluación del estado de la salud mental en el país.
       
  • Functional Analytic Psychotherapy among Mothers with Children with
           Disruptive Behavior

    • Abstract: : Children’s mental problems remain one of the central topics in governments’ agendas because of their negative impact at social and economic levels. The present paper is a single case study, with A-B design with concurrent control through behaviors and participants, aimed to identify the effect of the Functional Analytic Psychotherapy (FAP) on four mothers who informed about their children’s disruptive behaviors in the school setting. A case-control who received psychoeducation-based intervention was included in order to observe outcome differences among participants regarding the implemented treatment. Using the FIAT-Q and functional analysis, participants’ problematic behaviors were identified. Mothers treated with FAP were expected to display behavioral changes within the sessions and generalized them to the natural context in the interaction with their children in order to promote children’s prosocial behaviors. Results indicated that FAP reduced the frequency of mothers’ problematic behaviors and, in turn, children reduced disruptive behaviors. Implications of the study results are discussed both at theoretical and applied levels.Resumen: Los problemas psicológicos en niños constituyen una de las temáticas centrales en las agendas gubernamentales debido al impacto negativo que representan a nivel social y económico en la población. Mediante un diseño de caso único A-B con control concurrente, a través de conductas y participantes, la presente investigación tuvo como objetivo identificar el efecto de la psicoterapia analítica funcional en un grupo de cuatro madres cuyos hijos tenían comportamientos problemáticos. Se incluyó un caso control que recibió entrenamiento en el manejo de contingencias con el fin de observar diferencias en los resultados de acuerdo con el tipo de tratamiento. Por medio del análisis funcional y del FIAT-Q se identificaron las conductas clínicamente relevantes de las cinco participantes a ser modificadas durante la intervención para generalizar los cambios a la interacción con los hijos. Los resultados indican que las madres que recibieron tratamiento basado en FAP presentaron una disminución de los comportamientos problemáticos y, a su vez, sus hijos redujeron los comportamientos problemáticos en el contexto escolar. Las implicaciones de los resultados del estudio se discuten a nivel teórico y aplicado.
