  • Cognitive fusion in personality disorders: a contribution to research on
           mechanisms of change

    • Abstract: RESUMEN Diversos abordajes terapéuticos para personas con trastorno de personalidad (TP) postulan diferentes mecanismos de cambio. Investigamos si el constructo fusión cognitiva (FC) es relevante en el cambio terapéutico detectado en un grupo de personas diagnosticadas de TP grave (N = 110) tras 6 meses de intervención hospitalaria. Su nivel de FC es superior al de otras muestras publicadas (M = 38.5, DT = 8.98) y se asocia a mayor patología. Está relacionado con otros constructos como pensamientos automáticos (r = .529; p < .01) y actitudes disfuncionales (r = .368, p < .01). La FC se reduce tras el tratamiento (t = 4.897, p = .000, d = 0.65), pero no se confirma el supuesto del efecto obstaculizador de la FC sobre el cambio sintomático en la depresión, la perturbación global o la gravedad del TP. Se discuten los hallazgos en el contexto del solapamiento de variables como posible explicación de la constatación de que diferentes terapias produzcan beneficios similares y como acicate para seguir construyendo una práctica terapéutica basada en la evidencia.ABSTRACT Different therapeutic approaches for people with personality disorder (PD) hypothesize different mechanisms of change. We studied whether the cognitive fusion (FC) construct is relevant in the therapeutic change identified in a group of people diagnosed with severe PD (N = 110) after 6 months of intervention. The FC level is higher than that of other published samples (M = 38.5, SD = 8.98) and is associated with a more serious pathology. FC is related to other constructs such as automatic thoughts (r = .529, p < .01) and dysfunctional attitudes (r = .368, p < .01). The FC level is reduced after treatment (t = 4.897, p = .000, d = 0.65), but the hypothesized hindering effect of FC on the symptomatic change in depression, global disturbance, or severity of TP is not supported. The findings are discussed in the context of the overlapping of variables as a possible explanation for the observation that different therapies produce similar benefits and as an incentive to continue building an evidence-based therapeutic practice.
       
  • Contribution of neuroticism, pathological personality traits comorbidity,
           and coping to the prediction of clinical evolution: a five-year follow-up
           study of a sample of patients with adjustment disorder

    • Abstract: RESUMEN Numerosos estudios han demostrado que tanto la personalidad como las estrategias de afrontamiento utilizadas para manejar el estrés tienen un papel importante en el desarrollo de problemas de salud mental, entre los que se encuentra el trastorno adaptativo (TA). El objetivo de este estudio fue, en primer lugar, analizar las diferencias entre neuroticismo, comorbilidad y afrontamiento en un grupo de pacientes diagnosticados de TA con evolución clínica favorable (n = 58) y otro grupo con evolución desfavorable (n = 20); en segundo lugar, se comprobó si estas variables permitían predecir la evolución clínica a los 5 años. El grupo con evolución clínica desfavorable presentó un mayor neuroticismo, más presencia de rasgos comórbidos de personalidad patológica y un afrontamiento más desadaptativo. Además, los principales factores del riesgo de pertenecer al grupo con peor evolución fueron mayor neuroticismo, presencia de comorbilidad y uso del afrontamiento de negación, mientras que las estrategias de planificación y de apoyo instrumental redujeron dicho riesgo.ABSTRACT Numerous studies have found that both personality and the coping strategies used to manage stress have an important role in the development of mental health problems, particularly adjustment disorder (AD). The aim of this study was, firstly, to analyze differences in neuroticism, comorbidity and coping between a group of patients with AD with a favorable clinical course (n = 58) and another with unfavorable evolution (n = 20); and secondly, to analyze whether these variables could predict clinical evolution 5 years later. The group with unfavorable clinical evolution showed a greater neuroticism, more presence of comorbid personality pathological traits, and a greater use of maladaptive coping strategies. Furthermore, the main risk factors to belong to the group with unfavorable evolution were greater neuroticism, presence of comorbidity and the use of denial coping, whereas the use of planning and instrumental support strategies lowered this risk.
       
  • An extension of the concept of rule and its application to therapy

    • Abstract: Resumen: Proporcionar y seguir reglas se ha considerado un factor importante en la eficacia de los tratamientos psicológicos; sin embargo, hace falta una clarificación conceptual de lo que se entiende por emitir o seguir una regla y su aplicación en el ámbito clínico. Partiendo de una revisión sobre el concepto original de conducta gobernada por reglas, nuestro propósito fue extender la definición de esta conducta orientada al proceso terapéutico. Esta definición permite su identificación y estudio momento a momento a lo largo del proceso. La extensión de esta definición puede ayudarnos a identificar el papel funcional de las reglas en el cambio terapéutico y las variables involucradas que contribuyen al entendimiento de fenómenos clínicos como la generalización. Los hallazgos que se desprendan del estudio de las reglas desde esta perspectiva podrían contribuir a aumentar la información sobre el proceso de cambio terapéutico y la mejora del ejercicio clínico. : Providing and following rules has been considered an important factor in the effectiveness of psychological treatments; however, a conceptual clarification is needed of what is meant by issuing or following a rule and its application in the clinical setting. Based on a review of the original concept of rule-governed behavior, our purpose was to extend the definition of this behavior oriented to the clinical process. This definition allows its identification and study moment by moment throughout the therapeutic process. The extension of this definition can help us to identify the functional role of rules in clinical change and the variables involved that contribute to the understanding of clinical phenomena such as generalization. The findings that emerge from the study of rules from this perspective could increase the information on the therapeutic change process and, therefore, contribute to the improvement of clinical practice.
       
  • Psychological analysis of techniques used in the social phobia disorder in
           a psychology health center

    • Abstract: RESUMEN El objetivo del trabajo es analizar el tratamiento psicológico en la fobia social, identificando niveles de efectividad, técnicas de intervención utilizadas, duración y costes del tratamiento. La muestra consta de 51 pacientes, de edad promedio 30.14 (DT = 8.309), mayoritariamente mujeres (60.8%), solteras (78.4%) y estudiantes (64.7%). Se realizaron análisis descriptivos para las características sociodemográficas y el coste del tratamiento. Se subdividió la muestra en dos grupos (pacientes de alta y pacientes que abandonan), analizándose diferencias entre ellos. Se efectuaron técnicas de análisis de clústers y multivariados para conocer qué técnicas predecían éxito terapéutico. Se alcanzó un 61% de éxito, con una media de 20 sesiones y 1,200€ de coste del tratamiento. Las técnicas predictivas del éxito son la exposición y el uso de otras técnicas (p < .05). Se defiende el uso de protocolos para la fobia social que incluyan las técnicas mencionadas en la clínica cotidiana.ABSTRACT The purpose of this study is to analyze psychological treatment in social phobia, identifying levels of effectiveness, treatment length, treatment cost and techniques used. The sample consists of 51 patients, average age 30.14 (SD = 8.309), mainly women (60.8%), single (78.4%), and students (64.7%). Descriptive analyses for sociodemographic characteristics and therapy cost were performed; besides, the sample was divided into two groups (patients who achieved discharge and patients who dropped-out), and differences between the two groups were made. Finally, cluster analysis techniques were performed and multivariate analyses were conducted to know what techniques predicted therapeutic success. Of the patients, 61% are discharged, with an average duration of 20 sessions and a treatment cost of €1,200. The techniques which predict success are exposure and use of other techniques (p < .05). The use of protocols for social phobia which include the aforementioned techniques is suggested.
       
  • The Stockholm syndrome: a systematic review

    • Abstract: RESUMEN El síndrome de Estocolmo es un término utilizado para describir una experiencia psicológica paradójica en la cual se desarrolla un vínculo afectivo entre los rehenes y sus captores. Sin embargo, no existe actualmente una unificación de criterios con respecto a su diagnóstico y características e incluso a su denominación de “síndrome”. El objetivo del presente estudio fue revisar y sintetizar la literatura internacional existente sobre el síndrome de Estocolmo. Después de analizar los 23 artículos seleccionados a través de la búsqueda en PUBMED, Google Académico, Academic Onefile y EBSCO, se podría considerar que la extensión del término a diferentes casos o grupos es un indicador importante de que se trata de una respuesta instintiva universal de supervivencia, aunque la falta de estudios empíricos podría llevar a la conclusión de que muchas de las características del término se deben a un sesgo informativo.ABSTRACT The Stockholm syndrome is a term used to describe a paradoxical psychological experience in which an affective bond between hostages and their captors is developed. However, currently there is no unification of criteria with respect to its diagnosis and characteristics and even its denomination of “syndrome”. The objective of the present study was to review and summarize the existing international literature of the Stockholm syndrome. Databases (PUBMED, Google Scholar, Academic Onefile, and EBSCO) were systematically searched. After analyzing the 23 articles selected, it could be considered that the expansion of the term to different cases or groups is an important indicator that it is a universal instinctive response of survival, although the lack of empirical studies could lead to the conclusion that many of the characteristics of the term are due to an information bias.
       
  • Informal caregivers for dependent elderly people: a revision of
           psychological interventions over the last ten years

    • Abstract: RESUMEN En las últimas décadas se han realizado diversas revisiones sistemáticas para conocer la eficacia de las intervenciones para cuidadores informales de personas mayores, centrándose mayoritariamente en revisar aspectos específicos (e.g., cuidadores de personas mayores con demencia). Por ello en este trabajo realizamos una revisión actualizada de intervenciones con cuidadores de mayores dependientes en general. Las intervenciones que se incluyen en este trabajo (33) han sido obtenidas de Psicodoc, Psyke, PsycArticles, PsycInfo y Medline. La mayoría de las intervenciones que se aplican son cognitivo-conductuales y obtienen resultados significativos. Principalmente trabajan variables emocionales y carga. El formato de aplicación de las intervenciones es predominantemente grupal, con una eficacia equiparable a las intervenciones individuales. Conviene seguir mejorando las limitaciones que presentan estas intervenciones, entre las que cabe destacar la necesidad de incorporar seguimientos que permitan conocer la estabilidad de los resultados tras la intervención.ABSTRACT In recent decades, several systematic reviews have been carried out in order to know the efficacy of interventions for informal caregivers of dependent elderly, mainly focused on reviewing specific features (e.g., caregivers of elderly people with dementia). In this study, we conducted an updated review of interventions for informal caregivers of dependent elderly in general, using methodological restrictive inclusion criteria. Interventions included in this work (33) have been obtained from Psicodoc, Psyke, PsycArticles, PsycInfo, and Medline. The majority of interventions are cognitivebehavioral ones and achieve significant results. They mainly address emotional variables and burden. Interventions are predominantly applied in a group format and their efficacy is comparable to individual interventions. It is advisable to overcome the limitations of these interventions, especially including follow-up periods to know the stability of results after the intervention.

    • Abstract: RESUMEN Se describe el caso clínico de una mujer de 53 años con trastorno de estrés postraumático (TEPT), a raíz de un accidente ferroviario en Galicia (España) en julio 2013. El accidente le supuso diferentes lesiones e intervenciones quirúrgicas. El impacto a nivel psicológico aparece de forma demorada, presentando, además de sintomatología TEPT, alta ansiedad, síntomas depresivos y alta irritabilidad. En el tratamiento se aplicó “exposición prolongada” enmarcada en un tratamiento cognitivo-conductual más amplio, en el que se incluyó reestructuración cognitiva y entrenamiento en regulación emocional, para abordar las interpretaciones disfuncionales que favorecían la aparición de la ira. Se describe la aplicación del tratamiento y las principales dificultades encontradas, principalmente la evitación de la exposición al recuerdo del trauma y la solicitud de la paciente del cese anticipado del tratamiento. Se presentan los resultados, incluyendo los seguimientos a 1, 3, 6 y 12 meses, y se discute la evolución de la paciente.ABSTRACT This paper presents the case of a 53-year old woman with posttraumatic stress disorder (PTSD) after a train accident in Galicia (Spain) in July, 2013. The accident caused the patient several injuries and surgical interventions. The psychological impact is delayed and consists of PTSD symptoms, besides high anxiety, depression symptoms, and high anger. The treatment involved Prolonged Exposure into a broader cognitive-behavior therapy that included cognitive restructuring and training in emotional regulation to treat dysfunctional cognitive responses that promoted anger. The article describes the implementation of the treatment and the main difficulties encountered, namely, the avoidance of exposure to trauma memories and the premature end of the treatment at patient's request. We present the results of the treatment, including 1, 3, 6, and 12 months follow-ups and discuss patient's progression.
