  • Parental Choices of Flotation Devices for Children and Teen Swimmers and
           Waders: A Survey at Beaches in Washington State

      Abstract: Drowning at open-water areas is the second leading cause of unintentional death among Washington State children. Providing written and visual safety instructions and loaned life jackets at swim sites (“loaner boards”) may reduce the risk of drowning. This study sought to understand parental perceptions regarding loaner boards and behaviors related to bringing flotation devices for children to use while swimming. Of the 102 surveys administered to parents on the beach at 10 different open-water sites, 29 were collected from sites with loaner boards. Parents provided information about ages of family members present, flotation devices brought and perceptions surrounding loaner boards. The majority (85%) of families with young children (under six years old), brought some type of flotation device and 59% of them brought life jackets compared to 57% and 8% of families with only children six or older, respectively. Results provide evidence that parents of children younger than 6 years old more often plan for children to use life jackets while swimming; this may partially explain low rates of life jacket use among older children. Findings can guide efforts to increase the efficacy of loaner board programs and develop strategies to promote parental encouragement of life jacket use for older children.

  • Head Depth and Head Speed During Competitive Backstroke Ledge Starts

      Abstract: Recently, a commercially available starting ‘ledge’ designed to reduce foot slippage during the execution of the backstroke start was introduced in competitive swimming. For the purpose of identifying potential safety consequences, the present study investigated the effect of ledge use on head depths, speeds, and distances in backstroke starts of athletes with no prior or only novice familiarity of the ledge. Competitive backstroke starts were performed with and without ledges by high school-aged (14.5 to 19.2 yr, N = 61) swimmers in 1.52 m of water during a closed testing session. A SIMI Reality Motion System in a calibrated space using three cameras was employed for filming starts. Dependent measures were initial head height (Yset), distance from wall at entry (Xentry), entry angle (Angleentry), horizontal velocity at head entry (XVelentry), resultant velocity at entry (ResVelentry), maximum depth of the center of the head (Ymhd), resultant velocity at maximum head depth (ResVelmhd), and distance from the wall at maximum head depth (Xmhd).   • Cardiorespiratory Responses to Shallow Water Exercise: A Sex Comparison

      Abstract: This investigation examined physiological responses to shallow water exercise (SWE) and to a high-intensity interval SWE workout (HIISWE) in males (M, n=9) and females (F, n=9). Participants performed 5 X 5 min. SWE bouts (bts.) at ratings of perceived exertion (RPE) 9, 11, 13, 15 and 17 (Borg scale) and a maximal bout of SWE with metabolic, heart rate (HR), and blood lactate (BLa) responses monitored. The same measurements were performed during HIISWE (4 X 4-min bts., alternating 20-s “all-out” and 10-s rest).   • Can You Float' Part 2 - Perceptions and Practice of Lifejacket Use
           Among Young Adults

      Abstract: Personal flotation devices (PFDs), commonly referred to as lifejackets, have been identified as an extremely effective form of drowning prevention and was identified as a critical distinct water competency by Stallman and colleagues (2017). In this second phase of the Can You Float' study, perceptions and practice of a range of lifejacket tasks among students (N = 40) with known water proficiency were examined. Participants estimated exertion levels before and after practical testing of six simulated survival tasks when wearing lifejackets. All participants completed a 25m sprint swim, 5-minute endurance swim, 5-minute float, and 25m partner assist but many failed to complete a 15m underwater swim (63%) and deep water exit (63%). Students underestimated the level of exertion required to complete the underwater swim and deep water exit. Reasons for, and implications of, this underestimation are discussed and recommendations for the teaching of lifejacket competency in water safety programs are made.

  • An Investigation of Youth Swimming Skills and Method of Instruction

      Abstract: Drowning is a leading cause of death for US children. Teaching youth to swim in a formal setting from certified instructors is a consistent drowning prevention recommendation. Purposes for this investigation was to examine type of swimming instruction and ability to swim and compare to attitudes toward swimming among US youth. Methods were similar to previous USA Swimming studies in 2008 and 2010. YMCA associations in five cities were used to recruit adolescent survey respondents (n=600) aged 12-18 years. Results showed African American youth had the lowest rate of formal swimming instruction (29%) compared to White (32%) and Hispanic (42%) peers. Free/reduced lunch qualifiers reported a 23% formal instruction rate as compared to 43% of non-qualifiers. Formally instructed youth were 2.35 times more likely to report being a skilled swimmer (86%) compared to informally instructed youth (72%). Formal swimming instruction is recommended, and interventions need to target underserved populations.

  • Unintentional Bathtub Drowning Deaths Among Those Aged 65 Years and Older
           in Australia

      Abstract: When compared with other age groups, fatal drowning rates among seniors have stayed static. This study identifies causal factors in unintentional bathtub drowning deaths among people aged 65 years and over. This study is a 10-year (2003-2012) total population retrospective survey of all unintentional bathtub (baths, spa baths and showers) drowning deaths afflicting people aged 65 years and over. Data were sourced from the Australian National Coronial Information System. Risk factors and circumstantial variables were analysed including sex, age, activity prior to drowning, alcohol, drugs, pre-existing medical conditions, living circumstances, time until found, and performance of cardio-pulmonary resuscitation (CPR). There were 32 fatalities (20 females, 12 males); 41% drowned after a fall into the bath. High blood alcohol (≥0.05%) was disproportionately represented. Twenty-six people (81%) had a pre-existing medical condition, deemed contributory in 19 cases including cardiac (n=9) and sarcopenia or frailty (n=5). Of those with medical conditions, 69% had blood levels of prescribed drugs, commonly analgesics (n=10). Seven cases recorded both drugs and alcohol. In half of all fatalities (50%), the person resided alone. In ten cases (31%) the person was not found for one or more days. Bathtub drowning afflicting the elderly poses an unmet challenge. This study has identified five areas for targeted prevention: Pre-existing medical conditions, alcohol, falls in the bath, review of medications; and if practical, advising family members of bathing. Increased awareness of drowning among this age group (and carers) is required as the aged population increases.

      Abstract: The original question addressed in the scientific review was What is the most effective piece of rescue equipment for a lay responder to throw to a person in trouble in water' and the modified question was rephrased as What are the most effective types of aquatic rescue equipment for a layperson/bystander to use to rescue a drowning person' To thoroughly analyze the main question, the following sub-questions were examined: What are the most accurate and buoyant types of devices' What specific equipment is best for quick rescues' What specific equipment can be grasped most easily by drowning victims' This research topic is a valid area of inquiry because drowning is a serious public health issue which disproportionately impacts diverse populations (e.g., young children (1-4 years of age; adolescents and young adults (ages 15-24 years); minorities (African-Americans, Hispanics/Latinos) (CDC, 2017). Participation in unsupervised recreational aquatic environments remains popular and fairly constant; the need exists to identify the types of aquatic rescue equipment for layperson/bystanders to perform effective rescues. Answering the main question of this review has implications for identifying appropriate pedagogies and educational practices for water safety and learn to swim programs which will be addressed in separate reviews.

  • An examination of the severity of aquatic incidents

      Abstract: Lanagan-Leitzel (2012) found that lifeguards do not consistently report incidents when free-viewing aquatic scenes and miss some incidents that should be considered critical. This could have been because they did not know what incidents were critical to monitor or because they were busy monitoring other incidents. In the current study, lifeguards and non-lifeguards were presented with video clips of isolated incidents and rated the severity of each on a scale of 0 – 7. The lifeguards reported greater mean and maximum incident severity than non-lifeguards. Further analyses of lifeguard responses revealed that severity ratings were only moderately correlated to the report rate in Lanagan-Leitzel (2012). Some of the incidents, though under-reported in Lanagan-Leitzel (2012), were given high severity ratings when isolated in the current study. It is proposed that lack of report in Lanagan-Leitzel (2012) may have occurred due to attention being diverted to other critical incidents. Future research should utilize eye-tracking to assess the relationship between severity and monitoring.

  • Examining the Perceived Impacts of Recreational Swimming Lessons for
           Children with Autism Spectrum Disorder

      Abstract: The purpose of this study was to examine the perceived impacts of recreational swimming lesson participation for children with Autism Spectrum Disorder (ASD). Although swimming lessons are a suitable form of physical activity for children with ASD, minimal research has examined the impacts of these lessons. The author conducted semi-structured interviews with an Applied Behaviour Analysis (ABA) certified therapist and a swim instructor, each with experience working with children with ASD in swimming lessons. The participants suggested that swimming lessons encouraged children with ASD to socialise. Both participants agreed that distractions in swimming lessons and barriers in communication created challenges for developing swimming skills. Finally, the participants described techniques they found appropriate for teaching children with ASD. These results aim to provide insights into the perceived impacts of recreational swimming lessons and appropriate techniques for lessons. Hopefully these insights may inspire parents/guardians of children with ASD to include swimming lessons into the routines of their children while also considering the safety risks of aquatic environments.

  • Can You Float' Part I - Perceptions and Practice of Unsupported
           Flotation Competency among Young Adults

      Abstract: The capacity to maintain the airway through buoyancy control when immersed in water is critical in preventing drowning (Stallman, Moran, Quan & Langendorfer, 2017). In this first phase of the Can you Float' study, perceived and real unsupported flotation competency of a group of students (n=37) with known water proficiency was examined. Using a modified version of Borgs Rating of Perceived exertion (RPE), participants were asked to estimate exertion levels before and after a practical test of five stationary floating tasks of increasing difficulty ranging from treading water to motionless floating. Most participants (82%) were able to tread water for 2 minutes but only one third (31%) could perform a motionless float for the same duration. In all tasks students underestimated the level of exertion required. Reasons for, and implications of, this underestimation are discussed and recommendations for the teaching of unsupported flotation competency in water safety programs are made.

  • Risk Factors for Boating Incidents in Inuvik, Northwest Territories,
           Canada

      Abstract: Injury prevention programs that focus on boating and water safety in the Northwest Territories (NWT) have existed for decades; however, rates of boating incidents are much higher in the NWT than southern Canada. To better understand this health disparity, we engaged in community-based participatory research informed by postcolonial feminist theory to examine Aboriginal men’s understandings of the risk factors that contribute to boating incidents in Inuvik, NWT. Participants identified four main risk factors for boating incidents in Inuvik: 1) Gender, 2) age, 3) place, and 4) lack of boating safety education. As a result of these findings and the ways in which they are strongly related to culture, we argue that local community-based approaches should be employed to design and implement boating safety strategies in communities in the NWT.

  • Revised Scientific Review: Minimum Age for Swim Lessons

      Abstract: The question addressed in the original scientific review (2009) was rephrased as Does sufficient scientific evidence exist to support setting a minimum age for swimming lessons' for this triennial review. Addressing the research question is significant because strong differences of opinion about the appropriate age(s) for initiating learn-to-swim lessons have existed among the pediatric medical profession, aquatic professionals and agencies, and proprietary programs and swim schools for over four decades. The question has implications for the appropriate purposes, pedagogies, and outcomes associated with swimming experiences for infants and young children that will be addressed in a subsequent review. The revised statement now supports as a guideline that children older than 1 year of age can benefit from swim lessons with a lower risk of drowning; parents should make their decision to enroll based upon individual child considerations as proposed in this review.

  • Drowning Prevention: Define and Then Gather Evidence or Gather Evidence to

      Abstract: According to the World Health Organization, drowning fatalities are a neglected public health issue ranked as the third most frequent cause of unintentional death by injury. Data used to record drowning incidents are acknowledged as inaccurate. Recommendations on drowning prevention based on these data are currently not informed by accurate, detailed and reliable evidence. Two modifications to current practice are proposed: 1) an Aquatic Incident Report needs to be developed as an extension of the current workplace incident report to prompt for information commonly not included in current drowning reports; and 2) National Water Safety Plans should be modified to include a learn-to-swim program database registering the name/elements of programs taught. Worldwide implementation of these modifications is the goal. Using improved information databases will enable researchers to better identify patterns in the drowning data. Improved current data collection methods will lead to more informed recommendations on drowning prevention.
