      Abstract: The eccentric contraction has been proven to improve Glut-1 expression in muscle and fasting glucose levels in mice. The same eccentric contraction occurs in human being when walking or running while wearing high heels. Innovation on the eccentric contraction with high heels can be done by using a specific shoe insole. The design of the rear insole is made thicker than the front. The purpose of this study was to determine the effects of insole use specified for eccentric activities on daily work shoes on the skeletal muscle damage after wearing the specified insole. Skeletal muscle damage was measured based on sTnI level. This was experimental study with pretest-posttest design. The subjects involved were non-diabetic woman workers in Faculty of Medicine of Universitas Airlangga, and were divided into two groups. Each group represented group members who wore 5-degree and 10-degree insoles respectively. The angle used for the shoe insoles were 5° and 10° respectively. These insoles were worn on weekdays for one week. Blood was taken one day after the subjects stop wearing the insoles. Data results showed that the levels of fasting blood glucose (FBG) of subjects who wore the 10-degree insole had a value of p<0.05. The use of insole lead to decreased levels of fasting blood glucose (FBG). This study found that the sTnI levels in the 5-degree and 10-degree insole groups had no significant effect on troponin I skeletal muscle levels before and after treatment.

      Abstract: Red dragon fruit skin extract is used as an antioxidant to lead acetate toxicity. This study aimed to prove the difference in morphology of mice testicular spermatozoa exposed to lead acetate. Twenty-five mice were divided into 5 groups (5 mice per group). K- group was the control group without lead acetate and dragon fruit skin extract administration, K + group was treated with 8 mg/KgBW lead acetate administration for 14 days, P1 group was treated with the administration of 8 mg/KgBW lead acetate for 14 days + 250 mg/kgBW red dragon fruit skin extract for 21 days, P2 group was treated with the administration of 8 mg/KgBW lead acetate for 14 days + 500 mg/kgBW red dragon fruit skin extract for 21 days, and group P3 was a treatment group with the administration of 8 mg/KgBW lead acetate for 14 days + 1000 mg/kgBW red dragon fruit extract for 21 days. The results showed that there was significant difference (P<0.05) in normal sperm morphology among K-, K +, P1, P2, and P3. In conclusion, the administration of red dragon skin extract at a dose of 500 mg/kgBW (P2) is the optimal one which can be used as therapy to increase motility, morphology, and concentration of mice spermatozoa exposed to lead acetate.

      Abstract: Premature labor is a cause of high rates of perinatal morbidity and mortality. The use of tocolytics is one of the efforts to handle the risk of preterm labor. Tocolytics which are widely used in Indonesia is isoxsuprine and nifedipine. The purpose of this study was to identify the difference of effectiveness and safety of isoxsuprine as tocolytics in the risk of preterm labor. This was an observational study in the form of a case study that was done in detail and depth to the patients who were diagnosed as imminent preterm labor. In conclusion, there were differences in the effectiveness and safety of isoxsuprine and nifedipine as tocolytics in the risk of preterm labor.

      Abstract: Urothelial Cell Carcinoma (UCC), also called transitional cell carcinoma of the bladder, is the most common malignancy in urinary tract. The prognosis of this disease is highly dependent on the histological grading at diagnosis. Fas/CD95 has a role in apoptotic process, whereas HSP70 has an antiapoptotic role. This study aimed to analyze the expression of Fas/CD95 and HSP70 in low grade and high grade urothelial cell carcinoma in the bladder. This was an analytic observational study with cross sectional approach. Population and sample were paraffin blocks of urothelial cell carcinoma in the Laboratory of Anatomic Pathology, Dr. Soetomo Hospital, Surabaya in the period of January 2011-December 2016. Each grading of urothelial cell carcinoma was randomly sampled. Immunohistochemystry with Fas/CD95 and HSP70 were performed. Expression of Fas/CD95 and HSP70 were assessed semiquantitatively. Expression of Fas/CD95 and HSP70  were analyzed using Mann-Whitney test and Spearman test. The results showed there was significant different in expression of Fas/CD95 and HSP70 in low and high grades in urothelial cell carcinoma. There was no significant correlation between the expression of Fas/CD95 and HSP70 in urothelial cell carcinoma.  As a conclusion, the role of CD95 and HSP70 expression can be useful as marker for the diagnosis, especially in the determination of the grade of differentiation.

      Abstract: Intra Uterine Fetal Death (IUFD) is one contributor to infant mortality. Human Leukocyte Antigen-E (HLA-E) and Natural Killer Cells (NK cells) are believed to play an important role towards IUFD associated with immune maladaptation. This study aimed to determine the expression of HLA-E and NK cells on trophoblast on IUFD and normal pregnancy. The study used cross sectional approach undertaken at Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, networking hospitals, and Anatomic Pathology Laboratory of Faculty of Medicine, Sebelas Maret University. The number of samples were 32 subjects consisting of 16 subjects with normal pregnancy and 16 subjects with IUFD. The expression of HLA-E and NK cells on trophoblast of subjects who met the inclusion and exclusion criteria were examined using immunohistochemistry method and t-test statistical analysis. The mean value of HLA-E expression in the trophoblast of IUFD group was 17.30±6.69, in normal pregnancy was 57.06±32.04, with p=0.00 (p<0.05). The mean value of NK cell expression in trophoblast in IUFD group was 78.62±36.43, in normal pregnancy was 19.87±6.43, with p=0.00 (p<0.05). This study concluded that the expression of HLA-E was lower and NK cells wass higher in IUFD compared to those in normal pregnancy.

      Abstract: HPV infection is known to cause cervical cancer. This study aimed to identify the variant of HPV genotypes of cervical precancerous lesions from low grade squamous intraepithelial lesion  (LSIL) and high grade squamous intraepithelial lesion (HSIL). This was an explorative study using formalin fix paraffin embedded (FFPE) from cervical precancerous lesions at Dr. Soetomo Hospital, Surabaya. DNA was extracted from FFPE and hybridized for HPV genotyping using Ampliquality HPV Type Express kit (AB ANALITICA) by reverse line blot techniques. The results showed that there were variants of HPV genotype in LSIL. The variants were HPV16 (8/15), HPV18 (3/15), HPV52 (1/15), HPV6+31 (1/15), HPV6+18 (1/15), and HPV72+68 (1/15), and in HSIL which were HPV16 (4/10), HPV18 (2/10), HPV59 (1/10), HPV6+45 (1/10), HPV61+26 (1/10), and HPV16+31 (1/10). The characteristics of infection in LSIL were single infection of high-risk (hr) HPV and multiple infection of  low-risk (lr)+hr HPV, and in HSIL were single infection of HPVhr, multiple infection of HPVhr+hr and HPVlr+hr. In conclusion, HPV prevalence in cervical precancerous lesions is single infection by HPV16 (48%), HPV18 (20%), HPV52 (4%), HPV59 (4%), and multiple infection by HPV6+31, HPV6+18, HPV6+45, HPV16+31, HPV61+26, HPV72+68 is 4%.

           2-METHOXYETHANOL (2-ME)

      Abstract: The compound of 2-methoxyethanol (2-ME) is one of toxic materials that potentially damage male reproductive organs. This compound can enter the body by various means of direct contact through the skin, respiratory and digestive tract. This compound can cause oxidative stress in spermatozoa which is the main cause of spermatozoa dysfunction. Lycopene, also known as red pigment, is an antioxidant class of carotene. Lycopene has antioxidant activity  twice stronger than beta carotene and ten times stronger than vitamin E. Thus, the lycopene reaction as an antioxidant in the body is better than vitamin A, C, E, and other minerals. This study was conducted to determine the effects of lycopene on spermatozoa morphology in mice exposed to 2-ME. The study was conducted on 30 mice, divided into 5 groups. The K-group was the control group without 2-methoxyethanol and lycopene, the K + group was the group with 200 mg/kg BW 2-methoxyethanol on days 1-5, and the P1, P2 and P3 groups were the ones with 200 mg/kg BW 2-methoxyethanol on days 1-5 and lycopene doses of 5, 10 and 20 mg/kg BW on days 6-35. 2-ME was given intraperito-neally and lycopene was given per sonde. On day 36, the mice were sacrificed and the epididymis and vas deferens were removed for morphological examination of spermatozoa. The observations were performed using a microscope with 1000x magnification. The results showed that there was significant difference. In the morphology of spermatozoa, the administration of lycopene increases normal morphological percentage of spermatozoa in mice exposed to 2-ME.

      Abstract: Methyldopa is the main antihypertensive drug widely used in pregnant women with hypertensive disorder. It lowers blood pressure in pre-eclampsia by affecting a2-adrenoreceptors in central nervous system. However, it also decreases the production of proangio-genic factors that involved in the pathophysiology of hypertension in pre-eclampsia. Vascular Endothelial Growth Factor (VEGF) is one of proangiogenic and mitogenic factor that important for vasodilatation. VEGF is produced by the placenta and affected after treatment with methyldopa. The aim of this study was to analyze the effects of methyldopa on VEGF maternal circulating level as a proangiogenic factor in severe pre-eclampsia patients who were hospitalized at Obstetrics and Gynecology Department, Haji Hospital, Surabaya. This study was performed by cohort prospective observational method on August to October 2016.The data was assessed at before and 48 hours after methyldopa therapy. The study was approved by the ethical committee of Haji Hospital, Surabaya. There were 19 patients with severe pre-eclampsia who met inclusive criteria. The results showed that levels of VEGF before and 48 hours after 250 mg methyldopa therapy were 1178.37(281.97-3567.28) pg/mL and 1055.17 (129.79-4272.66) pg/mL, respectively. VEGF levels in severe pre-eclampsia patients were 1194.29 (175.68-3432.01) pg/mL at before treatment and 510.66 (214.34-1236.16) pg/mL after treatment with methyldopa 500 mg therapy. In conclusion, methyldopa could decrease VEGF level on severe pre-eclampsia patients, with a decrease of 10% at the dose of 250 mg and 57% at the dose of 500 mg.

      Abstract: Acne vulgaris (AV) is an inflammation of pilosebaceous unit especially in young adult. The pathophysiology is the elevation of sebum production, keratinization of abnormal pilocebaseous follicles, and inflammation caused by immune response to Propionibacterium acnes. Therapy combination of oral antibiotics (doxycycline) and physical therapy (blue light) in moderate-severe acne is one option to reduce antibiotic resistance. Doxycycline is a commonly antibiotic used. The effects of photosensitive can increase the penetration of blue light by sebaceous glands. There was a total decrease in sebum and clinical improvement of combination therapy of blue light and doxycycline in seven patients. The combination therapy has been shown to improve its therapeutic effect, but more clinical trials are needed to prove the effectiveness of blue light with doxycycline than without blue light.

      Abstract: EGFR-TKI is the first-line therapy for EGFR-mutant patients. Nevertheless, patients will have disease progression (median PFS 10 – 12 months) due to resistance. The treatment options are still limited in developing countries for such cases, thus double-platinum chemotherapy is the next option. Although IMPRESS study reported no difference in terms of PFS and OS between double-platinum alone and double-platinum plus EGFR-TKI, several local studies reported benefit of continuing EGFR-TKI in combination with double-platinum chemotherapy (treatment beyond progression). This study aimed to compare chemotherapy effects of double-platinum plus EGFR-TKI versus double-platinum alone on patients with NSCLC progression after EGFR-TKI treatment. This was an analytical descriptive study using prospective cohort design, involving 30 patients with disease progression following EGFR-TKI treatment that met inclusion criteria in Dr. Soetomo Hospital. Subjects were divided into two groups: arm A (double-platinum plus EGFR-TKI) and arm B (double-platinum alone). Subjects were observed until 4 cycles of double-platinum chemotherapy. Subjective response (body weight and EQ5D questionnaire) was analyzed, chest CT scans were evaluated using RECIST criteria, and adverse effects were monitored. This study was conducted in accordance with GCP principles and has received ethics certificate from Dr. Soetomo Hospital ethics committee (No. 08/Panke.KKE/I/2017). The results showed that subject characteristics between two arms were insignificantly different (p=0.05). The most common EGFR mutation was exon 21 (50% on arm A and 60% on arm B). Chi square was tested on subjective response parameter (EQ5D (p=0.483)). T2 free sample was tested on semi-subjective parameter (body weight (p=1.00)). Comparison test on both groups after cycle 2 and 4 showed p value=0.05. Statistical test on adverse effect between both groups showed p value=0.526. As a conclusion, there was no significant difference between double-platinum and double-platinum plus EGFR-TKI on patients who had disease progression following EGFR-TKI treatment.

      Abstract: Physical activity is one of the factors which determines one’s health and fitness, including the professors of Padjadjaran University, as it might support their academic performances. Global Physical Activity Questionnaire (GPAQ) may describe physical activity. The purpose of this study was to describe physical activity profile of the professors of Padjadjaran University based on GPAQ. Physical activity profile which was classified as gender, age group, recommendation fulfillment, and intensity variables by present-ing quantity (n) and percentage (%) was obtained from 141 professors who fulfilled the inclusion criteria. The result of this des-criptive-quantitative study showed that 109 of the respondents were male, and 32 were female. According to WHO age group cate-gories, 35 respondents were middle aged, 78 were elderly, and 28 were late elderly. Fifty-eight respondents (41.1%) did not fulfill minimal recommendation for physical activity, 114 respondents (80.8%) underwent moderate activity and 29 respondents (20.6%) underwent vigorous activity. Therefore, it can be concluded that professors of Padjadjaran University, which were mostly middle-aged men, did not fulfill minimal recommendation for physical activities, although many of them underwent moderate activities in their daily routines.

      Abstract: The esophageal caustic injury is rare, but it requires precise and complex management. A variety of reconstruction techniques have been done, but despite the increasing volume, the incidence of complications is still relatively high. We reported the experience in our center in handling esophageal reconstruction in patients with caustic esophageal injury that caused oesophageal stricture between 2014-2017. This study used case series method with literature review. The results showed that between 2014-2017, there were 3 patients with caustic esophageal injury. All patients undergoing esophageal reconstruction surgery were included under conditions of malnutrition. Two were caused by HCl and the rest by NaOH. All patients underwent a resection of stricture segment of the esophagus, either using partial or total esophagectomy. Anastomosis leakage occurred in all cases, but improved with conservative treatment. The average length of hospitalization was 27 days. The intraoperative blood loss in patients ranged from 450-700 cc. In conclusion, proper preliminary management can provide approppiate preparation of the patients for definitive or reconstructive surgery, especially to avoid malnutrition. The ideal reconstruction still could not be established, and the rate of postoperative complications was still high. The length of patient hospitalization was also relatively long.

      Abstract: This review examined the homeostasis of uric acid in human body and analyzed recent studies of the affecting major variables. Normal uric acid concentration in male is 3.5-7.2 mg/dL and in female is 2.6-6 mg/dL. Daily turnover of normal uric acid ranges from 498-1392 mg/day, miscible pool is 767-1650 mg, reabsorption is 8064 mg/day, renal excretion is 262-620 mg/day and intestine 186-313 mg/day. The dynamics of uric acid is influenced by factors of food, drink, age, history of disease, and genetic. High purine dietary consumption increases blood uric acid by 1-2 mg/dL, 213-290 g/day fructose drinks increases 0.52-1.7 mg/dL, 1.5 g/kgBW sucrose increases 0.61 mg/dL, and 10-20 ml/kgBW beer increases 0.50-0.92 mg/dL. The ABCG2 gene plays a role in bringing uric acid out of the body by 114.31-162.73 mg/dL, SLC2A9 of 5.43-20.17 mg/dL, and SLC22A12 of 5.77-6.71 mg/dL. The data described the homeostasis of uric acid and the magnitude of the impact of environmental (consumption of food, beverages, and lifestyle) and genetic factors. Understanding uric acid homeostasis and its disturbances is important in managing diseases as a consequence of hyperuricemia and hypouryscemia
