  • The Modified Dunn Osteotomy for SCFE: Clinical and Radiographical Results
           at 3 Years Follow-Up

      Abstract: The modified Dunn osteotomy (MDO) gained popularity for the treatment of moderate-to-severe SCFE over the past years, even though both indications and outcomes are currently debated. The aim of this study is to evaluate results and complications of MDO consecutively performed at a single institution for severe slipped capital femoral epiphysis (SCFE) (slip angle ≥ 60°). In this retrospective clinical study, 23 hips (22 patients) were treated with MDO (2009–2015). SCFE was classified according to stability, slip angle, and as chronic/acute. Clinical assessment was performed with clinical examination and nonarthritic hip score (NAHS), modified Harris hip score (mHHS), and short form 12 (SF-12) outcome scores. Residual deformity, avascular necrosis (AVN), and osteoarthrosis (OA) were measured on plain radiographs. The mean slip angle was 70° (range 60–90); 17 of 23(74%) patients had a chronic SCFE, 6 of 23(26%) had acute-on-chronic SCFE, and 1 of 23(4%) had an unstable SCFE. The mean age at surgery was 14 years (range 11–17). At 3 years follow-up (range 1–7), 4 of 23(17%) patients had an AVN or AVN progression, one of these needing a hip arthroplasty. All the cases of AVN or AVN progression were diagnosed within the first year after surgery and there was an association between AVN and operation date (with more AVN among patients operated earlier). Out of 23 patients, 2 (9%) had an implant mobilization or failure and needed a revision surgery without further complications, 1 (4%) developed an asymptomatic heterotopic calcification, and another a partial implant removal without clinical consequence. At follow-up, the slip angle was 9° (range 1–15) and outcome scores were improved. MDO is a demanding technique which allows a near anatomical correction of SCFE and the overall complication rate is not negligible. With actual data, the authors only recommend this procedure for specialized centers, performed by experienced high-volume surgeons, and for severe displaced SCFE (≥50°). The long-term results must be closely monitored and compared with the natural history and other treatment options. This study was a Level IV retrospective study in a case series.

  • Line-to-Line Reaming of Highly Porous Acetabular Components Improves
           Position and Reduces Variability of Component Placement

      Abstract: Accurate and precise acetabular component position in total hip arthroplasty remains challenging even for experienced, high-volume surgeons. Some variability may result from the press-fit technique of cementless components and maybe magnified by the use of highly porous cups with high-friction surfaces. The purpose of this study is to investigate the acetabular component positioning of a hemispherical, highly porous cup with line-to-line reaming compared with press-fit techniques. A retrospective cohort study was performed of 110 consecutive primary total hip arthroplasties performed by a single surgeon using a highly porous acetabular component. Half of the patients had the component implanted with a line-to-line reaming technique and the other half were implanted using a 1 mm press-fit technique. Postoperative radiographs were evaluated for component positioning and loosening at a minimum of 1 year postoperatively by two independent surgeons. Records were reviewed for subsequent surgery. There were no differences between the groups with respect to age, sex, body mass index, operative side, or acetabular component size. The press-fit group had an average abduction angle of 49.4 ± 5° and anteversion of 20.6 ± 7°.   • Increased Total Hip Arthroplasty Utilization, Osteoarthritis Diagnoses,
           and Comorbidity Burden in Patients Younger Than the Age of 65 Years:
           National Inpatient Trends 1998 to 2013

      Abstract: Primary total hip arthroplasty (THA) in patients younger than the age of 65 years demands long-term success to minimize lifetime revision surgery risks. Age at implantation, etiology of arthrosis, and comorbidity burden all have associations with revision risk. This study uses Nationwide Inpatient Sample (NIS) data between 1998 and 2013 to (1) report the annual utilization of THA in patients age 18 to 99 years divided into age groups, (2) determine trends in primary diagnoses, and (3) explore Elixhauser comorbidity burden trends. The authors reviewed 5,174,020 THA cases from the NIS. Cases were divided into age groups: 18 to 24, 25 to 34, 35 to 44, 45 to 54, 55 to 64, and 65 to 99 years.   • Complex Pelvic Reconstruction using Patient-Specific Instrumentation and a
           3D-Printed Custom Implant following Tumor Resection

      Abstract: Resection of a periacetabular sarcoma is a difficult surgical problem due to the complex bony and soft tissue anatomy. The challenge of pelvic tumor resection lies in achieving clear margins while also reconstructing the defect with a durable construct. Limb salvage surgery involving these types of tumors, as well as other tumors of the pelvis, has made great progress recently due to remarkable technological innovation in the field of computer-assisted surgery and custom implant design. In this study, the authors review the case of a 9-year-old child with an osteosarcoma arising from the proximal femur with local spread into the hip joint and a skip metastasis in the ipsilateral acetabulum. They undertook limb-sparing extra-articular resection of the sarcoma with a reconstruction consisting of a 3D-custom printed pelvic implant and a proximal femur extendible prosthesis. The resection of the pelvis was performed with patient-specific cutting guides, rather than computer-assisted navigation. The resection resulted in a clear margin and the patient has had no evidence of local recurrence. These patient-specific guides closely matched the planned resection in terms of orientation in the pelvis, which allowed for straightforward implantation of the custom prosthesis at the time of surgery. The result has been a durable reconstruction that has produced an excellent functional outcome for this patient.
