  • Strategies to Increase Pneumococcal Vaccination in Veterans: An
           Integrative Review
    • Authors: Mary Zekan; BSN, RN, HNB-BC
Abstract: Pneumonia is a common disorder, with 4 to 5 million cases diagnosed each year in the United States, with approximately one quarter of patients requiring hospitalization (Papadakis et al., 2019). In the United States, pneumonia is the most common cause of hospital admission for adults (American Thoracic Society, 2018). For adults aged ≥65 years, hospitalization for pneumonia has a higher risk of death compared to any of the other top 10 reasons for hospitalization (American Thoracic Society, 2018). Pneumococcal pneumonia is the deadliest infectious disease in the United States and the eighth

  • Voice Assistant Reminders for Pain Self-Management Tasks in Aging Adults
    • Authors: Marcia Y. Shade; PhD, BS, RN
Abstract: According to the U.S. Census Bureau, by 2050, 1.6 billion members of the total population will be adults age 65 years and older (Roberts et al., 2016). Pain is prevalent among older adults, with 60% to 75% reporting persistent pain (Dahlhamer et al., 2018). The impact of pain is significant, with symptoms being associated with three times the risk of disability in older adults (Eggermont et al., 2014; Stubbs et al., 2016). Appropriate pain management is necessary to promote quality of life for aging adults.
Pain management is challenging in older populations. Pain symptoms are frequently

  • Prevention of Delirium in Older Adults With Dementia: A Systematic
           Literature Review
    • Authors: Linda Schnitker; PhD, MN, BN
Abstract: The focus of the current article is the prevention of delirium, a disturbance of consciousness and attention (American Psychiatric Association [APA], 2013), in older persons with dementia (PWD), also known as delirium superimposed on dementia (DSD). Worldwide, there are approximately 50 million people with dementia (World Health Organization [WHO], 2019), which is a non-curable progressive terminal syndrome affecting brain function that is more prevalent in the older population. Each year, 10 million new dementia cases will be diagnosed (WHO, 2019). With increases in life expectancy and

    • Authors: Mallory L. Hacker; PhD, MSCI
Abstract: Neurological disorders and injury are common in the long-term care setting (Jones et al., 2009), where more than 1.4 million Americans reside. Two common sequelae of neurological conditions, such as spinal cord injury (SCI), stroke, cerebral palsy, multiple sclerosis, and traumatic brain injury (TBI), are spasticity and urinary incontinence (UI) (Chua et al., 2003; Jensen et al., 2013; Marciniak et al., 2014; Mehdi et al., 2013; Meng et al., 2010; Sommerfeld et al., 2004). Abstract: COVID-19 has certainly put a spotlight on so many challenges we face in health care. How do we provide care in times of uncertainty' How do we allocate resources equitably' How do we use technology to connect providers and families with older adults in their homes and care facilities' Another question that many of us are wrestling with now, for example, is how to continue learning about advances in research and care as conferences, meetings, and in-person gatherings remain on hold.

Informal Caregivers in the United States
    • Authors: Victoria F. Keeton; MS, RN, CPNP-PC, CNS
Abstract: Approximately 40 million individuals in the United States provide unpaid, informal care or assistance to adults who are in some degree incapacitated and require help (Family Caregiver Alliance, 2018; National Alliance for Caregiving [NAC], 2015). This number is likely to increase as the population ages, with the number of individuals aged ≥85 years projected to increase from 6 million to 20 million by 2060 (Mather et al., 2015). The physical strain, emotional distress, and financial burden associated with caregiving contribute to adverse health outcomes among caregivers, including stress,

Abstract: Telehealth provides for the promotion of health care, health education, and public health through the use of electronic information and telecommunication technologies (Health Resources and Services Administration, 2020). Telehealth may include a mixture of telephonic, video, and remote monitoring technologies that can perform subjective (e.g., patient/caregiver–reported symptoms) and objective (e.g., weight, blood pressure, medication adherence) functions.            COVID-19 Pandemic: Nurses' Contributions
    • Authors: Debora Rosa; PhD, RN
Abstract: On December 31, 2019, China reported a cluster of pneumonia cases of unknown etiology caused by a new coronavirus (SARS-CoV-2) associated with respiratory disease to the World Health Organization (WHO), which was later termed COVID-19. As of August 6, 2020, there were 18,614,177 confirmed cases worldwide since the beginning of the epidemic and 702,642 deaths (WHO, 2020).
The literature shows that age and some chronic diseases (e.g., cardiovascular, respiratory, hypertension, diabetes) seem to be linked to the pathogenesis of COVID-19 (Yang et al., 2020). Hypertension is prevalent in

Older Adults: A Call to Gerontological Nursing for Social Justice
    • Authors: Staja Booker; PhD, RN
Abstract: Nursing is in a strong position to serve as an instrument for the protection of humanity and social justice. Several nursing organizations, including the National Hartford Center of Gerontological Nursing Excellence, American Nurses Association (ANA), and the American Academy of Nursing, have issued recent formal statements denouncing systemic racism to ensure that all Americans, but specifically African Americans, receive equal treatment. Yet, few, if any, prominent gerontological nursing voices have addressed these issues specifically in diverse older adult populations to date.

Abstract: The channels through which people get information about health care are diverse, and include health care professionals, peers, and internet sources (Fox, 2011). Most people still consult with physicians or other health care providers about their health issues as a first choice; however, growing numbers of internet users search for health information online (Fox, 2011). • Factors Predicting the Onset of Amnestic Mild Cognitive Impairment or
Alzheimer's Dementia in Persons With Subjective Cognitive Decline
    • Authors: Sangwoo Ahn; PhD, RN
Abstract: In the United States and beyond, dementia cases are increasing in epidemic proportions. In 2017, it was estimated that approximately 50 million people have dementia worldwide, and 10 million new cases are expected to occur every year (World Health Organization [WHO], 2017). Alzheimer's disease (AD) dementia is an irreversible, progressive neurodegenerative disorder that accounts for 60% to 80% of all dementias. In 2017, it was estimated that approximately 5.5 million Americans had AD dementia and, by 2050, an AD dementia diagnosis will occur every 33 seconds (Alzheimer's Association, 2017).

Abstract: Older adults who reside in nursing homes have complex medical needs, and due to culture change efforts over the past 3 decades (Koren, 2010), promotion of their choice and dignity has been elevated along with the importance of clinical care. Facets of culture change promote individualized, person-directed care (PDC), which is supported through consistent staffing to facilitate meaningful relationships, and in the presence of a comfortable home environment (Miller, Lepore, et al., 2014; Saliba & Schnelle, 2002). There are comprehensive models of culture change that can be adopted,

Nurses are recognized as brave, resilient, and compassionate frontline caregivers during this unprecedented time. We add our voice of appreciation, but shift focus to the important, highly skilled care of nurses working in long-term care (LTC).
Nursing homes have emerged as places where COVID-19 has spread most aggressively and with tragic lethality. Approximately 30,000 LTC residents and employees have died from COVID-19 infection in the

Telephones, televisions, loud voices, and cleaning equipment can all be heard at a typical nursing home facility (McClaugherty & Burnette, 2001), where elevated noise levels are not only present during daytime hours, but also at nighttime (Schnelle et al., 1999). Abstract: Caregiver burden is a concept predominantly associated with family caregivers, with little previous research defining burden among formal caregivers who provide care to older adults in nursing homes. Subsequently, there is lack of understanding of what formal caregiver burden truly is and what it is not. Formal caregiver burden needs to be defined so that the consistency of phenomena can be identified and measured in nursing science. A conceptual definition that provides guidance for an operational definition will allow for the advancement in the science related to formal caregivers,

Abstract: A high level of physical activity is associated with preservation of physical function, lower risk of chronic disease, and improved mood and cognitive condition (Frandin et al., 2016). Theou et al.'s (2011) systematic review found that physical activity can improve body composition, food intake, muscle function, upper and lower limb flexibility, and can lower depressive symptoms in older adults. Previous studies have shown that low physical activity level is related to high risk of cardiovascular disease, all-cause mortality, and cancer mortality, and higher physical activity level can reduce

  • AGS COVID-19 Policy Brief Offers Roadmap to Government Action for a
Critical Group During Pandemics: Older Adults in Nursing Homes
    • Authors: Annie Medina-Walpole; MD, AGSF
Abstract: There are so many topics I hope to address during my tenure as the 2020–2021 president of the American Geriatrics Society (AGS)—but, candidly, it is hard to imagine writing anything in 2020 without first addressing COVID-19. By the time this article reaches you, I hope—as do all our nurses, pharmacists, physician assistants, physicians, social workers, and so many other inter-professional colleagues—the light at the end of this proverbial tunnel will be in sight. As we look toward a less distant horizon, though, I am reminded that we need to keep the past and present ever close. This pandemic

  • Qualitative Validation of the Nursing Home IT Maturity Staging Model
    • Authors: Kimberly R. Powell; PhD, RN
Abstract: Over the past 20 years, there has been an explosive trend in information technology (IT) system implementation in health care. Billions of dollars and countless resources have been spent implementing IT systems to improve efficiency, quality, and safety across different health care settings, including nursing homes (NHs). NHs are a critical component of health care systems around the globe, but particularly in the United States, where more than 16,000 facilities provide care to>1.7 million residents (National Center for Health Statistics, 2014). The need for NH services is expected to

  • Technology for Healthy Independent Living: Creating a Tailored In-Home
Sensor System for Older Adults and Family Caregivers
    • Authors: Erin L. Robinson; PhD, MSW, MPH
Abstract: Over the past decade, strides have been made in the development of sensing technologies to assist older adults in managing their health. Technologies such as environmentally embedded sensors, smart homes, robotics, and wearable devices help facilitate the aging process and allow older adults to age in place. In-home sensor systems, in particular, hold enormous potential for the early detection of health changes that can dramatically affect the aging experience. These systems use unobtrusive and environmentally embedded technologies that track health parameters, such as gait, walking speed,
