Egendorf, Howard W. MielkeNumerous toxicants contaminate soil and negatively affect the environments that children explore. Accurately measuring these toxicants and characterizing the level of soil contamination may be difficult and must include measurements of both the environmental concentrations and the exposure responses of human populations. This article reviews the current methods and technologies available for quantifying soil contamination. Several intervention strategies exist for limiting human exposure to contaminated soils and the strengths and weaknesses of these methods are discussed. Lastly, current policies on soil contamination and the importance of protecting vulnerable populations by developing means to improve health conditions for children are reviewed.

McLachlanWithin a remarkably short timespan the world population doubled and transitioned from an agrarian to an urban-industrial society. The transition was accompanied by the major expansion of industries that releases enormous amounts of toxicants into the air, water, and soil. Naturally occurring and synthetic chemicals compounds utilized the same signaling system as vertebrate internal cell signaling systems. The concept of environmental signals provides insights to address the impact of biochemically active toxicants on humans and the ecosystems that they share with other species. Disruption of the broad signaling systems has the potential for global change that transcends the biological systems of all organisms, including humans.

The focus of this concern, however, is typically limited to air and water pollution, leaving an important source under-studied and out of the concern of the general public. Soil pollution provides a unique threat to children's health, due to their increased exposure and susceptibility to its contaminants. The microbiome of a child is developed prior to birth and continues to evolve over their lifetime with each encounter to the outside world. The environment a child inhabits directly affects their microbiome and their overall health, and through interactions with contaminated soil, a child can accumulate adverse health outcomes. The aim of this article is to summarize the methods by which soil becomes contaminated and how children become exposed to the resulting toxicants.

Schachter, Howard W. MielkeSoil pollution is a global phenomenon, and children are uniquely susceptible to the wide range of toxicants that persist in topsoil. Given their increased exposure through mouthing activities, increased body surface area, likelihood of breathing air closer to soil, and immature immune and elimination systems, it is essential to understand the mechanisms of children's exposure and the potential health effects of toxicants found in soil. Here we describe the sources and toxicological profiles of a range of inorganic and organic soil contaminants, including arsenic (As), cadmium (Cd), lead (Pb), mercury (Hg), benzene, toluene, ethylbenzene and xylenes, chlorinated dibenzo-p-dioxins, polychlorinated biphenyls (PCBs), polycyclic aromatic hydrocarbons (PAHs), per and polyfluoroalkyl substances (PFAS), as well as agricultural and domestic sources of pollution. The aim of this article is to increase awareness regarding the risks and health impacts of contaminated soil, and to encourage further research and efforts aimed at mitigating children's exposure.

Systematic reviews suggest that organization-based interventions are more effective in reducing physician burnout than interventions targeted at individual physicians. This consensus review by leaders in the field across multiple institutions presents emerging trends and exemplary evidence-based strategies to improve professional fulfillment and reduce physician burnout using Stanford's tripartite model of physician professional fulfillment as an organizing framework: practice efficiency, culture, and personal resilience to support physician well-being. These strategies include leadership traits, latitude of control and autonomy, collegiality, diversity, teamwork, top-of-license workflows, electronic health record (EHR) usability, peer support, confidential mental health services, work-life integration and reducing barriers to practicing a healthy lifestyle. The review concludes with evidence-based recommendations on establishing an effective physician wellness program.
       
  • Cultivating the joy of medicine: A focus on intrinsic factors and the
           meaning of our work

StewartPhysician burnout is at epidemic levels. In our role as healers, the concepts of humanism and relief of suffering are central themes in our work, yet burnout and depersonalization can threaten these values. While working to mitigate burnout, we need to move towards a focus on health and well-being and develop preventive strategies to cultivate resilience. This manuscript discusses the intrinsic factors that motivate us to be physicians: medicine as a calling, finding meaning in our work, and seeking joy in practice. Some strategies that enhance our resilience will be discussed including individual reflective practices, in addition to organizational strategies such as creating team mission statements, and participating in debriefing, Balint groups, and Schwartz Rounds. All of these practices provide opportunities to acknowledge the emotional impact of our care of patients and to focus on our values and the meaning of our work. Strategies to cultivate joy in practice are presented alongside a framework from the Institute for Healthcare Improvement to guide organizations.

           well-being and resilience

RanaThe practice of medicine is rewarding on many levels, but demands of the work can result in mental and emotional exhaustion, self-isolation, burnout, depression, suicidal ideation and tragically, completed suicide. It is critical to have effective strategies to address the unique stressors of a medical career, mitigate burnout, and buffer the physiologic toll of chronic stress. Using Zwack and Schweitzer's widely published description of approaches to maintaining resilience and approaches to wellness, we have organized these strategies into three broad domains relevant to medical practice: (1) gratification (connection and communication, meaning and purpose); (2) resilience building practices (self-reflection, time for oneself, self-compassion, spirituality); and (3) useful attitudes (acceptance, flexibility, self-awareness). Several techniques are described including mindfulness-based stress reduction, narrative medicine, skillful communication, and practices in self-compassion, gratitude, and spirituality. The focus of the work is to introduce a spectrum of resilience strategies for individual consideration that can be tailored and combined to meet a physician's changing needs over the course of medical training and practice.
       
  • Selecting physician well-being measures to assess health system
           performance and screen for distress: Conceptual and methodological
           considerations

TrockelFollowing national calls to address burnout among U.S. physicians, healthcare systems across the nation are integrating measures of physician well-being into institutional assessments. In this paper, we review important conceptual and methodological considerations for selecting self-reported physician well-being measures to monitor health system performance and to screen individual physicians for symptoms of distress. First, we discuss the importance of selecting any given measure of physician well-being based on the degree to which evidence supports the validity of the measure within the context of its intended use. Second, we present a conceptual model explaining the relationship between physician well-being and the larger healthcare context, to assist health systems in identifying the intended goals of physician well-being assessment. Well-being assessments are metrics of individual-level physician wellness/distress and may be indicators of system-level performance. We highlight proposed roles of physician well-being as a performance metric (i.e., as a downstream effect of the medical practice environment, as a predictor of health system outcomes, and as a mediator of the practice environment's effect on health system outcomes). Using this framework, we review the evidence supporting the validity of four of the most commonly used measures of well-being in U.S. physicians, identify gaps in the literature, and present practical recommendations for healthcare organizations’ selection of appropriate measurement tools. We conclude by offering directions for future research to advance the measurement of physician well-being outcomes.
