  • A Novel Surgical Approach in the Management of Peters Anomaly With
           Glaucoma
    • Authors: Huda Sheheitli; MD
Abstract: Peters anomaly is a dysgenesis of the anterior segment characterized by congenital corneal opacity and iridocorneal (type I) or lenticulocorneal (type II) adhesion.1 Glaucoma may be present at birth or may develop, often as a complication of surgery undertaken to establish a clear visual axis. Patients with central corneal opacity and no lenticular involvement can usually achieve an adequate visual axis through an optical iridectomy.2 They thus avoid penetrating keratoplasty, which has poor results in infants with Peters anomaly.3 We describe a single operation for type 1 Peters anomaly with

           Cerebral Artery
    • Authors: Jeremy J. Kudrna; MS
Abstract: Pediatric cranial nerve palsies have many etiologies, including neoplasm, trauma, infection, and metabolic insult. One example of metabolic insult to a cranial nerve is cerebral vascular compromise. Only a fraction of these presumed perinatal ischemic stroke cases present acutely at birth. To our knowledge, this is the first published case of unilateral abducens nerve palsy following perinatal stroke of the middle cerebral artery.
A male infant first presented to the pediatric neurology service at 8 months of age because of non-use of his right arm (with right hand balled in a fist), which his

  • CRB1-Related Cystic Maculopathy in Twins Conceived Through Heterologous
           Fertilization With Variant-Carrying Oocytes
    • Authors: Stefano Paolacci; PhD
Abstract: Cystic maculopathy is rare in the pediatric age group and has been associated with several diseases. The most frequent cause of cystoid macular edema is not genetic but due to damage associated with vascular diseases. Less often, cystoid or schitic spaces in the macula may be present even if the blood–retinal barrier is intact and uninvolved, in cases such as optic pit,1 myopic traction maculopathy,2 or genetic disorders such as retinitis pigmentosa,3 or be the primary sign of cone dystrophy,4 enhanced S-cone syndrome,5 and foveal retinoschisis.6

           Vitamin A Deficiency in Infants
    • Authors: Meenakshi Wadhwani; MS
Abstract: Vitamin A deficiency occurs in nutritionally deprived children in developing countries despite the international and national programs involved with controlling this public health problem. In 2009, the World Health Organization's global estimate indicated that 5.2 million preschool aged children had night blindness, 190 million preschool aged children had low serum retinol concentrations (< 0.70 µmol/l), and approximately 7 million pregnant women had vitamin A deficiency.1,2 Vitamin A is a fat-soluable vitamin whose function is important to the visual cycle of the retinal pigment

           Facial Colliculus Syndrome
    • Authors: Mohadaseh Feizi; MD
Abstract: Paradoxical eye movements have been described in various congenital and acquired ocular disorders, and different central and peripheral mechanisms have been proposed for its pathophysiology.1–3 Convergence movements on attempted side gaze have been reported in patients with horizontal gaze palsy. These paradoxical movements may be part of a near reflex substitution in which miosis and myopic shift in refraction accompany convergence movements.2 We describe a patient with right-sided facial colliculus syndrome due to pontine arteriovenus malformation, which manifested as near reflex

  • Comparison of a New, Filter-Free Stereopsis Test (BEST) With the Randot
           Stereotest in a Pediatric Cohort
    • Authors: Oded Lagstein; MD
Abstract: Stereopsis is the perception of depth and is strictly a binocular phenomenon. It is expressed in seconds of arc (arcsec) and is considered the most developed and complex type of binocular vision.1,2
Stereoacuity depends on both sensory and motor abilities. Eyes are required to make convergent and fixating movements for standard near testing, and cortical fusion mechanisms, sensitive to retinal disparity, are also required.1 Because many factors influence stereopsis, its measurements can provide clinicians with essential information regarding the general health of the visual function, making it

    • Authors: Ari Leshno; MD
Abstract: Medicine is an ever-changing practice as a result of ongoing new scientific breakthroughs in both diagnosis and treatment options, requiring physicians to constantly pursue updates in published literature.1 Moreover, even while focusing on a certain subspecialty, clinicians still attempt to keep up with developments outside their chosen subspecialty. This is especially true in pediatric ophthalmology, because this is a broad area that has constant close interactions with many other subspecialties.

  • Peripheral Retinal Changes in Regressed Retinopathy of Prematurity
    • Authors: Jessica Minjy Kang; MD
Abstract: An 11-year-old boy who had been born 24 weeks premature and had a history of regressed retinopathy of prematurity (ROP) not requiring treatment during the acute phase presented for a routine examination. His fundus examination was notable for an atrophic retinal hole without surrounding subretinal fluid in the right eye (Figure 1A) and avascular fibrotic changes in both eyes (Figures 1B–1C). Although peripheral retinal abnormalities in premature children have been noted as distinct changes in patients treated with intravitreal bevacizumab,1,2 this case demonstrates that peripheral retinal

    • Authors: Rosie Brennan; FRCOphth, FRCSEd(Ophth)
Abstract: Partially accommodative esotropia continues to make up a significant proportion of a pediatric strabismus surgical workload. These children often do not demonstrate stereopsis following surgery.1 Full accommodative esotropia is a diagnosis of an esotropia that is fully controlled with demonstration of stereopsis of 800 seconds of arc or more on Lang stereo test, Frisby test, or Wirt test when hypermetropic correction is worn. Although the numbers of children needing surgery have decreased due to recognition that full hypermetropic correction must be prescribed,2 it is not clear whether the

    • Authors: Rehab R. Kassem; MD, FRCS(Glasg)
Abstract: Acquired esotropia is a leading cause of amblyopia in the pediatric population.1 The goal of management of acquired esotropia is to align the eyes as soon as possible to reestablish binocular fusion and prevent sensory and motor complications. The first line of treatment is to reverse any amblyopia and prescribe the full hyperopic correction. If a residual esotropia is present, strabismus surgery is performed.2 Various strategies have been proposed to determine the surgical target angle (the angle of esotropia on which the amount of muscle surgery is based),3 including the standard surgery

  • The Approach to the Newborn With Bilateral Leukocoria
    • Authors: R. V. Paul Chan; MD
Abstract: DeRespinis: Today's discussion is on leukocoria in newborns and infants. Every clinician who deals with children will at some point have to either diagnose, refer, or treat a patient with leukocoria. Depending on the cause, timely intervention may assist us in not only preserving vision and possibly the globe, but also even saving a life. The first patient we will discuss is a consultation in a newborn nursery. A full-term male infant has white reflexes in the pupils of both eyes and bilateral lens opacity that indicates bilateral congenital cataracts. What is your approach to the child'

Abstract: In a particularly relevant article in this issue, Leshno et al. describe the decline of published articles on pediatric topics in leading general ophthalmology journals. They cite multiple reasons for this trend, including a reduction in manpower leading to overworking and less time for significant research among pediatric ophthalmologists. This problem was addressed by our co-editor, Dr. Nelson, in a previous issue of the Journal of Pediatric Ophthalmology & Strabismus. Fortunately for our readers, a shift in the publication of pediatric ophthalmology articles from the top-rated

These disorders are highly associated with mutations of the FLNA gene on the X chromosome, which encodes the cytoskeletal protein filamin A.2 Ocular anomalies have been reported in confirmed cases of FLNA mutation.3–8 However, accessory iris membranes have not been described in

Abstract: Autistic children with selective diets have an elevated risk for vitamin A deficiency. The authors describe a 7-year-old boy with keratomalacia resulting from dietary vitamin A deficiency.
A 7-year-old boy with autism presented to the emergency department with red swollen eyes of 1-week duration despite a trial of topical olopatadine 0.1% twice a day for 5 days. He was non-verbal and communicated by pointing and moving objects. His parents noted that he was photophobic and had kept his hands over his eyes since onset. He had a selective diet of dry cereal, soda, and juice, which was reflected

Abstract: Familial exudative vitreoretinopathy (FEVR) is a rare hereditary disorder defined by failure of peripheral retinal vascularization. Described by Criswick and Schepens in 1969,1 this asymmetric, bilateral disease can range from asymptomatic vascular anomalies to retinal detachment. Diagnosis is made through fundus examination, fluorescein angiography, and genetic testing. Treatment relies on laser photocoagulation of avascular zones and vitrectomy surgery in the case of late stage retinal detachment. Mutations in the FZD4, LRP5, and TSPAN12 genes are identified in 50% of cases.2 Patients with
