The diseases of this synovial joint are similar to other synovial joints and include degenerative and inflammatory joint diseases, trauma and ankyloses, non-neoplastic lesions that behave like tumors and benign and malignant neoplasia. The imaging protocols and key diagnostic features of these diseases are discussed in the article below.
       
  • Sinonasal Neoplasms
    • Abstract: Publication date: Available online 9 March 2019Source: Seminars in RoentgenologyAuthor(s): Mohit Agarwal, Bruno PoliceniAbstractSinonasal neoplasms comprise 0.2% to 0.8% of all malignancies and only 3% of head and neck neoplasms. Despite their low incidence, sinonasal maligancies have a dismal prognosis. Squamous cell carcinoma is the most common malignant tumor and is most frequently seen in the maxillary sinus. Considerable overlap is seen in the imaging features of sinonasal tumors and the primary role of imaging is tumor mapping to delineate the size and extent of tumor, resectablility, surgical approach and to identify perineural tumor spread or distant metastasis. Despite great strides in the histopathological differentiation of sinonasal tumors and the introduction of several new tumor entities in the latest WHO classification, the primary roles of imaging remain unchanged. In this chapter, we will discuss sinonasal neoplasms with a separate section discussing new/emerging tumor entities.

Davenport, ME HarrisAbstractWith substantially growing prevalence of orbital implant and device placements, there is increasing frequency of incidental detection of such medical products on “non-dedicated” head and neck imaging studies such a head CT/MRI examinations. In addition, some patients might undergo dedicated orbital imaging to determine the extent of a suspected complication. Therefore, it is critical for the radiologist to be familiar with the expected and unexpected imaging findings of orbital implants and devices to facilitate accurate diagnosis, timely treatment of complications, and potential prevention of unnecessary implant revision in some patients with a high degree of confidence.
       
  • PERINEURAL TUMOR SPREAD IN HEAD AND NECK MALIGNANCIES

RathAbstractPresence of perineural tumor spread in head and neck malignancies worsens the prognosis and alters management. The tumor spread is usually retrograde and ‘skip’ lesions may be present. The trigeminal and facial nerves are most commonly involved. Connections between the trigeminal and facial nerves act as conduits of perineural tumor spread. Thin slice MRI is the imaging modality of choice for detection of perineural tumor spread where involved nerves enhance and are thickened, oftentimes with effaced perineural fat planes. It takes an astute radiologist to detect subtle perineural tumor spread. Knowledge of relevant nerve anatomy is critical for detection of perineural tumor spread.1Perineural tumor spread (PNTS) which is macroscopic spread of malignant tumor along the course of a nerve distant to a primary tumor should be differentiated from perineural invasion, the latter being a histological diagnosis; although both are associated with worse prognosis.2The trigeminal and facial nerves are most frequently affected. The tumor can ‘hop’ from one nerve to another, typically by way of the greater superficial petrosal and auriculotemporal nerves.3PNTS is usually in the vicinity of the primary tumor but can be present at remote “skip” locations, which makes it essential to look along the entire course of the nerve, emphasizing the importance of the knowledge of nerve anatomy.

CorneliusAbstractOsteosarcoma of the maxilla and mandible (gnathic osteosarcoma) is one of the pathologic entities to be considered in the differential diagnosis of masticator space masses. The mandible is more commonly affected, with peak incidence around age 30-40 years. Overall prognosis is favorable when compared to osteosarcoma of the long bones. However, higher grade at presentation and presence of disease recurrence are associated with decreased overall survival.

This is accomplished, in part, by observing radiographic signs or patterns that are more common in one or a small number of diagnoses.We will review the radiographic signs of diffuse lung disease seen on HRCT such as mosaic attenuation, honeycombing, and subpleural sparing. Because some of these signs are seen in more than one disease, we will discuss the sensitivity and specificity each sign. Ancillary features and historical clues that can further narrow the differential will also be discussed.

Chung, Luis LanderasAbstractIdiopathic pulmonary fibrosis is one of the most common and mortal types of pulmonary fibrosis. With the advent of therapies shown to slow the decline of pulmonary function in this condition, early accurate diagnosis has become imperative. However, categorization of imaging in suspected idiopathic pulmonary fibrosis continues to evolve. Two of the most important recent changes to the classification system are the probable and indeterminate for usual interstitial pneumonia categories. This review paper summarizes the data, which supports the introduction of the new categories as well as the other aspects of the new imaging classification system in suspected idiopathic pulmonary fibrosis.

HanrahanAbstractRadiology trainees and practicing radiologists are challenged by interpreting magnetic resonance imaging (MRI) examinations of the brachial plexus due to limited exposure during training and complex anatomy. We present a simple, easily applied pattern approach for identification and interpretation of important imaging findings, and a pictorial review of brachial plexus anatomy and pathology. Traumatic and non-traumatic pathology (including inflammation, infection, tumors, radiation plexopathy, and thoracic outlet syndrome) are presented with operative correlation and a highlighted section on brachial plexus birth palsy (also known as obstetrical brachial plexus palsy).
       
  • Advanced MR Imaging of Musculoskeletal Tumors: An Overview

TaljanovicAbstractMagnetic resonance (MR) imaging plays a major role in the management of musculoskeletal tumors. Conventional MR imaging offers information complementary to radiography essential in initial evaluation, post-treatment assessment, and surveillance. Advanced MR imaging techniques frequently add to the specificity of radiologists’ interpretations. These sequences interrogate tissues at a cellular level, and include diffusion weighted imaging, chemical shift imaging, dynamic contrast enhancement, and MR spectroscopy. While these sequences can improve specificity, they must be interpreted in conjunction with routine anatomic imaging sequences. This article describes these functional MR imaging techniques, while also highlighting limitations and technical considerations.

Etiologies are varied, and prompt recognition of these processes is critical in prevention of further morbidity and mortality, where possible. In this pictorial review, we discuss the pathophysiology, clinical presentation, and diagnostic imaging features of a curated selection of these entities, including avascular necrosis, bone infarct, sequestrum in chronic osteomyelitis, myonecrosis, necrotizing fasciitis, and gangrene. Typical radiographic, computed tomographic, and magnetic resonance imaging findings are outlined.

           Tumor-Like Lesions of the Foot

MRI is the modality of choice for the evaluation of soft tissue tumors of the foot, related to its intrinsic high soft-tissue contrast and multiplanar capability. This article reviews the MRI appearance of the common soft tissue masses in the foot together with their clinical, radiographic, and pathological features. Even though there is a large overlap between benign and malignant lesions in the foot, MRI characteristics can be useful to provide a practical guide for radiologists to establish a focused differential diagnosis.
