Arribas Díaz, C. Aparicio MartínezResumenLa infección endoprotésica es una entidad clínica de rara aparición y sintomatología insidiosa. Sin embargo, sus consecuencias clínicas son devastadoras, suponiendo alta morbimortalidad para el paciente. Por ello, su diagnóstico precoz e instauración de tratamiento antibiótico, así como el manejo quirúrgico, son primordiales para mejorar los resultados. Existen diversas opciones terapéuticas que varían desde el manejo conservador en pacientes con riesgo quirúrgico prohibitivo, hasta el drenaje percutáneo, la reconstrucción in situ utilizando materiales recubiertos con antibiótico o plata, o incluso de material protésico habitual no impregnado o el tratamiento clásico de reconstrucción extraanatómica dejando muñón aórtico. Sin embargo, en los últimos años la extracción completa del dispositivo y la reconstrucción aórtica in situ ha ido ganando adeptos por sus buenos resultados en cuanto a permeabilidad y el uso de otros materiales protésicos. Basándonos en nuestra experiencia clínica, por su durabilidad y alta resistencia a la reinfección, recomendamos el empleo de venas femorales superficiales como material autólogo y del parche de pericardio bovino como xenoinjerto, e incluso, una combinación de ambos materiales en caso de injertos más largos.AbstractThe endograft infection is a rare clinical entity that usually has insidious symptoms. However, its clinical consequences are devastating for the patient, with high morbidity and mortality in the published literature. Therefore, early diagnosis and establishment of antibiotic treatment, as well as surgical management, are essential to improve the results. There are several therapeutic options, ranging from conservative management in patients with prohibitive surgical risk, to percutaneous drainage, in situ reconstruction using antibiotic or silver coated prosthetic grafts, or even uncoated prosthetic material and extra-anatomic reconstruction, leaving an aortic stump. However, in recent years, complete removal of the device and aortic in situ reconstruction has gained popularity due to its good results in terms of patency and resistance to infection. Based on our clinical experience, due to its durability and high resistance to reinfections, it is recommended to use superficial femoral veins as autologous material and the bovine pericardium patch as a xenograft, and even a combination of both materials, if a longer graft is needed.
       
  • Consideraciones sobre la aplicación del derecho administrativo
           sancionador a las agresiones al personal sanitario

Sánchez-BenitoResumenLa aplicación del delito de atentado a las agresiones contra el personal sanitario ha suscitado cierta controversia. En primer lugar, los profesionales de la sanidad privada no se encuentran incluidos en el ámbito de esta protección, lo que genera críticas desde dicho sector. De otro lado, parte de la doctrina jurídico-penal entiende que el delito de atentado no es la figura idónea para enjuiciar los casos de violencia contra los profesionales sanitarios o de la educación. Algunos autores han ido más allá, cuestionando incluso la aplicación del derecho penal en el enjuiciamiento de estos actos. Entienden que se está vulnerando la necesaria proporcionalidad entre la conducta y la posible pena, por imponer esta rama del ordenamiento jurídico sanciones excesivamente severas ante hechos que no tienen gravedad suficiente. Defienden, además, que los propios principios del derecho penal (en especial los de mínima intervención y «ultima ratio») determinan que deba acudirse a otros recursos. En concreto, proponen que sea el derecho administrativo sancionador el que se utilice en exclusiva para castigar las agresiones contra el personal sanitario. Desde nuestro punto de vista, existen razones de peso para no prescindir de la intervención del derecho penal en el enjuiciamiento de este tipo de violencia. Entendemos que existen recursos para garantizar el respeto al principio de proporcionalidad y que la intervención en exclusiva del derecho administrativo sancionador plantearía problemas, sobre todo en los casos de agresiones a profesionales de la sanidad privada. Por ello, concluimos que la solución óptima estriba en la intervención de ambos órdenes (administrativo sancionador y penal), aunque reservando el delito de atentado para calificar únicamente las agresiones más graves.AbstractThe application of the crime of violence against health workers has raised some controversy. In the first place, private health professionals are not covered in the scope of this protection, which has raised criticism from that sector. On the other hand, part of the legal-criminal doctrine considers that the crime of attack against public servants is not the ideal term to prosecute cases of violence against health or educational professionals. Some authors have gone further, even questioning the application of criminal law in the prosecution of these acts. They think that the necessary proportionality between the conduct and the penalty to be imposed is being infringed, since this branch of law imposes excessively severe sanctions to acts that are not serious enough. They also defend that the principles of criminal law (especially minimum intervention and «ultima ratio») determine that other instruments should be used. In particular, they propose that the administrative sanctioning law must be used exclusively to punish assaults against health workers. From our point of view, there are compelling reasons not to dispense with the intervention of criminal law in the prosecution of this type of violence. We consider that there are resources to ensure respect for the principle of proportionality and the exclusive intervention of the administrative sanctioning law would cause problems, especially in cases of attacks against private health professionals. Therefore, it is concluded that the optimal solution lies in the intervention using administrative sanctioning and criminal law, although the crime of attack against public servants should only apply to the most serious assaults.
       
  • Paraparesia por oclusión aórtica aguda. Una manifestación
           clínica poco frecuente

Encisa de SáResumenEl síndrome de Leriche, consecuencia de la oclusión de la aorta abdominal o de ambas ilíacas, puede producirse de forma aguda, lo que conlleva un aumento de la morbimortalidad asociada. Aunque poco frecuente, el cuadro clínico en ocasiones es solo neurológico, manifestándose como una paraparesia aguda sin dolor asociado, lo cual generalmente produce un retraso diagnóstico y terapéutico, con el consecuente empeoramiento en el pronóstico del paciente. La cirugía abierta de forma urgente continúa siendo la primera opción terapéutica, aunque hay casos descritos de reparación endovascular. A pesar de ello, esta enfermedad presenta un mal pronóstico, con una mortalidad en torno al 30-50% en los primeros 30 días. Presentamos 3 casos clínicos de síndrome de Leriche agudo que se manifestaron como paraparesia aguda.AbstractLeriche syndrome, a consequence of occlusion of the abdominal aorta and/or iliac, can occur acutely, leading to an increase in the associated morbidity and mortality. Although uncommon, the symptomatology is sometimes only neurological, manifesting itself as an acute paraparesis without associated pain. This generally leads to a diagnostic and therapeutic delay, with the consequent worsening of patient prognosis. Urgent open surgery continues to be the first therapeutic option, although there are reported cases of endovascular repair. Despite this, the condition has a poor prognosis with a 30-day mortality of around 30-50%. The clinical cases are presented of 3 patients with acute Leriche syndrome that manifested as acute paraparesis.

Orgaz Pérez-GruesoResumenIntroducciónEl uso de los dispositivos de cierre arterial percutáneo (DCAP) se ha generalizado en los últimos años, lo implica riesgo de complicaciones específicas derivadas de la falta de efectividad hemostática y de su liberación inadecuada. La evidencia disponible referente a los CDAP está basada en datos de procedimientos coronarios, no extrapolares al intervencionismo periférico.ObjetivosDemostrar que la hemostasia del acceso arterial con DCAP tras procedimientos percutáneos arteriales periféricos es más rápida que con compresión manual (CM) sin disminuir la efectividad hemostática inicial y seguridad.Material y métodosRealizamos un ensayo clínico aleatorizado reclutando a todos los pacientes tratados mediante procedimientos percutáneos arteriales a través de introductores entre 4 y 8 French. Se asignó mediante aleatorización un sistema hemostático: CM, Angioseal®, StarClose® o Proglide®, registrándose la eficacia, el tiempo de hemostasia y la ocurrencia de complicaciones en el acceso arterial pasados 1 y 30 días de la intervención.ResultadosSe incluyó a 40 pacientes por grupo. La eficacia hemostática resultó igual en los 4 grupos (CM: 87,5%; Angioseal®: 87,5%; StarClose®: 85%; Proglide®: 85% [p: 0,97]).
