  • Cardiovascular Disease Risk Is Associated With Middle Cerebral Artery
           Blood Flow Velocity in Older Adults

      Abstract: imagePurpose: The aim of this study was to evaluate the relationship of cardiovascular disease (CVD) on middle cerebral blood flow velocity (MCAv) at rest and during exercise. A secondary aim was to explore the relationship between MCAv and (1) the presence of white matter lesions (WMLs) and (2) cognitive function.Methods: We recruited individuals who were cognitively normal older adults. Cardiovascular disease risk was assessed by the Pooled Cohort atherosclerotic CVD (ASCVD) risk score. Transcranial Doppler ultrasound measured middle cerebral artery at rest and during a bout of moderate-intensity exercise. We quantified WMLs from magnetic resonance imaging and cognitive function outcomes included executive function, language, processing speed, and attention.Results: Seventy-two participants 70.1 ± 4.7 years of age completed the study protocol. Atherosclerotic cardiovascular disease risk score was significantly associated with resting and exercise MCAv (P < .01) but not associated with WMLs (P> .468). We observed a significant association between resting and exercise MCAv and language processing (P = .010) but not other cognitive domains.Conclusions: In cognitively normal older adults, higher ASCVD risk score was associated with blunted resting and exercise MCAv and with lower language processing performance. These results highlight the need for CVD risk management to maintain optimal brain health.

  • Impact of a Blood Pressure Practice Initiative on Attitude, Practice
           Behavior, and Knowledge Among Outpatient Rehabilitation Providers: An
           Observational Study

      Abstract: imagePurpose: The purpose of this study is to describe the impact of a blood pressure (BP) educational initiative on the attitude, practice behaviors, and knowledge of outpatient (OP) physical therapists.Methods: An observational pre-test and post-test study design was initiated using a sample of convenience at 12 OP rehabilitation clinics. A 4-phase protocol included (1) evaluation of BP equipment, (2) pre-test questionnaire, (3) attendance at a 1-hour lecture on BP practice recommendations, and (4) performing BP measures on patient evaluations for 1 week followed by the post-test questionnaire. Descriptive statistics reported demographics, whereas a paired t test compared means of the presurvey and postsurvey responses with statistical significance set at P < .05.Results: Of the 55 therapists included, 70.9% were female and 29.1% male, whereas 41.8% and 58.2% reported having ≤15 and>15 years of experience, respectively. A significant improvement in BP attitudes (P = .02), practice behaviors (P = .001), and knowledge (P = .002) was identified.Conclusions: An educational initiative that provided BP assessment tools, a 1-hour evidence-based BP lecture, a decision-making reference, and a directive to perform valid and reliable BP techniques and interventions resulted in positive changes to an OP therapists attitude, immediate practice behavior, and knowledge of BP.

  • Physical Activity: Levels, Knowledge, and Attitudes of Physical Therapy
           Students in the United States

      Abstract: imagePurpose: To determine the physical activity (PA) level, knowledge of guidelines, and attitudes and role perceptions about promotion of PA in Doctor of Physical Therapy (DPT) students.Methods: In this cross-sectional design study, convenience sampling was used to recruit DPT students (n = 87, 24.6 ± 4.4 y). Each participant completed a survey that included as follows: International Physical Activity Questionnaire (IPAQ), knowledge, attitudes, and role modeling about PA. A subset (n = 15) used Fitbit to record PA over a 7-day period. Data were converted to metabolic equivalent (METS)/min/min to categorize participants into low/moderate/vigorous PA levels. Descriptive statistics and chi-square tests were conducted (P < .05).Results: On the IPAQ, 48%, 40%, and 12% participants reported a high, moderate, and low level of PA, respectively, and 62% participants met the guidelines. An agreement was noted between IPAQ and Fitbit data for meeting PA guidelines (P = .005). Respondents (61%) reported correct knowledge of PA guidelines and agreed/strongly agreed to the 7 questions (56%–86%) about the role of physical therapists in promoting PA.Conclusions: Based on our sample, most DPT students engage in and have knowledge of the recommended levels of PA, and value their role to give patients advice about PA.

  • The Effect of a Cardiovascular and Pulmonary Mobile Application on Student
           Learning of Assessment Skills: A Pilot Study

      Abstract: imagePurpose/Hypothesis: Advances in technology have recently resulted in the development of mobile learning applications (apps) for college students. Despite a recent influx of mobile apps, there is a lack of empirical data about the effectiveness of these apps on learning in the area of cardiovascular and pulmonary physical therapy (PT). The purpose of this study was to evaluate the effect of a researcher-generated cardiopulmonary mobile application on student skills in assessing blood pressure (BP), heart rate (HR), and pedal pulses.Number of Subjects: Twenty first-year PT graduate students were recruited for the study. Participants’ names were randomly chosen out of an envelope to assign them into 1 of 3 groups: demonstration (N = 7), mobile app (N = 7), or demonstration/mobile app (N = 6).Materials/Methods: After providing informed consent, each group participated in their randomized intervention. The demonstration group received demonstrations of each skill from a scripted therapist, followed by laboratory practice; the mobile app group received a 5-minute tutorial of how to use the app, followed by laboratory practice; the demonstration/mobile app group received both demonstration and the mobile app tutorial, followed by laboratory practice. Independent of group type, all participants were allotted 45 minutes in the laboratory to practice performing and interpreting BP, HR, and pedal pulses, after demonstration, demonstration/mobile app, or mobile app instruction. After the laboratory practice, participants were individually tested on the skills learned during the laboratory by a short competency examination administered by 3 licensed physical therapists. These physical therapists were blinded to each group's identity.Results: Total competency was highest among the demonstration/mobile app group as compared with the other 2 groups. The total competency examination score was statistically significant for different teaching methods with the use of demonstration and the mobile application being more effective than only using the mobile application P = .032. No significant difference was noted between the mobile app group and the group using demonstration only, P = .24. No other group differences were noted.Conclusion: Findings of this study suggest PT students demonstrate improved total competency examination scores (total score of BP, HR, and pedal pulses) when provided a mobile application combined with traditional demonstration.Clinical Relevance: There is currently a paucity of literature in PT education pertaining to mobile learning. The results of the current study support the use of a mobile app in addition to traditional learning methods. Future research should include a multisite design and validation of a clinical competency instrument.

  • Exploration of a Simulation-Based Learning Experience in Critical Care:
           The Use of Standardized Patients for Early Mobility Training

      Abstract: imagePurpose: The purpose of this study was to examine the influence of a simulation experience, using standardized patients (SPs), on physical therapy students' perceptions of comfort with early mobilization of patients in a critical care environment.Methods: To explore the influence of the simulation experience on students' confidence in the assessment and treatment of critically ill patients, a survey investigating the skills associated with cardiovascular and pulmonary physical therapy was developed and administered before and after the experience. In addition, focus group interviews explored the students' perceptions of the simulation scenarios.Results: Similar to studies using high-fidelity human simulators, students reported increased comfort with the skills required for early rehabilitation of patients in critical care settings. In addition, students discussed the realism of scenarios, the practical application of previously acquired knowledge, and the need to adapt to the patient response during the scenarios.Conclusions: With the advancement of the simulation technology, SPs can be used to accurately portray patients in critical care units. The use of SPs allows for the unrestricted ability to mobilize the patient and, therefore, the ability to practice the skills associated with early rehabilitation in the intensive care setting.
