  • One-day-only drug provocation test in beta-lactam non-immediate
           hypersensitivity reaction - Diagnostic efficacy

    • Abstract: Fundamentos: A prova de provocação oral (PPO) é considerada o gold standard no diagnóstico de hipersensibilidade aos betalactâmicos. Todavia, não existe ainda adequada uniformização entre os protocolos utilizados nos vários centros, surgindo a questão de se a administração de um curso terapêutico completo numa PPO alargada contribuiria para o aumento da sensibilidade diagnóstica da prova. Objetivos: Avaliação da eficácia diagnóstica da PPO de um dia com antibiótico betalactâmico num grupo de doentes com suspeita de reação de hipersensibilidade não imediata a este grupo terapêutico. Métodos: Dos 111 doentes consecutivos referenciados ao Serviço de Imunoalergologia do Centro Hospitalar de Setúbal por suspeita de hipersensibilidade aos antibióticos betalactâmicos, de Janeiro de 2012 a Janeiro de 2017, 56 apresentavam suspeita de reação não imediata. Após realização de PPO de um dia com o fármaco suspeito, 54 doentes não apresentaram sintomatologia, tendo a prova sido considerada negativa. Foram posteriormente avaliados telefonicamente com um questionário de efectividade sobre a realização de terapêutica com betalactâmicos após a exclusão do diagnóstico por PPO. Resultados: A análise do questionário mostrou que, do total de 54 doentes cuja hipersensibilidade aos betalactâmicos foi excluída pela PPO, 24 responderam que lhes tinha sido prescrita a terapêutica durante o período de follow-up abordado. Destes, 79,2% realizou a terapêutica e nenhum apresentou sintomatologia compatível com hipersensibilidade, sendo confirmada a exclusão do diagnóstico. Os restantes não cumpriram a prescrição por manterem receio de reação adversa. Conclusões: Na análise retrospetiva de um grupo de doentes com suspeita de hipersensibilidade não imediata aos betalactâmicos com PPO negativa, foi possível confirmar a posteriori que a utilização do protocolo recomendado de um dia apresentou total eficácia na exclusão do diagnóstico, questionando-se a necessidade de utilização de protocolos prolongados.Background: Drug provocation tests (DPT) are the gold standard in the diagnosis of beta-lactam hypersensitivity. However, no standardization exists concerning the protocols used between centers, raising the question if a full-course DPT would increase the effectiveness of the allergy workup. Objective: To evaluate the diagnostic efficacy of a 1-day-only DPT with beta-lactam antibiotic in a group of patients with suspected non-immediate hypersensitivity reaction to these drugs. Methods: From the 111 consecutive patients referred to the Allergy and Immunology Department of Centro Hospitalar de Setúbal for suspected beta-lactam hypersensitivity from January 2012 to January 2017, 56 had history of nonimmediate reactions. After completing a 1-day-only DPT with the culprit drug, 54 experienced no symptoms and the DPT were considered negative. Afterwards, an effectiveness questionnaire was administered to assess if patients had been prescribed and received beta-lactams after the negative DPT. Results: Of the total of 54 patients with excluded diagnosis of beta-lactam hypersensitivity with DPT, 24 answered that had been prescribed with beta-lactams during the considered follow-up period. Out of those, 79.2% affirmed that had received the antibiotic and hadn´t suffer from any symptoms of hypersensitivity, confirming the exclusion of drug allergy. The remaining patients did not get the therapy because the lack of confidence in the result. Conclusions: According with the retrospective analysis of our group of patients with suspected non-immediate hypersensitivity reaction to beta-lactams with a negative DPT, it was confirmed that 1-day-only DPT protocol was totally effective in the diagnostic exclusion, which questions the utility of long course protocols.
       
  • Anaphylaxis in Portugal: 10-year SPAIC National Survey
           2007-2017

    • Abstract: Introdução: A anafilaxia é a forma mais grave de doença alérgica; no entanto, a real prevalência em diferentes grupos populacionais é incerta por ausência de registos adequados desta entidade potencialmente fatal. Objetivos: Contribuir para melhorar o conhecimento epidemiológico e a abordagem da anafilaxia no nosso país. Métodos: Durante um período de 10 anos (2007 a 2017) foi implementado um sistema de notificação nacional de anafilaxia focalizado na notificação voluntária por clínicos com diferenciação em patologia imunoalérgica. Foram incluídas notificações de anafilaxia de 1783 doentes. Foi efetuada caracterização detalhada sobre etiopatogenia, manifestações e abordagem clínica em idade pediátrica e adultos. Resultados: A média de idade foi de 32,7±20,3 anos, 30% com idade inferior a 18 anos e 10% de idade pré-escolar. A relação de género feminino/masculino foi de 1,4. A maioria (68%) tinha antecedentes pessoais de doença alérgica, 33% com asma. A média de idade do primeiro episódio foi de 27,5±20,4 anos (com variação entre 1 mês e 88 anos). As principais causas da anafilaxia foram os alimentos (48%), os fármacos (37%), primeira causa nos adultos, e os venenos de himenópteros (7%). Os principais alimentos foram o marisco (crustáceos e/ou moluscos) 27%, frutas frescas 17%, leite 16%, frutos secos 15%, peixe 8%, ovo 7%, amendoim 7% e sementes 3%. Os principais fármacos foram os AINE, 43%, os antibióticos, 39%, e os agentes anestésicos, 6%. Outras causas identificadas foram a anafilaxia induzida pelo exercício, 3%, a anafilaxia induzida pelo látex, a anafilaxia induzida pelo frio e a anafilaxia idiopática, 2%, entre outras. Houve predomínio de manifestações mucocutâneas (96%), seguido de respiratórias (82%) e cardiovasculares (36%). A maioria dos doentes (80%) foi admitida em serviço de urgência, sendo que apenas 43% recebeu tratamento com adrenalina. Ocorreu recorrência da anafilaxia em 41% dos doentes (com ≥ 3 episódios de anafilaxia em 21%), tendo sido o dispositivo autoinjetor de adrenalina utilizado por 7% dos doentes. Conclusões: Os alimentos foram a causa mais frequente de anafilaxia em Portugal, sobretudo em idade pediátrica. Os fármacos foram os principais desencadeantes de anafilaxia na idade adulta. Destaca-se o subtratamento com adrenalina e a recorrência de anafilaxia, apontando para a necessidade de serem implementadas medidas estratégicas e educativas de forma a melhorar a abordagem diagnóstica e terapêutica da anafilaxia.Background: Anaphylaxis is the most severe manifestation of an allergic disease. However, the actual prevalence in different population groups is uncertain due to the lack of adequate records of this potentially fatal entity. Objectives: Contribute to the improvement on epidemiological knowledge and anaphylaxis management in our country. Methods: Over a 10-year period (2007-2017) a national anaphylaxis reporting system was implemented, focused on voluntary reporting by clinicians with differentiation in immuno-allergic diseases. Reports of anaphylaxis from 1783 patients were included. Detailed characterization of etiopathogenesis, manifestations and clinical approach was performed in pediatric and adult ages. Results: The average age was 32.7±20.3 years, with 30% under 18 and 10% of preschool age. The female / male gender ratio was 1.4. Most (68%) had a personal history of allergic disease, 33% with asthma. The average age of the first episode was 27.5±20.4 years (ranging from 1 month to 88 years). The main causes of anaphylaxis were food (48%), drugs (37%) (the first cause in adults) and hymenoptera venom (7%). The main foods were shellfish (crustaceans and/or mollusks) 27%, fresh fruits 17%, milk 16%, tree nuts 15%, fish 8%, egg 7%, peanut 7% and seeds 3%. The main drugs were NSAID 43%, antibiotics 39% and anesthetic agents 6%. Other causes identified were exercise-induced anaphylaxis 3%, latex-induced anaphylaxis, cold-induced anaphylaxis and idiopathic anaphylaxis 2%, among others. There was a predominance of mucocutaneous (96%) manifestations, followed by respiratory (82%) and cardiovascular (36%) manifestations. Most patients (80%) were admitted to the emergency department, and only 43% received epinephrine treatment. Anaphylaxis recurred in 41% of patients (21% with ≥ 3 episodes of anaphylaxis). The adrenaline autoinjector device was used by 7% of patients. Conclusions: Food was the most frequent cause of anaphylaxis in Portugal, especially at pediatric age. Drugs were the main triggers of anaphylaxis in adults. We highlight the undertreatment with epinephrine and the recurrence of episodes, pointing to the need to implement strategic and educational measures to improve the therapeutic and diagnostic approach of anaphylaxis
       
  • Objective measurements for diagnosis of exercise-induced
           Bronchoconstriction

    • Abstract: A broncoconstrição induzida pelo exercício (BIE) define-se como o aumento transitório da resistência das vias aéreas resultante da obstrução brônquica que ocorre após esforço físico. É uma entidade clínica muito frequente, que importa diagnosticar corretamente de forma a prevenir a resposta broncoconstritora ao esforço, de modo a que esta não constitua restrição à escolha de uma atividade física ou limitação ao nível do desempenho desportivo. A história clínica pode ser extremamente sugestiva, porém, são frequentes tanto situações de sobrediagnóstico de asma como de subdiagnóstico de BIE. Assim sendo, e face à importância do diagnóstico exato, pretende-se com o presente artigo apresentar os exames complementares no diagnóstico de BIE, abordando a sua utilidade e aplicabilidade.Exercise-induced bronchoconstriction (EIB) is defined as the transient increase in airway resistance resulting from bronchial obstruction that occurs after physical exertion. It is a very frequent clinical entity that must be correctly diagnosed in order to prevent the bronchoconstrictive response to the exercise, so that it does not restrict the choice of a physical activity or limits the sports performance. The clinical history can be extremely suggestive, however, both overdiagnosis of asthma and underdiagnosis of EIB are frequent. Therefore, and due to the importance of an exact diagnosis, this article intends to present the complementary exams in the diagnosis of EIB, addressing its usefulness and applicability.
       
  • Cytomegalovirus gastritis and ileitis as presentation of common variable
           immunodeficiency in an adolescente

    • Abstract: Objetivo: Descrever o caso de uma imunodeficiência comum variável (IDCV) cuja primeira manifestação foi diarreia crónica por infeção gastrointestinal por citomegalovírus (CMV). Esta é uma forma de apresentação rara e subdiagnosticada, cujo diagnóstico e intervenção precoces podem evitar a morbimortalidade associada a esta infeção. Descrição do caso: Adolescente de 16 anos com diarreia crónica desde os 10 anos e perda ponderal nos últimos 2 anos, sem antecedentes relevantes, referenciado à consulta com critérios diagnósticos de IDCV (diminuição de imunoglobulinas (Ig) G, A e M; ausência de anticorpos vacinais antiparotidite e antiHBs; exclusão de outras causas de hipogamaglobulinemia). Após início de terapêutica endovenosa com gamaglobulina, por manutenção de diarreia, realizou endoscopia digestiva alta e colonoscopia com biópsias, que mostrou gastrite e ileíte a CMV. Após terapêutica com ganciclovir endovenoso, verificou-se resolução clínica. Discussão: A infeção a CMV deve ser investigada em doentes com CVID e diarreia crónica simultaneamente com a exclusão de outras patologias gastrointestinais mais comuns, como infeção por Giardia lamblia, doença celíaca e doença inflamatória intestinal.Objective: To describe the case of a common variable immunodeficiency (CVID) whose first manifestation was chronic diarrhea due to cytomegalovirus (CMV) gastrointestinal infection. This is a rare and underdiagnosed presentation, whose early diagnosis and intervention may prevent the morbidity and mortality associated with this infection. Case description: 16 year-oldmale adolescent with chronic diarrhea from the age of 10 years and body weight loss in the last 2 years, with norelevant past clinical history, referred to our consultation with diagnostic criteria for CVID (low levels of immunoglobulin (Ig) G, A and M; absence of anti-mumps and anti-hepatitisB surface antibodies; excluded other causes of hypogammaglobulinemia).After intravenous Ig replacement was initiated, diarrhea persisted, which led to perform high endoscopy and colonoscopy.Biopsy immunohistochemical analysis revealed CMV gastritis and ileitis. After intravenous ganciclovir therapeutic, clinicalresolution was achieved. Discussion: CMV infection should be investigated in patients with CVID and chronic diarrhea simultaneously with the exclusion of more common gastrointestinal diseases such as Giardia lamblia infection, coeliac disease and inflammatory bowel disease.
