  • Effect of Dental Erosion and Methods for its Control on the Marginal and
Internal Adaptation of Restorations with Different Adhesive Systems

    • Authors: Janaina Emanuela Damasceno, Felipe Vilasboas Rodrigues, Luana Mendonça Dias, Patricia Akemi Nishitani Shibasaki, Max José Pimenta Lima, Roberto Paulo Corrreia de Araújo, Richard Mark Foxton, Andrea Nóbrega Cavalcanti
Abstract: AbstractThe dentin exposed to erosive challenges is often superficially protected to prevent progression of the lesion. This study investigated the marginal and internal adaptation of composite resin restorations made on surfaces that had first been treated with different methods for controlling erosion. Cavities with margins in dentin were prepared in bovine incisors (n=360) and were divided into three groups according to the method for controlling the erosive challenge: negative control, topical application of fluoride and resin-modified glass ionomer varnish. The specimens were then randomly divided into three sub-groups according to the exposure to simulated gastric acid solution (DES) (5% HCl, pH=2.2) and subsequent remineralization (RE): negative control, 9 and 18 cycles of DES-RE. Finally, teeth were divided into four groups, depending on the bonding agent used for composite resin restoration (n=10): conventional etch-and-rinse adhesive system (2 and 3 steps) and self-etching (1 and 2 steps). Front and internal images of the interface tooth/restoration were recorded in stereoscopic microscope (15x) to quantify the percentage of adhesive failures. Despite the promising results of the resin-modified glass ionomer varnish after 9 cycles; no protective material prevented increased internal defects after 18 erosive cycles. More continuous internal margins were noted with etch-and-rinse acid systems after more intense erosion. The maintenance of internal margins in eroded substrates was positively influenced by the resin-modified glass ionomer varnish and, under the most aggressive challenge, by the use of etch-and-rinse adhesives systems. Keywords: Tooth Erosion. Dentin. Acid Gastric. • Manifestation of Anxiety during Dental Treatment: Integrative Literature
           Review

    • Authors: Ana Carolina de Lima Cardoso, Ana Karlla Rocha, Bruna Rafaella Ramos Melo, Lyzandra Cardoso Calixto, Marilia Mattar de Amoêdo Campos Velo, Dayse Andrade Romão
Abstract: Abstract Even with the technical-scientific advance in Dentistry, it is still seen as a profession that causes anxiety in children and adults. Therefore, in addition to the technical knowledge, the dental surgeon must have different strategies in the management and conduct of this situation once anxiety if aggravated can become pathological and interfere in the patient's oral health and  his or her  quality of life. Thus, the present study was conducted in order to realize the main aspects of dental anxiety, aiming to improve patient-dentist relationship, avoiding the onset of phobic disorders. This is an integrative review of the literature, using the Lilacs and SciELO databases with the descriptors anxiety, phobia and dentist verified individually and later they were crossed with AND operator with search period from 2000 to 2019, articles in Portuguese and in fulltext. The exclusion criteria were: thesis, books, final paper and dissertations. 20 articles were identified which showed that anxiety directly interferes with the effectiveness of treatment in different age groups. Priority should be given to communication between the dental surgeon and his or her patient, to recognize possible causes of discomfort. In addition, it is necessary to carry out more studies because there are differences in the literature concerning the interference of social and economic factors and schooling  levels  in the anxiety development. Keywords: Anxiety. Dentist. Phobia. • Aesthetic-Functional Fixed Appliance as Treatment of Premature Loss of
           primary Anterior Teeth

    • Authors: Priscila Vieira da Silva, Juliana Severino Lupinetti Noberto, Ariane Barbosa Rodrigues Del Papa, Cíntia Aparecida Damo Simões, Sandrine Bittencourt Berger, Andreza Maria Fábio Aranha
Abstract: Abstract The early loss of anterior primary teeth is frequent in the pediatric dentistry clinic and it is associated with extensive caries or dental trauma. The purpose of this study was to report two cases of aesthetic-functional rehabilitation due to early loss of anterior primary teeth, with the aid of fixed space maintainers of the tube-bar type. In the first case, a 4-year-old female child referred for dental treatment is reported. During clinical examination, several restorations in the posterior teeth and absent upper anterior teeth were observed. The treatment plan consisted of installing a tube-bar type upper fixed space maintainer, considering the ease of installation and adaptation for young children. The second case reports a 5-year-old male patient with the main complaint of dental caries. Clinical examination revealed extensive coronary destruction in the upper central incisors, as well as in the posterior teeth that presented carious lesions in several teeth. For the case planning, it was determined the need for  the posterior teeth restorations and the exodontia of the anterior dental remnants. After oral rehabilitation, an aesthetic-functional space maintainer of the tube-bar type was made and installed. In both cases, prosthetic rehabilitation was fundamental to maintain the space until the eruption of the permanent successors, avoiding future damages, as well as to restore the children's self-esteem and introversion  behavior. Keywords: Deciduous. Space maintenance.

           Literature Review

    • Authors: Bruno Gomes Duarte, Jefferson Moura Vieira, Renato Yassutaka Faria Yaedú, Osny Ferreira Júnior
Abstract: AbstractThis paper aims to report a case of radicular displacement to the submandibular space and to review the literature seeking reports of dental displacements / fragments published in the period from 2007 to 2017 in PubMed evidencing risk factors, prevention and forms of treatment. In this article a case of displacement of dental fragments to the submandibular space with immediate removal is reported. The third molar or its roots displacement into facial spaces is a rare situation. The preoperative surgical planning is fundamental to avoid this type of complication. The association of (1) good surgical planning, (2) surgical technique utilization and adequate instruments, and (3) the surgeon experience are determining factors to avoid the occurrence of this complication. Keywords: Molar, Third. Surgery, Oral. Intraoperative Complications. • Conservative Substitution of Fractured Ceramic Onlays Using Self-Adhesive
           Cement: Case Report

    • Authors: Mariana Dias Flor-Ribeiro, Josué Júnior Araújo Pierote, Giselle Maria Marchi, Luis Alexandre Maffei Sartini Paulillo
Abstract: Abstract The advances in bonding of ceramics and self-adhesive resin cements to tooth structure make this treatment a feasible alternative to restore posterior teeth. This case report described the removal of two fractured onlays and the fabrication of new lithium disilicate onlays cemented with self-adhesive resin cement. The rationale for various choices in this treatment protocol is detailed with reference to the pertinent literature. • Double Retromolar Canal: a Case Report

    • Authors: Ana Paula Tulio Manfron, Alessandra Soares Ditzel, Flávia Gasparini Kiatake Fontão, Luciana Rei Azevedo-Alanis
Abstract: AbstractThe retromolar canal is an anatomical variation of the mandibular canal. Its neurovascular bundle may extend to areas of the temporalis tendon, and areas of buccinator muscle insertion in the alveolar process at lower third molar region, beside the retromolar fossa. The observation of the retromolar canal in conventional radiographs is limited by the presence of increased bone condensation in the region due to overlapping anatomical structures. The use of three-dimensional imaging techniques, such as cone beam computed tomography can contribute significantly for the treatment planning. Some complications may occur in surgical procedures involving the retromolar area when the retromolar canals are present, such as hemorrhage, failure in anesthesia and injuries to nerve branches. This study reported an unusual case of a double retromolar canal on the left side of the mandible, that led  to the change of the treatment planning in a 54-year-old male patient. Furthermore, the clinical and surgical consequences of these findings in oral rehabilitation were discussed. The cone beam computed tomography was  an important diagnostic tool in the observation of the presence and the exact location of retromolar canal. Retromolar canals may be detected on a panoramic radiograph. However, more precise information about the anatomical variation can be shown on cross sectional cone beam computed tomography images. Keywords: Anatomic Variation. Mandible. Cone-Beam Computed Tomography.

between the Y-TZP Zirconia and Composite Resin Interface

    • Authors: Lucas Campagnaro Maciel, Amanda Pádua Proeza, Hélyda Coelho Guimarães Balbino, Marcela Moráo Corteletti, Ricardo Huver de Jesus, Laís Regiane da Silva Concílio
Abstract: AbstractThe evolution of dental materials and the improvement of ceramic systems stimulated the increased use of Y-TZP zirconia-based ceramics. Despite the excellent mechanical performance, this material has low adhesion potential. The objective of this work was to evaluate the surface treatments and resin cements influence on bond strength between Y-TZP zirconia and composite resin interface. A total of 60 blocks of Y-TZP zirconia (3x8x8mm) were prepared and divided into 3 groups according to the surface treatments: (C) control - extra fine diamond bur, (J) sandblasting with Al2O3 and (JP) sandblasting with Al2O3 + ceramic primer. Each group was subdivided into two groups according to type of resin cement used for cementing composite resin discs (2mm thick x 5mm diameter): self-adhesive and conventional (n=10). The samples were stored in distilled water for 24 hours at 37±1°C in a incubator and subsequently submitted to the shear bond test to determine the bond strength (RU). There was no significant difference in RU among the  surface treatments when using conventional resin cement. For the self-adhesive resin cement, Al2O3 blasting and Al2O3 + primer blasting increased the RU but did not present significant differences between them (p<0.05). Comparing the cements, it was observed that regardless of the surface treatment, the highest values were for the self-adhesive resin cement (p<0.05). Application of the primer after blasting with Al2O3 did not increase RU. Keywords: Dental Prosthesis. Ceramics. Dental cements.

  • Conservative Esthetic Approach of Anterior Teeth Using Minimal
Intervention and Preservation of all Sound Tooth Tissue: a Case Report

    • Authors: Janaina Emanuela Damasceno, Priscila Matos Pedreira, Thiago Ozi Bueno, Mariana Valerio Pallone, Thais Escobar Fava, Giselle Maria Marchi
Abstract: AbstractThis clinical case report describes a conservative approach to a smile harmony rehabilitation performing a gingivoplasty, dental bleaching and resin-based composites to finalize the case. This clinical article describes the case report of a patient in need of an esthetic and corrective solution for the anterior teeth, which presented a disharmonic smile, in its shape and color, besides the gingival smile. As a result, gingivoplasty, dental bleaching and restoration with composite resin were done. Gingivoplasty with bone preservation was possible because there was a large volume of soft tissue, and a distance ≥ 2 mm from the bone crest to the junction cementum enamel was confirmed. Bleaching was performed with the use of associated techniques: office (Whiteness HP 35%) and supervised at-home (Whiteness Perfect 10%) bleaching were performed, and then the "buccolingual technique" using a nanocomposite resin IPS was used from a diagnostic wax-up to perform the restorative procedure for the anterior teeth. The use of minimally invasive techniques is an effective alternative for the  anterior teeth rehabilitation. As in this case, the gingivoplasty, dental bleaching with associated techniques, and restorations with nanocomposites allowed satisfactory esthetic results, even more so in relation to dental preservation, mainly in young patients. Keywords: Tooth Bleaching. • Lower Incisor Root Resorption after Orthodontic Alignment and Leveling

    • Authors: Ângela Graciela Deliga Schroder, Mariana Gonzaga Erthal Ribeiro, Alyssa Sales dos Santos, Amaro Ilidio Vespasiano Silva, Flavio Ricardo Manzi
Abstract: AbstractThe objective of this study was to evaluate the external apical root resorption in the lower incisors after orthodontic alignment and leveling using digital periapical radiographs. The sample consisted of periapical radiographs of 90 lower central incisors from 45 patients, 19 men and 26 women aged 12-36 years (mean 22.7 years). All the 90 lower central incisors were performed periapical radiographs in two phases: 1 - initial before orthodontic treatment and 2 - after alignment and dental leveling phase. The initial periapical radiographs were digitized through a transparency scanner. All patients were treated by the same orthodontist, the inclusion criteria were: never having been treated orthodontically; absence of exodontia, agenesis, malformation or defect in teeth, supernumerary teeth, endodontic treatment and impacted incisors. The time interval between the initial and post-leveling radiographs was from 6 to 12 months, with an average time of 9 months. It was observed that 47.8% of the incisors evaluated presented external apical root resorption, being most grade 1 (irregular contour up to 1 mm reabsorption). There was a positive association of external root resorption with the triangular root shape. There was no association between the severity of root resorption and the type of malocclusion.   An association was observed between age and resorption severity, that is, patients who have already reached bone maturation were more likely to develop apical external root resorption. Female patients were more likely, but when men developed external root resorption, it tended to be a more severe degree. Female adult patients, with a triangular root shape, anatomical apical pipette shape, regardless of malocclusion type are more likely to develop apical external root resorption in orthodontic treatment. Keywords: Radiography, Dental, Digital. Orthodontic. Root Resorption. Abstract: AbstractDental caries is still the most common pathology in childhood, caused mainly by lack of adequate prevention. The aim of this project was to evaluate the dental caries profile in children and the importance of multidisciplinary practice in dental caries. Material and methods: 1st stage- meetings were held with the teachers of the child education network and conferences addressed to parents and those responsible for the children. 2nd stage- Assessment and examination of oral health with education process and motivation for dental hygiene. Results: A total of 3390 children were analyzed, of which 859 had carious teeth, that is, 25.34% of the children had caries disease. There is a large presence of caries in the PRE II level, whose highest prevalence of caries is 41.73%, and the lowest recorded in Kindergarten I with 3.11%. This is mainly due to age, directly related to the period in which the basic oral health care program began in day care centers, as older children already had high caries rates when the preventive program was institutionalized to control the occurrences of caries and health promotion. Conclusion: The prevalence of caries in preschool children is still high, but the multidisciplinary approach has shown to be effective and able to reach the levels recommended by  ONU 2030 appointment book for dental caries control. Keywords: Dental Caries. • Optical Properties of a Glass Ceramic and an Indirect Resin Composite:
           Effects of Polishing and Staining

    • Authors: Fernando Zurlo Dellazzana, Camila Silva Rodrigues, André Mallmann, Letícia Borges Jacques
Abstract: AbstractLittle information is available about the optical behavior of glass ceramics and indirect resin composites. This study aimed to evaluate if an indirect resin composite can present similar behavior of color stability and translucency to a glass ceramic after polishing and aging in staining beverage. Specimens of a dental ceramic (IPS e.max Ceram) and an indirect resin composite (SR Adoro) were made. Half the specimens of each material were polished with disc-shaped tips. Groups were divided according to aging media: distilled water or immersion in red wine for 20 min/day during 30 days. CIE L*a*b* coordinates were measured with a spectrophotometer at baseline and after 30 days of aging. Color change was calculated by CIEDE2000 and translucency was calculated by contrast ratio (CR). Statistical analysis were performed with ANOVA and Tukey tests. Aging in red wine caused perceptible color change in both materials. Polishing only increased color change of indirect composite when aged in red wine. Ceramic groups showed greater opacity than the composite in all measurements. The indirect composite remained more translucent and results showed that it is capable of presenting color stability similar to a ceramic. However, polishing seems to increase its color change. Keywords: Ceramics. Color. Composite Resins. Cor. Polimento Dentário. Resinas Compostas.

  • Effect of Vitamin D Supplementation on Clinical and Radiographic
Evaluation of Oral Rehabilitation with Osseointegrated Implants

    • Authors: Adriano Piccolotto, Gisele Toyama, Mauro Busato, Adriane Yaeko Togashi
Abstract: Abstract   Vitamin D (VD) is important for the development and maintenance of bone tissue. This study used clinical and radiographic evaluations to analyze whether insufficient VD levels affect the periimplant health of patients with implant-supported prostheses. Data were collected for 33 patients with implant-supported prostheses: probing depth (PD); width of keratinized mucosa (wKM); bleeding index (mBI); periimplant plaque index (mPI); distance from implant to bone crest and VD level using chemiluminescence. After 1 year of implant-supported prosthesis installation, patients with levels under 30 ng/ml at baseline received 50000 IU of VD per week for 8 weeks (post treatment, PT), when clinical and radiographic evaluations were repeated for the control (n=19) and the VD (n=14) groups. Clinical and radiographic results were compared at baseline and at PT.  There were differences in PD (p=0.0247) and distance from implant to bone crest (p<0.0001), but there were not statistically different after supplementation.  wMK, mBI and mPI were not statistically different between groups.  VD levels were 24.95 ± 0.96 ng/mL in the VD group (42.42% of patients), whereas 57.57% of all participants had a mean VD value of 40.99 ± 1.23 ng/mL, a statistically significant difference (p = 0.0034). According to clinical and radiographic findings, VD serum levels do not seem to affect periimplant health. Keywords: Bone Density. Clinical Study.
