  • Child with Guillain-Barré Syndrome Responding to Plasmapheresis: A
           Case Report

    • Abstract: Intravenous immunoglobulin (IVIG) has long been regarded as the first-line treatment for Guillain-Barré syndrome (GBS), with plasmapheresis only being reserved for severe cases or used as an additional therapy of unproven efficacy. Here, we present the case of a 9-year-old girl with acute motor axonal neuropathy (AMAN), a rapidly progressive subtype of GBS that caused her to fall into respiratory failure. The patient failed to show a response 10 days after starting IVIG, but showed rather quick improvement with plasmapheresis. She received a total of 5 sessions of plasmapheresis on alternate days over a course of 8 days. Before starting plasmapheresis, her muscle strength was 2/5 in both upper limbs and 1/5 in both lower limbs, and she was dependent on mechanical ventilation. Following the first session, her power improved from 2/5 to 4/5 in the upper limbs, and the gag and sucking reflexes were recovered. On day 3, after the second session was initiated, she was extubated successfully (having been on a ventilator for 2 weeks) and remained on continuous positive airway pressure for the next 48 h, after which she was on room air. In addition, she was having hypertension from the first day of the diagnosis (which was due to autonomic instability), which improved after clonidine to maintain her blood pressure. She was also initially having urinary retention, then was off Foley’s catheter. The patient was discharged from the hospital 2 weeks following the first session of plasmapheresis, with power grade 4/5 in both her upper and lower limbs. Her cranial nerves had recovered fully, and she was able to walk with aids.

  • Bilateral Basal Ganglia Lesions in Patients with Heroin Overdose: A Report
           of Two Cases

    • Abstract: The prevalence of opioid use has increased worldwide. Two-thirds of deaths caused by drug overdose are opioid-related. Individuals with opioid use may present with a variety of complications. The available history in unconscious patients is often insufficient, which may cause diagnostic difficulty and delayed management. We present two cases of 54-year-old and 25-year-old male patients who were brought to our emergency department after being found unconscious at home, without any known preceding event. They were in a deep coma with a Glasgow coma score of 3/15. However, their brainstem reflexes were normal. Pinpoint pupils were observed bilaterally. Brain computed tomography (CT) demonstrated the presence of bilateral basal ganglia hypodensities in both patients, in addition to multiple hypodensities scattered in the cerebral hemispheres of one patient. Toxicology screening from both patients was positive for opioids. The patients were transferred to the intensive care unit for supportive management. One patient recovered completely, whereas the other remained in a vegetative state. The presence of bilateral basal ganglia lesions in brain CT in unconscious patients should alert the physician of opioid use disorder among these patients.

  • Total Unilateral Lung Atelectasis in a Child with Asthma and Its Rapid
           Resolution with Medical Management without Any Invasive Intervention: A
           Case Report

    • Abstract: Asthma is the most common chronic lung condition of childhood affecting approximately 6 million children in the United States. Right middle lobe syndrome or atelectasis is common in children with asthma though whole unilateral lung collapse is rare; only case series are reported. Bronchoscopy is usually required in addition to medical management of asthma and treatment of infection. We present a case of unilateral lung atelectasis in a young child which resolved with intense medical management without any endoscopy.

  • Pentalogy of Fallot Firstly Diagnosed before Emergency Vertebral Surgery
           in an Adult Patient

    • Abstract: Pentalogy of Fallot (POF) is a variant of the more common Tetralogy of Fallot. We report the case of a 32-year-old man with asymptomatic POF, who was admitted to the department of acute medicine due to fracture of the lumbar vertebrae, and lumbar fusion was performed timely. No report of an adult with asymptomatic POF exists in the current literature. After 1.5 years, the patient returned to the hospital in order to remove the lumbar internal fixation.

  • Central Airway Obstruction due to Mediastinal Malignant Lymphoma Requiring
           Venovenous-Extracorporeal Membrane Oxygenation

    • Abstract: Extracorporeal membrane oxygenation (ECMO) therapy might be controversial when patients with advanced malignant disease develop heart or lung failure refractory to conventional management. Especially as for the hematological malignancy patients, the induction of ECMO therapy must be considered carefully, since it is often associated with bleeding complications or infectious diseases. Here, we report a case of life-threatening airway obstruction requiring ECMO. The trachea of the patient was narrowed by an anterior mediastinal tumor too highly to ventilate both lungs, so she had to be connected to venovenous-ECMO (VV-ECMO) before pathological examination and radical treatment were planned. During the intensive care with ECMO, she was diagnosed with malignant lymphoma and the chemotherapy was started. The chemotherapy brought such an immediate result that the trachea regained its patency and ECMO was disconnected 9 days after the initiation of the chemotherapy. Then, the patient was able to leave the intensive care unit with no sequelae. Central airway obstruction is a life-threatening situation, in which prompt decisions are essential. On the other hand, if the airway is impaired by hematological malignancy, it might be generally challenging to consider ECMO as a bridge until the chemotherapy takes an effect. The prognosis of hematological malignancy differs according to its subtype or stage, so it may be reasonable to take ECMO therapy into consideration for some population.

  • Obstetric Management of Copperhead Snake Envenomation in Pregnancy: A Case
           Report

    • Abstract: Snakebites in pregnancy can result in significant maternal and fetal harm; however, the literature to guide management of this rare obstetric complication remains limited. We describe our approach to envenomation in pregnancy based on the currently available evidence. A 27-year-old G2P1 female presented at 27 weeks’ gestation after suffering a copperhead snakebite. She received antivenom and antenatal steroids without adverse maternal or fetal event. Antenatal testing was reassuring throughout admission, and she was discharged home with plans for close outpatient surveillance. She later developed preterm premature rupture of membranes and preterm labor, with delivery of a live infant at 33 weeks’ gestation. The risk of adverse maternal and fetal outcomes following snake envenomation in pregnancy may warrant closer antenatal surveillance than has been previously described.

  • Silent Acute Renal Impairment after Low-dose Paraquat Ingestion

    • Abstract: Paraquat poisoning causes fatal multi-organ failure soon after ingestion. Here we report a case of paraquat poisoning presenting acute kidney injury without any preceding signs and symptoms other than nausea and vomiting. A 77-year-old man was admitted to our hospital because of paraquat ingestion and received fluid resuscitation until the negative conversion of urine paraquat on day 4. Serum creatinine abnormality emerged on the same day, which improved gradually without any specific intervention. Paraquat poisoning can cause silent acute renal impairment. Abundant fluid resuscitation after renal impairment is not essential provided that sufficient fluid is administered beforehand. Repeated screening for renal impairment and sufficient fluid resuscitation are important in the early phase of low-dose paraquat poisoning.

  • Erroneously Analyzed F-Wave of Bradycardic Atrial Fibrillation as
           Ventricular Fibrillation for a Cardiac Arrest Patient

    • Abstract: The prognosis of out-of-hospital cardiac arrest has improved in Japan due to the increasing availability of automated external defibrillators. However, priority should be placed on specificity to not perform unnecessary waveforms rather than sensitivity to perform defibrillation on necessary waveforms. This report describes a case in which the f-wave of bradycardic atrial fibrillation was erroneously analyzed and defibrillated. Health care workers should be aware that the f-wave of bradycardic atrial fibrillation may be erroneously assessed as fine ventricular fibrillation; they should change it to the manual mode and respond as necessary.

  • Successful Conservative Treatment of Traumatic Intraperitoneal Bladder
           Rupture in a Child: A Case Report

    • Abstract: The urinary bladder is protected by the pelvic bones and hence is a site less susceptible to external force injury than other organs. As children’s bones are not fully developed, the risk of bladder injury by external force is greater in children than adults. Pelvic fractures often cause the bladder to rupture, as do strong external forces when the bladder is filled. Bladder rupture is categorized as extraperitoneal or intraperitoneal, and treatments differ between the two. Extraperitoneal injury usually heals after drainage alone, whereas intraperitoneal injury usually requires surgical intervention such as a combination of surgical repairs and drainage. Herein, we report a pediatric case of intraperitoneal bladder rupture successfully treated without surgical repair (i.e., with drainage via an intraperitoneal tube and a urethral catheter only). Our report suggests that nonoperative treatment is an option for pediatric patients with intraperitoneal bladder rupture if certain conditions are met.
