  • Comparison of skin staples versus nylon mattress sutures for closure of
           low midline cesarean section incision in patients undergoing cesarean
           section at Fang Hospital , Chiang Mai Province

      Abstract: ABSTRACT BACKGROUND                  Closure of low midline cesarean section incision , a variety of skin closure techniques can be used; for example interrupted suture, subcuticular suture or even skin staples. post-operative interrupted vertical mattress nylon suture less wound dehiscence but after stitch off  may be not beautiful. Staples used as an alternative to speed of skin closure ,less pain level and wound infection. Therefore ,choices of skin closure techniques depends on the experience or discretion of the surgeon and and satisfaction of patients. OBJECTIVE                 To compare post-operative pain after cesarean section ,time taken in skin closure ,rates of would infection and satisfaction of patients from two skin closure techniques- skin staples interrupted vertical mattress nylon suture. METHODS                 A retrospective descriptive study was conducted in 357 cesarean section patients from October 2016 - September 2017 in Fang  Hospital. Data collected from medical records comprised of patients history and physical examination. Descriptive statistics and Independent t test and Fisher’s Exact test were used to analyzed the data. RESULTS                 Time taken in skin closure was associated with statistically significant at 0.05. No incidence wound infections occurred in neither group. CONCLUSION AND DISCUSSION                 Staples was superior to conventional suturing in time consuming but had higher price. Therefore ,choices of skin closure techniques depends on the experience and discretion of the surgeon. or financial situations of hospitals.   • Innocent murmur in early childhood in Uttaradit province

      Abstract: ABSTRACT Background Heart murmurs are commonly diagnosed among children. Although most of them are primarily innocent murmur on physiologic condition without structural heart defects, it may be the first sign of a structural cardiac disease.  Heart murmur is the sign that should not be ignored since it may cause concerns for both parents and physicians. Objective The purpose of this study is to determine the incidence of innocent murmur in healthy early childhood who have heart murmur in Uttaradit province. Methods Retrospective review was conducted among healthy early childhood, aged between 1- 6 years old in Uttaradit province. They had been auscultated a heart murmur by pediatricians and further performed an echocardiogram between September 1st – 30th, 2018. Results According to the study, among 4,100 healthy early childhood (52.4% male), it was found that 38 of them had a heart murmur (0.9%). When further performed an echocardiogram for a definite diagnosis, 36 of them (94.7%) were innocent murmur (52.8% male) with a 4.3±1.5 years on average age, 16.7±5.1 kg on average weight, 101.5±11.4 cm on average height, 0.5±0.3 on average CT ratio. Therefore, overall average data indicated no statistically significant between males and females. The murmurs were systolic ejection murmur grade 2-3/6 and best heard at lower left parasternal border which is the characteristic of an innocent murmur, in Still’s murmur category. Chest X-ray and EKG examination showed normal findings in this group. Conclusion and discussion The innocent murmur is commonly found among healthy early childhood. Their heart murmur has a unique sound characteristic. A complete history and physical examination together with an initial chest X-ray and/or EKG examination are sufficient to diagnose an innocent murmur. To this extent, general practitioners should familiarize themselves with listening to innocent murmur in order to reduce unnecessary investigation.   • Oral Medication Use Behaviors among Elderly patients with chronic diseases
           in Tambom Hua Ngom, Phan District, Chiang Rai province

      Abstract:                 Nowadays, Thailand is becoming an aged society. Most elders have at least one chronic disease. Polypharmacy is also common and can lead to inappropriate medication use behaviors. This research aimed to study medication adherence and medication use behavior among elderly patients with at least 2 chronic diseases (Hypertension, Diabetes Mellitus, Dyslipidemia or Chronic heart disease) who received medication from Hua Ngom Sub-district Health Promoting Hospital, Phan District, Chiang Rai during 1 January - 1 December 2017. Data collection was an interview and questionnaire consisting of questions about demographic factors, medication use behavior and Morisky Medication Adherence Scale 8-item question in Thai version.                The results of the samples report appropriate medication use behavior 82.20 percent. Also, the Morisky medication adherence level was at high adherence for 79.03 percent. However, the inappropriate medication use behavior of checking the expiration date was at an inappropriate level of 90.30 percent. None of demographic factors were related statistically significant with Morisky medication adherence level.                This result strongly suggest emphasizing health education about medication expired date, giving strongly instruction about checking date before using drug and also advice appropriate medication use and behavior to patient with trustworthy behavior.   Keywords : Elderly patients, Chronic diseases, Medication used behavior, Medication adherence

      Abstract: ABSTRACT BACKGROUND                    Sepsis is status that increasing of incidence and it is the first of hospital death. The application of clinical practice guideline for sepsis by an effective multidisciplinary team can reduce the mortality rate of patients. OBJECTIVES          To compare clinical outcomes before and after application of clinical practice guideline and to study the operational quality of care for patients with sepsis. METHODS          The study was an observational study with historical control. The data of pre-guideline period collected by retrospective review of medical records during October 2016 – March 2017 were compare to those of post-guideline period collected during April 2017 – September 2017.  Data were analyzed by using descriptive statistics and chi-square test, Fisher Exact test and Independent t test respectively. RESULTS          246 patients with sepsis or septic shock were included. 85 patients were in pre-CPG periods and 161 patients were in post-CPG periods. The mortality rate was reduced from 28.2% to 21.1% meanwhile, rate of the hemocultures test before antibiotics prescription and initial therapy within 1 hour of sepsis onset decreased from 81.2.7% to 60.2% and 44.7% to 29.2% Furthermore, rate of fluid replacement therapy within 1 hour and rate of intensive management within 3 hour decreased from 20.0% to 19.3%.  CONCLUSIONS AND DISCUSSIONS  
               The CPG for sepsis is effective in reducing in-hospital mortality rate. The reciprocal decrease in process results indicates that the CPG implementation might imperfect and requires further improvement.

  • Risk Factors for Pulmonary Tuberculosis in Type II Diabetic patient in
           Primary Care Unit at Chiang Rai Prachanukroh Hospital

      Abstract: Diabetes increases the risk of pulmonary tuberculosis (PTB). Early detection and early treatment of PTB is necessary. Continuously increase number of diabetic patients can be a challenge for Thai public health policy and practice in screening for PTB. Identifying risk factors for PTB among the diabetic patients can be helpful for selective PTB screening policy. Objective: To determine risk factors for PTB in Diabetic patients in Primary Care Unit at Chiang Rai Prachanukroh Hospital. Method: A retrospective case-control study was conducted. Medical records of Diabetic patients with PTB (Case group) and Diabetic patients without PTB (Control group) from January 2012- December 2017 were reviewed. Multivariable logistic regression analysis was performed to identify the risk factors and interpreted with adjusted odds ratios (AOR) and 95% of confident intervals (95% CI). Result: a Total of 205 cases were reviewed. There were 52 cases in case group and 153 cases in control group. This study showed risk factors for PTB in Diabetic patients were: male gender (AOR 3.11; 95%CI 1.33-7.30;p-value =0.009), Body Mass Index (BMI) < 18.4 kg/m² (AOR 80.88; 95% CI 15.77-414.91;p-value=<0.001) HbA1c level at 7.1-8%, 8.1-9% and >9% (AOR1.33; 95% CI 0.45-3.99;p-value=0.605), (AOR2.06; 95% CI 0.66-6.41;p-value=0.210) and (AOR3.78;95% CI 1.25-11.42;p-value=0.018), respectively. Conclusion: Screening program and routinely monitoring for PTB among diabetic patients should focus on diabetic patients who are male, having poor glycemic control and low BMI. Early detection and early treatment will improve in mortality and control of the PTB.   Key words   pulmonary tuberculosis, Diabetes, HbA1C, risk factors
