  • Incidence of parasitic infections of in- and outpatients at Maharaj Nakorn
           Chiang Mai hospital, Chiang Mai University, Thailand 2012-2016

      Abstract: Objective The objective of this study was to determine the incidence of parasitic infections in in- and out-patients at Maharaj Nakorn Chiang Mai Hospital, Chiang Mai University, Thailand during the period 2012 through 2016. Methods Stool, blood, and sputum samples of patients were examined for parasite infections at the laboratory of the Department of Parasitology, Faculty of Medicine, Chiang Mai University. Stool samples were examined for intestinal parasites using the formalin-ether concentration technique, modified acidfast staining, and simple smears. Stool and sputum samples were cultured for Strongyloides stercoralis using an agar plate technique. Blood samples were examined for malarial parasites using Giemsa staining of thick blood films. Results During the five year study period, a total of 18,967 stool specimens were examined of which 1,268 (6.69%) were positive for at least one intestinal parasite. Helminthic infections were found in 6.02% of cases and 0.67% had protozoan infections. The most frequent helminth and protozoa among infected patients were S. stercoralis (58.60%) and Giardia lamblia (4.18%). The prevalence of malaria infection was 2.94%, with a higher positive rate for Plasmodium vivax (1.99%) than P. falciparum (0.95%). The annual incidence of intestinal parasites during the period 2012 through 2016 decreased significantly from 7.99% to 5.78%. Malaria infections between 2013 and 2016 also decreased from 3.23% to 1.76%, but the change was not statistically significant. Conclusion There has been a decrease in the incidence of parasitic infections in patients presenting at the hospital over the study period. This may reflect improvement of the hygiene of people in northern Thailand in general. However, this information may not be representative of the parasitic infection situation of particular groups, e.g., hill tribe communities and other groups living in isolated rural areas.

  • Effects of smog on respiratory muscle strength and risk of chronic
           obstructive pulmonary disease in healthy people

      Abstract: Objective  Smog impacts the risk of respiratory disease.  However, the effects of smog on the healthy people have not been intensively investigated. Therefore, this study aimed to evaluate the influences of smog on maximal respiratory pressure, peak expiratory flow rate (PEFR) and risk of chronic obstructive pulmonary disease (COPD) in healthy people. Methods  A total of 400 voluntary healthy subjects, aged between 30-87 years old were recruited.  All participants have been inhabited in Phayao province for at least 3 years. The maximum respiratory pressure was determined by using respiratory pressure meter (MicroRPM®).  The PEFR was assessed by using Mini Wright peak flow meter and the questionnaire was used for analysis the risk of COPD. Results  The results showed that there was no difference of PEFR level in the volunteers who had been long term exposed to smog compared with standard value, with the p=0.694. However, the maximum inspiratory pressure and maximum expiratory pressure of the participants were significantly reduced to 22.46±17.10 and 17.93±35.17 cm H2O, respectively, with the p <0.001.  Moreover, 4.5 percent of the healthy volunteers have risk to be COPD. Conclusion  Smog caused decreasing of maximum inspiratory pressure and maximum expiratory pressure and the people who have been long term exposed to smog have a high risk to be COPD.

  • Sport injuries of Chiang Mai University athletes and sport medicine
           management in The 44th University Sports of Thailand, academic year 2017

      Abstract: Objective  This study aimed to describe the data of sports injuries and sport medicine management of Chiang Mai University’ athletes in the 44th University Sports of Thailand, 2017. Methods  The study reported and analyzed data of sports injuries and sport medicine management of 236 CMU athletes (142 men, 94 women) who participated in 21 types of sports were collected for descriptive analysis (i.e., frequency, percentage).   Results  Analytic report found that top five of on-field sport injuries mainly occurred in taekwondo (57.94%), rugby (14.96%), hockey (10.28%), basketball (8.41%) and jujutsu (3.74%). Most of the injury parts were the region of thigh, ankle, knee, wrist & hand, leg and arm, respectively.  Majority of on field managements were cryotherapy, sport taping, sport massage, stretching, and cooling spray.  For treatment service at the CMU Medicinal Clinical Service Center, top five of sports that were treated including rugby (28.55%), hockey (21.15%), swimming (13.78%), taekwondo (9.62%) and volleyball (8.65%). Most of the regions that were treated comprising ankle, shoulder, leg, knee and thigh, respectively.  Most kinds of management methods at the CMU Medicinal Clinical Service Center were cryotherapy, therapeutic ultrasound, stretching, sport taping, sport massage and therapeutic exercises, etc.  Conclusion Majority of sport injuries occurred in the collision contact sports.  Lower extremities were the major parts of body, which were reported for injuries.   The main treatments that the athletes received were cryotherapy, sport taping, manual therapy techniques and exercises.  Therefore, descriptive patterns of sport injuries and treatment programs are crucial for sport medicine in both of man power preparation and sport injury management.

  • Efficacy of dose modification of gefitinib after hepatotoxicity in
           late-line treatment of EGFR mutated advanced non-small cell lung cancer: A
           case report

      Abstract: Targeted therapy with epidermal growth factor receptor tyrosine kinase inhibitors (EGFR TKI) is a standard treatment for advanced epidermal growth factor receptor (EGFR) mutated non-small cell lung cancer. There is information evidence demonstrating the efficacy of progression-free survival in either both first line treatment or and second-line treatment. However, there is only limited information on the efficacy of treatment and dose modification on the occurrence of medication side effects, especially in late-line treatment patients, and most is limited to case reports rather than clinical studies. In this study we describe the use of gefi tinib in late-line treatment where the gefi tinib dosage was adjusted reduced to 250 mg every 2 days aft er patients had developed hepatitis due to gefitinib.  There was a 21 week progression free interval which included a 13 week progression-free interval of following dose modification.

  • Oxytocin, breastfeeding, and postpartum depression: A review of the

      Abstract: More than 25% of perinatal women are affected by depressive symptoms that can lead to serious maternal, fetal and child consequences. There is growing evidence that biomarkers can predict postpartum depression, one of which, oxytocin, is one of the most studied hormones.  This review focused on associations between oxytocin, breastfeeding and postpartum depression.  Oxytocin plays an important role in breastfeeding and also has psychological effects, e.g., reducing stress and promoting social attachment. Accumulating evidence suggests that oxytocin level can predict postpartum depression in various onset following childbirth. As exclusive breastfeeding has been shown to have a normalizing effect on oxytocin level, breastfeeding might be a factor in protecting against postpartum depression. Integrated studies of oxytocin level, maternal behavior, breastfeeding and psychosocial factors, however, have been rare.

  • How Important are Posterior Tibial Plateau Fractures' A review of the
           incidence, fragment characteristics, and mechanisms of injury

      Abstract: Posterior tibial plateau fracture is a common feature found in complex tibial plateau fractures. However, the fracture characteristics of posteromedial and posterolateral fragments are different. Various mechanisms of tibial plateau injuries result in diverse fracture types which require appropriate treatment strategies. To emphasize the importance and diversity of posterior tibial plateau fractures, the incidence and characteristics of those fractures is reviewed. In addition, integration of knowledge of mechanisms of injury and fracture characteristics with the selection of surgical procedures is discussed.
