  • Intestinal Diverticular Disease: A Review of Radiologic Findings and

      Abstract: Intestinal diverticular disease, which once thought to be a disease of the elderly, is now also becoming more and more common in younger age group. However the prevalence of this disease is still increasing due to the rising of the average age. Most common complication of diverticular disease is acute diverticulitis. With the progression and advancement of radiologic investigation today, the managements of choices have been changing the past few years. The objectives of this article are to review indication of each imaging modality, to identify its radiologic findings and to update on choice of managements.

  • Therapeutic exercise in Systemic Sclerosis

      Abstract:           Physical therapy has significant role in systemic sclerosis. Therapeutic exercise delay or decrease hand deformity, expand the lung. Because systemic sclerosis increases rate of collagen fiber reproduction and found in every part of the body, especially in skin, hand and internal organ. Extremely collagen fiber reproductions have an affect on skin, hand and internal organ sclerosis. Decrease quality of life in finally time. Physical therapy and therapeutic exercise improve mobility of body, hand and breathing. Prevention or decrease disability and increase quality of life for systemic sclerosis patients.

  • Effects on maximum oxygen uptake and muscular strength during an interval
           training program for people working within confined spaces

      Abstract: Objectives  To study the effects on maximum oxygen uptake and muscular strength during an interval training program at 70 – 75 percent maximum heart rate (MHR) . Methods  A sample group of twenty-eight volunteers (all Bangpakong Power Plant workers with duties requiring them to work within confined spaces), participated in this study.  The interval training program exerted the subjects for three minutes at 70 -- 75 % MHR, with three-minute recovery periods at heart rates under 50 % MHR.  The interval training program took place three days a week for an eight-week training period.  The participants trained in 5 sets for the first four weeks (thirty minutes per day), and 6 sets in the last four weeks (thirty-six minutes per day).  The Astrand-Rhyming test and Grip and Leg strength tests were used to measure maximum oxygen uptake (VO2max) and muscular strength, respectively.             A dependent t-test was utilized for pre-training and post-training data analysis, with the significance level set at 0.05. Results  The results showed that after 8 weeks of interval training, the maximum oxygen uptake rates revealed statistically significant increases from 26.72 (SD 5.88) to 30.66 (SD 6.16) ml/kg-1/min-1.   Additionally, measurements of muscular strength showed statistically significant increases from 1.77 (SD 0.58) to 2.25 (SD 0.53) kg-1/weight (for leg strength). Conclusion  It was found that after 8 weeks of the interval training program at 70 – 75 % maximum heart rate, maximum oxygen uptake and muscular strength in people working within confined spaces have predominantly improved.

  • Surgical complications of Tenckhoff catheters used in Continuous
           ambulatory peritoneal dialysis (CAPD) in End Stage Renal Disease (ESRD)
           patients at Burapha University Hospital

      Abstract: Background: Half of all of the end stage renal disease patients in Thailand use the continuous ambulatory peritoneal dialysis. They need surgical intervention for catheter placement permanently. This type of renal replacement therapy carry on many surgical complications; peritonitis, catheter malfunction and hernia e.g. these complications need surgical treatment.    Objective: Analyze clinical characteristic and factor which associated with surgical complications that need surgical treatment in the end stage renal disease patients who use the continuous ambulatory peritoneal dialysis who were underwent peritoneal catheter placement at Burapha University Hospital.   Material and Method: Reviewed the medical record of the end stage renal disease patients who were underwent peritoneal catheter placement at Burapha University Hospital between 1 August 2010 to 31 July 2016, collected data; sex, age, body weight, height, underlying disease, anticoagulant or antiplatelet usage, surgical technique, operating time, onset of first peritoneal dialysis after catheter placement, volume of dialysate and surgical complication were analysed to find the factor that influence the surgical complication.         Results: One hundred fifty six patients were recruited. Mean age 59.5 year (19-89 years) mostly (58.3%) in 40 to 60 years old range, body mass index mostly normal range (64.7%), anticoagulant or antiplatelet usage 12.8%, diabetes mellitus 12.8%, mostly were below umbilical incision for catheter placement (79.5%), operating time mostly less than 1 hour (95.5%), first peritoneal dialysis mostly more than 7 days after surgery (97.4%) and dialysate volume mostly 1.5 litres (58.9%). Overall surgical complications were 78 patients (50%) mostly were infection (29.5%) (28.8 patient-month), catheter malfunction 14.1%, hernia 4.5%. Factor associated with infection was diabetes mellitus odds ratio = 2.78 (95% CI 1.0681 - 7.2239), p = 0.0362. Factors associated with hernia were diabetes mellitus odds ratio = 11.08 (95% CI 2.2732 - 54.0395), p = 0.0029 and first peritoneal dialysis less than 7 days after surgery odds ratio = 29.40 (95% CI 3.4139 - 253.1910), p = 0.0021.     Conclusion: The first three surgical complications of the end stage renal disease patients who use the continuous ambulatory peritoneal dialysis were infection, catheter malfunction and hernia. Diabetes mellitus increased risk of infection that need surgical removal of catheter and increased risk of hernia that need surgical repair. The first peritoneal dialysis less than 7 days after surgery increased risk of hernia in peritoneal dialysis patient.

  • Collected opinions of medical students on studying the history ofmedicine
           as part of their orientation program atSomdejphrajaotaksinmaharaj
           Hospital’s Medical Education Center in Tak Province

      Abstract: Introduction Somdejphrajaotaksinmaharaj Hospital’s Medical Education Center has implemented
      a training model on medical history as part of the orientation program for 4th year medical
      students who are preparing for their real, hands-ontraining in clinical rotation.
      Objective To study the opinions of medical students undergoing a training model on the history
      of medicine in theirorientation program in 2017.
      Materials and Methods This research undertaking was a qualitative study. The key informants
      were 4thand 5th year medical students in 2017, who participated in a new training program in
      the history of medicine. In-depth interviews with these students, upon the completion of their
      clinical rotation (as well asreflective papers), supplied the data for this research. The SCAT
      method was implemented to analyze the data.
      Results The opinions from these medical student safter haven taken the training model in
      medical history during their orientation program included not only a more equipped systematic
      thinking process,an increase in confidence and affirmative attitude, and a better understanding
      of patient feelings; but, alsothe study and review onthe basics of clinical history, which can
      promote opportunities for self-development. Futhermore, a good relationship occurred between
      the senior and junior year clinical students throughout the training period. After the students
      went through their clinical rotation, most participants agreed that this activity was very necessary.
      Conclusion A training model in the history of medicine helps medical students increase their
      essential studying skills during their clinical rotation.

  • The effect of cleaning solution residues on microbiological testing

      Abstract: Objective: To compare the effect of glassware cleaning solution residues and the different rinsing procedures on microbial testing in microbiology laboratory. Materials and Methods: The 2.0% glassware-washing solution for laboratory purpose (reagent type A) composing of polyethylene glycol dodecyl and ethylene oxide, 2.0% dish-washing reagent (reagent type B) composing of linear alkylbenzene sulfonate,  potassium salt and sodium lauryl ether sulfate, and 0.5% detergent (reagent type C) composing of anionic surfactant, zeolite, sodium carbonate, sodium carboxymethyl and cellulose were used to evaluate the detergent residues after performing 5 different rinsing procedures (No rinsing, rinsing for 1, 3, 6 and 12 times) on petri dishes. Sterile glass petri dishes were included as negative control. The pH of cleaning residue was determined using 0.04% Bromthymol blue. The 50-100 CFU/mL of Staphylococcus aureus (ATCC 6538) was used to explore the efficiency of bacterial growth on glass petri dishes with pour plate method and reported as mean CFU/mL with standard error of the mean (SEM). Statistical analysis was performed with either IBMÒ SPSSÒ version 23 or GraphPad PrismÒ version 6.01. The p-value < 0.05 indicated statistical significant at 95% confident interval.     Results: The reagent type A was alkaline and became neutral after rinsing for 3 times. Additionally, the reagent type B was acidic and turned into neutral after rinsing for 3 times. Moreover, the reagent type C was alkaline and became neutral after rinsing for 6 times. All 3 detergents affect microbial growth at statistically significant level (p < 0.05) in the conditions of no rinsing, rinsing for 1, 3 and 6 times. Using reagent type A or B following by rinsing for 3 times did not show statistically significant in bacterial growth (p > 0.05). However, the bacterial growth was not significant different in the use of reagent type C comparing with reagent type A or B. When considering the rinsing procedures less significantly affecting bacterial growth         (p > 0.05), the suggested protocols for using reagent type A or  was 3-12 times whereas the protocol for reagent type B was 6-12 times and the protocol for reagent type C was 12 times. Conclusion: Each cleaning solution has different pH properties in detergent residues, which is solved by rinsing protocols.  Moreover, different types of detergent and rinsing procedures significantly affect the bacterial growth. The suitable protocols for each washing solution are rinsing at least 3 time for 2.0% glassware-washing solution, rinsing at least 6 times for 2.0% dish-washing reagent and rinsing at least 12 times for 0.5% detergent, respectively.
