  • Hiperparasitemia por Plasmodium falciparum como criterio diagnóstico de
           malaria grave. Fotografía médica.

      Abstract: Paciente masculino de 37 años de edad, procedente de Santa Lucía, estado Miranda, quien refiere fiebre diaria no cuantificada de una semana de duración precedida de escalofríos y sucedida por diaforesis; concomitantemente cefalea, artralgias y lumbalgia. El paciente niega episodios maláricos anteriores. Es hospitalizado, encontrándose palidez cutánea, tos seca y hepatoesplenomegalia al examen físico. Se toma muestra de sangre periférica para realizar gota gruesa y extendido con coloración de Giemsa invertido y se observa a microscopía óptica (aumento de 1,000X, Nikon Eclipse E200®). Nótese la gran cantidad de trofozoítos anulares en la gota gruesa, mientras que en el extendido se observa un gametocito, varias formas appliqué y eritrocitos con poliparasitismo. Se evidencia aproximadamente un 20% de eritrocitos parasitados, con una parasitemia estimada de 98.850 parásitos/μL.
      Plasmodium falciparum es el agente etiológico más frecuentemente asociado a malaria grave, dentro de cuyos criterios diagnósticos destacan: hiperparasitemia, distress respiratorio, malaria cerebral, hipoglicemia, insuficiencia renal, entre otros [1]. La hiperparasitemia es uno de los criterios de laboratorio para severidad; una parasitemia >10%, incluso en ausencia de hallazgos clínicos de severidad, indica malaria grave [2,3]. Medical Photograph: hyperparasitemia by Plasmodium falciparum as a diagnostic criterion of severe malariaA 37 year-old male from Santa Lucía, Miranda state, complained of unquantified daily fevers for the past week, preceded by chills and followed by profuse sweating; also, he complained of headaches, as well as pain in his joints and lower back. The patient denied any previous episodes of malaria. Physical examination during hospitalization showed pallor, dry cough and hepatosplenomegaly. A peripheral blood sample was taken for thick and thin smears with an inverted Giemsa’s stain and later observed under optical microscopy (1,000X magnification, Nikon Eclipse E200®). Note the large amount of ring-form trophozoites in the thick smear, while the thin smear shows a single gametocyte, many appliqué forms and red blood cells with polyparasitism. There are approximately 20% of infected red blood cells, with an estimated parasitemia of 98,850 parasites/μL.Plasmodium falciparum is the species most frequently associated to severe malaria, which includes the following as its most distinguished diagnostic criteria: hyperparasitemia, respiratory distress, cerebral malaria, hypoglycemia, kidney failure, among others [1].   • Ausencia de la fisura interlobular horizontal, otro ejemplo de la
           variabilidad anatómica.

      Abstract: Anatómicamente, el pulmón derecho suele dividirse en 3 lóbulos, superior, medio einferior, separados los lóbulos superior y medio por la fisura horizontal y el lóbuloinferior de éstos por la presencia de la fisura oblicua [1]. Ésta clásica división permite al tejido pulmonar funcionante expandirseuniformemente y reacomodarse dentro de la cavidadtorácica durante el proceso de la ventilación [2].En la fotografía puede observarse la visiónlateral del pulmón derecho de un feto femenino de 33semanas de gestación donde se evidencia la ausenciade la fisura horizontal, a la disección del hilio pulmonarhomolateral se observa la presencia de las estructurasbronquiales, arteriales y venosas correspondientes alos lóbulos superior, medio e inferior.Esta variante, descrita como ausencia completade la fisura oblicua, es evidente entre el 4 y el 11% de lapoblación en general [2-4], la cual se ve involucrada enmúltiples fallas diagnósticas y terapéuticas, por lo quedebe de ser considerada al momento de la evaluación deestudios imagenológicos, radiológicos o tomográficos,así como procedimientos quirúrgicos como lalobectomía con el fin de garantizar la indemnidad deltejido pulmonar, disminuyendo el riesgo de la apariciónde complicaciones postoperatorias [4].

  • Competencias de los estudiantes del sexto año de medicina de la
           Universidad del Zulia (LUZ) para comunicar malas noticias.

      Abstract: En medicina, la comunicación de la verdad, cuando se desea informar malas noticias, se convierte en una situación estresante para los médicos, entendiéndose como mala noticia cualquier información que afecte negativamente la visión de un individuo sobre sí mismo y su futuro. Actualmente en la Facultad de Medicina de la Universidad del Zulia (LUZ) se evidencia déficit en la formación curricular de las destrezas de comunicar malas noticias, debido a que la educación se basa en enseñar aspectos técnicos y se descuidan aspectos éticos de la medicina. El propósito de esta investigación consiste en determinar las competencias para comunicar malas noticias de los estudiantes del último año de la Escuela de Medicina de LUZ. Se realizó un estudio de campo, descriptivo, de tipo encuesta transversal, con una población de 800 estudiantes del último año de medicina, con una muestra de 146, a quienes les fue aplicada una encuesta online a través de Google Forms, constituida por 38 ítems, evaluando diferentes dominios (cognoscitivo, actitudinal, metodológico, procedimental y ético-legal), analizando las dos preguntas más polarizadas de cada dominio. Los resultados indican que los estudiantes actuarían con acciones similares a las de los protocolos, aun cuando los desconocen, lo que indica que los conocimientos adquiridos son empíricos y no por educación formal. Un gran porcentaje desea instrucción sobre comunicar malas noticias y así abarcar al paciente integralmente. Finalmente, se determina que la mayoría se siente incapaz de comunicar malas noticias, por lo que se sugiere incluir este tópico en el diseño curricular de la carrera de medicinaCompetencies towards breaking bad news in sixth year medical students from the Zulia University (LUZ), VenezuelaIn medicine, conveying a truthful message when breaking bad news has come to be a very stressful situation for physicians; considering a bad news as any information that negatively affects a patient’s perception about himself or his future. Currently, in the School of Medicine of the Zulia University (LUZ) there is a noted deficit in curricula for the developing of skills in breaking bad news, mostly because of a lack in teaching ethical aspects of medicine in favor of more technical aspects of it. The purpose of this research consists in the determination of competencies for breaking bad news among senior medical students at University of Zulia’s School of Medicine. A descriptive, survey type, transversal field study was performed, recruiting a sample of 146 students, who responded an online form delivered through Google Forms, constituted by 38 questions, evaluating different domains of competency (cognitive, attitude, methodology, procedural, ethical and legal). We then analyzed the two more polarized items of each domain. The results indicate that students would act competently according to current guidelines, even in the absence of formal knowledge, which may indicate empirical competency in the context of lack of formal training. The majority of these students would like to be formally trained about breaking bad news in order to offer better care for their patients. Lastly, most of the students perceive their own competency on breaking bad news as poor, suggesting that this kind of training should be included formally into the curricular design of this career.

           Multidisciplinario de Investigación y Servicio 2015, Maniapure,

      Abstract:  En las comunidades indígenas de Latinoamérica persisten enfermedades relacionadas con necesidades básicas insatisfechas, siendo las principales registradas en los centros de salud las diarreas, el paludismo y las infecciones del tracto respiratorio. En base a esto, en el año 2013 la Organización Panamericana de la Salud plantea la transformación de los sistemas de salud: incluyendo y conjugando la medicina tradicional con la investigación. Por ello en agosto de 2015, la Sociedad Científica de Estudiantes de Medicina de la Universidad Central de Venezuela (Sociem-UCV) realizó el II Campamento Universitario Multidisciplinario de Investigación y Servicio en la región de Maniapure, estado Bolívar, Venezuela, en donde estudiantes de medicina de distintas universidades brindaron servicio médico a las comunidades de la zona, así como en el ambulatorio tipo II “la Milagrosa”. Los datos fueron recolectados con las historias clínicas y registros de morbimortalidad utilizados durante las jornadas. Se registraron datos de 305 pacientes, que abarcaban, tanto de la etnia Eñepá, como criollos. El motivo de consulta más frecuente fue tos en 69,83% de los pacientes, seguido de 30,16% fiebre y relacionados con el área gastrointestinal. Entre los principales diagnósticos: 45,3% de los pacientes atendidos eran pacientes sanos, seguido de 17,7% con rinofaringitis aguda y 6,9% síndrome diarreico. Los síntomas más frecuentes pudieran relacionarse con el principal diagnóstico realizado en las jornadas, la rinofaringitis aguda, encontrando que las infecciones aéreas superiores son la primera afección de las comunidades estudiadas, por encima de la diarrea y enteroparasitosis que han sido descritas clásicamente como las principales. Evaluation of Health volunteering: Multidisciplinary College Camp for Research and Service 2015, Maniapure, Venezuela.  In the native’s communities of Latin America there is a prevalence of diseases related to unsatisfied basic services, the prevalent registered in health centers are: diarrhea, malaria and respiratory tract infections1. Based on this, in 2013 the Panamerican Health Organization proposes the transformation of health systems: including and combining traditional medicine with the investigation. So in August 2015, the Scientific Society of Medical Students of the Central University of Venezuela (Sociem-UCV) made the II University Camp Multidisciplinary Research and Service in the region of Maniapure, Estado Bolivar, Venezuela, where medical students from different universities provided medical services to communities in the area as well as type II ambulatory “La Milagrosa”. Data were collected with medical records and records of morbidity and mortality used during the conference. 305 patients were attended, including the Eñepa ethnic and Creole. The most frequent reason for consultation was coughing in 69.83% of patients, followed by 30.16% fever and related with gastrointestinal area. The frequent diagnoses: 45.3% of patients were healthy patients, followed with 17.7% Acute Nasopharyngitis and 6.88% Diarrheal Syndrome. The most common symptoms may be related to the principal diagnosis made at the conference, acute nasopharyngitis, demonstrating that upper respiratory infections are the first condition of the communities studied, above diarrhea and intestinal parasites that have been described classically as most frequently observed in the natives

  • Inmunología y homeóstasis: conceptos actuales y perspectivas.

      Abstract: La inmunología es un campo que no solo ha tenido un crecimiento enorme en cuanto a su comprensión intrínseca y práctica clínica, sino además en el descubrimiento de su influencia en otros sistemas del organismo de los cuales parecía estar aislada. Es de importancia vital el hecho de que dicha interacción del sistema inmunológico con otros no ocurre solo durante una noxa externa, sino también durante la homeostasis fisiológica. Se ha descubierto que el sistema inmunológico, mas allá de su función de defensa, funge un rol esencial en la regulación de múltiples funciones biológicas, incluyendo la integridad de las mucosas, la hemodinámica e incluso el metabolismo, entre muchos otros. La independencia de los sistemas y sub-sistemas ha de ser revisada, pues una comprensión integradora es requerida. Modificaciones importantes han de ser implementadas en nuestros sistemas educativos a fin de lograr una enseñanza adecuada de los conceptos ubicuos de la inmunología. En esta revisión/artículo de opinión serán discutidos los conceptos modernos
      no-clásicos de la inmunología, con énfasis especial en: infiltración y residencia, microentorno (microenvironment), interacción con células no leucocitarias (estroma/parénquima), funciones no canónicas del sistema inmunológico y leucocitos reguladores. Esta revisión contiene además opiniones e hipótesis de las cuales como autor me hago enteramente responsable. Más que una revisión fáctica puntual, este artículo pretende dibujar la visión holística del sistema inmunológico en la homeostasis fisiológica basado en los conceptos y evidencias más modernas.Immunology and Homeostasis: Current Concepts and PerspectivesImmunology is a field that has grown enormously in terms of its intrinsic understanding and clinical practice. It has been also remarkable the discovery of its influence in other systems of the organism, of which it seemed to be isolated. It is key to point out that interaction of the immune system with others does not occur only during an external insult, but also during physiological homeostasis. It has been discovered that the immune system, beyond its anti-microbial function, plays a critical role in the regulation of many biological functions; including mucosal integrity, hemodynamics and even metabolism, amongst others. The independence of systems and sub-systems has to be revised, since an integrative understanding is required. Important modifications have to be implemented in our educational systems in order to achieve an appropriated teaching method of the ubiquitous concepts of immunology. In this review/opinion article, the modern non-classical concepts of immunology will be discussed, with special emphasis on: infiltration and residence, microenvironment, interaction with non-leukocyte cells (stroma/parenchyma), non-canonical functions of the immune system and leukocytes regulators. This review also contains opinions and hypotheses
      of which as author I am entirely responsible. Finally, more than a specific revision, this article aims to draw the holistic view of the immune system in physiological homeostasis, considering the most modern concepts and evidence.

  • Hipertensión endocraneana idiopática (Seudotumor Cerebri) en una
           adolescente obesa: a propósito de un caso.

      Abstract: La hipertensión endocraneana idiopática (Pseudotumor Cerebri) es una afección con incidencia de 1-3 por cada 100.000 personas, que afecta más mujeres que hombres. La enfermedad está caracterizada por aumento de la presión endocraneana sin causa determinada, la cual cursa con la tríada clásica de vómitos, cefalea y papiledema. Sin embargo, la edad promedio de presentación es de 31 años, por lo que es infrecuente en adolescentes. Se presenta un caso clínico de una adolescente de 12 años de edad, con índice de masa corporal (IMC) de 30,6 Kg/m2, quien presenta dolor de carácter punzante en columna cervical y dorsal, vómitos en escopetazo, diplopía y desviación del ojo derecho hacia el lado interno. Al examen físico impresiona papiledema bilateral, paresia del nervio abducens izquierdo y parálisis del homólogo contralateral. Durante su estancia hospitalaria se asocian nuevos síntomas visuales (escotoma central y disminución de la agudeza visual progresiva). Se realizan neuroimágenes (resonancia magnética cerebral y medular) las cuales no reportan alteraciones y punción lumbar con manometría, en la cual se evidencia elevación de la presión inicial del líquido cefalorraquídeo a 320 mmH2O. En virtud de dichos hallazgos se decide terapia con metilprednisolona, con la cual presenta mejoría parcial de la sintomatología visual y se plantea la realización de punción lumbar como estrategia terapéutica. Se presenta una revisión de la posible asociación entre este síndrome y la obesidad infantil, así como de los criterios diagnósticos de la enfermedad en este grupo etario, en el cual no es común el síndrome. Idiopathic intracraneal hypertension (Pseudotumor Cerebri) in an obese teenager: a case report. Idiopathic intracranial hypertension is a condition with incidence of 1-3 for every 100000 people, that affects more women than men. The disease is characterized by an increase in intracranial pressure with no determined cause, which courses with the classic triad of vomits, headache and papilledema. However, the average age of presentation is 31 years, making it uncommon in teenagers. A clinical case of a 12 years old teenager is presented, with a Body Mass Index (BMI) of 30,6 kg/m2, who presented stabbing pain in cervical and dorsal spine, projectile vomiting, diplopia and deviation of the right eye towards the inner side. Physical examination shows bilateral papilledema, left abducens nerve palsy and contralateral homologous paralysis. During her hospital stay new visual symptoms were associated (central scotoma and progressive decrease in visual acuity). Neuroimaging is done (Brain and spinal magnetic resonance images), which do not report alterations, and lumbar puncture with manometry, in which the initial pressure of the cerebrospinal fluid is elevated to 320 mm H2O. Based on these findings, therapy with methylprednisolone is decided, with partial improvement of the visual symptomatology, and the lumbar puncture is proposed as a therapeutic strategy. A review of the possible association between this syndrome and childhood obesity is presented, as well as the diagnostic criteria of the disease in this age group, in which the syndrome is not common

  • Bloqueo bifascicular en la cardiopatía chagásica:

      Abstract: se trata de una paciente de 50 años de edad, natural y procedente de Los Teques, Miranda, Venezuela, con serología positiva para Tripanosoma cruzi (IgG), su electrocardiograma de 12 derivaciones, revela ritmo sinusal, frecuencia cardiaca dentro de límites normales, QRS mayor de 0,12 segundos, rSR’ en derivación V1, y RS y QS en I y V6, acompañado de morfologías rS en II, III y AVF, qR en AVL y el eje del QRS entre -90° y -45°; cumpliendo los criterios de bloqueo avanzado de rama derecha y bloqueo de la subdivisión anterior de la rama izquierda de has de His [1]. La enfermedad de Chagas crónica suele manifestarse con trastornos de conducción intraventricular, por lo cual ante este electrocardiograma en un país endémico como Venezuela es necesario incluir la enfermedad de Chagas en los planteamientos diagnósticos, para instaurar terapia y prevenir la aparición de cardiopatía congestiva y arritmias ventriculares con riesgo de muerte súbita [2,3].  Bifascicular block in Chagas heart disease: Medical picture A 50 years-old woman who lives in “los Teques”, Miranda, Venezuela, with positive serology for Tripanosoma cruzi (IgG), has a 12-lead electrocardiogram with sinusal rhythm, normal heart rate, QRS larger than 0.12 seconds, rSR’ in lead V1, RS and QS complexes in leads I and V6, accompanied with rS complex in leads II,III, aVF, qR complex in aVL, and QRS angle between -90° and -45°. It fulfills the criteria of advance right bundle branch block and left anterior fascicular block of the bundle of His [1]. Chronic Chagas disease usually debuts with intraventricular conduction disorders, therefore in the context of a patient with this electrocardiogram in an endemic area like Venezuela it is necessary to consider Chagas disease within the diagnostic possibilities. In this way the therapy could be started, preventing heart failure and ventricular arrhythmias with risk of sudden death.[2,3].
