  • Press-Fit® (Natural Collagen Plug) Application in Perianal Sinus
           Treatmen

      Abstract: Objective: Although the pilonidal sinus is most commonly seen in thesacrococcygeal region, it may emerge in any region of the body, where the hairsmay penetrate. The pilonidal sinus that develops in the perianal area is called'endoanal' or 'perianal pilonidal sinus' and it is in the close neighborhoodwith the anal verge.  In this study, ourobjective was to present the patients with "perianal pilonidalsinus", whom we treated with the "Natural Collagen Plug", whichis mainly used for the perianal fistulae.Materials and methods: The data of four patients, who applied to ourclinic due to the perianal pilonidal sinus between June and December 2015 andtreated with “natural collagen plug” after curettage, were included in thestudy.  The characteristics of patientssuch as age, gender, treatment duration, complications and recurrence rateswere investigated. Findings: The mean age of the patients was 25.75 ± 3.5 years and all patientswere males. The treatment was administered twice to one patient and just onceto the remaining 3 patients. Only one patient developed abscess as acomplication. Average recovery time was 27.5 ± 3.3 days. During the follow-up(average: 5.75 ± 1.7 months) period, the disease recurred in one patient andone patient did not recover. The success rate of the procedure was 50% eventhough the number of patients was very limited.Conclusion: Although the perianal pilonidal sinus disease is a rare condition,it is difficult to treat due to the close neighborhood to the anal verge. Eventhe duration of healing is long and the success rate is relatively low in thesepatients, “natural collagen plug” application after a thorough local removal ofthe hairs and debridement can be taken into consideration. This method can beused safely around the anal region.

  • Control of Mitochondrial Integrity of Aging

      Abstract: Aging is a natural cause ofprogressive decline in tissue and organ functions, leading to an increased riskof disease and death. Among the various factors contributing to human aging,mitochondrial dysfunction is emerging as one of the most important factors.Mitochondrial dysfunction is linked to the development of age-relatedpathologies such as metabolic syndrome, neurodegenerative disorders, cardiovasculardiseases and cancer. Mitochondria are central to the regulation of energy andmetabolic homeostasis and have a complex system to limit mitochondrial damageand provide mitochondrial integrity and function. Severalmolecular and cellular pathways in eukaryotes are involved in controlling thequality and integrity of mitochondria. These pathways areconcerned with the functioning of the organism in a healthy manner throughoutits life cycle. The regulation of the integrity ofmitochondrial DNA (mtDNA) as well as the mitochondrial complexes that determinemitochondrial functions and the regulation of expression are necessary for thereshaping of single proteins. Mitochondria; Understanding of the underlyingmechanisms in genomic, proteomic, organellar and cellular levels is the basisfor intervening in age-related diseases resulting from mitochondrialdysfunctions, degenerative processes, aging and deterioration of mitochondria.Qualitycontrol (QC) systems prevent processes such as degenerative diseases and agingthat cause organ dysfunction. The subject of this review; themitochondrial regulation which causes the elucidation of the aging process,which is still not fully understood today. Pathways to disease andaging in mitochondrial QC; mtDNA repair and reorganization, regeneration ofoxyde aminoacids, refolding and disruption of heavily damaged proteins,degradation of mitochondria entirely by mitofargin, and eventually programmedcell death.

  • Clinical and Laboratory Evaluation of Culture-Proven Neonatal Sepsis in
           Premature Infants

      Abstract: Aim:Inthis study, who were followed up with diagnosis of sepsis in neonatal intensivecare unit in terms of underlying facilitating factors, causative pathogen,clinical course and mortality.Materialand methods: 114 cases of sepsis with proven-bloodculture were included in the study. Early onset neonatal sepsis (ENS), whichgrew in the blood culture sample within the first 72 hours after birth, and lateonset neonatal sepsis (LNS), which grew in the blood culture sample after 72hours. Platelet values <150x109/L were defined asthrombocytopenia, and those with ≥150x109/L were defined as normalplatelet counts. C-reactive protein (CRP) levels >10 mg/dl were divided intotwo groups as high level and ≤10 mg/dl as middle-low level.Results:30.7%of the cases were ENS and 69.3% were LNS. In ENS, group B streptococcus was themost frequently isolated agent in blood culture, whereas Klebsiella pneumoniaewas the most frequently isolated agent in LNS. When the ENS and LNS werecompared between gram positive and gram negative groups, mean platelet valueswere significantly higher in the gram positive group and mean CRP values weresignificantly higher in the gram negative group. Thrombocytopenia in ENS wasseen in only three cases, whereas high CRP values were seen in only one case.InLNS group, the incidence of  patientswith thrombocytopenia and high CRP levels were significantly higher in gramnegative-candida groups than in gram positive group. Conclusion:Gram negative neonatal sepsis is a major problem in neonatal intensive careunits. In preterm babies with LNS, thrombocytopenia and high CRP levels aremore frequent in those who was detected gram negative agent-candida than those who was detected  gram positive agent in blood cultures.

  • An Interesting Cause of Acute Abdomen: Stomach Perforation due to Foreign
           Body

      Abstract: Foreignbody ingestion is a frequent cause of emergency service admission. Althoughfrequent in children, it can be seen in alcoholics and adults with psychiatricdistress or mental retardation. Most of these patients do not require anyintervention, however only a few have surgery or endoscopic procedures. Surgeryis mostly performed in cases such as obstruction, perforation or hemorrhage. Inthis article, we present a case along with literature which was evaluated asacute abdomen in the emergency department. Theetiology explaining the case was notfound in the test sand perforation wasd etermined due to a foreign bodyafter laparotomy.

  • Retrospective Evaluation of the Effects of Albumin and Hemoglobin Values
           on the Duration of Hospital Stay and Mortality in Elderly Patients
           Operated for Hip Fracture

      Abstract: Aim: In this study, we aimed to make contribution to the literature byanalysis the effects of albumin and hemoglobin levels on the duration ofhospital stay and mortality in the patients with hip fracture.Material and method: After the approval of the Ethics Committeewith Decision 2017 / 3-19, 151 cases with a diagnosis of femur fracture afterminor trauma 60 years and over in our clinic between 2015-2016 were included inthe study. Patients' age, sex, mortality rates, duration of anesthesia and typeof anesthesia were examined. The preoperative serum albumin level was above 3.5g / dl, hemoglobin level was above 12 mg / dl and below the hematocrit levelwas above 36%. These groups were divided into two groups according to thelength of stay and mortality Results: The mean age of thepatients was 77.9 ± 9 years and 41.1% of the patients were male. It was foundthat 4.6% of the patients were discharged before the hospital discharge. Theassociation of the groups with the length of hospitalization and mortality wasnot statistically significant (p>0.05). Age and gender were not associatedwith mortality (p>0.05). The SBP, DBP and SPO2 values during the operationwere similar in all time periods in patients with exudative and living conditions(p>0,05). Survival rate of induction and first 30 minutes was significantlyhigher in living group (p<0.05). At other time periods, HR rates betweengroups were similar and the ratios were not statistically significant(p>0.05). The changes in SBP, DBP and SPO2 over time were similar for livingand exiting patients (p>0,05). Conclusion: There are many factors affecting mortality and length of stayin patients with hip fracture surgery, both of which are hemoglobin and albuminlevels. The albumin and hemoglobin values of the patients were found to be low,but the relationship between these values and mortality was not significant. Wethink that studies based on lower albumin and hemoglobin levels may give moremeaningful results.There are manyfactors affecting mortality and length of stay in patients with hip fracturesurgery, both of which are hemoglobin and albumin values. Albumin andhemoglobin values were low in these patients but the relation of these valueswith mortality was not significant. We believe that studies based on loweralbumin and hemoglobin will be more appropriate and we feel that our resultsare meaningless.

  • Distribution of vitamin D levels according to age and gender in patients
           with non-specific muscle pain.

      Abstract: SummaryAim: The aim of this study was to determine the rate of vitamin D deficiencyin patients with non-specific muscle pain and to investigate the distributionof vitamin D levels according to gender and age in this group.Material and methods: In this retrospective study, the data of 2.832patients diagnosed as non-specific myalgia who applied to our polyclinicbetween January 2016 and November 2017 were examined and 188 patients whosevitamin D levels were measured were included the study. Patients were dividedinto 4 categories according to serum 25 [OH]D level [I- Serious deficiencygroup (<10 ng/mL), II- Deficiency group (≥10 to <20 ng/mL), III-Insufficiency group (≥20 to <30 ng/mL) and IV- Normal group (≥ 30ng/ml)]. The mean age and serum 25 [OH]D levels of women andmen were compared. Correlation tests were performed to assess the presence anddirection of the potential relationship between age and serum 25 [OH]Dlevels.  Results: Of the 188 patients included in the study, 170 were female (90.4%) and18 were male (9.5%). The mean serum 25[OH]D levels in female (mean 11.61 ± 5.40ng/ml) were significantly lower than male patients (mean 18.28 ± 7.96 ng/ml).Of the patients, 77 were in the serious deficiency group (40,95%), 91 were inthe deficiency group (48,40%), 14 were in the insufficiency group (7.44%) and 6 were in the normal group (3.19%).There was no significant correlation between age and 25[OH]D levels.Conclusion: This study shows that in about 90% of patients withnon-specific muscle pain complaints were deficient or serious deficient ofvitamin D levels.  Therefore, vitamin Ddeficiency should be considered in the differential diagnosis of non-specificmyalgia and appropriate replacement therapy regimens should be applied in casesof deficiency or insufficiency.

  • Male and Female Players may Pomegranate Blood Lipoproteins Effects of Iron
           and Iron Binding Capacity

      Abstract: Objectives:The aimof this study is to determine the effect of pomegranate juice on the bloodlipoprotein and iron-iron binding capacity of young male and female soccerplayers with sports background, and how and to what extent they influence theperformance of footballers. Research; Şanlıurfaspor (U-19, 17 and 16) maleteams and şanlıurfa Youthspor female football team sports with the age of16-19, ranging from 22 Male players, 22 female, including a total of 44volunteer footballer was determined and the study was conducted.  Determined football players were divided intofour groups of 11. The groups were determined as Mr., Mrs. experiment andcontrol group. Footballers continued their training program, which includesfive days a week, four weeks of aerobic and resistance exercises andtechnical-functional exercises specific to football.Results: Astatistically significant decrease (P <0.05) was found in the Fe++and Iron Binding Capacity (P <0.05), and a statistically significantdecrease in VLDL and Triglyceride values in the male soccer players in theexperiment group, A statistically significant increase (P<0.05) in TotalCholesterol, Fe++, Iron Binding Capacity, and a significant decreasein LDL values were found in the female players in the experimental group beforeand after exercise (P<0.05).Conclusion: Maleand female soccer players receive 200 ml daily for four weeks and five days aweek. VLDL and Triglyceride values decreased in male soccer players, and iron(Fe++) and iron binding capacity values increased in the aftermathof giving daily pomegranate juice. Same time; while the increase in TotalCholesterol in female soccer players is statistically significant, it ispossible to increase performance and success in football due to positiveeffects of increase in the iron (Fe+ +) and iron binding capacity ofmale soccer players as it is within the normal reference limits for theathletes in the same age group. is thought to play a protective role.
