  • Protekcyjny wpływ nebiwololu na prewencję uszkodzeń narządowych w
           nadciśnieniu

      Abstract: Powikłania sercowo-naczyniowe ( przerost lewej komory, migotanie przedsionków, niewydolność serca, incydenty wieńcowe, starzenie się naczyń i przyspieszona miażdżyca) są główną przyczyną zgonów u chorych z nadciśnieniem tętniczym. Stres oksydacyjny i aortalne ciśnienie centralne odgrywają kluczową rolę w patofizjologii systemu sercowo-naczyniowego oraz powikłań sercowo-naczyniowych. Leki przeciwnadciśnieniowe, nawet w obrębie tej samej klasy, wywierają różny wpływ na wskaźniki sztywności tętnic i funkcję śródbłonka. Nebiwolol jest β-adrenolitykiem trzeciej generacji o działaniu wazodilatacyjnym. W jego funkcji rozszerzającej naczynia pośredniczy szlak śródbłonkowej L-arginyny/NO. Nebiwolol zwiększa biodostępność tlenku azotu. Ostatnie badania wykazały, że nebiwolol poprawia elastyczność naczyń w stopniu większym niż konwencjonalne β-adrenolityki (atenolol, metoprolol). Według wytycznych ESH/ESC 2013 niektóre β-adrenolityki o działaniu rozszerzającym naczynia, takie jak celiprolol, karwedilol i nebiwolol, zmniejszają ciśnienie centralne tętna i sztywność tętnic lepiej niż atenolol i metoprolol.

  • Dobowy rytm ciśnienia tętniczego i powiązane czynniki u chorych z
           nadciśnieniem tętniczym

      Abstract: Wstęp. Nadciśnienie tętnicze jest częstą, niemal bezobjawową, wykrywalną i poddająca się leczeniu chorobą. Nieprawidłowy dobowy rytm ciśnienia tętniczego wiąże się ze zwiększonym ryzykiem zaburzeń, takich jak problemy ze snem, zespół metaboliczny, choroby sercowo-naczyniowe i nowotwory. Jednak całodobowe monitorowanie ciśnienia tętniczego, metoda pozwalająca wykryć nieprawidłowości w zakresie nocnego spadku ciśnienia tętniczego, cieszy się stosunkowo małym zainteresowaniem wśród lekarzy. Badanie przeprowadzono w celu określenia dobowego rytmu ciśnienia tętniczego i powiązanych czynników u chorych z nadciśnieniem tętniczym. Materiał i metody. To analityczne przekrojowe badanie przeprowadzono w 2015 roku. Badana populacja obejmowała pacjentów gabinetów lekarskich i klinik w mieście Qom, a do analizy włączono próbę 183 chorych. Zgromadzono odpowiednie dane demograficzne chorych oraz wyniki całodobowego monitorowania ciśnienia tętniczego. Wyniki. Średni wiek chorych wynosił 52,08 ± 14,16 roku. Kobiety stanowiły 57,9% uczestników. Średnia czas od wykrycia nadciśnienia tętniczego wynosił 4,32 ± 4,96 roku. U 77% uczestników stwierdzono brak nocnego spadku ciśnienia tętniczego (profil non-dipper). Nie stwierdzono istotnych różnic pod względem wieku (p = 0,31), czasu od wykrycia nadciśnienia tętniczego (p = 0,93), płci (p = 0,55) i metody leczenia nadciśnienia tętniczego (p = 0,96) między osobami z prawidłowym nocnym spadkiem ciśnienia tętniczego (profil typu dipper) a osobami z profilem typu non-dipper. Wnioski. Częstość braku prawidłowego spadku ciśnienia tętniczego w godzinach nocnych była w badaniu autorów większa niż w innych tego typu badaniach. Z uwagi na znaczenie całodobowego monitorowania ciśnienia tętniczego w diagnostyce i leczeniu ciśnienia tętniczego zaleca się wykonywanie tego badania u chorych z nadciśnieniem tętniczym.

  • Plasma adiponectin in hypertensive patients with and without metabolic
           syndrome

      Abstract: Introduction. The metabolic syndrome is defined on the basis of a cluster of coexisting metabolic deteriorations, which increase the risk of cardiovascular disease. Earlier studies suggested a role of adipokines and proinflammatory cytokines in the pathogenesis of metabolic syndrome-induced complications. Some clinical studies reported the association of hypoadiponectinemia with cardiovascular diseases, diabetes mellitus, hypertension and dyslipidemia. Decreased adiponectin has been proposed as a useful biomarker of the metabolic syndrome. The aim of the study was to compare serum adiponectin levels in patients with primary hypertension with and without coexisting metabolic syndrome. Material and methods. The study group comprised 145 patients aged 18–50 years with primary hypertension. On the basis of IDF diagnostic criteria, all patients were categorized in groups with (HTMS; n = 73) and without (HTC; n = 72) metabolic syndrome. Study protocol included anthropometric measurements including waist circumference, 24 hour blood pressure measurement, serum levels of adiponectin, uric acid, lipids, insulin and glucose, and assessment of insulin resistance using HOMA-IR index. Results. The plasma levels of adiponectin were significantly lower in the subjects with hypertension and metabolic syndrome as compared with those without the MS (4.2 ± 3.1 μg/dL vs. 6.7 ± 6.5 μg/dL, p = 0.0026). In all patients with hypertension, adiponectin negatively correlated with insulin (r = −0.20; p = 0.014), HOMA-IR (r = −0.24; p = 0.003), triglycerides (r = −0.19; p = 0.025), uric acid (r = −0.25; p = 0.003) and positively with HDL-cholesterol (r = 0.33; p = 0.0001). In the ROC curve analysis, the cut-off value predicting metabolic syndrome in patients with hypertension was 4.1 μg/mL for adiponectin. Conclusions. In conclusion, low adiponectin levels should be taken into account as a potential non-classical biomarker of metabolic complications in patients with primary hypertension, not only with concomitant metabolic syndrome.

  • The relationship between blood pressure changes and the efficacy of
           treatment in patients with primary glomerulonephritis with special regard
           to kidney size

      Abstract: Introduction. Blood pressure plays a modulating role in the progression of glomerulonephritis. Kidney size could also constitute a factor influencing the efficacy of treatment. The study sought to determine renal length and the influence of blood pressure changes and renal length on the efficacy of treatment. Material and methods. This study included 53 adult patients (25 female and 28 male), aged 17 to 63. At the beginning of the observation period, the following tests were performed: percutaneous renal biopsy, anthropometric measurements, renal length in abdominal ultrasound scan. At the beginning of the observation period and after 24 months the following tests were performed: SBP, DBP, MAP, PP, serum creatinine level, GFR MDRD, DPL. Absolute renal length (D) was related to anthropometric parameters and values of relative renal length D/H, D/BSA, D/BMI were calculated. Results. D value ranged from 93.5 mm to 135.5 mm. Mean values of parameters were: D/H 0.67 ± 0.07 mm/cm, D/BSA 61.8 ± 8.7 mm/m2, D/BMI 4.67 ± 0.79 mm/kg/m2. No correlations were found of DPL changes and GFR MDRD changes with arterial pressure. A correlation was found between DPL changes and D. In patients whose DPL values decreased by at least 50%, mean values of D, D/H and D/BSA were higher. No correlations were found of GFR MDRD changes with D, D/H, D/BSA or D/BMI. Conclusions. No influence of arterial blood pressure on the efficacy of treatment was discovered. Renal length is not a prognostic factor for changes in glomerular filtration rate; however, it can be a prognostic factor for proteinuria changes.

  • Plasma concentration of tryptophan and pregnancy-induced hypertension

      Abstract: Introduction. Pregnancy-induced hypertension (PIH) is one of the main clinical problems of unexplained etiopathogenesis. New factors involved in the pathogenesis of this disease are still being searched. The available literature lacks data regarding the differences in tryptophan concentrations in physiological and PIH-complicated pregnancy. Previous studies have shown that L-tryptophan treatment reduces blood pressure in hypertensive rats. The direct vascular effects of tryptophan have not been fully explored. In this study, the stimulating effect of tryptophan on the development of PIH was revealed. The aim of the present study was to assess the differences in plasma tryptophan concentrations in physiological pregnancies and pregnancies complicated with hypertension in the third trimester. Material and methods. The study was carried on 105 complicated by PIH and 105 pregnant women with blood pressure within normal limits between 25 and 41 weeks of gestation. Tryptophan concentration was determined by the automated ion-exchange chromatography using an Amino Acid Analyser (AAA 400) by Ingos, Czech Republic. Tryptophan concentration was expressed in μmol/cm3 plasma. Results. The mean concentration of tryptophan in the third trimester of physiological pregnancy was found to be 0.035 ± 0.009 μmol/cm3, whereas in PIH — 0.099 ± 0.007 μmol/cm3. Conclusions. The development of PIH in pregnant women is likely to be caused by increased concentrations of tryptophan, which is a substrate for production of serotonin and tryptamine. Further studies are needed to analyse the kinetics of tryptophan metabolism.
