      Zoonoses are diseases and infections that are transmitted in nature between vertebrates and humans. Zoonoses consist of an interaction among at least three species: one pathogen and two hosts − animals and humans. This review aimed at shedding in depth light on zoonotic parasitic diseases in Egypt, with special reference to their relative incidence between humans, reservoir animals, sources of human infection and control policies. According to the available literature, many parasitic zoonoses are endemic in Egypt. In rural areas, intestinal parasitic zoonoses are widespread and are the leading cause of diarrhea, particularly among children. Some parasitic zoonoses are mainly found in certain areas in Egypt, for example, cutaneous leishmaniasis and zoonotic babesiosis in Sinai. Other locations in Egypt have a history of certain parasitic zoonoses, such as visceral leishmaniasis in the El Agamy area in Alexandria. Fortunately, control programs have led to a dramatic decrease in the prevalence of other zoonoses, such as intestinal schistosomiasis and fascioliasis in the country. In Egypt, animal reservoirs of parasitic zoonoses have been identified. These include rodents, stray dogs, and cats as well as domestic and farm animals and birds. Many vectors have also been revealed, typically mosquitoes and ticks, which pose real threats for disease transmission. Strict control strategies are needed to upgrade and complement current efforts at eradicating parasitic zoonoses in Egypt.

      Annual outbreaks of influenza infection are an ongoing public health threat, which kill thousands of people worldwide. Influenza A virus is the most clinically important subtype of the influenza virus. It is the cause of the four pandemics that occurred in the 20th and the 21st century and still causes thousands of deaths worldwide annually. Understanding the life cycle of influenza virus is a prerequisite to competing it. Inhibiting the virus before infecting the host cell eliminates the need of drug delivery into cells and prevents the storm of cytokine expression accompanying the virus infection, which is believed to be the cause of severe consequences of influenza infection. Consequently, viral entry, the first step during infection, is our interest. This is a systematic review where 110 manuscripts were included. Google scholar was the search engine with keywords that included influenza A virus, influenza A virus receptors, influenza A, and host cells. The aim of this review is to give an overview of the multiplicity of factors and cofactors that orchestrate the process of viral entry and to mark its features.

      Context Bladder urothelial carcinoma is one of the most common cancers in both men and women worldwide. Matrix metalloproteinases (MMPs) are a family of endopeptidases capable of degrading essentially all extracellular matrix components. MMP-9 has multiple substrates; however, collagen type IV is the most crucial substrate. The proteolytic activity of MMP-9 not only promotes invasion and metastasis but also releases matrix-bound growth factors and other signaling molecules, participating in growth, angiogenesis, and inflammatory responses. Aims To evaluate the expression of MMP-9 in urothelial carcinoma of the bladder immunohistochemically, to correlate the expression with stromal invasion, tumor grade, and histological type, and to test for the usefulness of MMP-9 as ancillary test to distinguish different histological types and tumor grades Settings and design A retrospective study included the collection of 37 formalin-fixed, paraffin-embedded tissue blocks. Overall, 32 blocks/cases represent urothelial carcinoma of urinary bladder and five blocks/cases of reactive (inflammatory) conditions. Patients and methods Two sections from each case were stained with hematoxylin and eosin and immunohistochemically with MMP-9 antibodies, using rabbit monoclonal anti-human MMP-9 as a primary antibody. Statistical analysis It was performed with statistical package for the social sciences (SPSS Inc., Version 18.0, Chicago) and Excel 2007 program. Data analysis was done using t test and χ2 test. Results Eighteen (56.25%) cases of the 32 total cases of urothelial carcinoma included in this study show a negative immunohistochemical expression for MMP-9 and 14 (43.75%) cases were positive for MMP-9. There was no statistical significant correlation between MMP-9 expression and tumor stage, grade, and histological type. Conclusion No statistical significance was found between MMP-9 immunohistochemical expression and the studied prognostic parameters: histological type, grade, and stage.

      Context Hepatitis E infection, caused by the hepatitis E virus (HEV), is a common cause of acute hepatitis in developing countries with poor sanitation and hygiene. Aims In this paper, we expressed HEV ORF2 C-terminal 802 bp and ORF3 C-terminal 107 bp fragments and its glutathione S-transferase (GST) fusion genes in Escherichia coli and tested the immunoreactivity of HEV antibody from sample serum with recombinant GST-HEV ORF2, 3 antigens by enzyme-linked immunosorbent assay (ELISA). Settings and design Eight primers designed for gene synthesis. Materials and methods HEV ORF2, 3 and its GST-fusion gene fragments were amplified by PCR. Expression of target genes in E. coli was induced by Isopropyl-β-D-Thio-alactopyranoside (IPTG) (0.1 mmol/l) and temperature (42°C). The detection of anti-HEV antibody with purified GST-HEV ORF2, 3 antigens was performed using the indirect ELISA method. Results When our ELISA was compared with the HEV-ELISA kit, the sensitivity of our assay is 97.9%, specificity is 100%, and consistency is 99.6%. The positive rate of anti-HEV antibody from the serum in acute hepatitis patients with recombinant GST-HEV ORF2, 3 antigens by our ELISA was 29.5 and 26.1%, respectively, and this result corresponded with the HEV-ELISA kit. Conclusion The new HEV ELIA developed in the present study is a highly specific diagnostic assay for the detection of anti-HEV antibody in serum specimens obtained from acute hepatitis patients.

Studies that tested the association between fibroblast growth factor (FGF)-23 and LV structural and functional changes in hemodialysis (HD) patients have shown conflicting results. Aims The aim of this work was to study whether FGF-23 has a direct association with LV structural and functional changes in HD patients. Settings and design This is a cross-sectional study. Materials and methods Demographic data of 95 HD patients were recorded in a predefined data sheets. The blood samples for FGF-23 and other laboratory variables were collected and measured by the standard methods. Echocardiography area length method was used to calculate left ventricular mass index (LVMI), and modified Simpson’s rule was used to measure ejection fraction. Transmitral Doppler and tissue Doppler of septal e′ were used to measure LV diastolic function. Statistical analysis Multivariate analysis was carried out. Results There were significant positive correlations between FGF-23 and LVMI (r=0.285, P=0.005) and LV E/e′ ratio (r=0.391, P=0.001), while there were insignificant correlations with other echocardiographic parameters including chamber dimensions, chamber volumes and PASP. Multivariate analysis showed a significant association between LVMI and FGF-23 level after adjustment for age and systolic BP (P=0.0001). Conclusion FGF-23 is independently associated with LVMI in HD patients. This result suggests a direct role of elevated FGF-23 in the pathogenesis of LV hypertrophy in patients with HD.

      Background Haematological malignancies represent ∼7% of all malignant diseases. Acute myeloid leukaemia (AML) is an aggressive and fatal disease. AML treatment basically remained unimproved in the last 20 years; it depends upon induction of cytotoxic chemotherapy. An average of less than 30% of AML patients survive for the long term. Mesenchymal stem cells (MSCs) are currently being investigated for an ever-expanding number of clinical indications based on their tissue-regenerative, immunomodulatory and anti-inflammatory effects. The leukaemic inhibitory factor gene (LIF) induces the differentiation of AML cells and inhibits their growth, while the interleukin-10 (IL-10) might be an efficient inhibitor of tumour metastasis. Aim The present work aimed to detect the effect of human umbilical cord blood-derived (HUCB-MSCs) on the expression of the LIF gene and on IL-10 in AML patients. Materials and methods The MSCs were separated from HUCB, and co-cultured with samples collected from peripheral blood of AML-insulted adults before chemotherapy. The expression of LIF gene and the IL-10 level were measured using the real-time PCR and enzyme-linked immunosorbent assay techniques, respectively, before and after the co-culture aiming to evaluate the immunomodulatory and anti-inflammatory effects of the MSCs. Statistical analysis The results were considered statistically significant if a ‘P value’ was found to be less than 0.05 based on one-way analysis of variance analysis and paired t-test. Results The present study revealed that the group of AML cells co-cultured with HUCB-MSCs showed a significant increase in the expression level of LIF gene compared with the untreated group. The group of AML cells co-cultured with MSCs showed a significant decrease in the IL-10 concentration compared to that of the untreated group. Conclusion Our data demonstrated that co-culture of AML with MSCs represents a simple approach to inhibit leukaemic cells in vitro.

      Background Diabetes is a metabolic disorder characterized by chronic hyperglycemia and has long standing complication including both micro and macro vascular ones. One of the common underdiagnosed complications of diabetes is cardiovascular autonomic neuropathy (CAN), which encompasses damage to the autonomic nerve fiber that innervates the heart and blood vessels. Objective The aim of this work was to assess the prevalence of CAN in Egyptian people with type 2 diabetes. Patients and Methods The study was conducted on 120 subjects with type 2 diabetes. CAN was assessed by a group of defined cardiovascular autonomic reflex tests including (Resting heart rate, Heart rate response to standing, Beat-to-beat HRV, Blood pressure response to standing, Diastolic blood pressure response to isometric exercise and QTc interval). Patients with CAN were subsequently divided into three groups, Early, Definite and severe CAN. Results and Conclusion The prevalence of CAN is high in Egyptian people with type 2 diabetes amounting to 60%. Percentage of patients that had early, definite and severe CAN was 15%, 36.7% and 8.3% respectively. Positive significant independent correlation was found between CAN, HbA1c and serum total cholesterol.

      Background Overall, 20–50% of patients with hepatocellular carcinoma (HCC) present at advanced stage where palliative treatment is the only reasonable management. Pain in HCC is a significant factor of morbidity, and celiac plexus neurolysis demonstrated good results in relief of pain as a result of pancreatic cancer; however, there are no studies concerning its use in management of HCC. Patients and methods A total of 30 patients with advanced stage HCC complaining of moderate to severe abdominal pain, requiring opioid analgesics to control, scored their abdominal pain according to the Numeric Rating Scale-11 before the procedure and were re-assessed every 2 weeks for 2 months. Results This study included 30 patients, comprising 24 men and six women, with a mean age of 61.73±7.9 years. A significant correlation between older age and better BCLC staging and pain reduction was found (P=0.045 and 0.08, respectively), whereas there is no significant correlation between pain reduction and clinical manifestations and medical history. There is a significant reduction in pain score after the procedure. Median pain score was significantly lower than pre-endoscopic median pain score, with a P value less than 0.001. Conclusion Endoscopic ultrasound-celiac plexus neurolysis should be a potential choice in patients with moderate to severe abdominal pain in patients with HCC who are not considered for therapeutic intervention.

It is characterized by congenital absence of the uterus and the vagina in a phenotypically and karyotypically normal female. Here, we report a case of atypical Mayer–Rokitansky–Kuster–Hauser in a 13-year-old girl who also had abnormalities in the ovaries.
