  • A prospective descriptive study of the management of miscarriages at two
           tertiary hospitals in Zimbabwe: variations from global trends

      Abstract: Background: Early pregnancy loss is the commonest reason for admission to gynaecological wards for women of reproductive age group.
      Objective: To assess the current management of incomplete miscarriage at Harare and Parirenyatwa Hospitals and to determine the proportion of patients with incomplete miscarriage managed with Manual Vacuum Aspiration (MVA), sharp curettage or medical evacuation.
      Design: A prospective descriptive study.
      Setting: Two teaching hospitals in Zimbabwe.
      Materials and Methods: Women admitted with incomplete miscarriage, missed, septic incomplete and complete miscarriage were followed up from admission until discharge between for April and May in 2014. Demographic details, management and clinical course was recorded prospectively on a form.
      Main Outcome Measures: Outcome assessed were method of evacuation, need for transfusion, antibiotic use and hospital stay.
      Results: Four hundred and eleven (411) women were admitted to the two hospitals. The mean gestation at admission was 12 weeks. Planned pregnancies were 62.3% with 37.8% unplanned. Those who used contraception prior to conception were 22%. Only 5.4% were diagnosed as septic and 2 patients (0.5%) had foreign bodies in the genital tract. The majority of patients had sharp curettage for evacuation of retained products of conception with very few patients being having MVA, suction curettage, expectant management or medical evacuation. Overall only 0.5% of patients had medical evacuation. The mean duration of hospital stay was 27 and 19 hours for Parirenyatwa and Harare respectively and was not statistically significant.
      Conclusion: We have shown that most patients are still having sharp curettage in theatre despite Zimbabwe being one of the countries to introduce MVA in early 1990s. Management of incomplete miscarriage has not kept up with global trends where suction blunt curettage is the preferred method

  • Exclusion of reticulocytosis as a cause of elevated mean corpuscular
           volume in HIV patients on antiretroviral treatment in Zimbabwe

      Abstract: Background: Macrocytosis is one of the haematological abnormalities that have been observed in association with the administration of antiretroviral drugs, especially zidovudine. Macrocytosis due to other antiretroviral drugs has also been reported. Several explanations have been made about the mechanisms and indications of macrocytosis but no study has been done in Zimbabwe to evaluate it.
      Aim: To investigate whether reticulocytosis is also responsible for macrocytosis observed in human immunodeficiency virus patients on antiretroviral therapy.
      Objective: To exclude reticulocytosis as one of the causes of macrocytosis associated with Antiretroviral (ARV) treatment.
      Methods: A cross-sectional study was carried out at Parirenyatwa Opportunistic Infections Clinic from January to April 2013 on two hundred and forty-one (241) adult human immunodeficiency virus patients with macrocytosis after antiretroviral treatment for more than six months. Reticulocyte count was determined on all samples using the Sysmex XT 2000i at Parirenyatwa Haematology Laboratory.
      Results: The mean relative reticulocyte count, mean absolute reticulocyte count and mean immature reticulocyte fraction determined in human immunodeficiency virus - patients with macrocytosis were within reference ranges. Reticulocyte count and immature reticulocyte fraction showed a statistically significant difference (p=0.004) between patients with significant macrocytosis (mean corpuscular volume =100- 115fL) and those with extreme macrocytosis (mean corpuscular volume >115 fL). There was a statistically significant difference between significant macrocytosis and extreme macrocytosis in relative reticulocyte count (p=0.004), absolute reticulocyte count (p=0.001) and immature reticulocyte fraction (p=0.001) respectively.
      Conclusion: Macrocytosis observed in human immunodeficiency virus patients on antiretroviral therapy for more than six months is not due to reticulocytosis, but may be an indicator of bone marrow dysplasia induced by antiretroviral drugs. A bone marrow examination would be recommended as a follow up study to confirm this.

  • Price and availability survey of essential medicines in the Harare
           province, Zimbabwe

      Abstract: Background: Access to essential medicines is both a fundamental basic right and necessity for everyone, thus concerted efforts should be made to ensure universal access to safe, quality and effective medicines.
      Objectives: To determine prices, sources and availability of essential medicines (their innovator and/or generic equivalents) in both private retail and public pharmacies.
      Setting: Private and public sector retail pharmacies in Harare metropolitan province, Zimbabwe.
      Materials and Methods: Forty medicines were selected for the survey. A standardised methodology developed by the World Health Organization and Health Action International (WHO/HAI) was used to survey the selected medicines. The survey was conducted in 110 private pharmacies and the two central hospital pharmacies in Harare. Prices were compared with International Reference Prices (IRPs).
      Results: In both sectors, availability of the selected medicines (Low Priced Generics) was high (>80%). Fewer innovator brands were found for the selected medicines. Median Price Ratios (MPR) of Lowest Priced Generics (LPG) showed better access to essential medicines is hindered by the high prices obtaining in the private sector (with median MPR of 4.52). The public sector showed significant progress towards procurement efficiency (median MPR of 1.5). More than 70% of the surveyed medicines were from manufacturers outside Zimbabwe with more than 60% from Indian generic manufacturers.
      Conclusion: Accessibility of essential medicines from private retail pharmacies in Harare, Zimbabwe is poor due to the high prices of these medicines as compared to public pharmacies. Promoting local production of essential medicines is necessary as it may be one of the practical ways of increasing access to quality, safe and effective medicines

  • Ulcerative Keratitis: incidence, seasonal distribution and determinants in
           a tertiary eyecare facility south east Nigeria

      Abstract: Background: Ulcerative keratitis is a potentially blinding eye disease found in eyes with predisposing risk factors.
      Objectives: To determine the incidence, distribution, causative organisms and risk factors for ulcerative keratitis in patients presenting at the University of Nigeria Teaching Hospital (UNTH)- a tertiary referral eye care facility, Enugu, Nigeria.
      Study Design: This was a hospital based cross-sectional study of 61 new patients with ulcerative keratitis who presented at the UNTH from October 2011-September 2012. Participants’ socio-demographics and predisposing risk factors were documented. Corneal scrapings were collected for microbial diagnosis.
      Results: A total of 11,056 new patients presented during the study period giving an incidence rate of 0.6%. Of the 61 participants, 44(72.1%) were males, 17(27.9%) were females with an age range of 6 – 75 years and a median age of 54 years. Of the 40(65.6%) culture positive corneal scrapings, 21(52.5%) were purely bacterial, 17(42.5%) were purely fungal while 2(5%) were mixed infection, caused by both bacterial (Staphylococcus aureus) and fungal (Aspergillus fumigatus) organisms in both cases. Commonest predisposing risk factor was non- surgical trauma 43(70.5%) followed by use of harmful Traditional Eye Medicine (TEM) 27(44.3%). The seasonal distribution of corneal ulcer was highest in month June 11(18.0%). Complications from the ulcers included central leucoma 19 (31.2%) and panophthalmitis 6 (9.9%).
      Conclusion: The incidence rate of ulcerative keratitis is 0.6% occurring highest in the month of June with more bacterial than fungal isolates. Many participants were exposed to non- surgical trauma and use of TEM. Eye health education (emphasizing the deleterious effects of TEMs) and protection (especially for artisans) if instituted can reduce the visual morbidity associated with corneal ulcers.

  • External auditory canal leech: a rare case report of paediatric external
           auditory canal haemorrhage

      Abstract: Leeches are blood sucking organism feed on human blood. While human bites are common, they rarely cause human internal infestation. We describe a rare case of a parasitic leech infestation of the External Auditory Canal (EAC).
      A two month old child presented to the Emergency department with a seven day history of left sided ear bleeding. On examination she was noted to have a whitish foreign body in the left EAC imbedded in fresh blood. Irrigation with warm saline extracted a 1.5 cm long leech. While very uncommon, parasitic infestation should be considered in a patient living in the tropics
