The reported incidence is 1/4500, and more than 80% occurs before the age of two as an acute abdomen or bowel obstruction. The most common site is Ileum (60%), while the colonic localisation is reported between 4 and 18%.Presentation of the caseHerein we report the case of a 35-year-old man, presented at the Emergency Department with fever and localised abdominal pain in the right iliac fossa. Preoperative abdominal ultrasound and CT scan showed a cystic mass of 44 × 43 × 70 mm adjoining the posterior wall of the right colon. He underwent explorative laparoscopy, laparotomy conversion, right hemicolectomy with an intra-operative diagnosis of colonic duplication cyst, confirmed by histology.DiscussionThe review of the literature showed as the intestinal duplication cysts are rare congenital anomalies. The clinical presentation is variable and depends on the site and the related complications. A surgical approach based on the resection of the involved bowel tract is the treatment associated with the best long-term outcomes.ConclusionIt is important to include intestinal duplication in the differential diagnosis of acute abdomen, to ensure the best therapeutic strategy.
  • Midline gallbladder makes a challenge for surgeons during laparoscopic
           cholecystectomy; case series of 6 patients

Abnormal location comprises the commonest one.The presence of an ectopic gall bladder is estimated to occur in around 0.1–0.7% of individuals, it can be truly ectopic locating under the left lobe of the liver or just to the left of falciform ligament.Gall stones are common disorder that may mandate cholecystectomy especially in symptomatic patients, surgery can be done laparoscopically safely in cases of abnormal anatomical position, but such cases may be at higher rate of complications especially if associated with other biliary tract anomalies.MethodsSix cases of symptomatic gall stones who underwent laparoscopic cholecystectomy included in this case series. During insertion of the telescope through the umbilical port, we found midline gallbladder under the falciform ligament instead being under right lobe of the liver. We did modification of the port sites by placing epigastric port in the left hypochonrdium.ResultsIn all the six cases the surgery had been done successfully laparoscopically without conversion to open technique. Follow up of the patients done for 2 months with no post-operative sequelae.ConclusionLaparoscopic cholecystectomy for midline gall bladder is technically difficult. Modifying the port sites make the surgery easier. MRCP preoperatively, intraoperative cholangiography, or fluorescent cholangiography may be needed if there is any concern about biliary anomalies or for real time detection of biliary injuries.
  • Declining interest in general surgical training – Challenging
           misconceptions and improving access at undergraduate level

The St. George's Surgical Society hosted a national surgical conference with an aim to explore the reasons for the decline and to investigate the effectiveness of a one-day intervention on students' perceptions of general surgery and surgical skills.Materials and methodsThe conference took place on 20th January 2018 at St. George's, University of London. During the conference, medical students received lectures on “careers in surgery” and small-group workshops introducing students to simulation-based laparoscopic machines. Delegates were invited to complete before and after questionnaires looking at various domains; (1) Perceptions of general surgery, (2) Simulation skills, and (3) Usefulness of the day.ResultsThere were significant impacts on student perceptions of the speciality such as increases in the views that "general surgery contained the opportunities for personal and professional development" (26%, p = 0.04), and of "general surgery as a rewarding speciality" (26%, p = 0.05). There were also negative changes such as an increase in the perception that “general surgery is female unfriendly” (+32%, p = 0.01). There were positive findings in all aspects relating to the use of laparoscopic simulation.DiscussionThe challenges faced in improving access to general surgery has been highlighted. More importantly the benefits of a one day intervention in addressing misconceptions and improving knowledge was seen. This study also shows the benefits of simulation teaching in the undergraduate curriculum.ConclusionThe intervention has improved the perceptions on general surgery, surgical skills and knowledge and provided a platform for engaging students and clinicians.
  • Are interventional radiology and allied specialities neglected in
           undergraduate medical education' A systematic review

Awareness and understanding of interventional principles require efficacious education and training methodologies. We performed a systematic review to identify all available interventional speciality learning modules or training opportunities available for undergraduate medical students. We also propose a standardised framework for relevant modules.MethodsWe searched PubMed and all Ovid databases with no language restriction for studies that report and evaluate interventional speciality educational modules or similar training initiatives. We followed a prospective protocol (PROSPERO registration: CRD42018110006). Internal and external validity of the included studies was assessed. Qualitative synthesis of data was performed to define performance improvement and/or motivation towards a career in an interventional speciality.ResultsOut of 6081 records, 17 studies met the inclusion criteria, 15 of which were focused on interventional radiology. More than half of studies (9/17) were surveys where student knowledge and interest were reported as poor. 5 out of 6 studies which assessed the effect of educational interventions concluded to improved knowledge or performance. Most surveys concluded that early exposure can increase interest towards such specialities, improve knowledge and relevant motivation.ConclusionsFew studies report teaching initiatives in interventional radiology and other interventional specialities, reflecting the poor relevant motivation and knowledge amongst medical students. Simple interventions e.g. introductory lectures and simulation sessions spark interest in students and also improve knowledge as proven in the case of interventional radiology. Standardisation of such efforts via a suggested framework, Strategy Development Framework for Interventional Radiology, can further optimise such outcomes.
  • Surgical repair of a large ventral hernia under spinal anaesthesia: A case

Up to 30% of all patients undergoing laparotomy develop an incisional hernia. Open ventral hernia repair is often performed under general anaesthesia but can also be performed under regional anaesthesia.Case reportWe report the case of an elderly man, who underwent open surgery of a large incisional hernia in spinal block. Regional anaesthesia was chosen due to the patient's additional diseases and disorders.ConclusionOpen surgery of large ventral hernia in spinal anaesthesia can be performed because the spinal anaesthesia provides adequate conditions for ventral hernia repair. The patient has to be in good physical condition in order for the surgery to be successful. During the surgery the patient has to be watched over vigilantly by the anaesthesiologist.
  • Apical periodontitis associated with a calculus-like deposit: a case
           report of a rare fan-shaped manifestation

Granulomas or cystic lesions are the most commonly found entities in endodontics. Surprisingly, this case report presents a rare radiopaque image, in a fan shape, of a calculus-like deposit in the periapical region of the maxillary left central incisor. Case presentation: A 34-year-old male, with a history of trauma, presented with apical periodontitis associated with an uncommon image, similar to a calculus-like deposit adhered to the apical region of the maxillary left central incisor. Nonsurgical endodontic intervention was performed, followed by apicoectomy and histopathological analysis of the collected material. The results of the biopsy were not compatible with a cyst or granuloma but showed fibrous connective tissue with calcified areas. Discussion: Correct diagnosis in endodontics is possible with a well-conducted anamnesis, complementary imaging exams and, in some cases, histopathological analysis. The periapical calculus-like deposit, associated with a periapical radiolucent lesion, was a result of the body's fight for healing, producing unusual radiopacity. Conclusion: The presence of the calculus-like deposit in a fan shape at the root surface represented dystrophic calcification as a manifestation of the attempt to heal. In the present case, apicoectomy and tissue biopsy for histological evaluation were fundamental for the correct diagnosis.
  • The relationship between femoral neck fracture in adult and avascular
           necrosis and nonunion: A retrospective study

This study was conducted to determine the relationship between the etiologies and management of FNFs at our institution and the development of AVN or nonunion.Materials and methodsThis study was a retrospective medical chart review of all adult patients admitted and managed for FNF.ResultsThere were a total of 69 FNF patients reviewed. FNF was caused by a fall in 37 patients (53.6%), a road traffic accident in 16 (23.2%), motorcycle and motorbike accidents in 8 (11.6%), and heavy exercise in 8 (11.6%). Twenty-four patients (34.8%) had fixation within 24 h of injury, and 45 (65.2%) went more than 24 h before fixation. The mean RUSH score at 6 months was 21.4 ± 5.1. There were 4 patients (5.8%) with a collapsed FNF and 4 patients (5.8%) had a nonunion FNF. AVN was documented in 12 patients (17.4%). Of the 12 patients who had AVN, 8 (66.7%) received fixation within 24 h from the time of the injury, whereas only 4 (33.3%) received fixation more than 24 h after the injury. There was a significant negative correlation between the time of fixation and AVN.ConclusionWe report a 17.4% incidence of AVN over 10 years in patients managed with FNF. AVN was found to be significantly correlated with the mode of injury (fall and RTA among younger male patients).

TennyAbstractIntroductionMassive pulmonary embolus (PE) is associated with a high mortality if not treated aggressively. Treatment classically includes thrombolysis, catheter embolectomy, or open surgical embolectomy. This is the case report of a 38-year-old female presenting with massive PE three weeks status post gastric sleeve resection.Presentation of case38-year-old female status post gastric sleeve resection presented to the emergency department with acute onset shortness of breath and dizziness. Computed Tomography (CT) chest angiography showed extensive PE with pulmonary artery saddle embolus, and an enlarged right ventricle suggesting strain. Her treatment consisted of anticoagulation, AngioVac suction embolectomy, EKOS catheter thrombolysis, fragmentation with catheter, extracorporeal membrane oxygenation (ECMO), and lastly surgical embolectomy due to refractory clinical course.DiscussionThis case report details the natural history of a complex massive pulmonary embolism presentation requiring multiple catheter-based measures, ECMO initiation, and subsequent surgical embolectomy.ConclusionThis case report should serve as encouragement for early adoption of surgical therapy in pulmonary embolism cases where patients present with a complex presentation.

Metastases of Thymoma are usually involved the pleura, pericardium or diaphragm, whereas the probability of extrathoracic metastases are extremely low. Success in the treatment of thymomas mainly depends on the radicalism of the performed operation.Materials and MethodsSince 2008 33 Thoracoscopic Thymectomies were performed in the thoracic department of the N.N.Blokhin Russian Cancer Research Center. The right-sided approach was in 23 (69,7%) patients, 7 (21,2%) operated on the left and 3 (9,1%) patients had combined approach. We include 26 patients with similar characteristics of the primary tumor operated from sternotomy approach to compare direct and long-term results of surgical treatment.ResultsThoracoscopic approach in comparison with sternotomy in surgical treatment of non-invasive Thymomas significantly reduces the incidence of postoperative therapeutic complications (p
