  • Association of Serum Uric acid and Gamma-glutamyltransferase with Obesity
           Related Metabolic Derangements in a Cohort of Children with Obesity in Sri
           Lanka

    • Abstract: Introduction: Gamma-glutamyltransferase (GGT) and serum uric acid (UA) are known to be associated with cardiovascular disease in obese children. Objectives: To determine the association of serum UA and GGT with components of metabolic syndrome (MetS) in a group of obese children and determine the validity of UA and GGT in predicting MetS. Methods: Cross sectional analytical study was conducted among 205, 5-15 year old obese children. After a 12- hour overnight fast, blood was drawn for glucose, lipid profile, alanine aminotransferase (ALT), aspartate aminotransferase (AST), insulin, UA and GGT. Oral glucose tolerance test (OGTT) was done with 2 hour plasma glucose. Height, weight, waist circumference,blood pressure and fat mass were measured. USS of abdomen was performed to assess hepatic steatosis. Results: Chi square test showed statistically significant associations between GGT and UA with triglyceride, insulin resistance (HOMA-IR), AST, ALT, AST/ALT ratio and fatty liver. Additionally UA showed a significant association with the OGTT. With existing cut offs (GGT >30 U/L and UA >330 μmol/L) the sensitivity and specificity of GGT in predicting MetS was 19% (95% CI, 13.63-24.37) and 88.4% (95% CI, 84.02-92.78) respectively while for UA was 28.6% (95% CI, 22.42-34.78) and 80.2% (95% CI, 74.75-85.65) respectively. A new cut off value of 19.5 U/L (sensitivity 56% and specificity 55%) for GGT and 275.5 μmol/L (sensitivity 61% and specificity 54%) for UA predicted MetS with greater accuracy. Conclusion: GGT and UA are strongly associated with metabolic derangements and these biomarkers are rather weak in predicting MetS.   • Prevalence, antibiotic sensitivity pattern and genetic analysis of
           extended spectrum beta lactamase producing Escherichia coli and Klebsiella
           spp among patients with community acquired urinary tract infection in
           Galle district, Sri Lanka

    • Abstract: Introduction: Community acquired urinary tract infections (CA-UTI) are commonly caused by Escherichia coli and Klebsiella spp which are known extended spectrum beta lactamase (ESBL) producers. Objectives: To determine, the prevalence and characteristics of ESBL producing of E. coli and Klebsiella spp in the community, and the association of risk factors with ESBL CA-UTI. Methods: Descriptive cross-sectional study with urine cultures performed from clinically suspected CA-UTI patients by CLSI standards. Conventional multiplex PCR was performed for gene analysis. Results: Cultures were positive in 178 (38%) patients from 465. Majority were females (103, 58%). Most frequently isolated was E. coli (149, 84%) with 68(46%)   ESBL producers   followed by 16(9%) Klebsiella pneumoniae with   4 (25%) ESBL producers.  Majority of patients with ESBL CA-UTI were >50 yrs (35/72, 49%) and 13 (18%) children <10 years were present. ABST of ESBL producers revealed high resistance rates for quinolones (41%) and   >80%   sensitivity   for   nitrofurantoin, fosfomycin, mecillinam, aminoglycosides and carbapenems. Presence of ESBL genes were 83% CTX –M, 71% OXA, 24% TEM and 9% SHV with one organism often producing more than one gene in 29 isolates (71%). Haematuria and structural abnormalities of urinary tract were significantly associated with increased ESBL CA-UTI (p<0.01). Conclusions: ESBL prevalence of this community was 40% in CA-UTI with   E.coli predominance among female   majority.   >80% ESBL   organisms   show   high sensitivity   for aminoglycosides, carbapenems, nitrofurantoin, mecillinam and fosfomycin.   • Vitamin D deficiency among children aged 10-18 years in Sri Lanka

    • Abstract: Background: Vitamin D deficiency (VDD) and insufficiency (VDI) are public health problems in many countries, and limited data is available on the prevalence of VDD/VDI in Sri Lanka. Objectives: To determine the prevalence and associated factors of VDD in children aged 10- 18 years. Methods: This was a cross-sectional study among school children aged 10-18 years at national level. A representative sample of 2525 children were recruited from July to November 2017. Serum 25(OH)D concentration and the patterns of vitamin D rich foods consumption were assessed. VDD and VDI cut offs were set at serum 25(OH)D concentrations of <12 ng/mL and 12-20 ng/mL, respectively as defined by global consensus in 2016. Results: The mean serum 25(OH)D level was 19.3±7.4 ng/mL. The prevalence of VDD and VDI were 13.2% (95%CI: 11.9%-14.5%) and 45.6% (95%CI: 43.7%-47.5%), respectively. The prevalence of VDD was highest in the central province (32.2%) and highest prevalence of VDI was  in the Inabaragamuwa province (58.9%). VDD and VDI were lowest in North Central province (0.7% and 34.7%, respectively). Significantly higher serum 25(OH)D levels were observed with male gender (p=0.000), BMI for age <-2SD (p=0.000), daily milk consumption (p=0.000) and residing in dry zone (p=0.000). Conclusions: Though Sri Lanka is a tropical country, VDD is prevalent among school children aged 10-18 years.   • Awareness and care seeking pattern for symptoms associated with
           endometrial carcinoma among postmenopausal women in the district of
           Colombo: a cross-sectional study

    • Abstract: Introduction: Variations in endometrial carcinoma awareness among postmenopausal women may explain the variations in care seeking pattern for symptoms associated with endometrial carcinoma. Objectives: To describe the awareness and care seeking pattern for symptoms associated with endometrial carcinoma among postmenopausal women in the district of Colombo. Methods: A community based descriptive cross sectional study was conducted among 1168 postmenopausal women in the district of Colombo, using multistage cluster sampling technique. A pre-tested interviewer administered questionnaire was used. Descriptive statistics were used to describe the awareness and care seeking pattern. Results: Of the sample, 83.6% (95% CI: 81.5-85.7, n=977) had heard about endometrial carcinoma. About 56% of women (n=654) were aware  that postmenopausal bleeding was a suggestive symptom of endometrial carcinoma. Only 24.0% (n=280) knew that never conceived was a risk factor, 20.6% and 20.9% knew that physical inactivity and obesity were risk factors respectively and 28.0% knew that hormone replacement therapy was a risk factor, for endometrial carcinoma. Of the women, 26.6% (n=311) had experienced some gynaecological symptoms similar to symptoms of endometrial carcinoma during their postmenopausal period. Majority of them had disclosed to the children (n= 155, 49.8%) about the symptoms, 70.7% (n=220) had sought treatment, and the most common reason for seeking treatment  was physical discomfort (n=83, 37.8%).   • Body image perception and body dissatisfaction among rural Sri Lankan
           adolescents; do they have a better understanding about their weight'

    • Abstract: Introduction Adolescents have high vulnerability for body image distortions which may result in many psychological and physical problems. 
      Objectives This study aimed to determine body image perception and dissatisfaction among rural schooling adolescents in Sri Lanka.
      Methods Adolescents (aged 13-16 years) were selected using probability-based sampling framework.  Self-administered figure rating scale was used to assess perceived current (PCBS) and ideal (IBS) body size and body discrepancy score (BDS = PCBS – IBS).  Pubertal stage was assessed with pubertal development scale.  Anthropometry was done according to WHO guidelines.
      Results Of 3128 students studied, 47.8% were boys.  Median PCBS was 4 (inter quartile range 1) for boys and 4 (2) for girls.  Boys aspire a larger body size compared to girls and the respective median IBS were 5 (0) and 4 (1).  More boys (70.4%, n = 1053) than girls (66.4%, n = 1084) were dissatisfied about their current body size; χ2 = 5.5, df = 1, p < 0.05.  Overweight students recorded higher PCBS and were more dissatisfied with their body size when compared to others.  Nearly half of boys and 29.6% girls wanted to have a larger body size than their current.  The main influencing factor of body dissatisfaction was body mass index.
      Conclusions Body dissatisfaction is evident even among rural adolescents, affecting boys more than girls.  Many students show a desire to have a larger body size than their current.  This study also provides valid and reliable tools to assess body dissatisfaction and pubertal development in Sinhala speaking adolescents.  Published on 2019-09-30 00:00:00

      Methods This is an observational study among patients with chronic RHD. One hundred patients (female 81 and 19 males) within ages 12 to 40 years (Mean age 27.3) were analyzed. A relevant clinical history including that of an initial episode of acute rheumatic fever (ARF) and recurrent episodes was obtained. 2D echo assessment of the cardiac valves was performed with an estimation of Wilkins score for the mitral valve (MV).
      Results Among the study population female: male ratio was 4:1. 30% had recurrent episodes of ARF. Only 60% had at least some evidence of ARF at any time in their life. The posterior mitral valve appears to be affected more than the anterior leaflet giving an average Wilkins score of 9.7 and 6.7 respectively. The total score had a positive correlation with Mitral stenosis (MS) (p <0.05). MV involvement was noted in 97%. 44% had significant mitral valve prolapse (MVP) but no statistical correlation was noted with mitral regurgitation (MR) (p>0.05). A regurgitant grade of 2 or more was found in 41%. High sensitive C reactive protein of more than 1mg/dl was noted in 55% of patients.  
      ConclusionChronic rheumatic MV disease can exist as MS, MR, MVP or simply an elevated valve score. Apart from recurrent streptococcal infections and chronic sub clinical inflammation, a number of different components of valve damage contribute towards the end result.
