  • Evaluation of the Nurses’ Level of Knowledge Regarding Hand Hygiene and
           Healthcare-Associated Infections: A Survey

      Abstract: The level of knowledge on hand hygiene (HH) and healthcare associated infections (HAI) directly influences the medical staff’s practice of HH and the rate of HAI. Continuous Medical Education (CME) is an important way of improving knowledge, increasing HH compliance and reducing HAI rates. The aim of the study was to asses the nurses’ knowledge regarding HH and HAI in order to identify their educational needs. In February-June 2018, a study was conducted, which included nurses participating in an educational program on HAI management theme. A questionnaire with 25 items from the following 6 specific areas: ”HAI”, ”HH”, ”Disinfection and sterilization”, ”Standard precautions and transmission-based precautions”, ”Accidental exposure to biological products” and ”The management of medical waste” was used to assess the knowledge. A total of 236 nurses were included in the study. The average score of the group was 8.38+1.03. There were identified 3 important specific areas where nurses encountered problems in identifying the right answer: ”HAI”, ”HH”, ”Disinfection and sterilization”. 67,8% of the nurses had inadequate knowledge of HH and HAI prevention, and 39% of the nurses had an insufficient level of knowledge, which may be a potential barrier to HAI prevention. Identifying specific areas where nurses have knowledge deficits facilitates the development of a CME strategy centered on the nurses’ educational needs.

  • Psychosocial Risk Factors of Elective Abortion: A Structural Equation
           Modelling Approach

      Abstract: Introduction: In Romania, although the number of elective abortions (EAB) has decreased, the percentage is still quite high, given the large variety of contraceptive methods and their ease of use. The principal factors that influence the decision of EAB are family, education, and income. Our study aimed to determine to what extent the psychosocial factors had an influence on EAB. Material and Methods: A sociological survey-based study was conducted, including women who presented for abortion on request during the period 2015-2018 at the "Bega" University Clinic of Obstetrics-Gynecology, Timişoara. The study investigated the degree of information and the use of contraceptive methods at the moment of pregnancy’s termination request. The extracted latent constructs were women’s general perception of abortion, emotional involvement regarding the pregnancy, relationship and family context, knowledge about the abortion’s consequences, and social status. A structural equation model of EAB was derived, describing the direct (and indirect) effects among latent variables. Results: The EAB model showed significantly adequate fit (χ2(45)=382.262, GFI (Goodness-of-fit index) =0.91, CFI (Comparative Fit Index) =0.951, RMSEA (Root Mean Square Error of Approximation) =0.03, and p<0.001). The model also showed adequate fit with significant indirect effects on EAB of general perception of abortion (β=1.132, p=0.004), emotional involvement regarding the pregnancy (β=0.036, p=0.492), knowledge about abortion consequences (β=0.184, p=0.001), relation and family context (β=0.268, p=0.034), and social status (β=0.061, p=0.422). Conclusions: Results support the use of structural equation modeling for investigating the moderating and mediating effects, while simultaneously measuring latent constructs. Also, results showed that five main factors have an influence on EAB.

  • Automated Identification of Red Blood Cells in Optical Microscope Images
           of Blood Smears Using Various Color Segmentation Methods

      Abstract: The identification and counting of red blood cells (RBCs) in microscopic blood images can provide useful information concerning the health of patients. Software-based cell counting has the advantages of objectivity, speed, and convenience over the manual method. Most of the automated RBC counting techniques in literature employed the grayscale or the green component of the red-green-blue (RGB) color images of blood smears. This work focuses on comparing the effect of using different color layers on the performance of software-based RBC counting. Ten color layers were extracted from different color models of blood smear images along with the grayscale conversion. Two comparisons were made: a comparison of contrast and a comparison of RBC counting performance using 52 blood smear images. The RBC contrast in the magenta layer of the cyan-magenta-yellow-key (CMYK) color model was at least 230% higher than that in the other layers. Additionally, our results indicated that using the magenta layer can provide better RBC counting performance when compared to the green, grayscale, and key layers with the p-values of p=0.0283, p<0.0001, and p<0.0001, respectively.

  • A Cross-Sectional Survey on Medical Learning Applications Used by
           Undergraduate Medical Students in Cluj-Napoca

      Abstract: Introduction: This study explores the utility of and attitude towards medical learning applications (MLAs) among undergraduate students of medicine in Cluj-Napoca and the use of these apps in finding solutions to five case-based medical scenarios. Materials and methods: A cross-sectional design using an online survey was conducted between June 8th and July 19th, 2019. The English/Romanian students were personally invited to participate in the survey via social media platforms (Facebook/WhatsApp student groups). A three-section questionnaire was developed to anonymously collect demographic data, study habits and the answers to the five case-based medical scenarios. Results: Ninety-three students between the age of 19 and 50 (median 24 years) participated. Most of the responding students (81.7%) were in the clinical years of study and studied up to 3 hours per day (67.7%). The use of MLAs ≥2 hours per day is reported by 18.3% of respondents. The MLAs were mainly used for studying throughout the semester (69.1%); 30.9% of the students used MLAs only for their exam preparation. The top-three MLAs were ‘AMBOSS’ (30/93), ‘Medscape’ (9/93) and ‘Dr. Najeeb Lectures’ (6/93). In around half of the cases, the students paid for the MLA (46.4%). The maximum obtainable score (6 points) in the clinical cases was reached only by 3 students without significant differences between the scores of those students who used or did not use resources (MLAs, books, other) to find the case solutions (Mann-Whitney test P=0.594, median of correct answers = 3 for both groups) or between the scores of those who used or did not use MLAs for learning (P=0.594, median of correct answers = 3 for both groups). Conclusion: One third of the participating students used MLAs but this is not reflected in the number of correctly answered clinical cases.

  • A Novel Framework for Accurate Segmentation of Brain Tumor Using Multiple
           Kernel Fuzzy Clustering Algorithm

      Abstract: Magnetic Resonance Imaging (MRI) is one of the prominent imaging techniques for assessment of brain tumor progression. Intensity inhomogeneity, partial volume effect (PVE) and diverse nature of tumor render a challenging task for automatic segmentation of brain tumors from MR images. Existing MRI brain tumor segmentation methods focus one or two of the above mentioned challenges. We aimed to present a framework for automatic brain tumor segmentation that effectively tackles all major challenges. In the proposed framework, first the intensity inhomogeneities in the MRI images are corrected using an Enhanced Homomorphic Unsharp Masking algorithm. Following intensity inhomogeneity correction, features are extracted. Finally, the extracted features are fused and clustered using Multiple Kernel FCM (MKFCM) clustering algorithm. The MKFCM clustering algorithm employed in the proposed framework overcomes the PVE and form more generalized clusters, thus proved to be effective for images with diverse tumor shape. To demonstrate the effectiveness of the proposed framework, it is compared with four other clustering algorithms using different validation measures.

  • Development of Graph-Based Algorithm for Differentiating
           Pathophysiological Conditions

      Abstract: Aim: Clinical diagnostic decision support systems, which use pathophysiological information to improve diagnostic accuracy, have historically required knowledge of various relations between pathophysiological states to handle complex cases. Developing a knowledge model centered on pathophysiological functions instead of pathophysiological states may reduce this unwieldiness. Materials and Methods: In this study, such a knowledge model is provided by a modified and generalized factor graph, the pathophysiological query (PPQ) graph. A PPQ algorithm that automatically suggests possible pathological conditions of patients in the form of PPQ graphs is also developed. To evaluate the model and the algorithm, a computer software that processes the PPQ algorithm and PPQ graph, which represent the acid-base regulatory functions, was developed. Four case reports were considered, and up to two-time points, used as evaluation data points, were selected for each case. The software was used to obtain the diagnoses suggested by the PPQ model, which were then compared to diagnoses formulated by three physicians. Results: The output acquired by the proposed method was in accordance with the diagnosis of the physicians in three out of the five cases. Conclusion: The PPQ model may be a valuable diagnostic tool for suggesting differential pathological conditions to physicians in complex cases.

  • Usability Evaluation of Hospital Information System: A Cross-Sectional

      Abstract: Introduction: Hospital information system (HIS) is software that collects, gathers, retrieves, and publish data and information and Usability evaluation result in objectives achievement. This study aimed to evaluate HIS usability in educational hospitals of Shiraz University of Medical Sciences (SUMS) in Shiraz, Iran. Material: The study conducted from February to April 2017 and the population comprised 689 users of 7 HISs in 7 hospitals of SUMS. The data collected using a questionnaire according to Isometric 9241-110 with a 5-point Likert scale. Results: The highest and lowest subscales of usability belong to the Conformity with user expectations and Self-descriptiveness, respectively. Except for the Suitability for individualization (p=0.156) and Suitability for learning (p=0.197), other subscales were significantly different between hospitals (p<0.05). Conclusions: The total system’s usability was evaluated above medium and users more focused on task-related items and less noted technical features.
