A.Acute and chronic osteomyelitis is currently one of the most important medical problems that need to be addressed due to the incidence, complications, mortality and economic damage It is well known that osteomyelitis pathogens are microorganisms of various species, including Staphylococcus, Pseudomonas, Candida progenitors, some representatives of the Enterobacteriaceae family (Salmonella spp et al.), Noclostridial and other anaerobes. Influence of treatment on microorganisms and macro-organisms on special and non-specific protective factors does not allow to study dynamics of immuno-microbiological patterns of osteomyelitis, development and occurrence of osteomyelitis.
       
  • Future of Reproductive System after Massive Obstetric Bleeding and
           Analysis of Childbirth Stories with Postpartum Hemorrhage

KarimovaThe aim of our study was to retrospectively examine the history of childbirth and the quality of primary care for bleeding and evaluate the prescribed rehabilitation measures for women who have suffered postpartum hemorrhage and massive bleeding. The materials and methods of the study were 242 birth histories with postpartum hemorrhage for the last 6 years (2013-2018) in the city maternity hospital of Bukhara. The average age of the patients was 26.7 ± 1.2 years. The obstetric pathology leading to bleeding mainly consists of uterus hypotonia - 143 (59.1) and large fetus - 68 (28.1), and preeclampsia and DIOV are equal amounts - 33 (13.64). Only about 20% of women who had postpartum hemorrhage underwent early rehabilitation in the form of prescribing contraceptives.The aim of the research was to study the effectiveness of modern principles of stopping postpartum obstetric bleeding. From 127 cases of bleeding in 101 women (79.5%) the childbirth were with the operational method. With the development of blood loss was renderedstepwise ways to stop bleeding. During hemostasis of the bleeding, ligatures were imposed on the ovarian arteries and the ascending branch of the uterine artery for ischemicization of the uterus, which was effective in 30 (30%) women, and in 114 (89.7%) women managed to achieve organ-sparing tactics.
       
  • Optimization of Treatment Tactics of Bleeding Gastric and Duodenal Ulcers

S.The aim of the study was to search for effective hemostasis of pyloroduodenal ulcers without surgical interventions. It would delay the operation to the stage of elective surgery, and to avoid it in number of patients. Material. The authors conducted treatment analysis of 674 patients admitted to the emergency surgery department with gastric and duodenal ulcer complicated by bleeding from 2001 to 2016. Methods. The authors developed an algorithm of diagnostic and treatment tactics for ulcerative pyloroduodenal bleeding. Results. The hemorrhage recurrence in the group of non-operated patients decreased from 7.7% (relapse was noted in 17 cases of 221) to 2.7% (in 9 patients of 329) (χ2 = 6.504; df = 1; p = 0.011) and the need for surgical treatment decreased from 30.6% (in 90 from 294 patients) to 15.8% (in 60 of 380 patients) (χ2 = 21.049; df = 2; p
       
  • Diagnostic Value of p53 Protein, Epidermal Growth Factor, and Endothelial
           Vascular Growth Factor (VEGF) in Polypous Rhinosinusitis

Material and methods: We examined 122 patients with PRS treated in the ENT department of the Khorezm Multidisciplinary Medical Center from 2015 to 2018. All patients were divided into 4 groups depending on the duration of PRS: Group I - 30 patients with a duration of PRS of up to 3 years; Group II - 31 patients with a duration of PRS of 3-6 years; Group III - 29 patients with a duration of PRS of 6-10 years; Group IV - 32 patients with a duration of PRS of 10-15 years. Results. Determination of the level of EGF revealed a significant difference in the median values of this indicator in blood plasma, while the median level of EGF was 1.4 times higher with PRS compared with the control group.   • Diagnostic Value Determination of Antibodies to Antigens of
           Micro-Organisms in Women with Inflammatory Diseases of the Pelvic Organs

OripovaTo study and determine the diagnostic value of determining the titers of antibodies to antigens of certain etiological PID agents in women of childbearing age. To achieve the goal, 304 women of reproductive age with inflammatory diseases of the pelvic organs were examined. ELISA settings were performed using diagnostic test systems for determining serum antibodies to antigens Chlamydia spp, Toxoplasma gondi, Mycoplasma spp, Cytomegalovirus, herpes simplex viruses type 1 and 2 - HSV 1,2 (test systems from XEMA, RF) Ureoplasmaurealyticum (test system of the company "Vector-Best", Russia). Antibodies to Mycoplasma spp were detected in the examined women; they were detected in 120 cases (39.5 ± 2.8%). It turned out that the monoculture of microorganisms is detected 4.2 times more often than associations, in 7.6% of cases negative samples are detected. Most antibodies were detected against antigens of other species Mycoplasma spp, Chlamydia spp, Cytomegalovirus and Herpes simplex virus types 1 and 2.

  • Duodenal Ulcer Complicated by Piloroduodenal Stenosis: Technical and
           Tactical Approaches of Treatment

Sh.Aim of the study was to improve surgical treatment results in patients with gastric and duodenal ulcers complicated by stenosis, aimed at reducing postoperative complications and reducing mortality to 0%. Material 233 patients with pyloroduodenal stenosis were divided into two groups. A comparison included 147 patients: 141 with duodenal ulcer and 6 with gastric ulcer. The main group was consisted of 86 patients: 82 cases with duodenal ulcer and 4 cases with gastric ulcer. Methods The authors developed a therapeutic and diagnostic algorithm for managing patients with gastric and duodenal ulcers, complicated by stenosis. Results The increase in the share of organ-saving surgeries led to a decrease in the frequency of specific complications from 13.6% (in 20 of 147 patients in the comparison group) to 7.0% (in 6 of 86 patients in the main group), and general complications from 10.2% (in 15 from 147) to 5.8% (in 5 of 86) and mortality from 1.4% (2 in the comparison group) to 0. Conclusion The expansion of the complex of preoperative preparation of patients with pyloroduodenal stenosis due to the inclusion of endoscopic bougienage along with other options (probing, conservative therapy) can improve not only the condition of patients, but also motor-evacuation function of the stomach.

Despite all the considerable technical efforts and intensive therapeutics, HNSCC survival rates have not improved significantly over the past two decades. HNSCC are subject to various recommendations for early detection and management in order to control this devastating disease. Therefore, it is essential to ascertain new prognostic molecular markers which may help to recognize the biological behaviour of the tumor and identify patients with high risk of disease recurrence. The aberrations in the p53 pathway in HNSCC imply that molecular and immunohistochemical analysis (IHC) of this critical tumour suppressor marker may be of diagnostic and prognostic utility in the clinical management of the disease and improve the survival of HNSCC patients. Aim & objective: The prospective study was aimed to determine the association of p53 protein expression in HNSCC cases with certain clinicopathological variables, disease recurrence, overall survival and disease free survival and to assess whether tumor suppressor p53 protein has any prognostic significance in primary HNSCC. Materials & Methods: Expressions of p53 protein were assessed in a series ofhundred histopathologically diagnosed, surgically treated, randomly selected and formalin fixed paraffin embedded (FFPE) biopsy specimens of HNSCC by IHC procedure. Clinical and histopathological data were gathered and patients were followed up for one year/death from the date of surgery. Pearsonchi-square test/ Fisher exact test were used to explore the statistical associations of p53 protein expression with clinicopathological parameters and disease recurrence of HNSCC. In order to investigate the relevance of p53 staining with overall survival and disease free survival, survival analysis was performed by using Kaplan-Meier log rank test. Results and Conclusion: The positive nuclear immunoexpression of p53 was found in 70% of the investigated malignancies, using a threshold of 10% stained cell nuclei. Out of 100 HNSCC cases, 41 died and 16 had disease recurrence during one year follow up. Statistically significant associations were observed for individual tumor stage (p = 0.010), early and advanced tumor stage (p = 0.004) and lymph node involvement (p = 0.028) with p53 immunoexpression. However, no significant correlation was found between p53 immunostaining and disease recurrence (p = 0.634), overall survival (p = 0.556) and disease free survival (p = 0.608) in HNSCC. Thus, our findings suggest that p53 serves as a poor potential prognostic biomarker in HNSCC. Clinical implications and prognostic relevance of p53 expression in HNSCC needed further studies to evaluate its role as a potential tumor marker.

IL-8 acts as one of the criteria for the severity of systemic inflammatory response in newborns. The authors argue that the level of IL-8 in urine can predict the outcome of both early and late neonatal period in newborns with infections. The criterion of severity of both early and late neonatal period in newborns with infections is the concentration of IL-8 in the urine. Prognostic criteria for the severity of CIRS in newborns with non-communicable diseases are: In the early neonatal period: - TNF-α> 20.3 PG / ml in urine; - INF-γ> 9.2 PG / ml in urine; In the late neonatal period: - IL-6> 56.9 PG / ml in blood; - TNF-α>39.0 PG/ml in blood; - IL-8>1.1 PG / ml in urine.
