           chamissoi y Chlorella peruviana contra el virus dengue serotipo 2

    • Abstract: RESUMEN Objetivo: extraer y evaluar la actividad antiviral de los compuestos de Chondracanthus chamissoi y Chlorella peruviana contra DENV-2 en células Vero-76. Materiales y métodos: se extrajeron el carragenano de Chondracanthus chamissoi, los carbohidratos solubles de Chondracanthus chamissoi y Chlorella peruviana y se realizó la prueba de toxicidad en células VERO-76 y la evaluación de la actividad antiviral. Resultados: se obtuvieron carragenanos de la fase de esporofito y gametofito de Chondracanthus chamissoi, los mismos que fueron identificados, mediante infrarrojo, como k-carragenano. Por cromatografía se identificaron nueve azúcares (ribosa, xilosa, arabinosa, fructuosa, manosa, galactosa, sucrosa, maltosa y lactosa) en la muestra de carbohidratos solubles de Chondracanthus chamissoi fase gametofito y cuatro azucares (glucosa, sucrosa, maltosa y lactosa) en la de Chlorella peruviana. Los compuestos de Chondracanthus chamissoi y la solución de carbohidratos solubles de Chlorella peruviana no presentaron efecto citotóxico; los carbohidratos del extracto crudo de Chlorella peruviana sí los tuvieron. Todas las fracciones del extracto crudo de Chondracanthus chamissoi fase gametofítica fueron positivas por la prueba de reducción del número de placas(50) a la dilución 1:5. El k-carragenano de Chondracanthus chamissoi en ambas fases y los extractos crudos de carbohidratos solubles de Chondracanthus chamissoi, Chlorella peruviana y la solución de carbohidratos solubles de Chlorella peruviana inhibieron el crecimiento del virus dengue, pero no los carbohidratos del extracto crudo de la Chlorella peruviana. Conclusiones: los compuestos obtenidos de Chondracanthus chamissoi y Chlorella peruviana presentan actividad antiviral contra DENV-2 por lo cual es necesario continuar los estudios del potencial antiviral de estos compuestos fraccionados y purificados.ABSTRACT Objective: to extract and determine the antiviral activity of compounds from Chondracanthus chamissoi and Chlorella peruviana against DENV-2 in Vero-76 cells. Materials and methods: carrageenan from Chondracanthus chamissoi and soluble carbohydrates from Chlorella peruviana were extracted, and toxicity tests in VERO-76 cells and antiviral activity were determined. Results: carrageenan from Chondracanthus chamissoi sporophyte and gametophyte phases were obtained, the compound was identified as k-carrageenan using infrared light. Nine sugars (ribose, xylose, arabinose, fructose, mannose, galactose, sucrose, maltose, and lactose) were identified using chromatography in the soluble carbohydrate mix from Chondracanthus chamissoi, and four sugars (glucose, sucrose, maltose, and lactose) from Chlorella peruviana were identified. Compounds from Chondracanthus chamissoi and Chlorella peruviana soluble carbohydrate solutions did not show any cytotoxic effect, carbohydrates from the raw extract from Chlorella peruviana did have this effect. All fractions from the raw extract from the gametophyte phase from Chondracanthus chamissoi were positive in the plate reduction test at 1:5 dilution. K-carrageenan from Chondracanthus chamissoi in both phases, as well as the crude extracts of soluble carbohydrates from Chondracanthus chamissoi and Chlorella peruviana as well as the soluble carbohydrate solution from Chlorella peruviana inhibited growth of dengue virus, but that was not the case with the carbohydrates of the raw extract from Chlorella peruviana. Conclusions: compounds obtained from Chondracanthus chamissoi and Chlorella peruviana show antiviral activity against DENV-2, so it is necessary to continue studies aiming to determine the antiviral potential of these fractioned and purified compounds.
       
  • Nivel de satisfacción en pacientes del servicio de oftalmología de un
           hospital de nivel II en los años 2014 y 2018, y su relación con calidad
           global

    • Abstract: RESUMEN Objetivo: comparar el nivel de satisfacción en los pacientes con la calidad de atención brindada en el servicio de oftalmología de un hospital de segundo nivel de atención entre los años 2014 y 2018, además determinar la calidad global en el 2018. Materiales y métodos: se realizó el estudio observacional, transversal, analítico. Se consideró una muestra de 678 pacientes y se utilizaron dos instrumentos: SERVQUAL que precisa el nivel de satisfacción en cinco dimensiones de calidad y SERVQHOS que mide la satisfacción por la calidad global del servicio recibido. Resultados : en el año 2014 la población (n=346) encuestada con SERVQUAL evidenció el nivel de "satisfecho" con la calidad de atención recibida, destacando la dimensión de seguridad; pero en el año 2018, la población estudiada (n=332) mostró que los niveles de satisfacción disminuyeron en todas sus dimensiones. Respecto al SERVQHOS los pacientes estuvieron satisfechos en su mayoría, con la calidad global y recomendarían este hospital a otras personas, su ingreso fue con el tiempo necesario, los profesionales y personal sanitario les hicieron pruebas con su permiso, conocen el nombre de su médico y reciben suficiente información sobre su estado de salud. Se determinó una débil asociación entre SERVQUAL y SERVQHOS directamente proporcional entre el nivel de satisfacción de los pacientes y la calidad global. Conclusión: en el 2014 los pacientes evidenciaron el nivel de "satisfecho" con la calidad de atención recibida comparado al 2018 que fue "levemente insatisfecho", y en la medición de la calidad global estuvieron satisfechos destacándose la atención del médico y el respeto mostrado a los pacientes.ABSTRACT Objective: to compare satisfaction levels of patients regarding quality of care in the ophthalmology service in a level II hospital between 2014 and 2018, and also, to determine overall quality in 2018. Materials and methods : this is an observational, cross section and analytical study. The sample comprised 678 patients, and two instruments were used: SERVQUAL, which measures satisfaction levels in five quality dimensions, and SERVQHOS, which measures the overall quality of services used. Results : in 2014, subjects (n= 346) interviewed with the SERVQUAL instrument declared being "satisfied" with the quality of care they received, particularly with respect to safety; but, in 2018, subjects (n= 332) declared their satisfaction levels were reduced in all scenarios. With respect to SERVQHOS, most subjects declared being satisfied with the overall quality of services and said they would recommend this hospital to other persons. Their encounters had enough time, healthcare personnel performed tests after asking for permission, subjects knew the names of physicians taking care of them, and they also said they received adequate information about their health condition. A weak association between SERVQUAL and SERVQHOS was found, which was directly proportional between patient satisfaction levels and overall quality. Conclusion : during 2014, patients declared "being satisfied" with the quality of care they received compared to 2018, when they declared being "mildly unsatisfied". When overall quality was measured, patients declared being satisfied, emphasizing medical care and respect towards them.
       
  • Características clínicas y epidemiológicas del estado nutricional en
           ingresantes a la facultad de medicina humana de una universidad privada

    • Abstract: RESUMEN Objetivos: determinar la frecuencia de malnutrición en los estudiantes ingresantes a la facultad de medicina humana de una universidad privada en el año 2018. Materiales y métodos: estudio transversal y retrospectivo, que consistió en la revisión de las historias clínicas de los estudiantes ingresantes en 2018 que pasaron evaluación médica en una facultad de Medicina de Lima, Perú. Se obtuvo frecuencias absolutas y relativas, se calculó Chi-cuadrado o Test exacto de Fisher para las variables cualitativas y Prueba de U de Mann - Whitney o T student para las cuantitativas. Se consideró como nivel de significancia al 95%. Resultados : La población de estudio fueron 263 personas; se obtuvo que el 53,99% tuvo malnutrición, conformado por un 36,5% con sobrepeso, el 11,03% obesidad y el 6,46% desnutrición. Se encontró asociación con malnutrición a: consumo de alcohol (p <0,001), tabaco (p <0,001), antecedentes familiares de diabetes tipo 2 (p < 0,001), distrito de procedencia (p= 0,014) y perímetro abdominal (p < 0,001). Conclusiones : más de la mitad de los estudiantes ingresantes a una facultad de medicina humana presentaron problemas de malnutrición, en mayor proporción por exceso de peso y estuvo asociada al riesgo cardiovascular por su aumentado perímetro abdominal y valores alterados en sus exámenes de laboratorio. Las variables asociadas a malnutrición fueron: consumo de alcohol, tabaco, antecedente familiar de diabetes tipo 2, distrito de procedencia y perímetro abdominal.ABSTRACT Objectives: To determine the frequency of malnutrition in students recently admitted to a private medical school in 2018. Materials and methods : This is a cross-sectional and retrospective study which consisted in reviewing the records from students admitted in 2018 to a private medical school in Lima, Peru. Absolute and relative rates were obtained, Chi-square test and Fisher’s exact test were used for qualitative variables and U Mann-Whitney’s test or Student’s t test were used for quantitative variables. Significance level was set at 95%. Results : The study population consisted of 263 subjects. More than half of them (53.99%) had malnutrition, which was established as 36.5% overweight persons, 11.03% were obese, and 6.46% were undernourished. The following findings were associated to malnutrition: alcohol consumption (p<0.001), family history of type 2 diabetes (p<0.001), district of origin (p= 0.014), and abdominal circumference (p<0.001). Conclusions : More than half of the students admitted to a private medical school had malnutrition, mainly because of being overweight, and this was associated with cardiovascular risk because of having large abdominal circumference and altered laboratory tests. Variables associated to malnutrition were as follows: alcohol consumption, tobacco use, family history of type 2 diabetes, district of origin, and abdominal circumference.
       
  • Factores asociados a la presencia de prostatitis aguda bacteriana en
           pacientes sometidos a biopsia prostática transrectal entre los años 2015
           y 2019 en un hospital de las Fuerzas Armadas de Lima, Perú

    • Abstract: RESUMEN Objetivo: determinar los factores asociados a la presencia de prostatitis aguda bacteriana en pacientes sometidos a biopsia prostática transrectal entre los años 2015 y 2019 en un Hospital de las Fuerzas Armadas de Lima, Perú. Materiales y métodos: estudio observacional transversal retrospectivo. Se evaluaron historias clínicas de 142 pacientes, seleccionados por muestreo no probabilístico. Analizamos los factores asociados a la presencia de prostatitis aguda bacteriana (PAB) en pacientes sometidos a biopsia prostática transrectal (modelo-1) y el número de factores asociados a PAB (modelo-2). Resultados: la presencia de prostatitis aguda bacteriana (PAB) fue 15,5%. La PAB fue mayor en pacientes diabéticos, hipertensos y los que padecían de insuficiencia renal crónica. En el modelo-1, el antecedente de diabetes mellitus tipo 2 (razón de prevalencia ajustada (RPa): 3,18; IC 95%: 0,92 - 11,03), la hipertensión arterial (RPa: 1,78; IC 95%: 0,53 - 5,98), la insuficiencia renal (RPa: 2,73; IC 95%: 0,60 - 12,46) y la prostatitis crónica (RPa: 4,5; IC 95%: 0,99 - 20,43) no se encontraron asociados (p >0,05). En el modelo-2, encontramos que aquellos sujetos entre 66-75 años, tienen 50% menor probabilidad de presentar PAB que aquellos sujetos de 50-65 años y que los sujetos mayores de 75 años tienen 2,7 (IC 95%: 0,86 - 8,13) veces mayor probabilidad que el grupo más joven. Finalmente, se observó que un mayor número de comorbilidades incrementa la probabilidad de presentar PAB, siendo 4,80 (IC 95%: 1,86 - 12,42) veces en aquellos con una enfermedad, 5,77 (IC 95%: 1,03 - 32,13) con dos enfermedades y 10,47 (IC 95%: 1,98 - 55,41) veces mayor, en aquellos con tres enfermedades. Conclusiones: en nuestro estudio, el antecedente de diabetes mellitus tipo 2 y el número de comorbilidades crónicas incrementan la probabilidad de presentar PAB. Se recomienda realizar un mayor número de estudios que permitan verificar nuestros resultados.ABSTRACT Objective : to determine the factors associated with the presence of acute bacterial prostatitis in patients undergoing transrectal prostate biopsy between 2015 and 2019 at an Army Forces Hospital in Lima, Peru. Materials and methods : this is a retrospective observational and cross-sectional study. Clinical records from 142 patients were reviewed, which were selected using non-probabilistic sampling. We analyzed factors associated with the occurrence of acute bacterial prostatitis (ABP) in patients undergoing a transrectal prostate biopsy (model 1), and the number of factors associated with ABP (model 2). Results : the rate of ABP was 15.5%. This condition was more prevalent in subjects with diabetes, high blood pressure and chronic renal failure. In model 1, a past history of type 2 diabetes mellitus (adjusted prevalence ratio (aPR): 3.18; 95% CI: 0.92-11.03), high blood pressure (aPR: 1.78; 95% CI: 0.53-5.98), renal failure (aPR: 2.73; 95% CI: 0.60-12.46), and chronic prostatitis (aPR: 4.5; 95% CI: 0.99-20.43) were not associated (p >0.05). In model 2, we found that subjects between 66-75 years of age had a 50% less likelihood for developing APB compared with those in the 50-65 years old group, and that subjects more than 75 years old have a 2.7-fold (95% CI: 0.86-8.13) likelihood for this condition compared with subjects from younger age groups. Finally, it was observed that a greater number of comorbidities increases the likelihood for developing ABP, being this 4.80-fold (95% CI: 1.86-12.42) in those with one concomitant disease, 5.77-fold (95% CI: 1.03-32.13) in those with two concomitant diseases, and 10.47- fold (95% CI: 1.98-55.41) in those who had three concomitant diseases. Conclusions : in this study, a past history of type 2 diabetes mellitus and the number of concomitant chronic conditions increase the likelihood for developing ABP. It is recommended to perform further research on this topic, so our results may be verified.
       
  • Desarrollo de un sistema microfluidico (lab-on-a-chip) accesible y de bajo
           costo para detección de células tumorales circulantes de cáncer de mama
           

    • Abstract: RESUMEN Objetivo: desarrollar un sistema microfluídico (lab-on-a-chip) para la detección de células tumorales circulantes de cáncer de mama (CTCs). Materiales y métodos: se diseñó el dispositivo en 3D y se fabricó usando fotolitografía suave y una cortadora láser. Se evaluó el funcionamiento del sistema y del arreglo magnético usando células Jurkat y células de cáncer de mama que poseen diferente expresión de los marcadores superficiales CD45 y EpCAM. Los anticuerpos contra los marcadores fueron unidos a perlas magnéticas. Adicionalmente se usaron nanopartículas de hierro para evaluar su atrapamiento. Resultados: las nanopartículas lograron atraparse de manera significativa en el área propuesta por el modelamiento de campos magnéticos. Las células tumorales marcadas con los anticuerpos magnéticos quedaron atrapadas. Conclusiones: se logró fabricar un lab-on-a-chip capaz de atrapar CTCs generando una excelente herramienta de diagnóstico y de análisis de la progresión de la enfermedad.ABSTRACT Objective. to develop a microfluidic system (lab-on-a-chip) for detecting circulating breast cancer tumor cells. Materials and methods . the device was designed using 3D technology, and it was manufactures using soft photolithography and a laser cutting machine. The system performance and its magnetic settings were assessed using Jurkat cells and breast cancer cells that show different expression of CD45 and EpCAM surface markers. Antibodies against these markers were bound to magnetic pellets. Additionally, iron nanoparticles were used for assessing their entrapment. Results . nanoparticles were significantly trapped in the area set by magnetic field modeling. Tumor cells labeled with magnetic antibodies became trapped. Conclusions . we were able to manufacture a lab-on-a-chip system that is capable to trap circulating breast cancer tumor cells, which may become an excellent tool for diagnosis and follow-up for this condition.
       
  • Efecto sobre la talla en niñas con pubertad precoz central de
           diagnóstico tardío tratadas con análogos de GnRH

    • Abstract: RESUMEN Objetivo: evaluar el efecto de los análogos de GnRH sobre la predicción de talla final adulta en niñas con pubertad precoz central de diagnóstico tardío. Materiales y métodos: estudio descriptivo, retrospectivo. Se incluyeron niñas atendidas entre 2012 y 2018 en la Unidad de Endocrinología Pediátrica del Hospital Cayetano Heredia con diagnóstico de pubertad precoz central idiopática de diagnóstico tardío. Se realizó evaluación antropométrica, evaluación de estadío Tanner y determinación de edad ósea con el método de Greulich y Pyle, estimando la predicción de talla final adulta mediante el método de Bayley Pinneau antes y después del tratamiento con análogos de GnRH. Se realizó t de student para comparar la predicción de talla antes y después del tratamiento. Se consideró significativo un p< 0,05. Resultados: se incluyeron 23 niñas con edad cronológica media de 8,4 ± 0,4 años y una edad ósea de 10,7 ± 0,8 años al ingreso. El 83,9% de las pacientes tuvieron exceso de peso y el 74,0% estadio Tanner 3 al diagnóstico. El tiempo promedio de tratamiento con análogos de GnRH fue de 20,5 ± 8,4 meses. El efecto del tratamiento calculado con la diferencia de predicción de talla final adulta menos predicción de talla inicial fue de 2,5 ± 4,1 cm (p<0,01). Conclusión: las niñas con diagnóstico de pubertad precoz central de diagnóstico tardío muestran un discreto beneficio sobre la predicción de talla final adulta, por lo que la terapia a esta edad debe ser individualizada.ABSTRACT Objective: to assess the effect of GnRH analogues on predicted adult final height in girls with a late diagnosis of central precocious puberty. Material and Methods : this is a descriptive and retrospective study. Girls seen between 2012 and 2018 in the Pediatric Endocrinology Unit of Hospital Nacional Cayetano Heredia with a late diagnosis of idiopathic precocious central puberty were included. Anthropometric assessments were performed, Tanner stage assessment and bone age determination using the Greulich and Pyle method were performed, and the final adult height was predicted using the Bailey Pinneau method before and after therapy. A Student-t test was used for comparing the height prediction before and after therapy. A p<0.05 value was considered as significant. Results : twenty-three girls with mean chronological age 8.4 ± 0.4 years were included. Their bone age was 10.7 ± 0.8 on admission. Most of the patients (83.9%) were overweight and 74.3% were Tanner 3 at the time of diagnosis. The average time using GnRH analogues therapy was 20.5 ± 8.4 months. The effect of therapy, which was calculated using the prediction difference for the final adult height minus the initial height prediction was 2.5 ± 4.1 cm (p<0.001). Conclusion : girls with a diagnosis of late diagnosis central precocious puberty show a discrete benefit on the prediction of the final adult height, so therapy at this age should be individualized.
       
  • Diagnóstico y tratamiento de la etapa aguda del accidente cerebrovascular
           isquémico: Guía de práctica clínica del Seguro Social del Perú
           (EsSalud)

    • Abstract: RESUMEN Objetivo: proveer recomendaciones clínicas basadas en evidencia para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud. Materiales y métodos: se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 8 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y -cuando fue considerado pertinente- estudios primarios en Medline y Cochrane Controlled Register of Trials durante el 2018. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y el flujograma de diagnóstico y tratamiento. Finalmente, la GPC fue aprobada con Resolución N° 128-IETSI-ESSALUD-2019. Resultados: la presente GPC abordó 8 preguntas clínicas, divididas en cuatro temas: tamizaje, diagnóstico, tratamiento, soporte y rehabilitación. En base a estas preguntas se formularon 28 recomendaciones (8 fuertes y 20 condicionales), 38 puntos de buena práctica clínica, 1 nota de implementación y 2 flujogramas. Conclusión: el presente artículo resume la metodología y las conclusiones basadas en evidencias de la GPC para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud.ABSTRACT Objective: to provide evidence based clinical recommendations for the diagnosis and therapy of the acute stage of ischemic cerebrovascular accident in EsSalud. Materials and methods : a group for producing the guideline was conveyed, including specialized physicians and methodologists. The group prepared 8 clinical questions to be answered by the guideline. Systematic searches of previous reviews were performed, and - when it was deemed necessary - primary studies in Medline and the Cochrane Controlled Registry of Trials for 2018 were reviewed. Evidence was selected aiming to respond each one of the proposed clinical questions. Certainty of the evidence was determined using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work sessions, the group used GRADE methodology for reviewing the evidence and generated recommendations, good clinical practice items, and designed flow charts for both diagnosis and therapy. Finally, the guide was approved with the decree N° 128-IETSI-ESSALUD-2019. Results : this clinical practice guideline focused in 8 clinical questions, which were divided into four topics: screening, diagnosis, therapy, support and rehabilitation. On the basis of these questions, 28 recommendations were formulated (8 strong and 20 conditional), 38 items for good clinical practice, 1 implementation note, and 2 flow charts. Conclusion : this paper summarizes both evidence based methodology and conclusions from a new clinical practice guide for diagnosis and therapy of the acute stage of a cerebrovascular accident in EsSalud.
       
  • Desorden esquelético con malformaciones faciales en cerámico de
           la cultura Moche

    • Abstract: RESUMEN La cultura Moche se desarrolló entre los años 200 y 850 d. C. en la costa norte del Perú. Sus vasijas de cerámica presentan un gran nivel de detalle, alto grado de realismo y se ha podido identificar distintas malformaciones físicas en sus representaciones. Se presenta el caso del cerámico C-00122 del Museo de Historia Natural y Cultural de la Universidad Privada Antenor Orrego (MHNC-UPAO), con un cuadro de desorden esquelético asociado a malformaciones faciales, además se plantean una serie de posibles diagnósticos etiológicos.ABSTRACT Moche is a culture that developed between 200 y 850 AD in the Peruvian northern coast. Their pottery artefacts were crafted showing many details, they were very realistic, and different physical malformations have been identified in these pieces. We present the case of the piece C-00122 that is kept in the Natural and Cultural History Museum of Universidad Privada Antenor Orrego (NCHM-UPAO), which shows a skeletal disorder associated to some facial malformations. We discuss the case and propose different likely diagnoses.
       
  • Ampliación del tamizaje de errores innatos del metabolismo en Perú:
           reporte de caso con trastorno del metabolismo de cobalamina

    • Abstract: RESUMEN El tamizaje neonatal de los errores innatos del metabolismo se instauró hace más de 50 años en el mundo. En Latinoamérica, Uruguay, Costa Rica, Chile, Brasil y Colombia han implementado esta política de salud pública de manera sostenida. La tecnología para detectar estas enfermedades ha ido progresando con un mejor costo/efectividad, haciendo que sea de acceso casi universal. Los trastornos del metabolismo intracelular de la cobalamina es un grupo heterogéneo clasificados en tres fenotipos bioquímicos. Reportamos al primer paciente en Perú con diagnóstico tardío de una variante homocigota c.394 C>T en el gen MMACHC, el cual pertenece al grupo de complementación cblC el cual produce aciduria metilmalónica y homocistinuria, caracterizado por talla baja, hipotonía, retraso del desarrollo psicomotor, convulsiones, anemia megaloblástica, trombocitopenia y neutropenia ondulantes; con homocisteína elevada, acidemia metilmalónica, y contradictoriamente aumento de vitamina B12 en sangre. Es importante el diagnóstico oportuno de enfermedades potencialmente tratables, evitando o disminuyendo la severidad del fenotipo, a través de la implementación de nuevas tecnologías en nuestro país.ABSTRACT Neonatal screening for innate metabolism disorders was instituted more than 50 years ago. In Latin America, countries like Uruguay, Costa Rica, Chile, Brazil, and Colombia have implemented this public health measurement in a sustained fashion. Technology for detecting these conditions has been steadily progressing, achieving a good cost/effectiveness ratio, so access for such test is practically universal. Intracellular cobalamin metabolism disorders constitute a heterogeneous group that is subdivided in three biochemical phenotypes. We report the first patient in Peru with a late diagnosis of a homozygous c.394 C>T variant in the MMACHC gene, which belongs to the cbIC complementation group, which leads to methyl-malonic aciduria and homocystinuria, characterized by low height, retardation of psychomotor development, seizures, megaloblastic anemia, and variable thrombocytopenia and neutropenia. Also, homocysteine levels are high, there is methyl-malonic academia, and there is a paradoxical vitamin B12 increase in peripheral blood. This paper emphasizes the importance of making a timely diagnosis of potentially treatable conditions, avoiding or reducing the severity of the implied phenotype, with the implementation of new technologies in our country.
       
  • Recomendaciones clínicas para la evaluación y manejo de riesgo de
           COVID-19 en personal de salud del Seguro Social del Perú (EsSalud)

    • Abstract: RESUMEN Actualmente estamos viviendo una pandemia de enfermedad causada por coronavirus (COVID-19). Un grupo vulnerable de ser infectado por este agente patógeno es el personal de salud. Por ello, el Instituto de Evaluación de Tecnologías en Salud e Investigación (IETSI) del Seguro Social del Perú (EsSalud) ha emitido recomendaciones clínicas sobre la Evaluación de riesgo y manejo de personal de salud en el contexto de la pandemia de COVID-19, que resumiremos en el presente artículo. Se incluye un flujograma de evaluación y manejo, así como una tabla para determinar el riesgo de presentar infección por COVID-19.ABSTRACT We are currently experiencing a coronavirus disease (COVID-19) pandemic. A vulnerable group of being infected by this pathogenic agent is health personnel. For this reason, the Health Technology Assessment and Research Institute (IETSI in Spanish) of the Peruvian social insurance (EsSalud) has issued clinical recommendations on risk assessment and management of health care personnel in the context of the COVID-19 pandemic, which we will summarize in this paper. An evaluation and management flow chart are included, as well as a table to determine the risk of developing COVID-19 infection.
       
  • Depresión perinatal en tiempos del COVID-19: rol de las redes
           sociales en Internet

    • Abstract: RESUMEN Las emergencias en salud pública como la pandemia del COVID-19 podrían llevar a situaciones de estrés extremo, ansiedad y depresión en mujeres en etapa perinatal. La depresión perinatal puede ocasionar a la madre, al recién nacido y al entorno familiar múltiples consecuencias negativas. Las redes sociales, bien utilizadas, pueden constituir una herramienta valiosa de apoyo para las mujeres en esta etapa de su vida. Este artículo describe el rol de las redes sociales en Internet como soporte de las gestantes y madres con depresión, particularmente en emergencias de salud pública a nivel mundial como la pandemia del COVID-19. Las redes sociales pueden contribuir a la salud y al bienestar al facilitar la interacción social, construir comunidades, normalizar el comportamiento de búsqueda de ayuda, inspirar cambios saludables en el estilo de vida y ayudar a la investigación médica. Es fundamental educar adecuadamente a las mujeres en etapa perinatal en alfabetización digital en salud que incluya el uso eficiente de las redes en Internet como soporte socioemocional y la evaluación de la calidad de información que se encuentra en las redes sociales y en Internet para una adecuada toma de decisiones.ABSTRACT Public health emergencies such as COVID-19 pandemic could lead to situations of extreme stress, anxiety and depression in women in the perinatal period. Perinatal depression may lead to multiple negative consequences for mothers, newborns, and their relatives. Appropriate use of social media may be a valuable supporting tool for women during this stage of their lives. This paper describes the role of social media on the Internet for supporting pregnant women and mothers with depression, particularly in global public health emergencies such as the COVID-19 pandemic. Social media may contribute to support health and wellness by facilitating social interaction, building communities, normalizing help-seeking behavior, inspiring healthy lifestyle changes, and promoting medical research. It is essential to adequately educate women in the perinatal stage in digital health literacy including the efficient use of social media for socio-emotional support and assuring the quality of the information circulating in social media and in Internet for proper decision-making.
       
  • Una visión peruana sobre los servicios de patología clínica: avances y
           agenda pendiente

    • Abstract: RESUMEN Los países realizan muchos esfuerzos, para poder implementar los objetivos de desarrollo sostenible, en búsqueda de la cobertura universal de la salud. Sin embargo, el acceso o la oportunidad de los servicios de Patología Clínica de alta calidad y oportunos, son necesarios para respaldar los sistemas de atención médica que tienen la tarea de lograr estos objetivos. Este acceso es más difícil de lograr en los países de bajos y medianos ingresos. En este artículo especial, se realizó un análisis de la situación global y nacional de esta problemática, en esta especialidad médica. Se identifican las cuatro barreras clave para la provisión de servicios de calidad y óptimos: recursos humanos o capacidad de la fuerza laboral insuficiente, educación y capacitación inadecuada, infraestructura y equipamiento inadecuado, y calidad, normas y acreditación insuficientes.ABSTRACT Countries do plenty of effort in order to implement the objectives for sustainable development, aiming to achieve universal health coverage. However, access or chances for having high quality Clinical Pathology services are both necessary for supporting medical care systems if such objectives are to be achieved. This access is much more difficult to achieve in low- and medium- income countries. This paper presents an analysis of both the global and local situation in this respect. Four key barriers for delivering high quality and optimal services were identified: insufficient human resources or limited capability of the working force, inadequate education and training, inadequate infrastructure and equipment, and insufficient quality, regulations, and accreditation strategies.

    • Abstract: RESUMEN El doctor David Matto Usandivaras realizó importantes contribuciones a la medicina peruana, en particular en las especialidades de Patología Clínica y Psiquiatría, hacia finales del siglo XIX e inicios del siglo XX, que coincidió con la corriente cientificista que comenzó a prevalecer paulatinamente en los años posteriores a la Guerra del Pacífico. No obstante, el trascurrir del tiempo difuminó tales aportes, por lo que el objetivo de este artículo es rescatarlos y situarlos en el lugar que merecen para memoria de todos los miembros de la comunidad médica nacional y en especial de los facultativos de ambas especialidades.ABSTRACT Doctor David Matto-Usandivaras made important contributions in Peruvian medicine, particularly in clinical pathology and psychiatrics, at the end of 19th and at the beginning of the 20th century, coinciding with the scientificist thoughts that became prevalent in the years after the war between Peru and Chile. Nonetheless, time passed by and his contributions became forgotten, so the purpose of this paper is to rescue Dr. Matto-Usandivaras contributions and place them in a proper context so members of the Peruvian medical community - particularly pathologists and psychiatrists - may become more knowledgeable of his valuable work.
