    • Abstract: Objetivo: Explorar los conocimientos, percepciones y prácticas de familias, actores locales y personal de salud (PS) de la provincia de Condorcanqui, respecto al consumo de micronutrientes (MMN) en niños de 06 a 35 meses de edad. Materiales y métodos: Estudio cualitativo realizado entre junio y agosto del 2015, en población Awajún y Wampis de los distritos de Río Santiago, Cenepa y Nieva; incluyó 38 entrevistas en profundidad, 13 grupos focales y 28 observaciones directas. Resultados:La comunidad entendió que la anemia causaba palidez o delgadez en el niño y pueden tratarla con sus propios ‘remedios’. El PS consideraba importante comprender al usuario y observaba problemas de acceso y adecuación cultural en su labor. Reconoció que la capacitación recibida fue insuficiente, pero cumplía con sensibilizar a la comunidad. Los MMN se dieron en la primera comida del día y con alimentos locales, observándose valoraciones positivas y negativas (causa diarrea, alergias o rechazo por su sabor u olor) sobre su aceptabilidad. Existe retroalimentación de la información monitoreada en la consulta y visita domiciliaria, pero no puede asegurarse el consumo y adherencia al MMN. Conclusiones: La intervención debe incorporar elementos culturales y mensajes claros sobre el beneficio y contenido de los MMN. Se deben generar alianzas estratégicas y promover el trabajo interinstitucional con diferentes actores sociales, para facilitar la sensibilización y acceso al MMN y alimentos básicos.Objective: To explore knowledge, perception, and practices within families, local actors, and healthcare personnel (HP) in Condorcanqui Province, with respect to micronutrient use (MMN) in 6- to 36- month old children. Materials and Methods: This was a qualitative study performed between June and August 2015 in Awajun and Wampis communities in Rio Santiago, Cenepa, and Nieva districts. The study included 38 in-depth interviews, 13 focus group meetings, and 28 direct observation activities. Results: Persons in the community understand that anemia leads to pallor and wasting in children, and that they may treat this condition with their own local ‘medicine’. Healthcare personnel understand that it is important to educate users and there are difficulties for access and cultural adaptation for performing their activities. They recognize that training may have been insufficient, but they do their best aiming to sensitize the community. MMN are given with the first meal of the day and combined with local food. Positive and negative comments (perceived as causative agents for diarrhea and nausea, or being rejected because of their taste and smell) were observed with respect to their acceptability. There is some feedback on information collected in office visits and household visits, but the use of MMN cannot be fully ensured. Conclusions: Any intervention must incorporate cultural elements and clear messages about benefits and contents of MMN. Strategic alliances should be established and inter-institutional activities should be performed with different social actors, in order to facilitate sensitization and access for using MMN as well as basic food components.
       
  • Cáncer colorrectal metastásico: supervivencia global con diferentes
           alternativas de tratamiento en Lima Metropolitana

    • Abstract: Objetivos: Evaluar la supervivencia global del cáncer colorrectal metastásico en Lima Metropolitana de acuerdo a la terapia empleada (independientemente del estado mutacional del gen K-ras) y comparar los resultados con los obtenidos en otros estudios similares en el mundo. Materiales y métodos: Estudio multicéntrico retrospectivo. Se analizaron las historias clínicas de pacientes con cáncer colorrectal metastásico, entre enero del 2010 y diciembre del 2014, en pacientes que fueron tratados en hospitales especializados de Lima Metropolitana. De los 419 casos con diagnóstico de cáncer colorrectal metastásico, 320 cumplieron con los criterios de inclusión correspondientes (76,4%), por lo cual fueron considerados en este estudio. Resultados: La mediana de la edad del total de participantes fue 63 años (rango: 15 a 90 años). El porcentaje de supervivencia a 2 y 5 años fue: 87,3% y 12,9% para aquellos que fueron sometidos a resección de metástasis y fue de 39,6% y 9,1% para aquellos pacientes con tratamiento quimioterápico (con o sin terapia biológica), la diferencia en estos dos grupos fue estadísticamente significativa en favor de la resección de las metástasis (p
       
  • Efecto de una emulsión lubricante en la sintomatología, daño a la
           superficie ocular e inestabilidad de la película lagrimal de pacientes
           con ojo seco asociado al síndrome visual informático

    • Abstract: Objetivo: Evaluar el efecto de una emulsión lubricante en la sintomatología, inestabilidad de la película lagrimal (PL) y daño a la superficie ocular de pacientes con ojo seco asociado al síndrome visual informático (SVI). Materiales y métodos: Estudio de serie de casos, que analizó el efecto de una emulsión lubricante en pacientes con ojo seco asociado al SVI. Los síntomas se evaluaron a través del cuestionario Ocular Surface Disease Index (OSDI), la inestabilidad de la PL a través del tiempo de ruptura de película lagrimal con fluoresceína (TRPL-F), y el daño a la superficie ocular a través del test de Oxford.   • Crecimiento físico de niños con hiperplasia suprarrenal congénita
           perdedora de sal durante los dos primeros años de vida en el Instituto
           Nacional de Salud del Niño

    • Abstract: Objetivo: Describir el crecimiento físico de niños con hiperplasia suprarrenal congénita (HSC) perdedora de sal durante los dos primeros años de vida, evaluados en el Servicio de Endocrinología del Instituto Nacional de Salud del Niño (INSN), Lima-Perú. Materiales y métodos: Es un estudio observacional, descriptivo y retrospectivo. Se incluyeron 55 niños con HSC que tuvieron seguimiento en el Servicio de Endocrinología del INSN del 2000 al 2016; se recopilaron datos como peso y talla al nacer y cada 6 meses, edad al diagnóstico, dosis de hidrocortisona y fludrocortisona y velocidad de crecimiento. Resultados: Se analizaron 55 niños con HSC perdedora de sal siendo el diagnóstico más precoz en mujeres (mediana: 21 días) que en varones (mediana: 52 días). Al nacimiento, su longitud promedio fue 0,07 + 0,75 DE; a los 6 meses fue -1,67 + 1,33 DE con mayor compromiso en varones (-2,29 + 1,18 DE; p=0,022); a los 12 meses fue -1,84 + 1,27 DE, con recuperación a los 24 meses (-1,51 + 1,10 DE). La velocidad de crecimiento fue -1,03 + 1,62 DE y -0,89 + 1,06 DE en el en el primer y segundo año, respectivamente. Conclusiones: Los niños con HSC perdedora de sal que tuvieron seguimiento en el INSN, tuvieron menor longitud a los 6 y 12 meses de edad y lograron recuperarse a los 24 meses.Objective: To describe physical growth features in children with salt-wasting congenital adrenal hyperplasia (CAH) during their first two years of life, in the Endocrinology Service of the Peruvian National Children’s Health Institute (CHI) in Lima, Peru. Materials and Methods: This is a retrospective, observational, and descriptive study. Fifty-five children with CAH underwent follow-up in the Endocrinology Service of the CHI from 2000 to 2016; data such as body weight and height were collected at birth time and every six months, as well as age at diagnosis, hydrocortisone and fludrocortisone doses, and growth velocity. Results: Fifty-five children with salt-wasting CAH were assessed, the diagnosis was more promptly made in females (median: 21 days) compared to males (median: 52 days). At birth, their average length was 0.07 ± 0.75 SD; at 6 months it was -1.67 ± 1.33 SD, and male subjects were more affected (-2.29 ± 1.18 SD; p= 0.022); at 12 months, this parameter was -1.84 ± 1.27 SD, and recovery was observed at 24 months (-1.51 ± 1.10 SD). Growth velocity values were -1.03 ± 1.62 SD and -0.89 ± 1.06 SD in the first and second years of life, respectively. Conclusions: Children with salt-wasting CAH who were followed-up at CHI had shorter length at 6- and 12- months of age, and their recovered at 24-monbths.
       
  • Efecto del protocolo de Rivers en la mejora de la hemodinamia y
           oxigenación en pacientes con shock séptico por neumonía grave del
           Hospital III EsSalud Chimbote

    • Abstract: Objetivos: Determinar la relación entre la aplicación del protocolo de Rivers y la mejora hemodinámica y oxigenatoria en pacientes con shock séptico por neumonía grave atendidos en el Hospital III EsSalud Chimbote durante el periodo julio a diciembre 2018. Materiales y métodos: Ensayo no controlado que incluyó a 30 pacientes con shock séptico por neumonía a quienes se aplicó el protocolo de Rivers, y se evaluó si efecto en indicadores hemodinámicos y oxigenatorios, según puntuación SOFA. Se aplicó la prueba de Wilcoxon para determinar el efecto de la aplicación del protocolo Rivers. Resultados: El score SOFA antes de aplicar el protocolo de Rivers presentó en la evaluación hemodinámica una mediana de 2,5 y de 2,0 al final (p < 0,01). Respecto a la evaluación oxigenatoria, la mediana inicial fue 2,5 y de 1,0 al final (p < 0,01). Se encontró que, luego de la aplicación del protocolo de Rivers, 21 de 30 pacientes presentaron mejoría en la hemodinamia y 27 de 30 pacientes presentaron mejoría oxigenatoria. Conclusiones: El presente estudio recomienda considerar la aplicación del protocolo de Rivers en pacientes con shock séptico por neumonía grave, con el fin de mejorar la hemodinamia y oxigenación.Objectives: To determine the relationship between the use of Rivers Protocol and hemodynamics and oxygenation status improvement in patients with septic shock due to severe pneumonia seen at EsSalud III Hospital in Chimbote from July to December 2018.   • Planificación virtual y modelo de impresión en 3D para reconstrucción
           mandibular con colgajo libre de peroné en ameloblastoma mandibular

    • Abstract: El ameloblastoma es un tumor benigno, localmente agresivo, que ocasiona deformidades faciales y con tendencia a la recurrencia. El objetivo del tratamiento del ameloblastoma es la remoción completa del tumor, la restauración de la función y el mantenimiento del aspecto estético. Se presenta el caso de un varón de 19 años que presenta tumoración en región mandibular y acude a consulta para tratamiento quirúrgico. Se realizó planificación virtual y modelo de impresión en 3D para reconstrucción mandibular con colgajo libre de peroné y modelo estereolitográfico para el manejo de su patología. Se observó una evolución post operatoria favorable; y una patología compatible con ameloblastoma mandibular intraóseo de patrón plexiforme. Si bien la cirugía es el tratamiento de elección para el diagnóstico de ameloblastoma; la planificación virtual y el modelo de impresión en 3D para la reconstrucción son herramientas útiles en el manejo de esta patología.Ameloblastoma is a benign, locally aggressive tumor that causes facial deformities and a tendency to recur. The objective of the treatment of ameloblastoma is the complete elimination of the tumor, the restoration of function and the maintenance of the aesthetic appearance. The case of a 19-year-old male presenting a tumor in the mandibular region is presented and a consultation for surgical treatment is accessed. Virtual planning and 3D printing model were performed for mandibular reconstruction with fibula-free flap and stereolithographic model for the management of its pathology. A favorable post-operative evolution evolved; and a pathology compatible with intraosseous mandibular ameloblastoma of the plexiform pattern. Although surgery is the treatment of choice for the diagnosis of ameloblastoma; Virtual planning and the 3D printing model for reconstruction are useful tools in the management of this pathology.
       
  • Tratamiento exitoso de síndrome hemofagocítico secundario a infección
           por virus de Epstein Barr: Reporte de caso

    • Abstract: Se presenta el caso de un paciente varón de seis años de edad, con síndrome hemofagocítico secundario a infección por virus de Epstein-Barr. El paciente inició el cuadro con fiebre, hepatoesplenomegalia, falla hepática, trastornos de la coagulación, ferritina, triglicéridos y disminución de todas las líneas celulares hematológicas. El aspirado de médula ósea evidenció la presencia de citofagocitosis, por lo que inició tratamiento específico según protocolo HLH 2004, respondiendo favorablemente luego de cuatro semanas. Actualmente, el paciente continúa sus controles por la especialidad sin recaída de enfermedad, con adecuado desarrollo y crecimiento. En nuestro país, son escasos los reportes de síndrome hemofagocítico; sin embargo, puede resultar una patología más frecuente de la que estimamos por lo que, es importante reportar estos casos y más aún el éxito del tratamiento a fin de continuar mejorando su manejo y reporte.We present the case of a six-year old male patient who presented with hemophagocytic syndrome secondary to Epstein Barr virus infection. The patient had fever, hepatosplenomegaly, liver failure, coagulation, ferritin, and triglyceride disorders, as well as a significant reduction of all blood cell populations. Bone marrow aspirate revealed the presence of autophagocytosis, so specific therapy according to the HLH 2004 protocol was started, and the patient satisfactorily responded after four weeks. Nowadays the patient still attends his control appointments, no relapse has been detected, and both his development and growth are adequate. Few reports on hemophagocytic syndrome have been published in Peru; however, this condition may be more frequent than previously thought. Therefore, it is important to report such cases, particularly successful experiences, aiming to improve management of this condition.
       
  • Degeneración nodular de Salzmann en paciente mujer de 20 años de edad:
           un reporte de caso

    • Abstract: La degeneración nodular de Salzmann es un trastorno corneal no inflamatorio, infrecuente, de mayor prevalencia en mujeres mayores de 50 años. Se caracteriza por la presencia de nódulos blanquecinos en la córnea y está asociada a diversas patologías. Reportamos el caso de una paciente de 20 años con historia de astigmatismo, miopía y síndrome de ovario poliquístico. Acudió a consulta por presentar sensación de cuerpo extraño en el ojo izquierdo y negó el haber padecido infección o trauma ocular recientes. Al examen oftalmológico se evidenció disfunción de las glándulas de Meibomio y nódulos opacos en la periferia de ambas córneas. Los hallazgos clínicos y la tomografía de coherencia óptica confirmaron el diagnóstico de degeneración nodular de Salzmann. El tratamiento con ciclosporina tópica demostró ser efectivo en la disminución de la inflamación causada por la disfunción de las glándulas de Meibomio, lo que evitó recurrencias nodulares a corto plazo.Salzmann’s nodular degeneration is an uncommon non-inflammatory corneal disorder, which is more prevalent in women more than 50 years old. It is characterized by the presence of whitish nodules in the cornea and it is associated to certain conditions. We report the case of a 20-year old female patient with past history of astigmatism, myopia, and polycystic ovary syndrome. She presented reporting a foreign body sensation in her left eye and she denied having had recent eye infection or trauma. The ophthalmological examination revealed Meibomian gland dysfunction and opaque nodules in the periphery of both corneas. Clinical findings and an optical coherence tomography (OCT) corroborated the diagnosis of Salzmann’s nodular degeneration. Therapy using topical cyclosporin proved to be effective in reducing the inflammation caused by Meibomian gland dysfunction, which prevented nodular recurrence in the short term.
       
  • Guía de práctica clínica para el diagnóstico y manejo de la
           osteoartritis en el Seguro Social del Perú (EsSalud)

    • Abstract: Este artículo resume la guía de práctica clínica (GPC) para el diagnóstico y manejo de pacientes con osteoartritis (OA) del Seguro Social del Perú (EsSalud). Para su desarrollo se conformó un grupo elaborador de la guía (GEG) que incluyó a especialistas clínicos y metodólogos. Para responder las 13 preguntas clínicas planteadas se realizaron búsquedas sistemáticas en PubMed y repositorios de GPC. Se usó la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE) para evaluar la certeza de la evidencia. Se emitieron 11 recomendaciones: tres fuertes (no solicitar de forma rutinaria radiografías para el diagnóstico de OA, brindar un plan individualizado de medidas de educación y autocuidado, indicar la realización de ejercicio físico) y ocho débiles (intervenciones para reducir el peso en personas con obesidad o sobrepeso, evitar el uso de paracetamol para el manejo de OA, promover el uso de AINE orales para el manejo de OA, evitar el uso de condroitín sulfato para el manejo de OA, evitar el uso de proloterapia para el manejo de OA, en adultos con OA y dolor refractario a manejo farmacológico considerar terapias adyuvantes como ultrasonido o similares, evitar el uso de terapia laser como adyuvante para el manejo de OA, evitar el uso de acupuntura para el manejo de OA); además, 20 puntos de buena práctica clínica y un flujograma de trabajo.This paper summarizes the clinical practice guideline (CPG) for the diagnosis and management of patients with osteoarthritis (OA) in the Peruvian Social Security health system (EsSalud). A working group was formed for producing this guideline, and this group included both clinical specialists and methodologists. Systematic searches in PubMed and CPG repositories were performed aiming to answer the 13 clinical questions that were asked. The GRADE (Grading of Recommendations Assessment, Development, and Evaluation) method was used aiming to determine the certainty of evidence. Eleven recommendations were issued: three were strong (do not request X-ray films as routine practice, give an individualized plan including educational and self-care measures, and prescribe physical activity; and eight were weak (weight-reducing interventions for persons with obesity or who are overweight, avoid the use of paracetamol for OA management, promote the use of oral non-steroidal anti-inflammatory drugs for OA management, avoid the use of chondroitin sulfate for OA management, avoid the use of prolotherapy for OA management in affected adults, particularly in those with refractory pain not responding to drug therapy, consider the use of adjuvant therapy, such as ultrasound and the like, avoid the use of laser therapies as adjuvant for OA management, and avoid the use of acupuncture for OA management); also, 20 items for good clinical practice and a workflow sheet were issued.
