  • The Menstruating Womb

      Abstract: Source: Volume 11, Issue 1-2, pp 21 - 60This paper analyses body and gender in East Asian medicine through a case study of Hŏ Chun’s 許浚 Precious Mirror of Eastern Medicine (Tongŭi pogam 東醫寶鑑, first ed. 1613). While Hŏ Chun’s Chinese sources classified menstrual ailments as a disease of women, Hŏ created a new nosological model that defined menstrual ailments as maladies of the ‘womb’, an internal body part found in men and women alike. I read back and forth between the Precious Mirror and the Chinese sources that Hŏ Chun cites to analyse the textual and intellectual processes by which he constructed his androgynous, menstruating womb. My findings engage with scholarship on the history of East Asian medical exchanges as well as with scholarship on menstruation, sex, and gender in world medicine.

  • Reconfiguring East Asian Modernity

      Abstract: Source: Volume 11, Issue 1-2, pp 61 - 99Around the turn of the twentieth century, Korean thinkers debated on the dilemma of how their country should deal with the challenge of modernity. Aiming to resist the westernisation of Korea, the physician Sŏk-kok took the unique approach of using the medical theories of Chinese antiquity to explain Korea’s propitious place in the world. In the context of Japan’s increasing influence leading to annexation in 1910, Sŏk-kok used the model of regional qi in China to explain variation of qi in the world. With China’s collapse, Korea had become the new centre of civilised learning, characterized by its increasing yang qi. Thus, identifying strengthening yang of the East as his solution, Sŏk-kok also challenged the traditional medical orthodoxy of nourishing yin qi by arguing for focusing more on strengthening people’s yang qi. To put theory into practice, the powerful toxic drug aconite became his metaphor to strengthen the yang of Korea as a civilisation through aconite-based drug therapies supporting the heart-minds of individuals.

  • The State-Centred Nosology

      Abstract: Source: Volume 11, Issue 1-2, pp 100 - 132Since 1951, when the Medical Service Act was enacted in South Korea, traditional Korean medicine has been an independent part of a dual medical system along with biomedicine. Despite several attempts to subordinate Korean medicine to biomedicine, Korean medicine has managed to remain an institutionally-independent system of medical care. However, in the years since the launch of the South Korean National Health Insurance Service in 1977, official regulations have required Korean medical nosology to be linked to biomedical nosology. In particular, during the four times of revision from 1979 to 2015, the originally separated disease classification of Korean medicine was gradually integrated into the Korean Standard Classification of Diseases (KCD), which heavily relies on biomedical nosology. The present study uses this nosological shift as a window to illustrate the transformation of Korean medicine in South Korea since the 1970s.

  • Tradition on the Move

      Abstract: Source: Volume 11, Issue 1-2, pp 133 - 159Studies of East Asian medicine have demonstrated that contemporary traditional medicine is by no means static but continuously changing, and current work often emphasises the impact of external forces on this transformation such as the modern state, globalisation, and biomedicine. This paper emphasises another significant—but under-examined—impetus for change: East Asian medical epistemology and its theoretical and clinical elaborations. Drawing on an ethnographic investigation of Korean medicine and focusing on ‘practice’ as a theoretical and methodological vehicle, this study illustrates that traditional medicine retains within great potential to transform itself. Three contemporary acupuncture methods—Sa-am, Eight-Constitution, and Hundred-Degree and their own ways of cultivating tradition (validation of pre-modern methods, combination of existing theories, and reinterpretation of classical texts)—show that East Asian medical theories and epistemology continuously serve as dynamic forces for tradition on the move. By analysing the emergence of new forms of traditional practices in Korean medicine, this study attempts to contribute to the discussion of how tradition exists in modernity.

      Abstract: Source: Volume 11, Issue 1-2, pp 161 - 170We can find several characteristics of Native Korean Herbal Medicine in the Preface of Compendium of People-Saving Prescriptions Made with Native Korean Herbs (Hyangyak Chesaeng Chipsŏngbang 鄕藥濟生集成方). First, it clarifies that saving people through medicine is part of the Neo-Confucianist ideal of ruling. Second, it emphasises that Korea’s medical tradition treats one disease with one herb, making it highly effective. Third, it points out that Korea is geographically different from China, and that treating disease with native Korean herbs has a faster and better effect than using foreign herbs. The geographic concept of Native Korean Herbal Medicine was inherited by the Treasured Mirror of Eastern Medicine (Tongŭi Pogam 東醫寶鑑) and had been the core idea of Chosŏn medicine until the early twentieth century.

  • Managing the Health of Chosŏn Kings: Illness and Treatment in the

      Abstract: Source: Volume 11, Issue 1-2, pp 171 - 191The Sŭngjŏngwŏn ilgi (Daily Records of the Royal Secretariat) is one of the major chronicles of the events of the Chosŏn Dynasty (1392–1910). Although the records prior to the year 1622 are no longer extant, the remaining records from the years 1623 to 1910 meticulously recount the daily activities of the reigning Chosŏn kings, including copious information on their physical and mental status. Because the king’s health was considered as important as other official affairs in many respects, detailed records were kept of royal ailments and how court doctors treated them. This article surveys the state of Korean-language scholarship on the medical content of the Daily Records and presents selected translations to demonstrate how this valuable historical source can shed light on both the social history of Chosŏn medicine and the political importance of kingly health at the Chosŏn court.

  • Cases from (ca. 1833–56) by Chang T’ae-gyŏng

      Abstract: Source: Volume 11, Issue 1-2, pp 192 - 206Chang T’ae-gyŏng 張泰慶 (1809–87) recorded the details of 47 medical cases and 130 prescriptions in his medical treatise Ujam’s Miscellaneous Writings (Ujam chapchŏ 愚岑雜著).1 As a scholarly physician, Chang treated people of all ages and both sexes in what is today Kwangju, South Chŏlla, the south-western province of Korea. In the hope of documenting his knowledge and experience for the benefit of later generations, Chang collated the various cases and prescriptions together with some brief associated writings, such as his own theory of acupuncture, parts of a poem that he had composed, his friend’s poem in response, an explanation of the meaning of his sobriquet ‘Ujam’ (ho 號), and a ‘written prayer’ (ch’ukmun 祝文) for the text. Although they were circulated locally, it appears that Chang’s cases were not officially published during his lifetime. Chang’s descriptions of his healing activities not only exemplify scholarly medicine in practice, but in them are also found his dialogic interaction with various patients and folk healers.

      Abstract: Source: Volume 11, Issue 1-2, pp 207 - 235The translation below is an essay, ‘On the Origin of Medicine’ (Ŭiwŏnnon 醫源論), taken from the textbook1 Eastern Medicine [for] Prolonging the World and Preserving People (Tongǔi Susebowǒn 東醫壽世保元, 1894),2 by Yi Chema 李濟馬 (1837–1900, styled Tongmu 東武).3 Yi Chema, hailing from the peripheral northern part of Korea, was a Confucian-cum-physician and also served as military officer and local magistrate during the Chosŏn 朝鮮 dynasty (1392–1910). Having no specific allegiances to any intellectual lineage in either medicine or Confucianism, he was able to translate his critical yet inspirational impulses into a distinctively novel style of clinical practice. His ideas built on the observation that sentimental/emotional, or better, psychosocial dispositions of a person are inextricably associated with the visceral dynamics of the person.4 He thus put forward as an overarching scheme of organisation the four constitutional types: Greater Yang (Taeyang 太陽) Person, Greater Yin (Taeŭm 太陰) Person, Lesser Yang (Soyang 少陽) Person, and Lesser Yin (Soŭm 少陰) Person. Later dubbed ‘Sasang Medicine’ (Sasangǔihak 四象醫學), Yi Chema’s distinctive way of knowing and style of practice became one of the main intellectual currents of Korean medicine during the twentieth century. Its history thus provides an illustrative example of the diverse landscape and historical dynamics of East Asian medicine.

      Abstract: Source: Volume 11, Issue 1-2, pp 237 - 255This report illustrates how the condition of infertility and its remedy in Korean Medicine is likened to the art of sowing, and how Korean medical professionals and their patients navigate their way against a backdrop of a dominantly biomedical scene. In South Korea, where both biomedicine and Korean Medicine are recognised as legitimate medical systems, a medically defined condition creates a crossroads where different epistemologies conflict and intermingle. While biomedicine perceives infertility as an absence or impairment of fertility, where the ability to become pregnant is immutable, Korean Medicine views it as something that could change according to conditions of bodily elements, and thus can be improved through shifting the bodily state. Factors involved in pregnancy are described metaphorically in the medical texts as the man as seed and the woman as earth. The doctor is described as playing the role of the farmer. This way of metaphorical thinking of infertility leads to a different assessment of what the problem is, and to different approaches in treatment. These differences can be seen in the interviews of Korean medical professionals that are the foundation of this practice report. The illustrations in this report show how practice can differ according to epistemology in the case of infertility, where on the one hand, the state of the art biomedical techniques for treatment remove the pregnancy process from the body by replacing its roles in the lab, while on the other hand, Korean medical practitioners consider fertility to be reflexive to the individual’s bodily state and deal with it on the more elementary level of the patient’s body.
