  • Cognitive representations of diabetes and their role in metabolic control
           and health: when a positive view of illness protects

    • Abstract: ABSTRACT A positive cognitive illness representation (CIR) of diabetes is associated with better metabolic control and health. The aims of this research were to assess CIR with a newly developed Spanish scale – the ERCE – and to predict metabolic control, health outcomes, and psychological distress. One hundred and fifty-two Type 1 diabetic patients were all administered the ERCE, the MOS SF-36, the PANAS, and the HADS scales. Data on glycated hemoglobin (HbA1c) were also collected. Scores on the ERCE led to the creation of two profiles (+/- CIR) by clustering the scores. After patients were grouped, multiple hierarchical regressions were performed. A negative view of diabetes (negative CIR) was associated with poor health outcomes. The ERCE scale demonstrated acceptable psychometric guarantees and validity evidences for being used in clinical contexts. This new instrument allows educators to adjust treatment to each individual, which leads to a more efficient and better support for people with diabetes in Spain.RESUMEN Una representación cognitiva de la enfermedad (RCE) positiva se asocia con mejor control metabólico y mayor salud en diabetes. Los objetivos fueron evaluar la RCE con una nueva escala –la ERCE– y predecir el control metabólico, los indicadores de salud y el malestar psicológico en 152 pacientes diabéticos tipo 1. Se administraron la escala ERCE, la MOS SF-36, la PANAS y la HADS. Se recogió también la hemoglobina glicosilada (HbA1c). Mediante un análisis de clusters con las puntuaciones de la ERCE, se crearon dos perfiles (+/-RCE). El análisis de regresión jerárquico mostró que una visión negativa de la diabetes (RCE negativa) se asoció con peor estado de salud. La ERCE demostró garantías psicométricas y evidencias de validez aceptables, que justifican su uso en contextos clínicos. Este instrumento permite a los educadores evaluar la RCE y ajustar el tratamiento, lo que conduce a una asistencia más eficiente para las personas con diabetes en España..
  • Relationship between mental health and level of functioning in people over
           65 years in Madrid Region

    • Abstract: RESUMEN El objetivo de este estudio es analizar la relación entre el nivel de funcionamiento (NdF) y las variables sociodemográficas y, salud física y mental de las personas mayores de 65 años. La muestra (N = 555) se tomó al azar en la Comunidad de Madrid. Los trastornos mentales se evaluaron con la CIDI65+ y el NdF con la WHODAS II y con la HoNOS65+. Se realizaron análisis de medias, ANOVA, ajuste de Bonferroni y un análisis de regresión lineal múltiple. Las mujeres presentan un NdF más bajo que los hombres y el NdF empeora con la edad. Los trastornos físicos y mentales tienen un impacto similar en el NdF. Las variables que ejercen un mayor impacto sobre el NdF son el número de síntomas psicológicos, la presencia de cualquier trastorno mental, una peor situación financiera, tener una mayor edad y otorgar poca importancia a las creencias religiosas.ABSTRACT The objective of this study is to analyze the relationship between the level of functioning (LoF) and a number of variables: sociodemographic variables, physical and mental health of people over 65 years. The sample (N = 555) was a random sample from Madrid Region. Mental disorders were evaluated with the CIDI65+ and LoF with the WHODAS II and the HoNOS65+. Analysis of means, ANOVAs, Bonferroni adjustment, and a multiple linear regression analysis were performed. Women have a lower LoF than men and LoF decreases with age. Physical and mental disorders have a similar impact on LoF. The variables that have a greater impact on LoF are the number of psychological symptoms, the presence of any mental disorder, a worse financial situation, being older, and giving little importance to religious beliefs.
  • The Connor-Davidson resilience scale (CD-RISC) amongst young Spanish

    • Abstract: ABSTRACT The interest in resilience has grown over the last few decades due to its relationships with health, well-being, and quality of life. Several instruments have been developed to measure resilience, with one of the most common being the Connor-Davidson Resilience Scale (CD-RISC). This scale has been validated in many cultures with divergent results. This paper investigates the factor structure of the CD-RISC. Participants were 3,214 students (62.1% female, mean age = 21.01, SD = 2.86) who were randomly divided into two equal subsamples (n = 1,607). One subsample was used to explore which models best fit the data and these models were fitted using the other subsample. Confirmatory factor analysis did not support the original 5-factor CD-RISC. The analyses supported unidimensional scale structures for both the 22-item and 10-item versions of the scale. The shorter instrument reduces the time needed to answer the questionnaire, allows it to be combined with other instruments, and does not require large sample sizes.RESUMEN El interés por el estudio de la resiliencia ha aumentado en las últimas décadas debido a su relación con la salud, el bienestar y la calidad de vida. Se han desarrollado diversos instrumentos para medir la resiliencia, siendo uno de los más comunes la Escala de Resiliencia de Connor-Davidson (CD-RISC). Esta escala ha sido validada en muchas culturas con resultados diversos. En este trabajo se analiza la estructura factorial del CD-RISC en una muestra de 3,214 estudiantes (62.1% mujeres, edad media =21.01, DT = 2.86) que fueron divididos aleatoriamente en dos submuestras iguales (n = 1,607). Se exploró en una de las submuestras qué modelos se ajustaban mejor a los datos y estos modelos fueron ajustados en la otra submuestra. El análisis factorial confirmatorio no avaló la estructura de 5 factores del CD-RISC, sino que confirmó una estructura unidimensional de 22 y de 10 items. La escala de 10 items reduce el tiempo de aplicación del cuestionario, permite combinarlo con otros instrumentos y no requiere muestras muy grandes.
  • Causal attribution and beliefs toward mental disorders among general
           population and psychologists. An exploratory study

    • Abstract: RESUMEN Con el objetivo de comparar las creencias sobre los trastornos mentales en población general y en profesionales de la Psicología y analizar las diferencias entre aquellos que presentan una atribución causal biomédica vs. psicosocial sobre estas creencias, se aplicó una versión adaptada del Illness Perception Questionnaire (IPQ-R) a una muestra de 166 participantes. El consumo de drogas y el desequilibrio bioquímico cerebral fueron las causas señaladas con mayor nivel de acuerdo por la población general y el estrés y el consumo de drogas por los profesionales de la Psicología. Los participantes de la población general que adoptaron una explicación biológica consideraron estos problemas como significativamente más crónicos que los se inclinaron por causas psicosociales. Los psicólogos señalaron los trastornos mentales como significativamente menos crónicos, menos cíclicos, más controlables por la persona y el tratamiento, más coherentes y asociados a una menor representación emocional negativa que la población general.ABSTRACT In order to compare beliefs toward mental disorders among the general population and psychologists and to analyze the differences on these beliefs between those who present biomedical vs. psychosocial causal attribution, an adapted version of the Illness Perception Questionnaire (IPQ-R) was administered to a sample of 166 participants. Drug use and brain biochemical imbalance were the causes most agreed upon by the general population, whereas stress and drug use were the causes most agreed upon by psychologists. Participants in the general population who adopted a biological explanation considered these problems as significantly more chronic than participants who chose psychosocial causes. Psychologists perceive these problems as significantly less chronic, less cyclical, more controllable by the individual and the treatment, more coherent, and associated with less negative emotional representation than the general population.

    • Abstract: ABSTRACT This study addresses the profile of at-risk subjects whose long-term psychological impact of presymptomatic testing (PST) for Familial Amyloid Polyneuropathy (FAP) TTR V30M is negative. The sample consisted of 177 subjects, aged over 20 years that were 50% at-risk for FAP, and performed the PST at least three years ago. Participants were contacted by mail, one time only, to answer the sociodemographic questionnaire and the Brief Symptom Inventory (BSI), the Self-Rating Anxiety Scale of Zung (SAS), and the Beck Depression Inventory (BDI). From the sample, 22.6% (BSI), 16.4% (SAS), and 9% (BDI) subjects presented negative psychological impact, after having performed the PST for more than 3 years. Subjects with clinically significant values in BSI, SAS, and BDI have an overlapping profile concerning the total sample, except regarding age, since clinically depressed subjects have a higher mean age. Married women or living in unmarried unions, aged between 30 and 45 years, employed, carriers, and having performed the PST test for 6-7 years are a group raising higher concern and requiring a more active role with respect to the psychological impact of the PST for FAP. The role of the clinical and health psychologist with these patients is critical in the adjustment to the presymptomatic test result as well as in adherence to the available treatments conducive to a better quality of life, in carriers.RESUMEN Este estudio aborda el perfil de sujetos en riesgo cuyo impacto psicológico a largo plazo de las pruebas presintomáticas (PST) para la polineuropatía amiloide familiar (FAP) TTR V30M es negativo. La muestra consistió en 177 sujetos mayores de 20 años que tenían un 50% de riesgo de FAP, que habían realizado el PST hacía al menos tres años. Se contactó con los participantes por correo, solo una vez, para responder el cuestionario sociodemográfico y el Inventario de síntomas breves (BSI), la Escala de ansiedad de autoclasificación de Zung (SAS) y el Inventario de depresión de Beck (BDI). El 22.6% (BSI), el 16.4% (SAS) y el 9% (BDI) de los sujetos de la muestra presentaron un impacto psicológico negativo después de haber realizado el PST durante más de 3 años. Los sujetos con valores clínicamente significativos en BSI, SAS y BDI tienen un perfil superpuesto con respecto a la muestra total, excepto con respecto a la edad, ya que los sujetos clínicamente deprimidos tienen una edad media más alta. Las mujeres casadas o que viven en pareja, con edades entre 30 y 45 años, que trabajan, son portadoras y han realizado la prueba PST durante 6-7 años son un grupo que suscita una mayor preocupación y requiere un papel más activo con respecto al impacto psicológico del PST para FAP. El papel del psicólogo clínico y de la salud con estos pacientes es decisivo en el ajuste del resultado de la prueba presintomática, así como en la adhesión a los tratamientos disponibles que conducen a una mejor calidad de vida en los portadores.
  • The influence of different monitoring means on the adherence to
           mindfulness meditation practice

    • Abstract: RESUMEN Los efectos beneficiosos de la meditación mindfulness (MM) se han observado en numerosos estudios, pudiendo ser la atención y conciencia en el momento presente una variable esencial. Sin embargo, la adherencia al entrenamiento de la práctica meditativa es en ocasiones baja. Se investigaron los efectos del entrenamiento en MM sobre la atención y conciencia y si el formato de registro de la práctica de MM, formato papel (PP) (n = 27) o aplicación móvil (APP) (n = 21), produce diferencias en la adherencia al entrenamiento y en la atención y conciencia. Los resultados mostraron un aumento de la atención y conciencia tras el entrenamiento, independientemente del formato de registro de la práctica. La APP redujo el abandono al entrenamiento y aumentó el tiempo de práctica de MM cada día que se practicó.ABSTRACT Beneficial effects of mindfulness meditation (MM) have been observed in many studies, with attention to and awareness of the present moment possibly representing an essential variable. However, adhering to the meditation regimes is poor at times. As such, the effects of MM training on attention and awareness were examined here, and whether the means of recording the practice of MM – using paper (PP, n = 27) or through a mobile phone application (APP, n = 21) – produced any differences in terms of adherence to the training, or in attention and awareness. The results demonstrate that training enhances attention and awareness, irrespective of the format with which it was recorded. The APP reduced the rate of withdrawal from the training and it increased the time dedicated to the practice of MM each day.
  • Comparison between mild and serious suicide attempts and suicide deaths in
           Navarre Region

    • Abstract: RESUMEN Este trabajo compara personas fallecidas por suicidio con personas que realizaron algún intento (leves o graves), identificando factores asociados a cada grupo. En el estudio se utilizan dos muestras en las que se recogieron variables sociodemográficas y clínicas de las personas fallecidas por suicidio en Navarra (2010-2013, n = 180) y de personas atendidas en urgencias de psiquiatría por un intento de suicidio (enero-octubre 2015, n = 207). Los resultados indican que un método de suicidio no violento, no estar jubilado y la ausencia de intentos de suicidio previos se asocian a quienes realizan intentos leves. Ser hombre, la ausencia de atenciones previas en urgencias de psiquiatría y el haber nacido en España se asocian a los fallecidos por suicidio. Se concluye que las diferencias encontradas entre los fallecidos y los intentos de suicidio subrayan la necesidad de abordajes diferenciados en la prevención de la repetición de la conducta suicida.ABSTRACT This paper compares people who committed suicide with people who made some (mild or severe) attempt, identifying factors associated with each group. This study uses two samples in which sociodemographic and clinical variables of people dying by suicide in Navarre (2010-2013, n = 180) and people attending psychiatry emergencies for an attempted suicide (January-October 2015, n = 207) were collected. The results show that a non-violent method of suicide, not being retired, and the absence of previous suicide attempts are associated with those who perform mild attempts. Being a male, the absence of previous attendances in psychiatry emergencies and being born in Spain are associated with those who have committed suicide. It is concluded that the differences between the deceased and suicide attempts underline the need for differentiated approaches in preventing the recurrence of suicidal behavior.
