  • Personality as unidimensional and bidimensional vulnerability: the
           mediator role of cognitive variables in symptom severity in a sample of
           people with severe personality disorder

    • Abstract: RESUMEN Los problemas de conceptualización de los trastornos de personalidad (TP) y su acomodo con el resto de las patologías siguen suscitando un prometedor esfuerzo investigador y clínico que va dando frutos al identificar procesos transdiagnósticos y proponer modelos heurísticos de interacción. Desde el marco de la personalidad como diátesis, estudiamos en una muestra de personas con TP grave (N = 310) el papel mediador de diversas variables en la gravedad sintomatológica. Mediante análisis de conglomerados hallamos una tipología bidimensional que divide de manera exhaustiva y exclusiva al 100% de los participantes. El análisis de mediación revela que el efecto de la personalidad como continuum unidimensional en gravedad sintomatológica está mediado por pensamientos automáticos negativos y fusión cognitiva; su efecto como tipología bidimensional (internalización y externalización) parece mediado solo por pensamientos automáticos. Se discuten los hallazgos e implicaciones de cara al nuevo paradigma de una ciencia clínica basada en procesosABSTRACT The problems of conceptualization of personality disorders (PD) and their adjustment within the rest of pathologies continue to boost a promising research and clinical effort that helps to identify transdiagnostic processes and suggestheuristic interaction models. Understanting personality from the framework of diathesis, we analized the mediating role of a number of variables in symptom severity in a sample of people with severe PD (N = 310). By means of cluster analysis, we found a two-dimension typology that divides exhaustively and exclusively 100% of the participants. Mediation analyses found that personality as a one-dimension continuum has an effect on symptomatic severity mediated by negative automatic thoughts and by cognitive fusion; its effect as a two-dimension typology (internalization and externalization) appears to be mediated only by automatic thoughts. These findings and their implications are discussed in the context of a new paradigm of a process-based clinical science.
       
  • Intention to breastfeed exclusively: a study based on the Theory of
           Planned Behavior

    • Abstract: RESUMEN La lactancia materna exclusiva (LME) aporta diversos beneficios a la salud del niño y de la madre. La Teoría de la Conducta Planeada (TCP) es un modelo enfocado en la predicción de algunas conductas relacionadas con la salud, como la LME. Elobjetivo de este estudio fue identificar la relación de las variables de la TCP con la intención de dar LME. Se llevó a cabo un estudio transversal con 442 mujeres embarazadas de la Ciudad de México. Las variables de la TCP se midieron con instrumentos previamente validados. Un análisis de regresión logística múltiple, ajustado por paridad y edad materna, identificó asociaciones significativas entre la intención de amamantar y la norma subjetiva, OR = 1.07 (IC 95% [1.01, 1.14]), y el control conductual percibido, OR = 1.05 (IC 95% [1.01, 1.11]), con la intención. Como conclusión, en esta muestra se asociaron dos constructos de la TCP con la intención de lactar de forma exclusiva.ABSTRACT Breastfeeding provides health benefits to children and mothers. WHO recommends it be exclusive until 6 months of age. The Theory of Planned Behavior (TPB) focus on the prediction of health-related behaviors, such as breastfeeding. Theobjective of this study was to identify the relationship of attitudes, subjective norm, and perceived behavioral control with the intention of exclusively breastfeeding in a sample of Mexican pregnant women. A cross-sectional study was carried out with 442 pregnant women from Mexico City. Psychosocial variables were measured with a set of scales previously validated. A multiple logistic regression analysis, adjusted for the number of previous children and maternal age, identified significant associations between intention to breastfeed and one dimension of the subjective norm (participants’ mothers opinions), OR = 1.07 (IC 95% [1.01, 1.14]), as well as perceived behavioral control, OR = 1.05 (IC 95% [1.01, 1.11]). In the case of this sample, TCP constructs associated to the intention to exclusively breastfeed.
       
  • Risk of addiction: its prevalence in adolescence and its relationship with
           security of attachment and self-concept

    • Abstract: ABSTRACT That study aims to analyze the prevalence of risk of addiction to psychoactive substances during adolescence, study the relationship between this risk and the representations of attachment and self-concept and analyze the relationshipbetween attachment and self-concept. The Youth Inventory 4 (YI-4) test was administered to assess risk of addiction in 668 participants between 13 and 19 years old. Representations of the attachment were evaluated with Cartes, Modèles Individuelles de Rélation, reduced version (CaMir-R), and self-concept dimensions, with Autoconcepto Forma 5 (AF5). The results indicate a high risk of substance addiction in adolescence, 19.5%, both for boys and girls, CI between 15.4 and 24.3, with age being a risk factor. The prevalence of addiction risk decreases with high scores on security, which correlates -.22 with such a risk and positive academic self-concept, correlating -.20.RESUMEN Este estudio tiene como objetivo analizar la prevalencia del riesgo de adicción a sustancias psicoactivas durante la adolescencia, estudiar la relación entre este riesgo y las representaciones de apego y autoconcepto y analizar la relaciónentre el apego y el autoconcepto. Se realizó la prueba de inventario juvenil 4 (YI-4) para evaluar el riesgo de adicción en 668 participantes de entre 13 y 19 años de edad. Las representaciones de apego se evaluaron con la versión reducidadel Cartes, cuestionario de evaluación del apego, versión reducida (CaMir-R) y dimensiones autoconceptuales, con Autoconcepto Forma 5 (AF5). Los resultados indican un elevado riesgo de adicción a las sustancias en la adolescencia,19.5%, tanto para niños como para niñas, CI entre 15.4 y 24.3, siendo la edad un factor de riesgo. La prevalencia del riesgo de adicción disminuye con altas puntuaciones en seguridad, que correlaciona con dicho riesgo -.22 y el autoconcepto académico positivo, que correlaciona -.20.
       
  • What mechanisms do children and adolescents with chronic pain use to
           manage their friendships'. A review of the literature

    • Abstract: ABSTRACT This study review the available literature about friendships of children and adolescents with chronic pain, focusing on the mechanisms they use to face challenges in their relationships and the theories studies use to address this topic. We conducted a search in PubMed, PsycINFO, and WOS from inception to August 2017. We included peer-reviewed primary studies with a specific subgroup analysis of friendships in children and adolescents (6-18 years old) with the followingconditions: lower limb pain, neck and shoulder pain, back pain, abdominal pain, headache and migraines, fibromyalgia, and complex regional pain syndrome. Twenty empirical articles were selected. From these studies, 18 mechanisms and 6theories were identified. Studies show a high variability in the methodologies employed, as well as in the way they define friendships experience. Finally, very few of the studies selected are theory-based.RESUMEN Este estudio revisa las publicaciones científicas disponibles sobre las amistades de niños y adolescentes que padecen dolor crónico centrándose en los mecanismos que utilizan para afrontar los desafíos que sus relaciones les plantean y las teorías utilizadas por los estudios para abordar este tema. Llevamos a cabo una búsqueda en PubMed, PsycINFO y WOS desde los inicios hasta agosto de 2017. Incluimos estudios primarios de revisión de pares con un subgrupo de análisis específico de las amistades en niños y adolescentes (entre 6 y 18 años), con las siguientes condiciones: dolor en las extremidades inferiores, dolor de cuello y hombros, dolor de espalda, dolor abdominal, dolor de cabeza y migrañas, fibromialgia y síndrome de dolor regional complejo. Se seleccionaron 20 artículos empíricos, a partir de los cuales se detectaron 18 mecanismos y 6 teorías. Los estudios muestran gran variabilidad en la metodología utilizada, así como en el modo de definir la experiencia de las amistades. Por último, muy pocos de los estudios elegidos están basados en la teoría.
       
  • Differential outcome of a therapeutic community program for internalizing
           and externalizing personality disorders: A change in cognitive processes
           mediates symptom improvement

    • Abstract: ABSTRACT The identification of transdiagnostic mediating processes involved in a therapeutic change and their relationship with personality can contribute to a better adjustment of a therapeutic technique, enhancing its effectiveness. In a sample of 158 adults diagnosed with personality disorder who complete an inpatient therapeutic community program for 6 months, the hypothesis of a differential reduction in symptom intensity according to the externalizer/internalizer typology is tested, and cognitive variables mediating change are explored. A pre-post change (p = .000, ηp2 = .50) is observed, along with a difference between externalizers and internalizers (p = .002, ηp2 = .06), and an interaction effect (p = .037, ηp2 = .03). The effect of personality type on symptom change (β = .43, p = .009) is no longer significant when negative automatic thoughts and dysfunctional attitudes are considered as mediators (β = .06, p = .549). Findings are discussed from the perspective of personality disorder as a vulnerability factor.RESUMEN La identificación de procesos mediadores transdiagnósticos intervinientes en el cambio terapéutico y su relación con la personalidad puede contribuir a un mejor ajuste de la técnica terapéutica, potenciando su eficacia. En una muestra de 158 adultos diagnosticados de trastorno de personalidad inscritos en un programa de comunidad terapéutica hospitalaria durante 6 meses se pone a prueba la hipótesis de una reducción diferencial de laintensidad sintomatológica según la tipología externalizador/internalizador y se exploran las variables cognitivas mediadoras del cambio. Se observa un cambio pre-post (p = .000, ηp2 = .50), la diferencia entre externalizadores e internalizadores (p = .002, ηp2 = .06) y el efecto de interacción (p = .037, ηp2 = .03). El efecto del tipo de personalidad sobre el cambio en los síntomas (β = .43, p = .009) deja de ser significativo cuando se consideran los pensamientos automáticos negativos y las actitudes disfuncionales (β = .06, p = .549) como mediadores. Se discuten los resultados desde la perspectiva del trastorno de personalidad como factor de vulnerabilidad.
       
  • Misconceptions about child and adolescent depression and its treatment

    • Abstract: RESUMEN Se analiza la veracidad de cinco ideas muy populares sobre la naturaleza y el tratamiento de la depresión infantojuvenil que dificultan que los niños y adolescentes reciban un tratamiento apropiado y que favorecen la farmacoterapia en perjuicio de la psicoterapia. Esas ideas se contrastan con los resultados de los metaanálisis y estudios más recientes localizados en PsycINFO y MEDLINE, que contrariamente a esas ideas indican que: 1) la psicoterapia cura la depresión infantojuvenil, 2) la psicoterapia, especialmente la terapia cognitivo-conductual para niños y adolescentes y la terapia interpersonal para adolescentes, es el tratamiento de primera elección tanto para la depresión leve como para la moderada o grave y por delante de la medicación antidepresiva, 3) la psicoterapia para la depresión infantojuvenil suele ser un tratamiento de corta duración, 4) la depresión infantojuvenil se considera un trastorno mental, no una enfermedad mental y 5) no se ha demostrado empíricamente que la causa principal de la depresión infantojuvenil sea biológica, sino que esta es solamente una hipótesis más.ABSTRACT This paper analyzes the veracity of five very widespread ideas about the nature and treatment of child and adolescent depression that makes it difficult for children and adolescents to receive adequate treatment and that promote pharmacotherapy to the detriment of psychotherapy. These ideas are contrasted with the results of the more recent meta-analyses and studies located in PsycINFO and MEDLINE. These results indicate that, contrary to those ideas: 1) psychotherapy cures child and adolescent depression; 2) psychotherapy, especially cognitive behavioral therapy for children and adolescents and interpersonal therapy for adolescents, is the first-line treatment over antidepressant medication for both mild and moderate-to-severe depression; 3) psychotherapy for child and adolescent depression is usually a short-term treatment; 4) child and adolescent depression is understood as a mental disorder, not as a mental illness; and 5) it has not been empirically proven that the main cause of child and adolescent depression is biological, instead this is only one more hypothesis.
