  • Treatment of chronic noncancer pain in patients on opioid therapy in
           primary care: A retrospective cohort study
    • Authors: Arden R. Barry, Chantal E. Chris
      First page: 52
      Abstract: Canadian Pharmacists Journal / Revue des Pharmaciens du Canada, Ahead of Print.
      Background:This study sought to characterize the real-world treatment of chronic noncancer pain (CNCP) in patients on opioid therapy in primary care. Methods:A retrospective cohort study from 2014-18 was conducted at a multidisciplinary primary care clinic in Chilliwack, British Columbia. Included were adults on daily opioid therapy for CNCP. Patients receiving palliative care or ≤1 visit were excluded. Outcomes of interest included use of opioid/nonopioid pharmacotherapy, number/frequency of visits and proportion of patients able to reduce/discontinue opioid therapy. Results:Seventy patients (mean age 53 years, 53% male, 51% back pain) were included. Median follow-up was 6 visits over 12 months. Sixty-two patients (89%) reduced their opioid dose, 6 patients had no change and 2 patients required a dose increase. Mean opioid dose was reduced from 183 to 70 mg morphine equivalents daily. Twenty-four patients (34%) discontinued opioid therapy, 6 patients (9%) transitioned to opioid agonist therapy and 6 patients (9%) breached their opioid treatment agreement. Nonopioid pharmacotherapy included nonsteroidal anti-inflammatory drugs (64%), gabapentinoids (63%), tricyclic antidepressants (56%) and nabilone (51%).Discussion:Over half of patients were no longer on opioid therapy by the end of the study. Most patients had a disorder (e.g., back pain) for which opioids are generally not recommended. Overall mean opioid dose was reduced from baseline by approximately 60% over 1 year. Lack of access to specialized pain treatments may have accounted for high nonopioid pharmacotherapy usage. Conclusions:This study demonstrates that treatment of CNCP and opioid tapering can successfully be achieved in a primary care setting. • Community pharmacists as catalysts for deprescribing: An exploratory study
           using quality improvement processes
    • Authors: Barbara Farrell, Chantalle Clarkin, James Conklin, Lisa Dolovich, Hannah Irving, Lisa McCarthy, Lalitha Raman-Wilms
      First page: 37
      Abstract: Canadian Pharmacists Journal / Revue des Pharmaciens du Canada, Ahead of Print.
      Background:There is growing international emphasis on deprescribing, involving the monitored reduction or stopping of medications that are no longer needed or that cause more harm than benefits, especially for the elderly. Community pharmacists are well positioned to partner with patients and their other health care providers in facilitating deprescribing activities. Objective:To build community pharmacists’ capacity to integrate deprescribing into their daily practices through training and workflow strategies. Methods:This study used an exploratory mixed-methods (primarily qualitative) design. Staff at 4 Ontario pharmacies were trained to use deprescribing guidelines. Qualitative data were collected through field observations, notes from advisory group meetings and documented Plan-Do-Study-Act (PDSA) plans. Quantitative data were derived from process and output measures reported by the pharmacies. Iterative PDSA cycles allowed the project team to appraise and accelerate process improvements over time and to summarize findings on facilitators, barriers and the adaptation of processes. Results:All 4 pharmacies identified individual and common goals related to deprescribing; however, drugs targeted and use of professional services to identify and address deprescribing opportunities varied. Each demonstrated that deprescribing activities could be integrated into daily practice and workflow. Common themes characterized approaches taken by each pharmacy: (1) processes used for capacity building among staff to identify patients for possible deprescribing, (2) approaches for preliminary interactions with patients, (3) in-depth medication reviews and (4) follow-up and monitoring. Approaches changed over time. Conclusion:Deprescribing appears to be feasible in community pharmacies. Data derived to populate a business model canvas informs the development of an in-depth business model for deprescribing. Can Pharm J (Ott) 2019;152:xx-xx.

  • Professional identity formation: The experience of regulated pharmacy
           technicians in Ontario
    • Authors: Paul Gregory, Zubin Austin
      First page: 46
      Abstract: Canadian Pharmacists Journal / Revue des Pharmaciens du Canada, Ahead of Print.
      Background:Despite being regulated and spoken about as professionals, there is little formal research examining professional identity formation among regulated pharmacy technicians. Methods:A semistructured interview protocol was generated, based on Holden et al.’s typology of professional identity formation (PIF). Regulated pharmacy technicians in Ontario with a minimum of 2 years’ experience working a minimum of 32 hours/week were recruited to participate. Interviews were transcribed, coded and analyzed based on professional identity formation. Results:A total of 15 regulated pharmacy technicians from southern Ontario participated in this study. Regardless of demographic background, most participants demonstrated identity splinting as the dominant form of professional identity formation. Issues related to social valuing of the role of the technician and environmental opportunities to develop and grow were highlighted as significant barriers.Interpretation:These findings suggest regulated pharmacy technicians have incomplete professional identities due in part to reasons linked to pharmacists and the pharmacy profession. The type of professional identity expressed by participants in this study may limit opportunities for full and optimal expression of their role. Conclusions:Further work is necessary to better understand the professional identity formation of regulated pharmacy technicians, to help support the evolution of this role. • Cannabis in pregnancy and breastfeeding: The pharmacist’s role
    • Authors: Erin Davis, Tiffany Lee, John T. Weber, Shawn Bugden
      Abstract: Canadian Pharmacists Journal / Revue des Pharmaciens du Canada, Ahead of Print.
      Background:The recent legalization of cannabis use in Canada requires pharmacists to be able to support their patients with accurate knowledge of its known risks and benefits. Certain populations, such as pregnant and breastfeeding women and their developing children, may be at higher risk than other populations. Methods:The authors independently searched the literature for clinical reports or reviews of the literature regarding the safety of cannabis use in pregnancy and breastfeeding using search terms such as cannabis, marijuana, pregnancy and breastfeeding. Results:This review combines the relevant pharmacological, pharmacokinetic and clinical evidence for the effects of cannabis in this special patient population. The literature demonstrates that some of the constituents of cannabis can reach children in utero and through breastmilk. Given that Δ⁹-tetrahydrocannabinol can be present in breastmilk as quickly as 1 hour after consumption and last up to 6 days, it may not be possible to use cannabis and avoid infant exposure. There is evidence that this exposure may result in cognitive, social and motor defects. Some of these effects may be long term, lasting years. The pharmacist must be able to educate and screen patients regarding marijuana use in pregnancy and breastfeeding, with the ultimate aim of harm reduction.
