  • Hearing loss following spinal anaesthesia among obstetric patients at the
           University College Hospital Ibadan

      Abstract: Background: Sensorineural hearing loss (SNHL) is a rarely reported complication of subarachnoid block and may go unnoticed unless audiometric test is performed.Objective: The aim of this study was to determine the prevalence of sensorineural hearing loss following spinal anaesthesia in obstetric patients.Patients and Methods: Seventy patients of ASAphysical status classes I and II scheduled for elective Caesarean section were randomized into two groups (A& B). Lumbar puncture was performed using 25 gaugepencil point spinal needle type in group Aand 25 gauge Quincke cutting tip type in group B. Spinal anaesthesia was achieved in both groups with 12.5mg (2.5ml) of 0.5% hyperbaric bupivacaine subarachnoid injection in the sitting position. Each patient had a preoperative pure tone audiometry (PTA) test done before spinal anaesthesia as a baseline; this was repeated on the first and third days after the surgery for each patient.Results: Twelve (34.3%) patients in group Aand 24 (68.6%) in group B had more than one attempt at lumbar puncture; and 5[1 (2.9%) from group Aand 4 from group B (11.4%)] developed Post Dural Puncture Headache(PDPH), p= 0.36. Of the 4 patients in group B who developed PDPH, 2 had associated transient sensorineural hearing loss.Conclusion: This study shows that spinal anaesthesia may lead to transient hearing loss among few obstetric patients without long term sequelae. Use of Quincke spinal needle type was more likely to cause PDPH and transient hearing loss.Keywords: Caesarean section, Spinal anaesthesia, Hearing loss, Spinal needles

  • Haemodynamic effects of propofol-ketamine and propofl-fentanyl
           combinations for total intravenous anaesthesia for short surgical
           procedures

      Abstract: Background: Various methods have been tried in preventing haemodynamic instability caused by propofol when combined with other intravenous analgesic agents, because itlacks analgesic property. Hence, it becomes necessary for propofol to be used in combination with other intravenous analgesic agents.Aim: The aim of the study was toevaluate thehaemodynamic effects of propofol-ketamine and propofol-fentanyl for total intravenous anaesthesia in short surgical procedures.Patients and Methods: This was a randomized double-blinded trial prospective study of all adult patients aged 18-50 years of either gender with ASA I and II, scheduled for elective short surgical proceduresat the AminuKanoTeaching Hospital, Kano. The clinical and demographic data were gathered from the patient's case note and the proforma of the study. Patients enrolled into the study were randomly allocated into two groups K and Fcomprised of 54 patients each. Group K received propofol-ketamine, whilegroup F received propofol-fentanyl for induction and maintenance of anaesthesia. The HR, SBP andDBP were recorded before and 1min after induction of anaesthesia, 5 minutes till the end of procedure and at recovery till the patients were fully recovered.Results: There was no statistically significant difference with respect to ages, weight and sex between the two groups (P>0.05). The M: F ratio was 1.8:1 in group K and 2.4:1 in group F (p=0.68). The types of surgical procedures were comparable between the two groups (P>0.05). There was an increase in the mean PR in groups K and F at induction of anaesthesia but the increase in group K was higher than the group F (p=0.01). The SBP in group F showed a statistically significant lower value compared to group K after induction of anaesthesia (p=0.01). There was a fall in DBP during induction of anaesthesia in both groups, but the DBPin group F was significantly lower compared to group K (p=0.01).Conclusion: Both propofol-ketamine and propofol-fentanyl combinations produced safe and effective anaesthesia withfew side effects. Propofol-ketamine results in a more stable haemodynamicprofile.Keywords: Propofol, fentanyl, ketamine, short surgical procedures

  • Comparing the effectiveness of phenylephrine infusion versus bolus regimen

      Abstract: Background: Phenylephrine is a preferred drug in preventing and treating hypotension during spinal anaesthesia for Caesarean section. Different techniques may be used to administer to get optimal blood pressure control.Objective: To compare an infusion technique (group A) against a bolus regimen (group B) in maintaining blood pressure closer to baseline values in patients undergoing Caesarean section under spinal anaesthesia.Patients and Methods: 96 patients were randomised to group Aand Bin a case-control study. The infusion was started at 100μg/minute immediately after administering spinal anaesthetic in Group A, and was maintained unless the patient developed hypertension (20% above baseline). In Group B, a 100μg bolus was given immediately after administration of spinal anaesthesia and additional 100μg boluses if hypotension occurred (20% below baseline). The primary outcome measure was degree of change in blood pressure from baseline level measured in mmHg.Results: The infusion group had mean change in BPof +1.65mmHg (SD = ±6.88) and the bolus group had mean change in BP of -10.77mmHg (SD = ±9.16). There was a significant difference in mean blood pressure changes (p<0.01) between the groups. The infusion group had less incidences of hypotension (p< 0.05). Reactive hypertension and bradycardia were noted in Group Abut not in group B. The blood pressure was maintained within 20% of baseline using either technique of phenylephrine administration.Conclusion: Phenylephrine infusion maintains BP closer to baseline with less hypotension, nausea and vomiting compared to bolus technique. However infusions at 100μg/minute may cause reactive hypertension and bradycardia.Keywords: hypotension, caesarean section, spinal anaesthesia, phenylephrene

  • Laryngeal mask airway for adenoidectomy in an infant with difficult airway
           in a resource-limited environment: a case report

      Abstract: Anaesthesia for adenoidectomy may be challenging to the anaesthetist because of shared airway. The standard airway device for adenoidectomy is endotracheal intubation. The usage of Laryngeal Mask Airway (LMA)[Classic and Flexible] has been described as an alternative airway with some researchers suggesting it might be superior to endotracheal intubation in some aspects. We described a case of adenoidectomy performed with the use of a Classic LMA to administer anaesthesia after failed multiple attempts with endotracheal intubation in a 22-month old child.The aim of the case report is to demonstrate the usefulness of laryngeal mask airway in a difficult paediatric airway for adenoidectomy in a resource limited environment.Keywords: Adenoidectomy, Difficult airway, Failed Intubation, Laryngeal Mask Airway
