  • Medical translation: the neglected human right

      Abstract: International Journal of Human Rights in Healthcare, Volume 10, Issue 4, Page 228-238, September 2017.
      Purpose The purpose of this paper is to highlight that the lack of a specific right to a medical translator under International Law can be considered an outlier when viewed within the context of the copious legislation regarding translation in general. Given the lack of specific legislation guaranteeing the right to a medical translator under International Law, the paper further aims to highlight the resulting effects on medical providers and patients. Design/methodology/approach The paper opted for a detailed historical legal analysis regarding the history of translation under International Law in general, as well as specific international, intranational, and regional legislation regarding the right to a translator in medical settings. The data were complemented by a thorough review of documentary analysis of existing scholarship, detailing the experiences of medical providers and patients. Findings The paper provides insights as to how international legislators have traditionally viewed medical translation: whether as a matter of international relations, access to care, discrimination, or as a fundamental part of the Right to Health. The paper finds that differing views on the subject have result in nations, regions, and medical providers having great discretion in deciding which patients are provided with a translator. The paper finds that such decisions are often made on a basis other than that of patient health. Research limitations/implications Because the provisioning of translators in medical settings currently inevitably falls to a nation or single institution, research into which patients receive a translator and why lacks generalizability (because empirical data are not available for every region of the world). Researchers in future are encouraged to further develop the empirical evidence found in their regions with a more quantitative approach, documenting the non-provisioning of translators in their areas and categorizing the motives behind the decisions of medical providers in a given area. Practical implications The paper includes implications for patients who have suffered adverse events after miscommunication (or lack of communication) with their medical providers. The paper aims to investigate in what venue may they seek legal remedy, and on what grounds. The paper also has implications for national and regional governments. Given the lack of binding International Law regarding medical translation, national and regional governments attempt to guarantee the provisioning of translators to some patients and not others. Such decisions may become political and have unintended consequences for medical providers and patients alike. Social implications The paper includes implications for international legislators and national legislators. The paper also includes implications for medical providers and patients, as language barriers are becoming a more common feature in medical facilities around the world due to globalization and migration. The rate of patients suffering adverse events after not being provided with a competent medical interpreter is bound to rise. Originality/value This paper fulfills a need to examine medical translation in the context of other types of translation under International Law. This paper fulfills a need to study how the lack of specific International Legislation guaranteeing the right to medical translation has implications for national/regional legislators, medical providers, and patients alike. This paper fulfills a need to discuss the legal remedies available to patients who have suffered adverse medical events after not being able to communicate with their medical provider.

  • Students with disabilities’ satisfaction with their
           universities’ services

      Purpose The purpose of this study is to examine the satisfaction aspects among disabled students in some universities in the Middle East and to examine the impact of socio-demographic factors on such satisfaction with regard to the services provided by their universities. Design/methodology/approach The sample consisted of 251 subjects from Oman, Qatar, Jordan and Yemen. Comparative means were used to examine the importance of satisfaction aspects and analysis of variance (ANOVA) as well as multi-analysis of variance (MANOVA) were used to examine the role of the socio-demographic factors in explaining the disabled students’ satisfaction. Satisfaction aspects included: social, psychological, educational, environmental and health, while the socio-demographic variables included were: country, specialization, kind of disability, gender GPA and degree of disability. Findings The findings revealed that social satisfaction was the greatest, followed by psychological, educational, environmental and health satisfaction in order. The results of the ANOVA revealed that all socio-demographic factors impacted significantly on the satisfaction of disabled students, except gender and the degree of disability. While, the MANOVA findings reported that Jordanian and Qatari students had higher educational and psychological satisfaction than Omani and Yemeni students. Research limitations/implications The present findings can be considered as limited and hard to generalize on all regions’ universities because the gap between them in disability services is very large. Some of universities have very good services, while the disability services of other universities are very limited or not existed. Practical implications The findings will be helping the educators and administrative people to offer more services to disabled students. Originality/value This is the first comparative study in the Middle East area.

  • Evaluation of strategic thinking and its affecting factors among managers
           and personnel in a medical sciences university in Iran

      Purpose Prediction of future changes and making appropriate strategic decisions require strategic thinking in any organization. It helps managers to create new opportunities. The purpose of this paper is to measure strategic thinking and its affecting factors at Kurdistan University of Medical Sciences. Design/methodology/approach This is a descriptive-analytic as well as a cross-sectional study which was conducted in 2016. Its statistical community included 300 managers and personnel of Kurdistan University of Medical Sciences. The sample size was equal to statistical community. Data were collected using a standard questionnaire. Data were then entered into SPSS20 and were analyzed using statistical tests such as Freedman, Whitney and Kruskal-Wallis. Findings Overall, strategic thinking was evaluated “good” and “somewhat good” among managers (5.0±72.28) and personnel (6.0±25.48), respectively. Moreover, among the elements of strategic thinking, conceptual thinking obtained the highest score among both managers (6.0±06.32) and personnel (5.0±53.52), which was evaluated in a “good” level. There was a significant difference between managers’ and personnel’s strategic thinking score (p=0.001).   • Evolution of opportunities for early childhood development in Arab
           countries

      Purpose The purpose of this paper is to evaluate opportunities for early childhood development (ECD) regarding children’s prenatal care, access to nutrition, health, parental care and cognitive-developmental activities, in 33 surveys from 13 countries. A total of 15 indicators for children’s opportunities are assessed including their typical level, inequality across demographic groups, and factors responsible. Design/methodology/approach Probability regressions estimate the effects of various household circumstances on children’s engagement in development opportunities. Dissimilarity indexes and human opportunity indexes are computed for each ECD dimension. To understand the impact of each household characteristic, Shorrocks-Shapley decomposition is performed. Findings ECD opportunities are poor but improving and becoming more equal across many countries. Progress is uneven. As may be expected, household wealth affects inequality for ECD opportunities facilitated by markets or governments, but not non-market opportunities. For preventive healthcare and preschool enrollment, access is deteriorating, reflecting low priority given to them in public policy. Children’s height falls behind in the first two years of children’s life, suggesting the need for targeted institutional interventions. Surprisingly, countries experiencing uprisings see conditions improving, while other Arab countries see them stagnating or deteriorating. Originality/value Local and national policy should tackle the identified opportunity gaps. Policymakers should allocate proper investment in medical and educational infrastructure and better coordinate support for disadvantaged families to ensure proper prenatal and ECD. International organizations should provide assistance with these programs.

  • Interprofessional working: adoption assessments for people living with HIV

      Purpose The purpose of this paper is to propose that interprofessional working between professionals who work with people living with HIV (PLWHIV) contributes to improvements in the health, social care needs and long-term outcomes of PLWHIV. Interprofessional working initiatives have been useful in promoting successful frameworks used towards improving various aspects of the HIV disease family planning and transmission prevention. The paper proposes that interprofessional working is important in elevating stigma and discrimination that sometimes prevent PLWHIV from successfully achieving parenthood through adoption. The objective of this study is to contribute to social work practice and literature that supports adoption. Design/methodology/approach This paper draws on an interpretative phenomenological analysis (IPA) study looking at the experiences of PLWHIV going through the adoption process. The study relied on in-depth interviews with six PLWHIV who had gone through the adoption process and presented views regarding a need for better collaborative working by those assessing PLWHIV going through adoption. The sample was purpose and homogeneous. Interviews were recorded, transcribed and analysed using IPA framework. Transcripts had been written up and analysed individually. Following which a cross-case analysis to create meaning and conceptual understanding that was common among all cases. Findings Drawing on themes around interprofessional working, this paper argues that there is a need for improved and transparent interprofessional working models within adoption systems. The paper provides conceptual understanding around interprofessional working and how this can be brought about to support the needs of PLWHIV seeking to adopt children. It proposes that working in isolation will leave PLWHIV feeling that the adoption process is ambiguous and unfair, yet efforts to combat this are evident in healthcare settings. Research limitations/implications Limitations to this study include an acceptable but small convenience sample within IPA methodological approaches. This is a hard to reach sample and results may not be generalisable. Practical implications This paper opens a dialogue for discussing issues around the adoption for PLWHIV and informing professionals about increasing opportunities for PLWHIV to adopt children where there is a high demand for adoptive parents. Social implications Placing the views of participants in this study within the body of knowledge could influence meaningful collaboration between adoption social workers and those supporting PLWHIV within health, social care and voluntary sectors. This may influence change and reduce stigma and barriers preventing some PLWHIV from successfully adopting children. Originality/value This paper meets an identified need to explore how PLWHIV can be supported to achieve parenthood. The paper expands on existing knowledge around the need to provide fertility treatment to PLWHIV. It suggests that child adoption can be promoted through child adoption and ultimately promoting normalcy around the desires of PLWHIV to achieve parenthood using non-traditional methods of conception.
