Aparicio MartínezResumenIntroducciónExiste cierta relación entre las velocidades de las arterias tibiales y la progresión de la enfermedad arterial periférica (EAP). Asimismo, el índice tobillo-profunda (ITP) ha sido descrito como posible herramienta diagnóstica en la EAP. Los objetivos son validar y establecer criterios de referencia como guía en la interpretación de la ecografía dúplex (ED) en pacientes con EAP, así como cuantificar, correlacionar y validar el ITP como método diagnóstico en la EAP.Material y métodosDurante un periodo de 6 meses se examinaron con ED pacientes consecutivos con EAP y controles sanos. Se midieron la velocidad pico sistólica (VPS) en el origen de la arteria femoral profunda (AFP) y en el segmento distal de los tres vasos tibiales de cada extremidad inferior. Se calcularon la VPS tibial media y el ITP definido como la VPS tibial media entre la VPS de la AFP. Se analizaron y compararon los datos entre los dos grupos.ResultadosSe obtuvieron datos de 35 extremidades con EAP (18 categorías Rutherford 3 a 6; 7 categorías Rutherford 1 y 2) y 25 controles sanos. Nuestro objetivo fue comparar los resultados de las variables biomecánicas, obtenidas mediante el análisis de elementos finitos, entre 2 grupos con aneurismas pequeños (grupo I 40-49 mm, grupo II 50-54 mm).MétodosRealizamos un estudio prospectivo y monocéntrico con 23 pacientes con aneurisma de aorta abdominal pequeño (grupo I N = 14; grupo II N = 9), a los que se les realizó una angio-TC toracoabdominopélvica. Se midió y calculó el diámetro máximo, estrés pico de pared (PWR), riesgo de rotura pico de pared (PWRI), diámetro equivalente de riesgo de ruptura (RRED) y volumen del aneurisma y trombo mediante el análisis de elemento finitos, así como la diferencia entre el diámetro máximo y RRED (variabilidad).ResultadosHemos observado un mayor PWS, PWRR y RRED en el grupo II respecto al grupo I. Además, 5 de los pacientes (21,74%) mostraron una variación positiva (>0,5 mm de diferencia) y 2 de ellos (8,70%) obtuvieron un PWRR>0,5 asociado a un mayor riesgo de rotura del aneurisma.ConclusiónPWS y PWRR son mayores en el grupo II, posiblemente con relación a un mayor tamaño del aneurisma. Son necesarios más estudios con mayor tamaño muestral, antes de utilizar este software para las decisiones clínicas. IntroductionThe diagnosis and management of abdominal aortic aneurysms currently relies on the maximal aortic diameter, which grows in an unpredictable manner. The aim of this study was to compare the results of the biomechanical variables using finite elements analysis between the 2 groups of small aneurysms (group I 40-49 mm, group II 50-54 mm).MethodsA total of 23 patients with abdominal aortic aneurysm (group I N=14, group II N=9) who had undergone a thoraco-abdominal angio computed tomography in this hospital were prospectively identified. Abdominal aortic aneurysms diameter, peak wall rupture (PWR), PWR Index, Rupture Risk Equivalent Diameter (RRED) abdominal aortic aneurysms and thrombus volume were measured and calculated using finite element analysis software. The variability between maximum abdominal aortic aneurysms diameter and RRED was also determined.ResultsAn increased PWS, PWRR, and RRED, was observed in group II. Moreover, 5 (21.74%) patients had a positive variability (>0.5 mm), and 2 of them (8.70%) had a PWRR>0.5, which is associated with a higher aneurysm rupture risk.ConclusionPWR and PWRR are elevated in group II, perhaps secondary to a larger abdominal aortic aneurysms diameter. Further studies are required before using this software in clinical decisions.

           deterioro postoperatorio de la función renal en la supervivencia

Repáraz AsensioResumenIntroducciónLa reparación endovascular del aneurisma de aorta abdominal (EVAR) ha supuesto desde su llegada y desarrollo una disminución en la mortalidad precoz con respecto a la cirugía convencional. Como factores predictivos de mortalidad se han identificado el deterioro de función renal preoperatorio, la enfermedad pulmonar obstructiva crónica y la cardiopatía isquémica.ObjetivoEvaluar la influencia en la supervivencia a largo plazo de los factores de riesgo preoperatorios y del deterioro de función renal perioperatorio en los pacientes sometidos a EVAR en nuestro centro.Material y métodosSe realizó un estudio observacional retrospectivo en pacientes sometidos a EVAR en nuestro centro entre los años 2008 y 2012. Se hizo un análisis de la supervivencia a medio y largo plazo, llevado a cabo mediante curvas de Kaplan-Meier. Se estudió la influencia de los factores de riesgo preoperatorios y del deterioro de función renal perioperatorio sobre la supervivencia empleando el modelo de regresión de Cox.ResultadosSe incluyeron 79 pacientes con una edad media de 75,2 años (57,6-85,9). La mediana del tiempo de seguimiento fue de 38 meses (0,4-83,4). Durante este periodo de tiempo se registraron 26 muertes (32,9%). La probabilidad de supervivencia al cabo de un año fue del 93,7%; a los 2 años del 82,3%; a los 4 años del 68,9%; y a los 6 años del 56,5%. Una mayor tasa de supervivencia se asoció con cifras analíticas de creatinina menores de 1,2 mg/dl a las 24 h y a los 7 días, y filtrado glomerular mayor de 60 ml/min a las 24 h y a los 7 días. El análisis multivariante evidenció una probabilidad de muerte 2,39 veces mayor en los pacientes que presentaron un valor analítico de creatinina a las 24 h mayor de 1,2 mg/dl con respecto a los que mostraron una cifra menor de 1,2 mg/dl (HR: 2,39; IC95%: 1,06-5,42); p = 0,037).ConclusiónEl deterioro de función renal tras la EVAR es un factor independiente de mal pronóstico a largo plazo. Tanto en la preparación preoperatoria como durante el postoperatorio deben ponerse los medios necesarios encaminados a corregir las circunstancias que provoquen un deterioro de la función renal. BackgroundSince its introduction and development, endovascular aortic repair (EVAR) has shown an improvement in short-term mortality compared to conventional surgery. Pre-operative renal function impairment, chronic obstructive pulmonary disease, and ischaemic heart disease, have been identified as mortality predictors.ObjectiveTo assess the influence on long-term survival of pre-operative risk factors and peri-operative renal function impairment in patients that underwent EVAR in our centre.Material and methodsA retrospective observational study was conducted on patients subjected to EVAR in our centre between the years 2008 and 2012. The Kaplan-Meier curve was used to analyse the long and medium-term survival. The influence of pre-operative risk factors and perioperative renal impairment on survival was analysed using the Cox regression model.ResultsA total of 79 patients were finally included in the study. The mean age was 75.2 years (57.6-85.9). The mean follow-up period was 38 months (0.4-83.4). During this period 26 (32.9%) deaths were registered. Survival probabilities during the first, second, fourth, and sixth years were 93.7%, 82.3%, 68.9%, and 56.5%, respectively. A greater survival rate was associated with creatinine levels lower than 1.2 mg/dL at 24 hours and at 7 days, and a glomerular filtration rate higher than 60 mL/min at 24 hours and at 7 days. Multivariate analysis showed a HR 2.39 higher in patients with a creatinine level higher than 1.2 mg/dL at 24 hours compared to patients with a creatinine level lower than 1.2 mg/dL (HR: 2.39, 95% CI: 1.06-5.42; P = .037).ConclusionRenal function impairment after endovascular aortic repair of abdominal aortic aneurysm represents an independent long-term poor prognosis factor. During pre-operative preparation and post-operative care necessary steps should be taken aimed at correcting the circumstances that cause renal function impairment.

Se registraron: datos demográficos, factores de riesgo cardiovascular, comorbilidad, parámetros analíticos, cuestionario Mini Nutritional Assesment, tratamiento realizado y evolución. Se estudiaron las tasas de mortalidad, salvamento de extremidad y tiempo libre de reingreso y el impacto de los parámetros descritos sobre estas variables resultado mediante las curvas de Kaplan-Meier y la regresión de Cox.ResultadosIncluimos 133 pacientes, 103 (77,4%) hombres, con edad media de 74,8 años (DS 10,4; rango 52-93). En 97 casos (72,9%) presentaban lesiones tróficas. Se revascularizaron 87 pacientes (65,4%), 48 mediante técnicas abiertas y 39 endovasculares, 5 (3,8%) sufrieron una amputación mayor primaria, tratándose 41 (30,8%) de forma conservadora o mediante amputación menor. La supervivencia fue 85% y 81% a los 6 y 12 meses, con tasas de salvamento de extremidad de 84% y 82% y tiempo libre de reingreso de 52% y 31% para los mismos tiempos. La hipoalbuminemia aumentó el riesgo de mortalidad (p = 0,024) y una puntuación baja en el Mini Nutritional Assesment incrementó el riesgo de amputación mayor (p = 0,021). Hubo más reingresos en pacientes revascularizados mediante técnicas endovasculares (p = 0,001) y en los que presentaban lesiones tróficas (p = 0,001).ConclusionesLa malnutrición es un factor muy prevalente, potencialmente tratable, y determinante en el pronóstico, vital y funcional, de los pacientes con isquemia crítica de extremidades inferiores. ObjetiveTo study the factors that affect the vital and overall functional prognosis of patients with lower limb critical ischaemia (LLCI), including any diseased sector and treatment methods.Material and methodsA prospective cohort study was conducted on patients with LLCI. A record was made of their demographic data, cardiovascular risk factors, comorbidity, blood test parameters, Mini Nutritional Assessment (MNA) questionnaire, treatment, and outcome. An analysis was made on the mortality, limb salvage, and freedom from re-admission (FRR) rates, as well as the impact of the described parameters on these outcome variables, using Kaplan-Meier curves and Cox regression.ResultsA total of 133 patients were included, 103 (77.4%) men, with a mean age of 74.8 years (SD 10.4; range 52-93). There was tissue loss in 97 (72.9%) cases. A total of 87 (65.4%) patients were revascularised, 48 using open techniques and 39 endovascular. A primary major amputation was performed on 5 (3.8%) patients, and 41 (30.8%) were treated conservatively, with or without minor amputation. Survival rates were 85% and 81% at 6 and 12 months, respectively, with limb salvage rates of 84% and 82% and FRR of 52% and 31%, respectively, for the same time periods. Hypoalbuminaemia increased the risk of death (P = .024) and a low score in the MNA questionnaire was associated with increased risk of limb loss (P = .021). More re-admissions were observed among patients revascularised with endovascular techniques (P = .001) and those with initial tissue loss (P = .001).ConclusionsMalnutrition is a very prevalent and potentially treatable factor, which has great impact on the vital and functional prognosis of patients with lower limb critical ischaemia.

Conocemos la importancia de la bomba soleogemelar sobre el sistema venoso durante la marcha, por lo que proponemos un método de estudio de la hemodinámica venosa durante la marcha, un estudio que trata de correlacionar los hallazgos hemodinámicos con la secuencia de contracción y relajación muscular durante la marcha.Material y métodosRealizamos un estudio descriptivo prospectivo sobre un total de 50 adultos sanos en el Laboratorio de Análisis del Movimiento del Hospital Universitario Infantil Niño Jesús de Madrid. El sistema de captura del movimiento fue el SMART-D de BTS Bioengineering, formado por 8 cámaras optoeléctricas y un software para reconstrucción en 3 D. Se realizó EMG de superficie (EMGs) (Free EMG 300 de BTS Bioengineering) de los músculos gastrocnemio medial y lateral, y de los músculos tibial anterior y posterior. Los sujetos caminaron sobre una cinta sin fin BH (modelo Marathon) y se realizó ecodoppler venoso poplíteo (Nemio MX Toshiba) durante la marcha.ResultadosSe estudió a 29 varones (58%) y a 21 (42%) mujeres, con una media de edad de 32 años. Se realizó el análisis y correlación del estudio de la marcha con la EMGs y el ecodoppler venoso y se obtuvo una onda positiva en el ecodoppler que se correlaciona con la contracción excéntrica del músculo gastrocnemio medial, que estabiliza la rodilla; una segunda onda de presión negativa como consecuencia del cierre valvular y relajación muscular y una tercera onda de presión positiva correlacionada con la contracción concéntrica del músculo gastrocnemio medial durante la flexión plantar y de la rodilla durante la fase de despegue de la marcha.ConclusionesDescribimos un método de análisis de la bomba muscular gemelar en relación con la hemodinámica venosa y el efecto fisiológico de la bomba muscular sobre el sistema venoso profundo. ObjectivesThe gait is the pattern of movement of the human being. The physiological interrelation between the muscular and vascular system has not been sufficiently studied.The importance of the soleus-calf muscle pump on the venous system during walking is known; therefore a method is proposed in order to study venous haemodynamics during walking. The aim of the study is to correlate the haemodynamic findings with the muscle contraction and relaxation sequence during walking.Material and methodsA prospective descriptive study was conducted on a total of 50 healthy adults in the Movement Analysis Laboratory of the Niño Jesús Children's University Hospital in Madrid. The movement capture system was the SMART-D of BTS-Bioengineering, consisting of 8 optoelectronic cameras and software for 3 D reconstruction.A surface electromyography (EMGs) was performed, using a Free EMG 300 (BTS Bioengineering) of the medial and lateral gastrocnemius muscle and the anterior and posterior muscle. The subjects walked on a treadmill (BH Marathon) and a popliteal venous Doppler ultrasound (Nemio MX Toshiba) was performed whilst walking.ResultsThe study included a total of 50 participants, with a mean age of 32 years, and 29 (58%) men and 21 (42%) women. The analysis and correlation of the study was performed, with the EMGs and the venous Doppler ultrasound, with a positive wave being obtained with the Doppler ultrasound that correlated with the eccentric contraction of medial gastrocnemius muscle, stabilising the knee. There was a second wave of negative pressure as a consequence of valvular closure and muscle relaxation. A third wave of positive pressure was then recorded that correlated with the concentric contraction of the medial gastrocnemius muscle during plantar and knee flexion during the take-off phase of the gait.ConclusionsThe haemodynamic evaluation of the venous system by non-invasive tests can provide an objective basis for the diagnosis, evolution, and prognosis of patients with chronic venous insufficiency.

Gómez PalonésResumenEl uso actual del término endotensión en el tratamiento del aneurisma aórtico resulta controvertido. Originalmente fue propuesto para definir aquella situación en la que existe un crecimiento del saco aneurismático tras su exclusión endovascular y en la que no existe evidencia de endofuga. El término fue acuñado con la pretensión de que transmitiera la posibilidad de que fueran causas distintas a la presión las que ejercieran un estrés sobre la pared del aneurisma. Se realiza una revisión de la bibliografía con el objetivo de establecer una actualización de los avances del conocimiento de las posibles teorías etiológicas propuestas. Los estudios experimentales cuestionan que sean causas distintas a la presión las que condicionen el crecimiento del mismo. Se propone el término «crecimiento aneurismático sin endofuga identificada» por ser más preciso y ajustado a lo que se pretende definir. La evidencia sugiere que los mecanismos más probables de persistencia de presión elevada intraaneurismática son que la endofuga exista y no sea identificada (probablemente tipo i o tipo ii de bajo flujo) o porque se transmita a través del trombo en el caso de que este ocluya conexiones anchas y cortas entre la luz arterial permeable y el saco aneurismático excluido (ya sea en los puntos de anclaje de la endoprótesis o a través de los orificios de ramas colaterales). The current use of the term “endotension” in the treatment of aortic aneurysm is controversial. It was initially proposed to define the circumstance in which there is a growth of the aneurysm sac after endovascular repair without a demonstrable endoleak. The term was established with the aim of transmitting the possibility of causes other than pressure applying stress to the aneurysm wall. A review of the literature was performed with the aim of presenting an update on the advances of knowledge of the possible aetiological mechanisms. The experimental studies call into question that causes other than pressure determine the increase in the aneurysm. It is proposed that the term ‘aneurysm growth without identified endoleaḱ is more precise, and adjusted to what it is aimed to define. Evidence suggests that the more likely mechanisms of persistent pressurisation of the aneurysm sac are an unidentified endoleak (likely type I or low-flow Type II), or thrombus occluding wide and short channels that connect with the excluded aneurysm sac (at the attachment sites of the stent-graft or at the branch vessel orifices).
