  • The question of the closure of judiciary psychiatric hospital

      The author, Pietro Pellegrini (Department of Integrated Assistance, Mental Health Pathological Dependency - Ausl of Parma) discusses the recent closure of the judiciary psychiatric hospitals in Italy. Besides an historical reconstruction and an overview of the laws pertinent to the subject, the author underlines in particular the importance of coping with the delicate phase of change which we are witnessing. He takes into account the complex and multifaceted (bio-psychosocial) approach needed for helping the maturation of changes in the entire social network in order to prevent the new structures proposed (Rems) from becoming nothing more than replicas of the judiciary psychiatric hospitals

      The author, starting from his critique of the individualistic discourse in the ethics of traditional psychoanalysis, shows as he goes along the importance that psychoanalysis be always more understood as a social psychology in order to avoid drifting towards a reductive and fractional conception of man. With referral to the fundamental principles of complexity theory, he describes what he means by the word Person through a continual weaving of psychoanalytic, philosophical and existential dimensions. According to the author, only by understanding the individual as a Person can we arrive at a therapeutic (but also existential) prospective which instead of being mutilating can generate relations capable of reciprocity

  • Sexuality and the residents of psychiatric structures: clinical and
           institutional ascpect

      The residents of psychiatric structures often raise questions regarding sexuality, creating objective situations of difficulty for the staff who must manage extremely delicate situations from both clinical and ethical points of view. Scientific literature on the subject is scarce. The present contribution represents an attempt to find indications for a method of useful intervention within the organization of a group psychiatric structure. Some critical points are developed using situations taken from clinical practice that seemed paradigmatic and/or descriptive of the seriousness of the situations with which the staff is called upon to cope. In the presentation of clinical cases, some "key points" are highlighted that also call attention to the lived experience of the staff. The sexual questions raised by the patients are often not faced in institutional settings. Excluding the sexuality of the patient from the work of the team means subtracting the instruments necessary for reading one’s own lived experience, one’s own "counter-transference,- with the consequent isolation, solitude and abandonment of the staff member in respect to themes so emotionally involving

  • Emotions between body and language

      The author examines the fundamental importance of language in his work as an analyst. The article uses clinical flashes and examples from his daily practice which help the reader understand how he uses words with his patients. He demonstrates in particular how in psychosomatic disturbances the body becomes language and how the individual expresses everything he cannot put into words with his body. Emotional experiences, if not elaborated, are evacuated into the body, and the fundamental aim of analysis is to help the patient realize the link between thoughts and feelings. The "sickness" that is treated in analysis is fundamentally a sickness of language. The treatment must therefore be a treatment of the words

  • Why aggression' a synthesis of some psychoanalytic hypotheses from
           freud to current times

      The history of the psychoanalytic hypotheses on the genesis of aggression is briefly reviewed. Firstly, Sigmund Freud’s thought is examined, following a 1981 essay by Gian Vittorio Caprara where Freud’ papers are discussed in chronological order, including the pivotal paper "Beyond the pleasure principle" of 1920. The positions of the following psychoanalysts are then briefly reviewed: Alfred Adler, Anna Freud, Melanie Klein, Wilhelm Reich, Otto Fenichel, Heinz Hartmann, Erich Fromm, Heinz Kohut, Otto Kernberg, Joseph Lichtenberg, Drew Westen, and Peter Fonagy. The aggressive drive (also called death instinct, thanatos, mortido, etc.) has been generally explained according to these two basic hypotheses: as originating from the inside (aggression as instinctual drive) or from the outside (aggression as a reaction to frustration of important needs). An abandonment of this dichotomy is suggested, also because it is based on an outdated conception of mind development. Current research on neurobiology and philosophy of mind shows a continuous interplay, from birth onward, between the brain and the environment
