  • Psychotic onset and cognitive behavioural therapy

      The authors present two exemplary clinical cases that outline two different psychotic onset frameworks. The theoretical frame of reference is of cognitive-behavioral matrix: from the clinic to the model, it places emphasis on the construction of the therapeutic alliance with psychotic patients, the assessment, the formulation of the case, the discovery of personal history and finally, the definition of the schemes. Therapeutic interventions appropriate to psychotic manifestations will follow.

  • Psychotic onset or developmental breakdown. A difficult dilemma

      The article, first of all, analyses the deep change caused by the transition from a paradigm of care to one of prevention and then studies the problems resulting from this transformation. The new conceptual model of schizophrenia as neurodevelopmental disorder is described as well as the subsequent research on identifying at-risk mental states referencing SPI-CY. After some clinical vignettes showing the difficulty of applying the traditional diagnostic categories to the teenagers’ mind, an evaluation and intervention model is proposed which focuses on the evolutive dimension and on the new research related to affection regulation.

  • The open dialogue approach for early psychotic stages. The return of
           family therapy in mental health Departments

      The author, after a synthetic premise addressing the major experiences in the relational-systemic area in the early psychotic stages, deepens the therapeutic approach of public mental health services in Finland. These experiences, which began at the end of the sixties with Alanen, took a new impetus and achieved excellent clinical results in the context of the clinical intervention, thanks to the work coordinated by Seikkula, in Lapland. This work focused on the individual with psychotic onset, his family and social context which prove to be emotionally significant. The Open Dialogue Approach is then enhanced compared to the trial that has been launched in Italy through several DSMs.

  • Beyond the walls of silence. Essay on the mental hospitals madness

      The author proposes an interesting reflection that highlights the profound analogies between the work of Anton Cechov (Ward n. 6 - 1892) and the reformist thought promoted in Italy by Franco Basaglia, more than 70 years later. Cechov’s narration proves to have a prophetic value somehow, proposing in a transparent and profoundly bitter way the reality of the institution of mental hospitals. This reality picture is described through vivid images of everyday life inside the structure and through the dramatic confrontation between patient and physician, in whose dialogues all the frustrating sense of the institutional incompleteness and its intrinsic therapeutic failure is noticeable. In the lecture, the ascertainment of the destiny of marginalization assigned to the institutionalized mental patient, who is dehumanized and deprived of any dignity and will, allow for a strong ethical and emotional resonance to emerge, which is still current.

  • Early intervention unit. The building up and clinical experience of a
           community-based unit for early intervention

      The ten-year experience of Early Intervention is presented in the early stages of psychosis, implemented in the Mental Health Department of the ASL Roma 3. In it, a synthesis was sought between the competences already present in the Department and the scientific and organizational innovations coming from the Literature and from the international experiences (intervention models, neuro and cognitive partner rehabilitation, tutoring, etc.). The challenge is to provide a timely, intensive, pro-active intervention, in which the various components are activated simultaneously (drugs, psychological support, family intervention, maintenance of social roles - school, work, free time). The goal is to achieve not only a clinical recovery but also a functional one, by activating personal and family skills to allow a lasting change and re-adaptation, respectful of the needs and aspirations of the patient and his context, but also to help recognize and accept limits and difficulties.

  • From psychotic thought to emotional-behavioural dysregulation... And back
           again. The young Anna

      A diagnostic formulation of a clinical case of a 12-year-old patient is presented which is studied through a continuous symptomatic analysis, which identifies the individual vulnerability, the exposure to early and perpetuated traumatic factors, the occurrence of negative symptoms (social withdrawal, rage explosions, negativism and cognitive deterioration) and the reference idea. This symptomatic pattern is compared with the child’s personal and familiar history and with the attachment figures relationship, considering specifically the features of "protectiveness and control", which are reflected in interpersonal relationships. The separation and the subsequent reunification with the mother and the family acts as a trigger for the development of a related symptomatology associated with disorganization of thought processes. The analysis and the understanding of the symptoms allows the identification of an association between the rigid and persevering thought patterns, the reference idea, the occurrence of negative emotional states, and the development of behaviors constituted by threats and self-hetero-aggressive conduct, including suicidal ones.

  • The child and her maps. Comment on Anna’s clinical case

      The contribution to the lecture of Anna’s case starts from the well-established interpretations of psychotic symptoms, in particular to reference ideas, as an expression of the attempt, from the person, to restore a sense of ambiguity, which is compatible with central schemes. The emotional, cognitive and behavioral detachment that derives from it, is therefore understood as a consequence of the impossibility of accessing dimensions that are closer to reality as they are excessively destructuring, overwhelmingly denied by the context of reference and confusedly perceived, but misunderstood, by the patient herself.

  • System approach to the psychotic onset. Ethical motivations and prevention
           opportunities. Comment on Anna’s clinical case

      The commentary on Anna’s clinical case highlights the need to take charge of the whole family in situations of serious psychopathological emergencies. On the one hand, this takes place by recovering the old intentions of family therapy by giving back to the family therapist those suffering systems that have inspired the birth of the systemic approach to psychological care. On the other hand, nowadays it is proposed as a tool that allows risk prevention of chronicization more effectively, if group activation (therapeutic on one side and family on the other) takes place from the onset.

  • Psychotic onsets. Clinical implications of attachment styles,
           metacognitive beliefs and doctor-patient communication

      During the last decades of the century it has become increasingly evident that the early phase of a schizophrenic psychosis and the period preceding the onset of positive symptomatology are of crucial importance for subsequent outcomes of the disease and for future developments in the patient’s life and their families. The possibility of early implementation of appropriate therapeutic interventions can significantly improve the prognosis of these disorders. In this paper the authors present a brief review of the research aimed at identifying some aspects (related to attachment styles, cultural identity, metacognitive beliefs, doctor-patient communication) that can significantly affect both the evolution of the disease and the effectiveness of the therapeutic interventions implemented during the early stages of the disorder.

      The Author traces in G. Amelio’s latest film "Tenderness" the relationship between words and non-verbal communication, actions between family members or couples and affection affinities. The movie deals with tri-generational families as has long been the case, for example, in the systemic psychotherapy of the family. Words are not enough. It is necessary to take into consideration the body’s reactions to identify the psychological situation.
