      Mindfulness has been defined as "the awareness that emerges through paying attention on purpose, in the present moment, and non-judgementally to the unfolding experience moment by moment". It has been cultivated through several forms of meditation derived from Buddhist meditative practices and is a skill which can be developed with practice.

      It does seem wrong to suggest that the depressed elderly could benefit from doing cognitive behaviour therapy over the internet (iCBT). Surely older people who are depressed need empathic understanding of their changing life circumstances, not tuition from a cold and impersonal computer program. Recent data suggest that this may not be so.

      Metformin is widely used for the treatment of Type 2 diabetes but there have been questions about its association with lactic acidosis. Metformin is primarily excreted unchanged in urine and its renal clearance is approximately proportional to the clearance of creatinine. Lactic acidosis is uncommon during treatment with metformin but excessive accumulation may exacerbate lactic acidosis.

      Steroids such as prednisone, methylprednisolone, hydrocortisone and dexamethasone have many benefits and are in widespread use, particularly in the elderly population. They are affordable and potent anti-inflammatory drugs used to treat a broad spectrum of conditions. Although the efficacy of glucocorticoids for treating a broad spectrum of inflammatory and autoimmune diseases is relatively well established, their tendency to induce potentially serious and often irreversible side effects, especially with long-term, high-dose treatment, continues to be a management challenge.

      GPs are often the first point of contact for people who are worried about their memory. Whilst some reduction in memory abilities is to be expected in later life, dementia is not a normal part of the ageing process. It is important for GPs to consider the possibility of brain pathology in anyone complaining of memory loss, and to be alert to any early signs of global cognitive change.

      Urinary tract infections (UTIs) are common in older patients, particularly women, and can be associated with significant morbidity. The lifetime risk of developing a UTI is approximately 50% for women compared with 5% in men. The incidence increases with advancing age and is particularly high in post-menopausal woman. In addition to pain, UTI may present with systemic manifestations including anorexia, nausea, fatigue, delirium and sepsis - common reasons for hospital admission. An acute geriatric syndrome, including falls, delirium, immobility and incontinence, may be the presenting symptom of a UTI.

      In 2009, the Australian Diabetes Society released a position statement on the individualisation of glycaemic targets for adults with diabetes mellitus. These guidelines support the notion that targets should be individualised based on the type of diabetes; duration of disease; nature of the diabetes medications being taken; presence of cardiovascular disease; risk and awareness of hypoglycaemia; as well as presence of co-morbidities.

      Wounds can be extremely painful, debilitating and often require frequent medical and community nursing attention. While the relationship between nutritional status and wound healing has been well documented in the literature it is frequently overlooked in practice. A chronic, slowly healing or dehiscent wound will lead to the continued depletion of lean body stores. It may result in pain, decreased mobility and an overall decreased quality of life.

      When it comes to incontinence, compared to women, males are the "minority group" within the 4 million Australians coping with poor bladder or bowel control. In older age groups, the incidence becomes more equally divided between genders and because of its co-morbidities, incontinence is more prevalent and severe in the over-75s, where up to 40% of people are affected.

           Residential Care

      The number of persons with dementia in Australia has been projected to triple between 2005 and 2030, and it is highly unlikely that the numbers of residential aged care beds will increase commensurately. In recognition of this stark reality, the Federal government is already placing an increased emphasis on the use of home-based care through the provision of both Community Aged Care and Extended Aged Care at Home (EACH) packages. It is vital, therefore, that GPs are well versed in the management of the Behavioural and Psychiatric Symptoms of Dementia (BPSD).
