  • Volume 15 Issue Autumn - Fish Oil: Phytoplankton and the Food Chain
    • Abstract: Colquhoun, David
      Concentrated fish oil and fish oil derivatives are one of the most effective therapies to prevent first and recurrent vascular events - heart attack, stroke and sudden death. In 2002, the American Heart Association Nutrition Committee recommended that all patients with coronary artery disease should have 1 gram of omega -3 fatty acids Eicosapentaenoic acid (EPA) plus Docosahexaenoic acid (DHA) per day. The European Societies of Cardiology and the Australian National Heart Foundation made similar recommendations in 2007 and 2008. The key pivotal randomised trial that showed benefit beyond reasonable doubt was the GISSI-P.

      You are a general practitioner in a city on the outskirts of the Brazilian Amazon. A previously well 35-year-old presents febrile, dyspnoeic and complaining of chest pain and myalgia. Clinical examination reveals the signs of severe heart failure. Your key differential diagnosis as a GP in the southern cone of South America should be Chagas' disease. This condition, largely unheard of in the rest of the world, is the leading global cause of infectious myocarditis and is an enormous burden on the health systems in numerous countries in South America.

      There is increasing recognition that clinical advice on nutritional issues should be couched in terms of foods and patterns of food consumption rather than percentages of fats, carbohydrates etc. In fact the most consistent findings from prospective studies in >200,000 women (Nurses Health Study) and men (Health Professionals Study) are that two patterns of eating led to large differences in future cardiovascular outcomes.

      The Coronary Slow Flow Phenomenon (CSFP) was initially described some 40 years ago but has been neglected until recently. The diagnosis is made in patients with chest pain who have no evidence of significant coronary artery disease (CAD) on coronary angiography. Thus to place the condition in perspective, it is important to consider other causes of chest pain in patients with normal angiography. In the following sections the basic principles in evaluating patients with chest pain and normal angiography will be discussed followed by the coronary disorders that may produce angina in the context of normal or near-normal angiography, including the CSFP.

           Cardiac Conditions'

      Cardiac pathology is rather uncommon amongst the Young so therefore investigating young athletes for cardiac conditions is not routine practice everywhere. However undiagnosed cardiac conditions can not only compromise effort tolerance and hinder performance but can also cause sudden cardiac death (SCD) in the young athlete. The primary objective of appropriate screening and investigating potentially risk prone young athletes is to prevent the dreaded consequence of SCD. Given the rarity of this occurrence there is no consensus yet on the best and most cost effective means of screening to identify the susceptible individuals.

      Chest pain is a common presentation to health services across the world. Early presentation to hospital after the onset of acute chest pain has been promoted by organizations such as the American Heart Association, and the Heart Foundation of Australia, so that patients with acute myocardial infarction can benefit from early revascularization therapy. There has been a proportionately greater increase in admissions with noncardiac or what has come to be known as 'atypical chest pain '.

      A cardiac murmur is defined as turbulent blood flow that is sufficient to cause noise audible by auscultation of the precordium. Cardiac murmurs are identified in all stages of life and can be benign and innocent, functional, or pathological. In the adult population the identification of a new or previously undocumented murmur raises various questions. It mandates, at the least, correlation with the clinical picture, physical examination, and thorough history taking. An ECG, chest x-ray and echocardiography are considered standard in the initial investigation of a new murmur. These investigations will often reveal a benign murmur which has simply not been heard before. There are, however, certain circumstances where the presence of new murmur is a sign of significant underlying pathology.

           Registry

      Over the last two decades, major advances have been made in our understanding of the genetic basis of heart disease. Currently there are over 40 different cardiovascular diseases which are directly caused by mutations in key cardiac genes. These genetic (inherited) heart diseases span many aspects of cardiovascular medicine including cardiomyopathies, primary arrhythmogenic disorders, congenital heart diseases and metabolic disorders. Interestingly, almost all these genetic heart disorders are inherited in an autosomal dominant fashion, meaning the risk of transmission of the disease from one generation to the next is 50%.

      Atrial fibrillation is an increasingly common rhythm disorder which challenges clinicians looking after all forms of heart disease. It is now clear that its prevalence is increasing due to the ageing population and the rising prevalence of chronic heart disease. It is estimated that 5-15% of those age 80 or older will develop this arrhythmia. Overall, 2% of the population may be affected, taking into account the presence of undetected arrhythmias.

      Beta-D-glucan (beta-glucan) from oats is a mixed linkage polysaccharide soluble fibre resulting in a random coiling of molecules. This characteristic causes viscosity in solution and is responsible for wide ranging physiological effects, including the well-known benefits of cholesterol lowering, improvements in glycaemic response and improved blood pressure control. More recent research has shown that beta-glucan may improve satiety, and potentially weight control, either by mechanisms related to its glycaemic actions or to actions on appetite hormones.

      NSAIDs (Non-Steroidal Anti- Inflammatory Drugs) are the cornerstone of treatment in a variety of acute and chronic pain conditions, most notably musculoskeletal complaints. Their efficacy is well established. Whilst contemporary analgesia protocols emphasise the role of 'simple' paracetamol based analgesia, there remains a clear role for NSAIDs, as evidenced by strong patient preferences for this form of therapy.
